Abstract:
The gall bladder is a sac which lies underneath the liver and stores and concentrates bile produced by the liver. As food enters the duodenum, it stimulates the release of cholecystokinin, which in turn stimulates the release of bile, which passes via the cystic duct to the common bile duct, which connects to the duodenum at the sphincter of Oddi. Bile is required in digestion, especially for the emulsification and absorption of fat. Biliary disease can take several forms. Cholelithiasis refers to the presence of gallstones in the gall bladder, whereas choledocholithiasis refers to gallstones in the biliary tree. Cholecystitis is inflammation and infection of the gall bladder. Cholangitis is inflammation and infection of the biliary tree. Sphincter of Oddi dysfunction (SOD) is characterized by symptoms of biliary obstruction, with no structural cause. Other forms of biliary disease are gall bladder polyps, primary biliary cholangitis, and primary sclerosing cholangitis.

Abstract:
Biliary tract cancers encompass tumors of the gallbladder, extrahepatic bile ducts, and ampulla of Vater. In the United States, biliary tract cancer is the fifth most common malignant neoplasm of the digestive tract, accounting for about 3,700 deaths per year. The gallbladder is the primary subsite for 40% of biliary tract cancers, followed by the extrahepatic bile ducts (33%), ampulla of Vater (20%), and unspecified subsite (8%). Gallbladder cancer occurs twice as often in women than men, while other biliary tumors are more common in men. Risk of gallbladder cancer is elevated in Amerindians, including the Pima Indians in the United States and the Mapuches in Chile, and in certain Hispanic populations. While a significant fraction of these tumors are related to underlying gallstones (cholelithiasis), information on other risk factors is limited, due to the rarity of the tumors, the often rapidly fatal course, and small number of epidemiologic studies.

Abstract:
Obesity is a major public health problem associated with significant morbidity and mortality. Obesity is also a well-known risk factor for gallstone formation. Bariatric surgery is the most effective therapy for morbid obesity; however, the metabolic alterations it imparts can cause specific mid- or long-term complications, such as biliary lithiasis. The rate of development of cholelithiasis after bariatric surgery has been reported to be around 32% to 42% with studies ranging from 2–52% depending on use of pharmacologic prophylaxis. Therefore, prophylactic cholecystectomy during gastric bypass was historically recommended in the era of open surgery. The current management of gallbladder pathology remains a point of contention, ranging from prophylactic cholecystectomy, to cholecystectomy in the setting of biliary symptoms or presence of stones on preoperative ultrasound, to expectant management. This chapter discusses the current evidence regarding biliary complications and management in bariatric surgery.
