Full textThe majority of patients undergoing surgery for a solid tumour are over the age of 701. About 30% of these patients can be considered frail. They have comorbidities and are at a higher risk of developing postoperative complications and experience functional loss. So far there is no simple screening tool that allows the treating physician to identify those patients most at risk of developing postoperative problems. Identifying those at risk is a first step in the process towards preventing unnecessary postoperative complications such as delirium, cardiovascular complications and overall functional loss leading to loss of independency. This study will be performed to test the predictive value of the GFI, the VES-13 and the timed “up and go” test compared to components of PACE in elderly patients of the age of 70 years and above undergoing surgery for a solid tumour.

Full textIn the present research project, from november 2011 to november 2013, patients with primary bone tumors of the extremities, pelvis and sacrum have been treated using computer assisted surgery evaluating the histological examination results of tumor resection margins and the clinical and radiological outcome. Materials and methods: We analized 16 patients 9 males and 7 females with a mean age of 31 years (range 12-55) and the primary bone tumors had different location, histology and grade. Of all the patients evaluated 8 had a localization to the lower limbs, 4 to the pelvis and 4 to the sacrum. Only those with parostale Osteosarcoma, Chordoma and Condrosarcoma have not been undergone to antiblastic treatment. Only one patients was subjected to postoperative radiotherapy for a local recurrence. All other patients were not treated with radiotherapy because of the adequacy of resection. There were no intraoperative complications. In the early postoperative period occured one neurological bladder, one sciatic paresis, two cases of infection one of which superficial and one deep; all the four patients with a sacral localizzation developed delayed wound healing and of these three developed sphincter incontinence. In all cases we obtained an excellent clinical and radiographic result, with patient satisfaction, proper contact between the osteotomy and the implant with a good stability to the first outpatient controls (FU 19 months). Results: Computer-assisted tumor surgery, with CT and MRI images fusion, has helped to improve the quality of the bone resections planned by the navigator and this may help reduce the risks of local recurrences. This technology is effective and useful for the treatment of musculoskeletal tumors, especially in complex anatomical sites like pelvis, sacrum and in intercalary resections and in all that cases where tumor resection could be difficult due to distorted surgical anatomy.

Full textThe ethical questions about the face transplant seem unavoidable. What risks can be tolerated, not irresponsibly, but in conscience, in the interest of the patient ? To what extent the ethical issues have been addressed by surgical teams prior to the completion of transplants ? Can ethics justify this intervention? A study of the scientific literature and general press of every team having performed the face transplant was conducted to determine the ethical issues raised by this transplantation. They affect both the donor and his family and the recipient. From these items, a questionnaire survey was conducted with the same surgical team to assess the management of these ethical issues. To open the debate, involving the community for educational and trace the way of the new acceptances, 1000 French surgeons were also interviewed by a questionnaire. The results were used to explore the issue of consent of the donor's family, the place of family in respect of memory and the will of the deceased. Explorer also respect the body, medical confidentiality, respect for donor anonymity, the role of media in the first major surgery, and finally the concept of consent of a vulnerable patient and that of the risk / benefit ratio for treatment whose benefits are still uncertain. As part of the face such questions are essential human dimension ... To testify early work focused on the symbolism of the face, disfigurement, historical, legal, institutional, and religious challenges

Full textRobot assisted surgery is slowly invading our world, taking side with regards to medical ethics. In a not too far future, one might watch this type of surgery operated between surgeons and patients thousands of miles apart. Can we then accept the image reflected on the screen as a valid substitute to the hand and eyes of the technician? Are we then accepting the surgeon as a technician whose tools would be limited to sticks and a screen? In fact, recent evolution in the field of medical ethics show that patients expect much more than the mere physical repair and consider that human direct contact with the surgeon is essential. This attitude appears as an invitation to avoid placing excessive emphasis on the technical aspect of surgery to the detriment of clinical aspect and human relationship

Full textThe skin of the child has histological, immunological and physiological characteristics responsible for different dermatological manifestations in children than adults, so the pediatric dermatology is gaining increasing importance as a specific branch in the general dermatology and pediatrician. An important problem which is encountered in the management of pediatric dermatosis is related to diagnostic difficulties, which often involve the need to perform a skin biopsy. As epidemiological studies relating to the frequency of pediatric dermatological diseases are widely reported in the literature, the data and revisions to the pediatric dermatological surgery are reduced. During the three years of research, my intent was aimed at assessing the possibility of organizing an outpatient service for biopsies in children, with only the aid of topical and local anesthetics. In particular 296 samples were performed. The biopsies were divided into 3 groups: diagnostic biopsies of skin diseases (108 pcs, 36%), biopsies of skin tumors (174 pcs, 59%) and biopsies of follicular lesions (14 pcs, 5%). Of each group were evaluated pathologies, age, sex, the use of topical anesthetic associated with local anesthetic. In 180 (61%) patients were applied stitches. We have evaluated the advantages and disadvantages of ambulatory activity compared to samples taken using deep sedation / general anesthesia.

Full textSurgeon is, between all doctors, the one who gets the highest rate of responsibility. His action is so typical, especially because of the need of specialisation and the need of team works, that judges are keen on increasing his responsibility. However, surgeon's responsibility is based on common rules of responsibility and that means that victims often fail in their quest of indemnity. Then, the system should be changed into one which would separate responsibility and indemnification. First of all, victims should be indemnified. An automatic system of indemnification based on a double private insurance should be set. It would work just by noticing victims' damage and implication of the surgeon's action in this injury. Then after, surgeon's responsibility should be considered. This responsibility should have a repressive nature and be restricted to the most important faults by increasing the weight of disciplinary responsibility, which would be demonstrated before civil or penal responsibility

Full textHepatocellular carcinoma (HCC) is the fifth most common cancer worldwide, and the third most common cause of cancer-related death. It is a major health problem worldwide, which represents the most prevalent primary liver cancer and constitutes the third most frequent cause of cancer-related deaths. The major risk factor for HCC is cirrhosis. All types of cirrhosis predispose to HCC, but the incidence is particularly high in persistent infection with hepatitis B (HBV) and hepatitis C (HCV) and in alcoholic liver disease. The clinical presentation and management of HCC depends on whether the liver is cirrhotic and whether there is underlying viral hepatitis. Therapeutic options fall into four main categories (1) surgical interventions, including tumor resection and liver transplantation, (2) percutaneous interventions, including ethanol injection and radiofrequency thermal ablation, (3) transarterial interventions, including embolisation and chemoembolisation and (4) drugs as well as gene and immune therapies. Potentially curative therapies are tumor resection, liver transplantation, and percutaneous interventions that can result in complete responses and improved survival in a high proportion of patients. Liver resection offers the greatest impact on survival when patients do not meet transplantation criteria and this is considered as the optimal treatment for HCC. The objective of this thesis, based on a retrospective survey, was to give an overview on conditions of realization and the results of the resection of HCC in France, in the period from 1990-2005. All the French centers of excellence in the hepatobiliary surgery were contacted and most of them accepted to participate. All the contacted units were essentially localized in University Clinical Centers, all of them experts in hepatic surgery and most of them were centers for liver transplantation. More than 2590 cases with hepatic resection were collected in this study. The file consisted of 102 questions and contained following headings: demographic data, underlined liver pathology, circumstances of diagnosis, imaging, evaluation of underlined liver pathology: biological, morphological, histological, preparation for resection: neoadjuvant treatment of the tumor, portal embolization, surgical intervention: approach, clamping, vascular control, nature and the extent of the exeresis, anatomic or non-anatomic features, histopathological analysis of the removed tissues, results: mortality, morbidity, recurrence, survival, lost from analysis. In total, this survey enabled us to collect a considerable sum of data in order to give a more precise overview on predictive criteria of per operative mortality and survival, as well as recurrence rates, after the resection of normal and pathological livers. It confirmed the development and the quality of the hepatic surgery in France
