Abstract:
This thesis handles the issue of Project Management, especially formation and structuring of the plan of project control (Project Management Plan). Formation of the Project Management Plan is one of the activities, which is performed by project manager in cooperation with customer in the first phase of project life cycle. The content of this thesis is a proposal of optimised structure of Project Management Plan with predefined description and extent of individual parts (chapters). In addition, there are different methodologies and their comparison described in this thesis, which are usually used in Project Management. These are PMBOK, Prince2, ČSN ISO 10006 and the project methodology of Siemens, which is called Chestra. These methodologies form the base for a newly formed structure of Project Management Plan. In the supplement of this thesis, there is a template of the document Project Management Plan, with which a project manager is able to form a new management plan of particular project easily. This template was created on the basis of particular project analysis within the scope and work of Siemens IT Solutions and Services Company and reflects not only theoretical knowledge, but also the needs of the particular company.

Abstract:
This thesis is a cross-disciplinary study of one of the culture-specific words important for a given society ("concepts") – concept chest' (honor) - that has a considerable weight in the Russian cultural tradition. The study aims to transcend disciplinary boundaries in order to examine the cultural construction of honor in the Russian Worldview. "Concept" is not just a lexical item but a distinctive "file" containing semantic and aesthetic information. "Concepts" reflect and pass on people's values, ideals, attitudes as well as a way of thinking about the world. They provide important clues to the understanding of culture. Elucidation of a concept chest' marked by a moral significance is thus able to provide a better understanding of a particular period of Russian cultural history – the first four decades of the nineteenth century.<br>The study analyzes the integrated structure of concept chest' which includes different components (Chapter 2). The analysis uses a variety of methods, including etymological and componential approaches, followed by an examination of relevant conceptual metaphors and the correlation between such concepts in the Russian Worldview as honor – conscience (chest' – sovest'), honor – dignity (chest' – dostoinstvo), honor – shame (chest' – pozor), conscience – shame (sovest' – styd). The gender component of concept chest' is also examined.<br>Since concept chest' is one of the key words of Russian Romanticism and has a culture-specific meaning that reflects society's past experience, Chapter 3 not only discusses the evolution of the concept connected to the cultural changes, but also traces the reconstruction of the concept chest' in the literary context of the period focusing on the works of K.Ryleev, A.Bestuzhev-Marlinskii, and M.Lermontov. Concept chest' was shaped in a gentleman's code of honor and bound to a dueling ritual (duel of honor) and gambling (debt of honor). Although it was the golden age of noble personal honor, the explication of the given notion in Lermontov's works shows the beginning of the concept's transformation that led to the subsequent devaluation of the meaning of chest' in society.<br>Cette thèse constitue une étude interdisciplinaire des mots spécifiques à une culture, qui sont importants pour une société donnée (des "concepts") – et plus précisément le concept tchest' (honneur), ayant un poids considérable dans la tradition culturelle russe. L'étude a comme but de transcender les frontières disciplinaires afin d'examiner la construction culturelle de l'honneur dans la perception russe du monde. Les « concepts » ne sont pas seulement des termes de vocabulaire, mais également des « dossiers » contenant de l'information sémantique et esthétique. Les « concepts » reflètent et transmettent des valeurs humaines, des idées, des attitudes, ainsi qu'une manière déterminée de percevoir le monde. Ils fournissent des pistes importantes permettant de comprendre une culture. L'élucidation du concept tchest' d'une perspective morale permet de mieux comprendre une période particulière de l'histoire culturelle russe, soit les premières quatre décennies du XIX siècle.<br>Cette étude analyse la structure intégrée du concept tchest' prenant en considération différents composants (chapitre 2). L'analyse utilisée s'appuie sur une variété de méthodes, incluant les approches étymologique et componentielle, suivies d'un examen de métaphores conceptuelles importantes et d'une corrélation des concepts dans la conception du monde russe tels que : honneur – conscience (tchest' – sovest'), honneur – dignité (tchest' –dostoinstvo), honneur – honte (tchest' – pozor), conscience – pudeur (sovest' – styd). Le composant du genre du concept tchest' est également abordé.<br>Étant donné que le concept tchest' est un des mots-clefs dans le romantisme russe et possède une signification culturelle qui reflète l'expérience sociale découlant du passé, le chapitre 3 discute non seulement de l'évolution du concept reliée aux changements culturels, mais aussi redéfinit le concept tchest' dans le contexte littéraire de cette période, se centrant sur les œuvres de K. Ryleev, A. Bestuzhev-Marlinskii et M. Lermontov. Le concept tchest' fut bâti dans le code d'honneur des gentilshommes et était relié à un rituel de duels (duels d'honneur) et de jeux (dettes d'honneurs). En dépit du fait que c'était l'époque dorée de l'honneur personnel des nobles, l'explication de ce concept dans l'œuvre de Lermontov montre le début de la transformation du concept qui a véhiculé la dévaluation subséquente de la signification de tchest' dans la société.

Jag har i mitt projekt jobbat med mönsterformgivning och produktion av vävda möbeltyger, digitaltryckta tapeter- och textilier för en fiktiv hemmiljö. Mina designbeslut grundar sig på en fiktiv karaktär, Mr Chester Jester, hans estetiska preferenser och behov i samrådan med mina egna. Arbetets resultat är en gestaltning av världen han lever i, komprimerad till ett hemma hos reportage på gränsen mellan verklighet och illusion. Jag har jobbat med dubbelhet och speglingar - av mönster, av rummet, av tekniker och mig själv. Jag har undersökt illusionens verklighet och verklighetens illusion.My bachelor project is a work of pattern design and production of woven upholstery fabrics, digital prints on textile materials and wallpaper, made for a fictional home environment. The desicions for the designs are based on a fictional character, named Mr Chester Jester. His preferences and needs combined with my omn are the result of this project. The outcome of the work is a formation of his world, made into a study of his home on the edge of reality and illusions. "Chester Himes (1909-1984), écrivain tragique", est une analyse de la tragédie des oeuvres de Chester Himes. L'ouvrage a trois partie. La première partie s'ouvre sur des analyses sur la tragédie de l'histoire américaine, de la vie de Himes depuis l'Amérique jusqu'à son exil en France et en Espagne où il est mort en 1984. Puis, j'introduis les seize romans de mon aire de recherche. La deuxième partie est une réflexion sur la littérature orale, sur la monstruosité des villes américaines, sur le style annonçant la tragédie (métaphore, proverbe, analepse, prolepse). La troisième partie est consacrée à l'étude d'une trentaine de thèmes tragiques, dont la peur, la fuite, l'absurdité de la vie, la violence et l'eschatologie du héros de Himes. Né à Kiev en 1866 et mort à Paris en 1938, Léon Chestov, de son vrai nom Lev Isaakovitch Schwarzmann, demeure l'une des figures les plus originales et les plus énigmatiques de la renaissance philosophique russe du début du siècle. Il reprend et approfondit un thème qu'avaient traité les slavophiles, celui de la fausseté et du poison du rationalisme, mais avec une audace, une rigueur et une intransigeance qui surpassent toutes les revendications irrationalistes que connaît l'histoire de la philosophie. Chestov repense la connaissance ainsi que l'éthique dans le contexte du mystère du péché originel, tel qu'il est révélé dans la Bible. Une substitution désastreuse s'est produite : l'homme a pris l'arbre de la science pour l'arbre de la vie. De créature libre et souveraine il est devenu l'esclave des évidences, des lois et des contraintes rationnelles. Il n'y a que la foi, conçue sur le mode du saut dans l'absurde kierkegaardien, du par-delà le bien et le mal nietzschéen ou des tenebrae fidei clamees par Luther, qui peut délivrer l'âme vivante de "l'enchantement et de l'assoupissement surnaturels" et de la tyrannie exercée par la raison autonome, donc, indifférente aux aspirations les plus profondes de l'existence. En effet, "la raison conduit à la nécessité et la foi à la liberté". Cathedrals do not really have a role in Britain's secular society, do they? Powerful socio­cultural pressures have forced religious suppliers, like Chester Cathedral, to innovate and to develop a tourism offering. Theoretically and empirically, cathedral studies have been dominated by tourism and quantitative strategies. Drawing on marketing scholarship, within the tight parameters of this interdisciplinary study, the thesis aims to challenge the status quo of cathedral studies. In particular, by using relevant branding principles as an illuminating framework, and theoretical tools from the sociology of religion and tourism, the aim is to qualitatively explore how end-users invest meaning in their 'visitor journeys/ Methodologically, the development of a spectrum/continuum of cathedral visitors, which was framed around the visitor journey, contributes to this area. Overall, the research is based upon interview and safari focus-group data. A broad range of actors were interviewed, including cathedral stakeholders, worshipping congregation, tourists and nonusers. The 'sacred brand', as it is developed in this thesis, highlights how visitors, irrespective of their background, creed and purpose of visit, are all able to connect with Chester Cathedral, in some form or another. Although the congregation, tourist and nonusers all use the Cathedral in different ways, the aesthetic value of the space often engendered a powerful emotional response, which was felt to be distinct from the mundane. This underlined the Cathedral's rich emotional economy, in which the sacred space seemed to resonate with visitors allowing them to invest their visitor journey with a deeper meaning than one would normally expect at a heritage site. The power of the sacred brand lies in these sources of significance, whether personal, religious, historical or socio-cultural. The thesis demonstrates the need to protect the Cathedral's legacy, and also, how secular societies still require 'sacred space'—although that space may not always be interested in 'religious' terms. Most broadly, there appears to be a need for an inclusive space that offers even the most ardent atheist sources of significance. As this thesis shows, cathedrals are well placed to provide this space and can be marketed accordingly, in order to meet both the commercial pressures and the need for a spiritual and a liturgical environment. To this end, the thesis also develops a particular method to be used in religious marketing and development of sacred brands.

Abstract:
This thesis addresses the live music scene in Chester in the mid-20th Century, and in particular jazz-based styles of dance music, played for the most part by local musicians. The basis of the study is a set of interviews with musicians, promoters and fans who were all active in the Chester area during the period between 1925 and 2008, in settings ranging from military bands and youth clubs to resident dance hall bands, touring concert parties, summer season shows and radio broadcasts. Thirty interviews were undertaken, and along with many hours of taped conversation, these yielded over 200 photographs and other pieces of evidence In this thesis I have synthesised existing theoretical approaches from a number of fields to account for the large number of part-time dance-band musicians who were active in the Chester area, especially during World War II and in the decade that followed. Ideas from popular music studies and jazz studies were part of this framework, but were not sufficient, as both fields have historically had a tendency to concentrate on musicians and places considered to be highly significant or exceptionally influential, rather than routine and local. I have therefore turned to other disciplines in search of appropriate analytical approaches, and used ideas from geography, economics and sociology as alternative lenses through which to view the problem. In the process, I have shown that this dance band scene grew from the people and entertainment infrastructure of the previous, inter-war, period. In turn, the musicians, promoters and venues of the dance band scene, combined with changes in technology and society which fundamentally changed the economics of live entertainment, formed essential parts of the environment in which much better-known rock and pop musicians of the 1960s and 70s emerged and developed.

Materials and Methods: Cross‑sectional data were collected from randomly selected patients in the German Sächsische Epidemiologische Studie in der Allgemeinmedizin 2 (SESAM 2) and analyzed from the Dutch Transition Project. Results: Overall, 270 patients from the SESAM 2 study consulted a general practitioner due to chest pain (3% of all consultations). Chest pain was more frequent in people aged over 45 years. The most common diagnostic interventions were physical examination, electrocardiogram at rest and analysis of blood parameters. For the majority of cases, the physicians arranged a follow‑up consultation or prescribed drugs. The transition project documented 8117 patients reporting chest pain with a frequency of 44.5/1000 patient years (1.7% of all consultations). Physical examination was also the most common diagnostic intervention, and physician’s advice the most relevant therapeutic one. Conclusion: The most common causes for chest pain were musculoskeletal problems followed by cardiovascular diseases. Ischemic heart disease, psychogenic problems, and respiratory diseases each account for about 10% of the cases. However, acutely dangerous causes are rare in general practice.

Abstract:
<p>Feedback control of Chest Acceleration or Chest Deflection is believed to be a good way of minimizing the risk of injury. In order to implement such a controller in a car, an observer estimating these responses is needed. The objective of the study was to develop a model of the dummy’s chest capable of estimating the Chest Acceleration and the Chest Deflection during frontal crashes in real time. The used sensor data come from car accelerometer and spindle rotation sensor of the belt, the data has been collected from dummies during crash tests. This study has accomplished the aims using a simple linear model of the chest using masses, springs and dampers. The parameters of the model have been estimated through system identification. Two types of black-box models have also been studied, one ARX model and one state-space model. The models have been tested and validated against data coming from different crash setups. The results show that all of the studied models can be used to estimate the dummy responses, the physical grey-box model and the black-box state-space model in particular.</p><br><p>Genom att använda återkoppling av storheterna bröstacceleration och bröstintryck antas man kunna minska risken för skador vid krockar i personbilar. För att kunna implementera detta behövs en observatör för dessa storheter. Målet med denna studie är att ta fram en modell för att kunna skatta accelerationen i bröstkorgen samt bröstintrycket i realtid i frontala krockar. Sensordata som använts kom från en accelerometer och en givare för att mäta rotationen i bältessnurran. Detta har gjorts genom att modellera bröstkorgen med linjära fjädrar och dämpare. Dess parametrar har skattats från data från krocktester från krockdockor. Två s.k. black-box-modeller har också tagits fram, en ARX-modell och en på tillståndsform. Modellerna har testats och validerats mha data från olika sorters krocktester. Resultaten visar att alla studerade modeller kan användas för att skatta de ovan nämnda storheterna, den fysikaliska modellen och black-box-modellen på tillståndsform fungerade bäst.</p>

Abstract:
The goal of this research was to improve the speed and accuracy of reporting by clinical radiologists. By applying a technique known as eigenimage processing to chest radiographs, abnormal findings were enhanced and a classification scheme developed. Results confirm that the method is feasible for clinical use. Eigenimage processing is a popular face recognition routine that has only recently been applied to medical images, but it has not previously been applied to full size radiographs. Chest radiographs were chosen for this research because they are clinically important and are challenging to process due to their large data content. It is hoped that the success with these images will enable future work on other medical images such as those from CT and MRI. Eigenimage processing is based on a multivariate statistical method which identifies patterns of variance within a training set of images. Specifically it involves the application of a statistical technique called principal components analysis to a training set. For this research, the training set was a collection of 77 normal radiographs. This processing produced a set of basis images, known as eigenimages, that best describe the variance within the training set of normal images. For chest radiographs the basis images may also be referred to as 'eigenchests'. Images to be tested were described in terms of eigenimages. This identified patterns of variance likely to be normal. A new image, referred to as the remainder image, was derived by removing patterns of normal variance, thus making abnormal patterns of variance more conspicuous. The remainder image could either be presented to clinicians or used as part of a computer aided diagnosis system. For the image sets used, the discriminatory power of a classification scheme approached 90%. While the processing of the training set required significant computation time, each test image to be classified or enhanced required only a few seconds to process. Thus the system could be integrated into a clinical radiology department.

In the Republic of South Africa, an autopsy is required in all cases of unnatural death, or in cases where the cause of death is unknown in terms of the Inquest Act of 1959. These are performed at the Salt River Medicolegal Laboratory by Forensic Pathologists and Registrars of the Department of Forensic Medicine and Toxicology of the University of Cape Town. The Salt River Medicolegal Laboratory serves the greater Cape Town area with a population of approximately 2,5 million, including the magisterial districts of Cape Town, Wynberg, Mitchell's Plain and Simonstown. Cape Town has one of the world's highest homicide rates and in 1986 the incidence was 56, 91 100000 population per annum. In contrast, Singapore has a homicide rate of only 2, 5/100 000, while the United States has an overall homicide rate of 7,7/ 100 000 population. In order to document the true impact of penetrating chest injuries, and to place mortality data in perspective, a retrospective descriptive study of all cases with fatal penetrating chest injuries admitted to the Salt River Medicolegal Laboratory in Cape Town during 1990 was undertaken. In 1990, a total of 5 758 cases was admitted to the Salt River Medicolegal Laboratory of which 1834 cases (39%) were the result of homicide. Of the homicide cases, 408 (22%) were the result of firearm injuries. A total of 2044 (35, 5%) cases admitted was deemed to have died of natural causes. This study identified a total of 841 cases of fatal penetrating injuries of the chest admitted during 1990, which constituted 22,6% of all non-natural cases admitted.

Previous research has suggested the existence of socio-economic and gender variation in uptake of hospital cardiology services but it is not known at which stage of the care pathway these variations arise. This thesis aims to address this gap in the literature by exploring the perceptions of an behaviour in response to chest pain of men and women living in two socio-economically contrasting areas of Glasgow. It has 3 objectives: to describe respondents' perceptions of and actions in response to chest pain; to ascertain whether there are differences in perceptions and behaviour associated with socio-economic status or gender; and, to relate any observed differences to the known variations in uptake of cardiology services. Respondents' interpretations of chest pain were related to their perceived vulnerability to heart disease and their previous illness experience. Their decisions of whether to present to a general practitioner were influenced by the quality of previous encounters with doctors and by their lay care and self-care. Women generally felt less vulnerable to heart disease than men, and often believed that their health problems were secondary to those of family members. These factors led women to report a reluctance to present with chest pain. Compared with more affluent people, those from the deprived area generally felt more vulnerable to heart disease but the perception that they were to blame for their ill-health often led to reluctance to present with chest pain. Differences in perceptions and behaviour might partly explain gender inequities in access to hospital cardiology services but socio-economic inequities in uptake of services have proved more difficult to explain.

Background: Both in primary care and emergency departments, chest pain has been a common presentation. However, it has frequently been identified having a non-cardiac pathology. This non-cardiac chest pain (NCCP) has been diagnosed for as much as 50 percent of referrals to cardiologists (Mayou, 1997). Nevertheless, the pain often persists after heart related diseases and other aetiologies have been ruled out. Psychological treatment studies have shown some positive results, but recruitment was low. Often, the sufferers who did not engage in the research believed that the NCCP had no psychological components. The aim was to gain a better understanding of the experience of NCCP from the sufferer's perspective. Method: Five NCCP sufferers were recruited from a rapid access chest pain clinic. They were interviewed by a psychologist for up to one hour. The semi-structured interviews were transcribed and the data analysed using Interpretative Phenomenological Analysis (Smith, 1996). Analysis: Three main themes emerged from the data. (1) Understanding: which included sub-themes cause, not knowing, comparison and context. (2) Impact: which included sub-themes daily activity, emotion, symptoms, control, image and chest pain results. (3) Support: which included sub-themes self-support, availability of support and support from others. Implications: The main clinical implication related to: communication with health care professionals interventions and information dissemination.

The research described in this thesis reports the results of a study into the adoption of e-learning strategies based on the use of the World Wide Web (WWW) and Internet. Through an extensive and critical literature review, it exemplifies how higher education uses intranets to deliver learning and support services to their student population. The overall aim of this research was to investigate how e-learning at the University of Chester might more effectively support students' learning needs, thereby improving their experience of e-learning. Students were given a mode of study, either face-to-face (64 subjects) or experimental using online intranet delivery (66 subjects). The course used for this study was a 13 week, Level Two undergraduate computer course taken by non-computing students. Quantitative and qualitative data were collected and analysed. The results reveal significant differences between the performance of the e-learning and face-to-face groups with e-learning students performing poorly when compared to their face-to-face peers. A lack of responsiveness in tutor support and student motivation were established as being major contributing factors as well as differences in the students' individual learning profiles. The research concludes that e-learning, although promoted as being anytime and anywhere is limited in its flexibility and responsiveness in the context in which it was assessed. Most e-learning activities at the University of Chester can be described as 'one size fits all'. They require students to read printed text, carry out further work, research or exercises, and post written comments to a discussion board. There is little evidence that individual student needs and preferences are being considered or supported. With the move towards blended learning in educational institutions, e-learning strategies are being used as a regular part of the curriculum to enhance the student experience. This research provides alternatives for the development and delivery of more individually tailored e-learning courses and provides strategies for supporting students in virtual environments more effectively. The thesis concludes by proposing a new model for e-learning based on these results coupled with a self-critical review and proposals for further research.

Alternative Food Networks (AFNs) are a system of food provision which is considered as the embodiment of the Sustainable Development (SD) agenda. They typically operate counteractively to conventional food networks (CFNs) seeking to reconnect all members in the supply chain through ethical and sustainable engagements. They are grounded by the theoretical underpinnings of quality conventions (Murdoch, 2000; Thévenot, 2002) and embeddedness notions such as alterity, valorisation, and appropriation (Dansero & Puttilli, 2014; Kirwan, 2004). Many scholars have focused on exploring AFNs in various contexts, initially focusing on binary notions of dichotomy between AFNs and CFNs, then developing discourse in terms of assessing hybridity (Holloway et al., 2006; Maye, 2013; Ponte, 2016; Renting, Marsden, & Banks, 2003; Tregear, 2011). Recent studies have indicated the potential for further research concerning social media based AFNs (Bos & Owen, 2016; Reed & Keech, 2017; Wills & Arundel, 2017). Therefore a contribution in terms of further understanding this issue arises from this thesis. The research was conducted in the midst of the referendum for the UK to withdraw from the European Union, the subsequent ‘leave’ vote resulting in a level of uncertainty in terms of policy implications. One policy implication may be that the UK will have to readdress the way it engages and supports its food and agriculture sector post-Common Agricultural Policy, therefore this research comes at a timely juncture. This research adopts an interpretivistic epistemological stance, with a constructivist ontological position. Social network analysis (SNA) of Twitter connections was conducted in order to assess connectivity and density of the AFN that was present in Chester and its region. Content analysis of this network was then conducted in order to understand SD related terms and shortlist pertinent actors for further analysis. Interviews were conducted with nine actors from this network in order to critically evaluate their perceptions of SD from an online and offline perspective. The results of the SNA suggest that the AFN of Chester and its region was not particularly well connected in terms of density. However, the SNA was a useful data collection tool, especially concerning the replicability and transferability of participant selection strategy. Further results suggested that there was a need for more organisational structures to support AFNs in becoming more mainstream and collaborative. It was also clear that there was still a degree of opposition between CFNs and AFNs, despite hybridity. A final finding of the research is the consideration of smart localism. The implications of this research are discussed, along with suggestions for future research including; the need to better understand leadership, relations between AFNs and CFNs, the role played by intermediates, and the expansion of social media based research.

This thesis analyses the Lord Edward’s lordship of the county of Chester between 1254 and 1272 and the impact it had on local inhabitants. The first chapter investigates the general administration of the county, including its financial structure, and the officials and men recruited by Edward to control his Cheshire estates. This chapter shows that Edward, in spite of his father’s influence, led an independent policy by relying primarily on the justiciar and on local tenants to govern the county. The second chapter tackles the organisation of justice and the law administered in Cheshire. It demonstrates that the judicial machinery in Cheshire was preserved during Edward’s lordship with its distinctive characteristics inherited from the earls of Chester, and that Edward was far more involved in the administration of justice in the county, and in the rest of his apanage, than previously envisaged. The third chapter considers the relationship Cheshire maintained with Wales in the thirteenth century, and more precisely, how Edward’s lordship of the county disrupted the stability that existed in the area. It shows that he was not directly responsible for the tensions that existed, but that his inability to address them eventually led to the outbreak of 1256. It also demonstrates that Edward failed to protect his estates despite multiple attempts, and how he progressively lost interest in his Welsh dominions. The fourth chapter discusses Edward’s relationship, and the difficulties he had, with his Cheshire tenants, especially during the baronial movement of reform and rebellion (1258-67). It analyses how problems in the county mirrored those elsewhere in the realm, because of the financial pressure and of the desire of local communities to have their laws and customs preserved, and how Edward’s considered approach and political discernment were eventually successful in restoring order in the county.

Abstract:
Factors affecting the risk of ribcage injury in adult subjects during CPR were investigated using the torso region of the THUMS model, a full human body FE-model, representing an average adult male. The thoracic dynamic response of the model was compared to experimental PMHS hub loading impact data and live-subject CPR data found in the literature. The model was then used to study the risk of obtaining injuries in various simulated CPR conditions, also varying the stiffness of the costal cartilage. Parameters that are known to predict induced injuries were extracted from the model simulations, i.e. chest deflections, and maximum 1st principal strain and von-Mises stress in the ribs and sternum, as well as the pressure in the heart muscle. These were compared with values that have been reported to have the potential to cause injury. The predictions were compared to experimental findings of the probability of CPR resulting in fractures of the ribs and sternum. The previously mentioned parameters did not reach high enough values to predict fracture occurrences, but interesting trends were highlighted with regards to the different loading conditions investigated. It was demonstrated that human body FE-model simulation studies can be useful for investigating the influence of different CPR related loading conditions on the risk of occurrences of rib and sternal fractures.

Objectives: This thesis aims to explore evidence for the effectiveness of cognitive behavioural therapy (CBT) for non-cardiac chest pain (NCCP). Design: The systematic review aimed to evaluate evidence for CBT as an effective intervention for anxiety in the NCCP population. Study one describes the chest pain characteristics, illness beliefs and prevalence of anxiety in a NCCP sample in a cross-sectional design. Study two explores the acceptability and clinical effectiveness of a CBT-based self-help intervention for NCCP patients, using a between subjects, repeated measures design. Methods: A systematic review was completed via a comprehensive literature search for comparative studies examining CBT-based interventions for NCCP including a measure of anxiety. In the empirical study, participants completed measures of anxiety, illness beliefs and indices of chest pain (self-reported frequency, severity and impact on activities) at baseline. Comparisons between illness beliefs and anxiety were undertaken using descriptive statistics and Pearson correlations. Participants were randomised to receive a CBT-based self-help intervention booklet or treatment as usual, with questionnaires re-administered at three-month follow-up. ANOVAs were used to evaluate whether the intervention led to improvements in anxiety levels, or increased belief in participants’ personal control of symptoms. Results: Ten studies met inclusion criteria for the systematic review, with four studies showing evidence regarding the effectiveness of CBT for anxiety. Approximately two thirds of the thesis research sample reported on-going pain following clinic attendance, for the majority this was ‘very mild’ or ‘mild’ pain. Almost half (47%) reported experiencing clinically significant anxiety. Stress was the most common causal attribution advocated by the sample to explain their chest pain. Anxiety scores were significantly associated with psychological attribution scores, but not with personal control or illness coherence beliefs. In study two, 87 participants completed the study and ITT analyses were completed on 119. There were no significant differences between the groups in terms of reduced anxiety or self-reported belief in personal control of symptoms. The intervention booklet was evaluated largely positively by those who reported reading it. Conclusions: CBT-based self-help appears an acceptable intervention for those diagnosed with NCCP. Further research is needed to identify those who are most likely to benefit from such self-help intervention.
