Abstract:
ABSTRACT Aim To assess the correlation of symptoms with findings on esophageal high-resolution manometry (HRM) in Indian patients. Materials and methods Prospective data collection of all patients undergoing esophageal manometry was done at two centers in India—Indore and Chennai—over a period of 18 months. Symptom profile of the study group was divided into four: Motor dysphagia, noncardiac chest pain (NCCP), gastroesophageal reflux (GER), and esophageal belchers. The symptoms were correlated with manometric findings. Results Of the study group (154), 35.71% patients had a normal study, while major and minor peristaltic disorders were noted in 31.16 and 33.76% respectively. In patients with symptoms of dysphagia, achalasia cardia was the commonest cause (45.1%), followed by ineffective esophageal motility (IEM) (22.53%) and normal study (19.71%). In patients with NCCP, normal peristalsis (50%) and ineffective motility (31.25%) formed the major diagnosis. Of the 56 patients with GER symptoms, 26 (46.4%) had normal manometry. An equal number had ineffective motility. Of the 11 esophageal belchers, 7 (63.6%) of these had a normal study and 3 had major motility disorder. Dysphagia was the only symptom to have a high likelihood ratio and positive predictive value to pick up major motility disorder. Conclusion Dysphagia correlates with high chance to pick up a major peristaltic abnormality in motor dysphagia. The role of manometry in other symptoms in Indian setting needs to be ascertained by larger studies. ABSTRACT BACKGROUND: The high-resolution manometry has been a significant advance in esophageal diagnostics. There are different types of catheter and systems devices to capture esophageal pressures that generate variable data related to Chicago Classification (CC) and consequently influence normal values results. There are not normative data for the 24-channel water-perfused high-resolution manometry system most used in Brazil with healthy volunteers in supine posture. OBJECTIVE: To determine manometric esophageal normative values for a 24-channel water-perfused high-resolution manometry catheter in supine posture using healthy volunteers according to CC 3.0 parameters. METHODS: A total of 92 volunteers with no gastrointestinal symptoms or medications affecting GI motility underwent esophageal high-resolution manometry by standard protocol. Age, gender and manometry parameters analyzed using Alacer software were collected. The median, range, and 5th and 95th percentiles (where applicable) were obtained for all high-resolution manometry metrics. Normal value percentiles were defined as 95th integrated relaxation pressure, 5th-100th distal contractile integral, and 5th distal latency. RESULTS: The mean age was 40.5±13.2 years. Our normative metrics were integrated relaxation pressure <16 mmHg and distal contractile integral (708-4111 mmHg.cm.s) distal latency was <6 s and peristaltic break size (>4 cm). For EGJ-CI the range 5th-95th was 21.7-86.9 mmHg.cm.s. CONCLUSION: This is the first report of normative data for the 24-channel water-perfused system in supine posture. It revealed higher integrated relaxation pressure and distal latency duration which suggest the need to change CC 3.0 cutoffs for this system. It is observed that there is a tendency that DCI >7000 mmHg.cm.s may represent the lower limit of hypercontractility, and when <700 mmHg.cm.s (<5% percentile) interpreted as ineffective esophageal motility or failcontraction. Also compared to Chicago 3.0, higher integrated relaxation pressure and duration of distal latency were found. We emphasize that these data must be confirmed by future studies.

Summary The wide-spread conception of Bloomfield as a cold, unfeeling person, devoted only to a naïve scientism, is discussed and refuted. The very intensity of his feelings led him to repress them and to give vent to them only indirectly, often which quiet but bitter wit and sarcasm. He consequently distanced himself from many of the normal concerns of every-day life and of university politics. This had unfortunate results in the failure of the University of Chicago’s administration to recognize his merits; his move to Yale in 1940; his wife’s resultant mental break-down on separation from her Chicago environment; and his ensuing stroke in 1946. His contribution to linguistics during the war-years (1941–45) was thus outweighed by the loss of further influence he might have had on the development of American “structuralism” after 1946.

Abstract:
Local news may be one vehicle through which television helps, inadvertently, both to preserve and to transform cultural values. Content analysis on the evening news on four Chicago television stations over a lengthy period suggests local television responds to viewing tastes of black audiences. However, data on these Chicago television news programs suggest racism still may be indirectly encouraged by normal crime and political coverage that depict blacks, in crime, as more physically threatening and, in politics, as more demanding than comparable white activists or leaders. Ironically, widespread employment of black television journalists suggests to viewers that racial discrimination is no longer a significant social problem. The mix of these two views of blacks encourages modern white racism—hostility, rejection and denial toward black aspirations—the study argues.

Abstract Factor IX Chicago-2 and prothrombin Madrid were purified from patients with hemophilia B and congenital dysprothrombinemia, respectively. Each protein displays defects in zymogen activation secondary to the failure to cleave one of the sessile bonds whose cleavage is necessary for full coagulant activity. These proteins were isolated by immunoaffinity chromatography using conformation-specific antibodies directed at either factor IX or prothrombin. Factor IX Chicago-2 is cleaved abnormally by factor XIa, yielding a pattern consistent with the failure to cleave the sessile bond between Arg 145 and Ala 146. Prothrombin Madrid is cleaved abnormally by factor Xa, yielding a pattern consistent with the failure to cleave the sessile bond between Arg 271 and Thr 272. Peptide mapping was performed on reduced and alkylated factor IX, factor IX Chicago-2, prothrombin, and prothrombin Madrid, and the hydrolysates were separated by high-performance liquid chromatography. The mutant peptide in factor IX Chicago-2 was identified by automated Edman degradation as residues 143 through 188 of factor IX, and had a histidine substituted for arginine at residue 145. The mutant peptide identified in prothrombin Madrid corresponds to residues 267 through 285 of prothrombin and has the substitution of cysteine for arginine at residue 271. These mutations, each occurring at arginines, are identical to those in factor IX Chapel Hill and prothrombin Barcelona. These results suggest that a limited repertoire of point mutations, many affecting arginine residues, may be responsible for hereditary defects of the vitamin K-dependent proteins in patients with normal antigen levels.

Abstract:
Las normas de bioseguridad tienden a reducir o eliminar accidentes. Los estudiantes de enfermería están propensos a riesgos ocupacionales; siendo indispensable el refuerzo constante en conocimiento. El objetivo de esta investigación fue determinar el nivel de conocimiento de las normas de bioseguridad en estudiantes del V al IX ciclo de enfermería de una universidad privada; la metodología del estudio fue tipo cuantitativa, descriptiva, corte transversal; tuvo como muestra censal 134 estudiantes de enfermería del V al IX ciclo, y un muestreo no probabilístico, se utilizó un cuestionario de preguntas cerradas. Los datos fueron procesados en Microsoft Excel (análisis estadístico descriptivo). Los resultados a nivel de conocimiento fueron: 37.31% bajo, 32.09% medio y 30.60% alto en normas de bioseguridad. En ciclos académicos: IX entre nivel bajo y alto (8.21%); V (11.94%) y VI (5.97%) nivel bajo; VIII (6.72%) nivel medio; y VII (5.97%) alto. Según dimensiones: Generalidades, medio (48.51%), alto (26.81%) y bajo (24.63%). Dimensión precauciones universales, medio (51.49%), alto (44.03%) y bajo (4.48%). Dimensión manejo y eliminación de material contaminado y desechos, alto (55.97%), medio (41.04%) y bajo (2.99%) y en dimensión exposición ocupacional, alto (57.46%), medio (40.30%) y bajo (2.24%). En conclusión: los estudiantes de enfermería tienen un nivel de conocimiento bajo de normas de bioseguridad; pese a que recibieron temas de normas de bioseguridad en asignaturas alternas, asimismo realizaron prácticas en laboratorio y hospitalarias durante la formación universitaria.La investigación cumplió con criterios de rigor científico y éticos.

Debido a la competencia y estándares de calidad en el sector, así como la necesidad de optimizar los procesos en el Molino Capricornio Import S.A.C. de la ciudad de Chiclayo, se ha determinado como objetivo de estudio: Elaborar la propuesta de implementación de un Sistema de Gestión de Calidad (SGC) basado en la norma ISO 9001:2015. Dada la complejidad del proceso y la necesidad de ir paso a paso, se adoptó el enfoque de auditoría interna para diagnosticar la situación actual de la empresa con respecto a las exigencias de la norma. El diseño de la investigación fue de tipo cualitativo – cuantitativo (mixto). La investigación ha sido de tipo descriptiva, no experimental y de corte transversal. No se ha considerado hipótesis debido a la naturaleza de la investigación y la aplicación de la Norma ISO 9001:2015, misma que ya se encuentra validada. La muestra considerada fueron los seis trabajadores fijos del Molino Capricornio Import S.A.C. (igual a la población). Los métodos y técnicas de recolección de datos fueron dados mediante observación, análisis documental y cuestionarios (encuestas y entrevistas), mismos que han sido plasmados en las listas de verificación que exige la norma. Encontrando que existe un nivel de cumplimiento del 48% en total en la empresa con respecto a la norma y el mayor porcentaje de cumplimiento es en los apartados de competencia y liberación de productos y servicios, ambos con 75%.

La Municipalidad de José Leonardo Ortiz, es una empresa autónoma de derecho público con personas jurídicas y patrimonio propio, que se encarga de administrar a un grupo de ciudadanos llamados comunas, que tiene como objetivo satisfacer sus necesidades primordiales. Para ello se necesita desarrollar un sistema web que ayude a mejorar los procesos del trámite documentario en la Municipalidad. La organización del estado presenta muchos problemas en los procesos del trámite como es: el tiempo que demanda en registrar, enviar y dar respuesta al trámite, además la Municipalidad no cuenta con formatos establecidos, para obtener la información del cliente quien realiza el trámite. Durante el estudio de los procesos documentarios se pudo observar que la municipalidad no contaba con un seguimiento de trámites y además cuando un trámite era finalizado no existía mensajes de aviso al usuario. Para eso planteamos como objetivo ayuda a mejorar el proceso documentario en la municipalidad de José Leonado Ortiz a través de la implementación de un sistema web utilizando una norma ISO 15489.Esto permitirá comprobar nuestra hipótesis haciendo una comparación de resultados con la prueba de pres test y post test. De los resultados obtenidos de las pruebas, se evaluó que el grado de satisfacción del cliente con el sistema web disminuyo en un 60 % esto se debe porque el sistema se implementó un módulo de seguimiento para clientes y mensajes aviso cuando un trámite esta culminado.

En el Perú, según el MINSA, cada una hora cuatro ciudadanos peruanos son diagnosticados de TB, por ende, la exposición del personal de salud a pacientes infectados por el bacilo de Koch es un riesgo que ha incrementado asociado también a las malas condiciones de bioseguridad y limitadas medidas de prevención y control en todos los niveles de los servicios de atención. El objetivo fue describir el conocimiento y prácticas del personal médico e internos de medicina sobre normas de bioseguridad para prevención de tuberculosis en un hospital del MINSA durante el 2018. Fue un estudio descriptivo transversal con muestreo no probabilístico censal. De la población de 196 participantes se incluyó al personal médico e internos de medicina que desearon participar y estuvieron presentes durante la captación en el Hospital Docente Las Mercedes durante el año 2018, obteniéndose una muestra de 99 participantes. Se consideraron las variables “conocimientos” y “prácticas” en base a 5 dimensiones: conocimiento de la enfermedad, uso de mascarilla, lavado de manos, uso de guantes y uso de mandilón, las cuales fueron evaluadas por un cuestionario y una guía de observación previamente sometidos a juicio de expertos y una prueba piloto. En la muestra estudiada los médicos asistenciales demostraron un mayor conocimiento sobre bioseguridad para prevención de TB, sin embargo, fueron los médicos residentes los que en mayor proporción cumplieron con la práctica siendo el ítem más cumplido el uso correcto de guantes en un 66,7%. Se concluyó que los internos de medicina y los médicos asistenciales y residentes demostraron un adecuado conocimiento sobre normas de bioseguridad para la prevención de tuberculosis, sin embargo, sólo una minoría las cumplía durante la práctica médica.

La presente tesis tiene como propósito mejorar la manera en que se realiza una investigación científica del tipo experimental, evaluando dos aspectos que abarcan tanto sistemas de información como también ingeniería de software. Entre ellos tenemos la gobernabilidad de TI y la calidad de software. Ambos, temas que competen a la época en que vivimos, dado que, con el gran crecimiento de la población, y el avance de la tecnología, las empresas (tanto privadas como públicas) se ven en la necesidad de renovar sus procesos, con el fin de lograr rapidez en ellos y un mejor y mayor control. Entrando un poco más a lo que respecta esta investigación, sabemos que una de las empresas que brindan la mayor cantidad de servicios a la población es el mismo estado como entidad, por ende se piensa que con la mayor disponibilidad de dinero, la estricta selección por la que pasan sus trabajadores (en teoría) y la posición que deben mantener frente a los demás países, sean estos los primeros en tener la mejor tecnología, brindar mejores servicios y obtener así una mejor comunicación con el ciudadano. Sin embargo, vemos muchos casos en los que no se da de esa forma (tanto internacional como de manera local). Entre los obstáculos para lograr una buena gestión de Gobierno Electrónico, tenemos la parte de control, que sería la más influyente de todas, y sin embargo la menos desarrollada y agilizada de todo el proceso.

Abstract:
La Empresa Cerámicos Lambayeque busca mejorar su productividad con la gestión de sus operaciones considerando la implementación de mejoras como son la compra de maquinaria, adecuado personal, reducción de tiempos por operaciones totales, entre otros. Para ello, se planteó como hipótesis que la filosofía six sigma a un nivel de 2,13 mejora la productividad de ladrillo de techo N° 15 en la Empresa Cerámicos Lambayeque. Durante el diagnóstico de la línea de producción de este tipo de ladrillo, se determinó que el número de unidades procesadas, con los recursos que contaban hasta la fecha 2014, en total fueron de 1 783 500 unidades dentro de los cuales se determinó que el número de defectos detectados llegan a un total de 1 029 000 unidades. Determinándose que la productividad hasta el análisis asciende a 57,7% con un nivel sigma de 1,69. Las mejoras correspondientes implican la reorganización de las operaciones que se siguen para obtener el producto final, la compra de maquinaria adecuada para el proceso de quemado, la gestión necesaria del personal, entre otras mejoras relacionadas. Las nuevas unidades procesadas estimadas para el año siguiente ascienden a un total de 2 743 000 unidades, lo que determina que el número de unidades defectuosas ascenderá a 729 000 unidades, recordando que la filosofía Six Sigma busca reducir las cantidades de defectos. Esto demuestra el aumento en la productividad del 57,7% al 73,4% bajo un nivel sigma de 2,13.

Abstract:
Para la obtención de dicha información y recolección de datos se consideró conveniente el uso de las técnicas de recolección de datos tales como encuestas, entrevistas, así como fichas de observación, como medio para poder extraer la información y su posterior interpretación; y de esta manera medir la realidad problemática apoyado en el uso de la Norma ISO/IEC 27001, lográndose determinar las deficiencias para mejorar los niveles de seguridad y confiabilidad en los sistemas de información de dicha institución. Los resultados obtenidos permitieron determinar de forma real que, al incorporar la norma ISO/IEC 27001 basada en una Guía de Implementación. Se logró incrementar los procedimientos utilizados en favor de la Institución permitiéndole la detección de anomalías en la seguridad de la información, reflejado en distintos mecanismos de seguridad para salvaguardarla. Con el Plan de tratamiento de Riesgos, se permitió la disminución de los niveles de riesgos con respecto a los activos de información, considerados amenazas y vulnerabilidades en la institución, esto manifestado en un plan adecuado para abordarlos y tomar las precauciones necesarias que minimicen sus impactos. Finalmente con el Plan de Capacitación y Concienciación puesto en marcha en la Institución, se pudo incrementar el porcentaje de conocimiento por parte del personal en temáticas orientadas a políticas, estrategias de seguridad que beneficien a la institución, teniendo como resultado personal comprometido con la seguridad en favor de la institución. Una correcta implementación de la Guía desarrollada en el presente trabajo de investigación permite incrementar el nivel de seguridad en la misma.

Los resultados obtenidos permitieron determinar de forma real que, al incorporar la norma ISO/IEC 27001 basada en una Guía de Implementación. Se logró incrementar los procedimientos utilizados en favor de la Institución permitiéndole la detección de anomalías en la seguridad de la información, reflejado en distintos mecanismos de seguridad para salvaguardarla. Con el Plan de tratamiento de Riesgos, se permitió la disminución de los niveles de riesgos con respecto a los activos de información, considerados amenazas y vulnerabilidades en la institución, esto manifestado en un plan adecuado para abordarlos y tomar las precauciones necesarias que minimicen sus impactos. Finalmente con el Plan de Capacitación y Concienciación puesto en marcha en la Institución, se pudo incrementar el porcentaje de conocimiento por parte del personal en temáticas orientadas a políticas, estrategias de seguridad que beneficien a la institución, teniendo como resultado personal comprometido con la seguridad en favor de la institución. Una correcta implementación de la Guía desarrollada en el presente trabajo de investigación permite incrementar el nivel de seguridad en la misma.

This chapter details Regina's years in Normal, Illinois; and then shifts to her return to Chicago and her college experiences at Wilberforce University. It was in Normal that she attended school with the future Illinois governor and presidential candidate Adlai Stevenson II. In terms of her experiences in Normal, Regina later credited an understanding librarian as a guiding influence in her early life and training which has brought success in her chosen field. Meanwhile, Regina's experiences at the Chicago Public Library were mostly negative. However, she later said she was influenced by Vivian G. Harsh—Chicago Public Library's head librarian. The current Vivian G. Harsh Collection of Afro-American History and Literature at the Carter G. Woodson Regional Library is named after her.

This chapter focuses on the advent of gospel quartet singing in Chicago around the late 1920s, when Norman McQueen, Charles Bridges, and other quartet trainers migrated to Chicago from the South. McQueen, Bridges, and others introduced to Chicago a style of part singing that was earthier and more vocally percussive than what jubilee quartets were used to singing. Thanks to the quartet sound, gospel music became extremely popular nationally. This chapter documents the history of jubilee quartets in Chicago, beginning with the Standard Quartette followed by the Sunset Four, and proceeds with a discussion of the contributions of McQueen, Bridges, the Soul Stirrers, the Famous Blue Jays of Birmingham, and other gospel quartets to the growth of the Chicago quartet movement. Finally, it looks at some Chicago-based female quartets such as the Four Harmony Queens, the Crooning Sisters, the GoldenTone Female Quartet, the Four Loving Sisters, and the Jubilee Four Female Quartet.

Abstract:
The fifth chapter explores how theological education was opened to women, African Americans, and working class whites. Congregationalist Mary Lyon founded Mt. Holyoke Female Seminary (1837) to provide a rigorous education built on the liberal arts, theology, personal discipline, and domestic work—all designed to produce independent women for missions. Other women, like Methodist Lucy Rider, founded religious training schools for women in their denominations. For African Americans, pioneers like AME Bishop Daniel Payne, who revived Wilberforce University (1856), developed a blend of liberal arts and theological education. W. E. B. Dubois fought for this model as the way to educate “the talented tenth” needed for racial uplift. The other model, pioneered by Samuel Armstrong at the Hampton Institute (VA) and Booker T. Washington at Tuskegee (Alabama), combined a religious training school with industrial work so that black pastors and teachers could be self-supporting. Finally, Bible colleges, like that of Dwight Moody, opened theological studies to working people with only a basic education. Emma Dryer brought practical, normal school approaches to the beginnings of the Moody Bible Institute (MBI) in Chicago. Under Dr. R. A. Torrey, MBI combined a literal reading of Scripture with experiential holiness, spiritual healing, end-times prophecy, and practical business methods—all of which marked the future fundamentalist movement.

Big Bill left the Deep South for Chicago in the early 1920s for Chicago, a rapidly industrializing city with an expanding black population in the city’s south side. There, he discovered Bronzeville, the African American section of Chicago. At the time, Bronzeville was fully engaged in the New Negro Renaissance of the age where black southern migrants like Big Bill challenged established class, political, cultural, and social norms in a contest centered on remaking American blackness over as modern. Black Chicago and its expanding consumer marketplace became one of the nation’s centers for transforming African American identity and culture of the age.

This chapter compares liberal and traditionalist understandings of empire, race, and gender to explain why liberals often declined U.S. citizenship and why traditionalists became more amenable to U.S. naturalization. Mexican liberals defined themselves as anti-imperialist mestizos and, in Chicago, they joined with Puerto Ricans and Nicaraguans to protest U.S. imperialism. Most liberals rebuffed U.S. naturalization, in part, because they were put off by U.S. imperialism, racism, and gender norms. By contrast, traditionalists celebrated the United States for granting them religious freedom, and challenged the liberals by exalting all that was Catholic and thus Spanish and white in Mexican culture. In so doing, traditionalists became more open to a U.S. understanding of whiteness and citizenship, and many traditionalists decided they could create new lives for themselves in the United States.

By recognizing and not underestimating the significance of everyday forms of resistance and the politics of culture, as well as institutions and organizations not normally seen as vehicles for everyday and working class change, we can delve into the strategies that helped Mexicans in interwar South Chicago cope with the oppressive environment that surrounded them. Individual Mexican immigrants and Mexican Americans in interwar South Chicago, including steel workers, shop owners, union organizers, and social workers, formed a community that was able to change its physical and cultural environment to help its members and create a degree of resistance that helped Mexicans persevere against intimidation and prejudice. These individual and community histories—the stories of people, organizations, and their physical surroundings—shed light on Mexicano life in a place far from the border and at the industrial heart of the United States.

The book closes with an experimental conclusion that retells three stories about Chicana/o art in Los Angeles and the ways that museum visitors and curators inhabit and reimagine mainstream institutions. Therefore, the last chapter embodies the methodology of the remix by favoring exploration and reconstruction, and thus refusing closure. More importantly, remixing is not simply a reordering of Chicana/o art but a rethinking of the conventions and norms that create its invisibility within American and Latin American art history. The three stories each convey how Chicana/o museum visitors and curators bring their transnational and transmodern interpretations of art, culture, and identity into sites of power. Their interpretations suggest that the future of art history depends upon an inclusive, expansive, and critical methodology in the study of art.

In this chapter, some of the types of methodologies currently in use for isolation and analysis of cellular phospholipids will be outlined. Such techniques can be readily applied to experiments designed to explore the involvement of phospholipids in cellular events, such as stimulus-induced activation. Primary attention will be paid to the human platelet. If you need to justify the choice of human platelets as the cell of choice, a number of highly creditable reasons can be cited. Only three need to be considered at this point. First, the circulating platelet is of paramount importance in hemostasis, and there is convincing evidence that its membrane phospholipids are intimately involved in this process. Second, these cells can serve as excellent targets or model systems for stimulus-induced activation in which the membrane phospholipids play an important role. Third, human platelets can be isolated from whole blood by a simple, convenient centrifugal approach. Human donors are available at a very reasonable cost, and the platelets obtained from a varied spectrum of donors show remarkable consistency. Thus, one can undertake their isolation using the following method and have them available for immediate experimentation. Nonfasting venous blood is drawn (with informed consent) from male or female subjects between the ages of 20 and 40 years, who are considered to be normal and healthy and had not ingested platelet-active medication for at least 10 days prior. Blood is obtained by insertion of a butterfly infusion set (12-in. tubing from Abbott Hospitals, Inc., North Chicago, IL) with a 1-in. x 19-gauge needle into the antecubital vein. A few milliliters of blood is allowed to flow before a 60-ml plastic syringe containing 7.5 ml of an ACD [10.8% citric acid, 2.2% trisodium citrate, and 2% dextrose (w/v)] solution was attached. Four syringes were filled to the 50-ml mark and inverted gently, and the contents were transferred carefully into 50-ml plastic tubes that are then capped. The blood-to-ACD volume ratio is 6:1 (v/v). The tubes are centrifuged at 2000 rpm (830g) in a Sorvall RT 6000 centrifuge at 24°C for 15 min.
