Abstract:
Abstract Background: IM, an oral molecular targeted therapy, has demonstrated impressive efficacy in treating advanced CML. In China, IM has been approved to treat all phases of CML. However, it was not considered for reimbursement at national level. Chemos such as Homoharringtonine combined with cytarabine (HA) or Theprucine in combination with cytarabine (TA) have been used and reimbursed to treat advanced CML patients. The objective of this study was to conduct a total cost of treatment analysis comparing IM with chemo (HA or TA) in combination with IFN in AP and IM vs. chemo in BC CML in China. Methods: Total cost of treatment contained drug cost, office visits, lab tests, hospitalizations, and cost of managing adverse events (AEs) and supportive care. The cost of managing AEs included the use of G-CSF, anti-infective medications, and blood transfusions. Treatment protocols for AP and BC CML from tier 3 hospitals in China were used to estimate the costs associated with treatment. Drug prices were based on the listed retail price in China. Dosages for IM and IFN were based on the approved product labels in China. Results: AP patients received on average 3 courses of chemo in the hospital during a year and were treated with interferon-while off the chemo. BC patients received one course chemo every two months. As shown in the table, the 12-month cost of treatment for an AP patient treated with IM was RMB119,850 vs. RMB153,900 with chemo plus IFN. The corresponding numbers for a BC patient were RMB133,450 with IM vs. RMB18,5400 with chemo. Hospitalizations and costs of managing adverse events and supportive care were the key drivers of the savings for IM. The health care costs savings related to taking IM outweigh the drug cost difference between IM and chemo plus IFN in AP or IM and chemo in BC. Conclusion: AP CML patients treated with IM incurred over RMB30,000 less health care costs in 1-year than those treated with chemo plus IFN. The savings were even more in BC: the 1-year cost of treatment with IM was over RMB50,000 lower than that with chemo. The cost of chemo used in this analysis was based on the 1st line chemo used in China. If patients failed the 1st line chemo, more expensive 2nd line chemos were usually used. Therefore, the total cost of treatment with chemo in advanced CML could be even more if 2nd line chemos were considered in this analysis. Providing advanced CML patients access to IM will not only improve clinical outcomes but also reduce the overall economic burden to the Chinese health care system. Abstract:
In this work an epitaxial layer of Indium Phosphide (InP) has been grown on top of a silicon substrate using the Corrugated Epitaxial Lateral Overgrowth (CELOG) technique. The grown InP CELOG top layer typically has a poor surface roughness and planarity. Before this surface can be used for any processing it has to be smooth and planarized. For this purpose a two-step Chemical Mechanical Polishing (CMP) technique has been investigated and developed. In the first step commercially available Chemlox has been used to planarize the sample. In the second step Citric Acid (CA) and sodium hypochlorite (NaClO) has been mixed together to form abrasive-free polishing slurry. The second step has been developed to remove defects introduced by the first step. This surface is prepared to demonstrate that a photonic device such as a quantum well can be realized in a Photonic Integrated Circuit (PIC). A quantum well was grown on the polished CELOG InP/Si sample and measured with Atomic Force Microscopy (AFM), Scanning Electron Microscopy (SEM), X-ray Diffraction (XRD) and Photoluminescence (PL). The roughness was improved with CMP from 33.2 nm to 12.4 nm. However the quantum well did not give any response in the PL measurements.<br>I detta arbete har ett epitaxiellt lager indiumfosfid (InP) blivit växt på ett kiselsubstrat med hjälp av en korrugerad epitaxial lateral överväxt (CELOG) teknik. Det översta lagret av den CELOG växta ytan har ofta en väldigt ojämn yta. Innan denna yta kan användas till någon fortsatt utveckling måste den vara slät och plan. Det översta lagret har polerats med hjälp av en två-stegs kemisk mekanisk polerings (CMP) teknik. I det första steget har komersiellt tillgänglig Chemlox använts för att planarisera ytan. I det andra steget har citronsyra (CA) och natrium hypoklorit (NaClO) blandats samman för att bilda ett partikelfritt polermedel. Det andra steget har tagits fram för att ta bort defekter introducerade I det första steget. Ytan är preparerad för att demonstrera att en fotonisk enhet, t.ex. en kvantbrunn kan realiseras I en fotonisk integrerad krets (PIC). Abstract:
The enzyme transketolase (TK) [E.C.2.2.1.1] catalyses the stereoselective transfer of a two-carbon ketol unit from a donor substrate such as lithium hydroxypyruvate (LiHPA, i) to an α-hydroxyaldehyde. Research into the use of TK from <I>Escherichia coli </I>as a process catalyst for asymmetric carbon-carbon bond formation has required the development of synthetic routes to novel acceptor substrates. The preparation of novel α-hydroxyaldehydes in enantiomerically pure form from chiral pool α-hydroxy-or α-amino acids, and of racemic α-hydroxy-aldehydes of utility in the synthesis of natural produce analogues, is described. Formerly, biotransformations mediated by TK involved the use of an excess of aldehyde substrate in buffered aqueous solution. An alternative protocol is presented, in which the biotransformations are performed in unbuffered medium; the natural pH change during the biotransformation is offset by use of a pH autotitrator to maintain the solution pH at the process optimum of 7.0. The synthetic utility of the chiral triols produced from TK-mediated biotransformations has been demonstrated through the development of synthetic routes to the natural product nectrisine, ii, and the <I>N-</I>hydroxypyrrolidine sugar analogue iii from the triols iv and v, available <I>via</I> TK-mediated condensation of LiHPA with the appropriate α-hydroxyaldehyde.

Abstract:
Using a state of the art galaxy formation software package, GCD+, we model the formation and evolution of galaxies which resemble our own Galaxy, the Milky Way. The simulations include gravity, gas dynamics, radiative gas cooling, star formation and stellar evolution, tracing the production of several elements and the subsequent pollution of the interstellar medium. The simulations are compared with observations in order to unravel the details of the Milky Way's formation. Several unresolved issues regarding the Galaxy's evolution are specifically addressed. In our first study, limits are placed on the mass contribution of white dwarfs to the dark matter halo which envelopes the Milky Way. We obtain this result by comparing the abundances of carbon and nitrogen produced by a white dwarf-progenitor-dominated halo with the abundances observed in the present day halo. Our results are inconsistent with a white dwarf component in the halo 5% (by mass), however mass fractions of ~1-2% cannot be ruled out. In combination with other studies, this result suggests that the dark matter in the Milky Way is probably non-baryonic. The second component of this thesis probes the dynamical signatures of the formation of the stellar halo. By tracing the halo stars in our simulation, we identify a group of high-eccentricity stars that can be traced to now-disrupted satellites that were accreted by the host galaxy. By comparing the phase space distribution of these stars in our simulations to observed high-eccentricity stars in the solar neighbourhood, we find devidence that such a group of stars - a 'stellar stream' - exists locally in our own Galaxy. Our next set of simulations demonstrate the importance of strong energy feedback from supernova explosions to the regulation of star formation. Strong feedback ensures that the building blocks of galaxy formation remain gas-rich at early epochs. We demonstrate that this process is necessary to reproduce the observed low mass and low metallicity of the stellar halo of the Milky Way. Our simulated galaxy is shown to have a thick disk component similar to that observed in the Milky Way through an abrupt discontinuity in the velocity dispersion-versus-age relation for solar neighbourhood stars. This final study suggests that the thick disk forms in a chaotic merging period during the Galaxy's formation. Our thick disk formation scenario is shown to be consistent with observed properties of the thick disk of the Milky Way.

OBJETIVOS/INTRODUCCIÓN ChemSex es el término usado en la comunidad científica para referirse al uso intencionado de drogas psicoactivas (CHEM) para mantener relaciones sexuales (SEX), entre hombres (HSH), durante largos periodos de tiempo, con múltiples parejas, y uso de apps para buscar contactos y drogas. Se asocia a riesgos con el consumo, salud sexual e integridad física, psicológica y social, siendo prioridad de salud pública estudiar este fenómeno y el desarrollo de programas de tratamiento eficaces. Nuestro objetivo es profundizar en el conocimiento y mejora de la intervención en pacientes que acuden a un dispositivo de atención a las adicciones (CAS) con práctica de Chemsex. MATERIAL Y MÉTODOS Estudio de caso clínico y revisión bibliográfica RESULTADOS Y CONCLUSIONES Derivación al CAS desde el Servicio de Enfermedades Infecciosas hospitalario de paciente de 48 años, VIH positivo, y otras ITS, inmigrado, activo laboralmente con alta especialización. Refiere trayectoria de psicopatología de larga evolución con dos periodos de agudización, el primero coincidiendo con la llegada a Barcelona y el segundo hace 2 años en contexto de ruptura de pareja e inicio de la práctica de Chemsex, con policonsumo (OH, cocaína, speed, MDMA, poppers, GHB i MA) y uso de apps. Destaca el malestar subjetivo agudo, sentimiento de culpabilidad y conductas autodestructivas, incluyendo alteración de la conducta alimentaria y de la sexualidad. Se observa disociación de varios aspectos de la personalidad, no integrados. Se establece plan de tratamiento individualizado. Destacamos la importancia de ofrecer un programa de tratamiento que identifique e intervenga en los determinantes multifactoriales del inicio y mantenimiento del uso problemático del Chemsex, y en el abordaje de las repercusiones derivadas de la práctica y psicopatología concomitante.

Introducción Nuestro objetivo fue explorar la ocurrencia de experiencias traumáticas en la infancia y la presencia de dificultades de regulación emocional en una muestra de usuarios del programa de Chemsex de la ONG Apoyo Positivo(AP+). Material y métodos Se ofreció realizar una encuesta on-line a los usuarios del programa que habían practicado Chemsex en el último año y a un grupo control sin dichas prácticas. Los grupos no difirieron en edad, sexo y orientación del deseo(hombres gais /bisexuales). Incluimos cuestionarios relacionados con consumo de drogas (DUDIT), regulación emocional(DERS) y experiencias traumáticas tempranas(EARLY-G). Presentamos el análisis descriptivo del grupo de usuarios(n=41). Comparamos usuarios y grupo control en variables relacionadas con trauma y regulación emocional(n=39). Resultados y conclusiones La media de edad de los usuarios fue 38,4 años, 56% nacieron en España y 34% en Latinoamérica. El 71% fue diagnosticado de VIH. Las drogas más consumidas en contexto de chemsex fueron: mefedrona(100%), metanfetamina inhalada(88%), GHB(90%), cocaína(66%) y ketamina(68%). Otras sustancias utilizadas en las sesiones de sexo y drogas fueron: poppers(78%), sildenafilo o similar(68%) y alcohol(27%). El 66% inyectó drogas en dichas sesiones(slamsex). Todos los usuarios presentaron puntuaciones sugerentes de consumo de drogas de riesgo y el 34% probable dependencia en el test DUDIT. Los usuarios refirieron significativamente mayor frecuencia de abusos sexuales tempranos que los controles (22 vs. 5% en la familia cercana y 23 vs. 10% fuera de la familia), y problemas económicos graves en la infancia(58 vs. 5%)(p&lt;.05). Los usuarios obtuvieron puntuaciones asociadas a mayor desregulación emocional general que los controles (DERS total, X(DT)=73,1(20,6) vs. 59,4(15,3)) y peores puntuaciones en las sub-escalas de descontrol, rechazo e interferencia (p&lt;.05). Algunas personas que practican Chemsex parecen presentar experiencias traumáticas tempranas y dificultades de regulación emocional. De confirmarse nuestros resultados, esta población podría beneficiarse específicamente de terapias centradas en trauma y regulación emocional.

Abstract:
El chemsex o uso intencionado de drogas para mantener relaciones sexuales entre hombres que tienen sexo con otros hombres (HSH) ha aumentado en los últimos años en España. Pese a tratarse de un fenómeno emergente existen pocos datos sobre sobre las intervenciones eficaces. - Objetivo: describir una muestra de pacientes que demandaron tratamiento por chemsex en una unidad de conductas adictivas en Barcelona, entre enero de 2017 y junio de 2019. - Material y métodos: se incluyeron un total de 53 pacientes a través de la revisión de historias clínicas. Se realizó un análisis descriptivo a partir de los datos sociodemográficos y clínicos obtenidos. - Resultados: se atendieron un total de 53 pacientes, 100% hombres. El 44,9% fueron derivados del servicio de enfermedades infecciosas. La media de edad era de 37,7 años. La mayoría pidieron tratamiento por consumo de metanfetamina (81,1%). La vía de administración más utilizada fue fumada (74,4%), el 16,3% inyectada (slamming) y el 11,6% utilizaba ambas vías indistintamente. El policonsumo fue elevado, siendo el GHB consumido por el 75,5% de la muestra. Se detectó psicopatología en el 77,4% de la muestra, siendo la más prevalente la psicosis (28,3%). El diagnóstico de VIH y otras ETS fue frecuente (69,8% VIH+; 49,1% otras ETS). Excepto un paciente, el resto seguían tratamiento antirretroviral y un 75,5% tenía una carga viral indetectable. Durante el tratamiento el 24,5% consiguió la abstinencia y un 39,6% redujo el consumo. Un 45,1% abandonó el tratamiento. - Conclusiones: la metanfetamina es la droga más consumida en Barcelona para practicar chemsex. El policonsumo es elevado. Es frecuente que coexistan problemas de salud mental con la adicción, siendo los síntomas psicóticos los más prevalentes. Se recomienda la coordinación con los servicios de enfermedades infecciosas y la búsqueda de estrategias para mejorar la adherencia al tratamiento.

Abstract:
This work provides a computational model of the chemo-mechanical behavior of based refractories used in waste to energy plant (WTE) linings. In this application, oxygen gas present in the atmosphere diffuses through the porosity and reacts with the refractory producing silica (SiO2). This new phase clogs gradually the pores and causes swelling of the refractory lining. The proposed thermo-chemo-mechanical model which simulates these phenomena is briefly summarized. The results obtained from the model implemented in a F.E code prove the ability of the model to reproduce qualitatively the swelling of post-mortem bricks taken from WTE linings.
