Abstract:
In Malaysia, the shear strength and corresponding deformation characteristics of residual soils are normally investigated by isotropically consolidated undrained (CIU) triaxial test. In the present study, anisotropically consolidated undrained compression (CAU) tests were carried out and explored in an attempt to facilitate understanding on the fundamental behaviours of a selected residual soil. A triaxial apparatus setup which was capable of performing the CAU test was first developed. Through a monotonic strain-controlled triaxial test apparatus, soil properties and stress paths could be obtained experimentally. It was confirmed that the newly fabricated triaxial apparatus could provide reasonable experimental results. As distinguished from the isotropically consolidated undrained compression (CIU) test, anisotropic stresses were induced in the stress path test. It follows that the soil responses were different between the CIU and CAU tests. The coefficient of earth pressure at rest (Ko) values as obtained in the present experiment showed a good agreement with the values estimated using equation reported by Jaky (1948).

Abstract:
This paper discusses results of laboratory tests carried out with a residual soil originated from the weathering of eolian sandstone from southern Brazil. Parent rock features, like microfabric and particle bonding, are remarkably well preserved within this residual soil. Stiffness and shear strength properties were evaluated with consolidated drained (CID) and consolidated undrained (CIU) triaxial compression tests. Undisturbed specimens were tested with two different orientations between the specimen axis and bedding surfaces (i.e., parallel (δ = 0°) or perpendicular (δ = 90°)) to investigate the effect of anisotropy. When CID triaxial tests were performed with δ = 0°, the yield surface associated with the structure was much larger than when tests were performed with δ = 90°. Coincidently, CIU tests with δ = 0° showed peak shear strengths much greater than for δ = 90° at comparable test conditions. Once the peak shear strength was surpassed, CIU tests followed collapse-type effective stress paths not shown by corresponding tests with remolded specimens. A near coincidence was observed between the yield surface determined with CID tests and the envelope of collapse-type effective stress paths for δ = 0° and δ = 90°.

Abstract:
The Correct Information Unit (CIU) analysis for measuring the communicative informativeness and efficiency of connected speech (Nicholas & Brookshire, 1993) was applied to the naturally occurring conversation of a person with moderate aphasia. Results indicated that, in this instance, reliable CIU measures could not be obtained. Intrarater reliability for CIU and %CIU was low, reaching only 72%, and interrater reliability was never greater than 63%. However, reliability of word counts was good. Post hoc analysis of rater disagreements in application of the CIU analysis revealed that the majority (72%) resulted from insufficiencies in the scoring rules that were originally designed to measure single speaker connected discourse. Two descriptive categories of disagreements were identified: interpretations of informativeness and absence of rules. The remaining 28% of disagreements were attributable to human error in the application of scoring rules. Comparison of findings with previous research and implications for future research are discussed.

Background. Estimation of efficacy of antihistamines in patients with various severity levels and various clinical and laboratory variants of chronic idiopathic urticaria (CIU) . Methods. The retrospective study of the anamnestic clinical laboratory findings in 196 patients with CIU was performed. Results. The frequency of patients with full effect of antihistamines was 69%, partial efficacy — 11%, lack of efficacy — 20%. Lack of efficacy was dominated in patients with severe course of the disease and the presence of clinical manifestations of autoimmune disorders, non-specific inflammation and / or signs of cutaneous vasculitis. The full effect of antihistamines was dominated in patients with mild to medium CIU, with a normal clinical blood count and with a negative skin test with autologous serum. Conclusion. Lack of efficacy of of antihistamines in patients with severe course of disease, signs of autoimmune disorders specify on others nonhistamine mediators in CIU pathogenesis and application of the second and third line therapy of CIU.

Abstract:
The purpose of this study was to show the association between urban upbringing and compulsive internet use (CIU). The interview data of the sample (N = 2431) was obtained from the World Mental Health Japan Second Survey and a multilevel model was used to investigate the association. Multiple imputation was also conducted in this study. Growing up in a large city was significantly associated with higher Compulsive Internet Use Scale (CIUS) scores (γ = 1.65, Standard Error (SE) = 0.45) and Mild CIU + Severe CIU (Exp(γ) = 1.44; 95% Confidence Interval (CI) (1.04–2.00)) compared to growing up in a small municipality after adjusting for both sociodemographic characteristics and psychopathology. This study showed a possible association between urban upbringing and CIU. Future studies with longitudinal design are needed to better understand this association.

IntroductionIn a recent study developed with a community sample, we have found that although PNT mediated the relationship between perfectionist cognitions and bulimic behavior, the effect of perfectionism on other disordered eating dimensions, such as diet, was independent of the PNT levels (Monteiro et al., 2015).ObjectiveTo investigate if PNT mediates the relationship between perfectionism and disordered eating behaviors in a clinical sample.MethodsFifty-two patients with eating disorders/ED (mean age = 22.54 ± 7.637; mean BMI = 20.07 ± 4.192; 14.5% Anorexia Nervosa cases; 7.8% Bulimia Nervosa and 9.0% EDNOS) were assessed with the ED section of the Diagnostic Interview for Genetic Studies and fill in the Portuguese validated versions of Eating Attitudes Test/EAT-25 (to evaluate Bulimic behaviors/BB, Diet and Social pressure to eat), Multidimensional Perfectionism Scales (to evaluate perfectionism composite dimensions Evaluative Concerns/EC and Perfectionistic Strivings/PS) and Perseverative Thinking Questionnaire/PTQ-15 (to evaluate Repetitive Thought/RT, Cognitive interference and unproductiveness/CIU). Only variables significantly correlated with the outcomes (EAT-25_Total and its dimensions) were entered in the regression models. Mediation analyses using Preacher and Hayes bootstrapping methodology were performed.ResultsEC, PS, CIU and RT were significant predictors of EAT_Total. PA, CIU and PR were significant predictors of BB. EC and PS were significant predictors of Diet. CIU partially mediated the relationship between EC and EAT_Total (95% CI = 0.0025–0.3296) and between EC and BB (95% CI = 0.0037–0.1877).ConclusionAlso in a clinical sample, CII, the most pernicious dimension of PNT, mediates the relationship between perfectionism and disordered eating behavior, particularly bulimic behavior; diet is predicted by perfectionism independently of PNT.Disclosure of interestThe authors have not supplied their declaration of competing interest.

Abstract:
CONTEXT AND OBJECTIVE: An association between chronic idiopathic urticaria (CIU) and autoimmune thyroid disease (ATD) has been reported. However, there have not been any reports on whether ATD raises the risk of angioedema, which is a more severe clinical presentation of CIU. Thus, the aim of the present study was to evaluate whether the risk of angioedema is increased in patients with CIU and ATD. DESIGN AND SETTING: Case-control study including 115 patients with CIU at a tertiary public institution. METHODS: The patients were evaluated with regard to occurrence of angioedema and presence of ATD, hypothyroidism or hyperthyroidism. RESULTS: Angioedema was detected in 70 patients (60.9%). There were 22 cases (19.1%) of ATD, 19 (16.5%) of hypothyroidism and nine (7.8%) of hyperthyroidism. The risk among patients with ATD was 16.2 times greater than among those without this thyroid abnormality (confidence interval, CI = 2.07-126.86). The odds ratio for hypothyroidism was 4.6 (CI = 1.00-21.54) and, for hyperthyroidism, 3.3 (CI = 0.38-28.36). CONCLUSIONS: Patients with CIU and ATD presented greater risk of angioedema, which reinforces the idea that a relationship exists between this allergic condition and thyroid autoimmunity. This finding could imply that such patients require specifically directed therapy.

Background The pathophysiology of chronic idiopathic (spontaneous) urticaria (CIU) is poorly understood. Objective We hypothesized that a study of gene expression in active lesions from patients with CIU would uncover unexpected associations. Methods We enrolled eight patients with CIU and six healthy controls, and obtained 4 mm punch biopsy specimens of active lesions and unaffected skin of patients with CIU and of skin from normal controls. Routine histologic evaluation was performed, RNA was isolated, and gene expression data were assessed. Due to technical reasons, the final evaluation included six samples of lesional skin, six samples of nonlesional skin, and five samples of normal skin. Results As expected, lesional skin had more inflammatory cells per high-powered field (mean ± SE, 96 ± 6) than did samples from nonlesional skin of the subjects with CIU (17 ± 2) (p < 0.01). Lesions of CIU showed significant upregulation of 506 genes and reduced expression of 51 genes. Those most upregulated were predominantly involved in cell adhesion (e.g., selectin E [SELE]), cell activation (e.g., CD69), and chemotaxis (e.g., CCL2). Twelve independent canonical pathways with p ≤ 0.001 were identified (including intracellular kinase pathways (RAs-related nuclear protein [RAN] and Janus activated kinase/interferon), cytokine signaling pathways (IL-9, IL10, and IFN), a strong inflammatory response (interferon, IL-9, IL-10, inducible nitric oxide synthase and glucocorticoid pathways) and increased cell proliferation (RAN signaling, cell cycle control, and tRNA charging). Conclusions This preliminary study describes a method to study gene activation in urticarial lesions and demonstrated a strong inflammatory response with a large variety of activated genes that are distinct from those reported with other dermatologic conditions.

Aim:Chronic idiopathic urticaria (CIU) is a frequently occurring disease that seems to be associated with a number of psychological factors. This study aims to determine the frequency of psychiatric disorders and to assess the depression and anxiety symptom severity in a group of children with chronic idiopathic urticaria (CIU).Method:The study group included 30 children, ranging in age from 8 to 16 years, with CIU who attended to the Pediatric Allergy Clinic of the Istanbul Faculty of Medicine and 30 healthy subjects matched for age and sex. The psychiatric disorders were evaluated by using Schedule for Affective Disorders and Schizophrenia for School Age Children-Present and Lifetime Version (K-SADS-PL). State-Trait Anxiety Inventory (STAI), Children’s Depression Inventory (CDI) and Child Behavior Checklist (4-18) were used to examine the levels of depression and anxiety.Results:The K-SADS-PL revealed a psychiatric diagnosis in 63% of the patients. In terms of the distribution of psychiatric diagnoses, the most frequently occurring diagnosis was anxiety disorders (33%), namely simple phobia (21%) and social anxiety disorder (18%). State anxiety scores were significantly higher in children with CIU than controls but not trait anxiety and depression scores. No significant relationships were found between urticaria activity score, duration of illness, age onset and depression and anxiety scores.Conclusion:Psychiatric disorders were frequent in the group children with CIU. Clinicians should be more aware of accompanying psychiatric symptoms in this group of cases.

La fertilización nitrogenada a gran escala y con productos o dosis inadecuados produce contaminación del suelo y napas, como así también la eutrofización de cuerpos de agua y la contaminación del aire, llevando a la formación de lluvias ácidas. En sitios marginales para la producción agropecuaria y forestal, los ambientes son más frágiles y el riesgo de contaminación es mayor que en aquellos con mejor aptitud productiva. La utilización de fertilizantes nitrogenados de liberación lenta pueden contribuir a aportar a los cultivos la dosis de nitrógeno necesaria para no disminuir la fertilidad de los suelos, y en consecuencia, disminuir la lixiviación de formas inorgánicas de nitrógeno y la contaminación ambiental asociada. Realizamos la síntesis y descripción de los complejos de inclusión de urea con ácido esteárico (CIU). El objetivo fue corroborar que los microorganismos del suelo o las enzimas ureasas libres, son capaces de producir la ureólisis, es decir, la liberación del amonio de la urea cuando ésta se encuentra formando parte del CIU. Evaluamos que los cristales de CIU, que presentan muy baja solubilidad en agua, pueden ser fuente de nutrientes para las plantas, es decir, que en contacto con el suelo, liberan amonio y nitrato que son las formas minerales en que las plantas pueden absorber el nitrógeno. Además, la estructura tridimensional de los cristales de CIU pueden ser utilizados para incorporar otros nutrientes o moléculas orgánicas en su estructura, como hormonas, funguicidas o insecticidas, las que serían liberadas paulatinamente, a medida que las ureasas del suelo producen la ruptura de la urea y de los cristales de CIU.

Abstract:
<p class="abstract"><strong>Background:</strong> Appearance of wheals daily for more than six weeks is chronic urticarial (CU). No cause is identified in about 50-70% of chronic urticaria patients and are labelled as chronic idiopathic urticaria (CIU). The aim of the present study is to study the clinical and etiological pattern of chronic urticaria and to find out the incidence of autoimmune urticaria by performing autologous serum skin test (ASST) in patients with CIU.</p><p class="abstract"><strong>Methods:</strong> This was a cross sectional study enrolling 100 chronic urticaria patients satisfying including and excluding criteria. The study was done for a period of 1 year. ASST was done in all the CIU patients after recording detailed history, complete physical and systemic examination.</p><p class="abstract"><strong>Results:</strong> Most of the patients (33%) were in 21-30 years age group with female preponderance (66%). Students (38%) followed by house wives (27%) were majorly involved. 21% patients had history of atopy and 8% had abnormal thyroid function tests. Causative factors noticed in 46% patients and remaining were idiopathic (54%). Infections (32.6%) constituted the major etiological factor followed by physical urticaria (30.4%), food (23.9%), medication (11%) and inhalants (2.1%). In infective agents, 46.6% were bacterial followed by helminthic (33.3%). In 29.6% of CIU patients, the ASST was positive indicating auto-immune urticaria.</p><p class="abstract"><strong>Conclusions:</strong> The etiology cannot be identified in most number of patients and hence they were labelled as CIU and the common causative agents observed were infections followed by physical urticaria, food and medication. ASST is considered as the relevant screening test to detect autoimmune urticaria.</p>

Abstract:
The objective of this study was to evaluate the efficacy of treating chronic idiopathic urticaria (CIU) with levamisole in combination with levocetirizine. This was a multicentre, randomized, double-blind, controlled trial that included 132 patients with active CIU who were treated for 6 weeks with either levocetirizine alone (control group; n = 65) or levamisole plus levocetirizine (treatment group; n = 67). Response to therapy was evaluated by measuring the efficacy rate. After 2 weeks of treatment, there was no significant difference in the efficacy rate between the treatment and control groups (54.84% and 42.37%, respectively). After 6 weeks of treatment, a statistically significant difference in the efficacy rate was observed between the groups (76.27% and 54.39% for the treatment and control groups, respectively). This study demonstrated that a combination of levamisole plus levocetirizine is more effective than levocetirizine alone and potentially provides a new, promising approach to the treatment of CIU.

Abstract:
Objective. To assess the efficacy of bloodletting therapy (acupoint pricking and cupping) in patients with chronic idiopathic urticaria (CIU) in a randomized, control, parallel-group trial. Methods. A total of 174 patients with CIU enrolled from March 2018 to October 2019 were randomized into three groups: group A treated with bloodletting therapy and ebastine, group B treated with placebo treatment (acupoint pseudopricking and cupping) and ebastine, and group C treated with ebastine only. The intention-to-treat analysis was conducted, and the primary outcome was the effective rate of UAS7 score being reduced to 7 or below after treatment phase. Results. The effective rates at the end of treatment phase were different among the three groups ( P < 0.05 ), which were 73.7% in group A, 45.6% in group B, and 42.9% in group C. Multiple analysis indicated differences between groups A and B ( P < 0.0125 ) and groups A and C ( P < 0.0125 ) and no difference between groups B and C ( P > 0.0125 ). No severe bloodletting therapy-related adverse events were observed. Conclusions. In this study on patients with CIU, one month of bloodletting therapy combined with ebastine is clinically beneficial compared with placebo treatment combined with ebastine and treatment with ebastine only. Thus, bloodletting therapy can be an effective complementary treatment in CIU. This trial is registered with ChiCTR1800015294.

Pemakaian energi dalam sektor transportasi di dunia terus meningkat seiring dengan pertumbuhan populasi penduduk. Untuk mengimbangi tingkat permintaan sumber energi yang ada, terutama minyak bumi, salah satu bahan bakar alternatif yang dibutuhkan adalah etanol. Namun, dengan adanya peralihan menggunakan bahan bakar alternatif tersebut menyebabkan kebutuhan alkohol di beberapa negara meningkat sehingga perlu pemanfaatan alkohol seefektif mungkin. Ciu Bekonang merupakan minuman tradisional khas Solo dari fermentasi tetes tebu yang mempunyai kadar alkohol masih rendah, yaitu ±30% yang selama ini belum dimanfaatkan dengan baik dan hanya digunakan sebagai minuman keras. Penelitian yang dilakukan akan mengkaji pengkayaan alkohol dari Ciu Bekonang secara destilasi adsorptif menggunakan zeolit alam dan silika gel sebagai adsorben. Hasil penelitian menunjukkan bahwa destilasi adsorptif menggunakan zeolit alam dan silika gel sebagai adsorben mampu meningkatkan kadar etanol dari Ciu Bekonang, diperoleh kemurnian etanol tertinggi dengan zeolit alam dan silika gel berturut-turut adalah 76% dan 91,61%. Penggunaan silika gel sebagai adsorben lebih efektif dan selektif menyerap air daripada zeolit alam.

Objectives:Antipsychotic medications are registered for the use in psychosis, mania and agitation. the aim of our study was to assess the off-label use of atypical antipsychotics (AA) at the Crisis Intervention Unit (CIU).Methods:Hospital records of 280 patients that were treated at the CIU in year 2007 were included to the observational study. Patients were screened for diagnosis (ICD-10), gender, age, suicidal behaviour and for prescribed psychotropic medications. Off-label use of atypical antipsychotics for diagnoses other than psychosis was evaluated.Results:Most hospitalised patients suffered for stress related disorders: adjustment disorder F43.2 (34%), acute stress disorder F 43.0 (2%), mixed anxiety-depression disorder F41.2 (19%), depression (24%), anxiety disorders (4%) and other disorders (17%). Patients were treated with antidepressants (92%), benzodiazepines (55%), hypnotics (44%) and mood stabilising drugs (9%) during their stay at the CIU. at discharge from hospital benzodiazepines (31%) and hypnotics (12%) were prescribed in lower percentages. 51% of patients received AA at least once during their stay at the CIU, 49% received AA for off-label use and it was prescribed for most of the hospital stay (for 87% of the time). Average daily CPU at admission to the hospital was 107.52 and at the discharge it was 100.33. No significant differences were noted among the different diagnoses for off-label AA use.Conclusions:Atypical antipsychotics are widely used for indications other than psychosis, even though the long-term effects of their use are not yet known and safety issues remain to be examined further.

Background Chronic idiopathic urticaria (CIU) is a complicated skin disease with unknown pathophysiology. MicroRNAs (miRNA) have been shown to be active in cellular regulation. The goal of this pilot study was to examine whether miRNAs may be involved in the regulation of CIU or as biomarkers for CIU. Methods Four groups of three patients each were selected: patients with either active hives or no hives and with positive or negative chronic urticaria (CU) index results. MiRNAs were isolated from patient plasma and analyzed by using miRNA microarray technology to determine the amount of each of the 2567 known human miRNAs. Results A total of 16 miRNAs were found to be differentially expressed in patients with active hives. Among them, five (2355–3p, 4264, 2355–5p, 29c-5p, and 361–3p) were significantly increased in samples with positive CU index results, which could be useful biomarkers for patients with chronic autoimmune urticaria. The miRNA data bases were used to find the targets of these selected miRNA sequences. These potential targets were then compared against a list of 154 urticaria-related genes. Twenty-five genes were found to match. These included eight that were significantly downregulated and eight that were significantly upregulated; however, seven of the eight downregulated genes (FBXL20, OPHN1, YPEL2, STARD9, EZH1, KLHL24, ING4) and five of the eight upregulated genes (BYSL, PNO1, ADAMTS9, STEAP4, SRGN) have no reported roles in signaling. For the 13 genes with reported roles in signaling, the following pathways were found: transforming growth factor beta signaling pathway (NRC31, KITLG, THBS1, CCL2), glucocorticoid receptor signaling pathway (NR3C1, SELE, CCL2), p53 signaling pathway (CCNG2, THBS1, CCL2), p21-activated kinase pathway (PAK1IP1, KITLG, CCL2), phosphoinositide-3 kinase protein kinase B signaling pathway (KITLG, CHRM, THBS1), and neuroactive ligand-receptor interaction (NRC31, HRH1, CHRM), which could play important roles in CIU. Conclusion A better understanding of those genes with undefined function and simultaneous quantitation of both miRNAs and messenger RNAs are needed to fully understand CIU disease.

Consumption of liquor such as Ciu in excessive doses can cause a decrease in hematological status. Balimo stem is an alternative treatment to improve hematological status due to excessive alcohol consumption because it contains alkaloids, flavonoids, and other secondary metabolic compounds, that have functions as antioxidant effects. This study aims to examine and analyze the effect of Balimo immersion water on the hematological status of mice with the observed variables, namely the erythrocytes count, hemoglobin levels, hematocrit value, and total count of leukocytes in rats that had been given Ciu. The study used 20 Rattus norvegicus male rats which were divided into 4 groups. The data were analyzed using one-way ANOVA. The results showed no significant differences (p <0.05) on the Balimo immersion water treatment, but if it was seen from the difference in the mean data of each variable, it could still be seen the difference from each treatment. In this study, it can be concluded that Balimo immersion water was able to improve the hematological status of rats that had been given Ciu liquor with a 0,2 mL dose.

Studies have shown the possible involvement of Helicobacter pylori (H. pylori) infection in chronic idiopathic urticaria (CIU), but the relationship remains controversial. Objective: To quantitatively assess the association between H. pylori infection and chronic idiopathic urticaria. Materials and Methods: This was a case-control type of analytical study and 100 patients were enrolled fifty patients of CIU fulfilling inclusion criteria with no identifiable cause were taken as case and patients without urticaria were taken as control, attending in the department of Dermatology & Venereology, CMH Dhaka for treatment from May 2015 to Aug 2016. Helicobacter pylori infection was confirmed by serum IgG for H. pylori test. Results: The result showed that H. pylori infection significantly affected a high percentage of patients 30 (60%) with chronic idiopathic urticaria. Male respondents were more positive 16(53.3%) than female 14(46.7%), in the middle aged 31-50 year about 18(60%) and triple drug therapy was successful in 16(53.33%) patients in whom H. pylori was detected. Conclusion: Urticaria is a common clinical disorder with complex triggering factors. Chronic urticaria has provided evidence that enteric infection with H. pylori may induce the disease. In this case control study, it was evident that chronic idiopathic urticaria was associated with positive serum IgG for H. Pylori. In this paper, we present the potential of Explainable Artificial Intelligence methods for decision support in medical image analysis scenarios. Using three types of explainable methods applied to the same medical image data set, we aimed to improve the comprehensibility of the decisions provided by the Convolutional Neural Network (CNN). In vivo gastral images obtained by a video capsule endoscopy (VCE) were the subject of visual explanations, with the goal of increasing health professionals’ trust in black-box predictions. We implemented two post hoc interpretable machine learning methods, called Local Interpretable Model-Agnostic Explanations (LIME) and SHapley Additive exPlanations (SHAP), and an alternative explanation approach, the Contextual Importance and Utility (CIU) method. The produced explanations were assessed by human evaluation. We conducted three user studies based on explanations provided by LIME, SHAP and CIU. Users from different non-medical backgrounds carried out a series of tests in a web-based survey setting and stated their experience and understanding of the given explanations. Three user groups (n = 20, 20, 20) with three distinct forms of explanations were quantitatively analyzed. We found that, as hypothesized, the CIU-explainable method performed better than both LIME and SHAP methods in terms of improving support for human decision-making and being more transparent and thus understandable to users. Additionally, CIU outperformed LIME and SHAP by generating explanations more rapidly. Our findings suggest that there are notable differences in human decision-making between various explanation support settings. In line with that, we present three potential explainable methods that, with future improvements in implementation, can be generalized to different medical data sets and can provide effective decision support to medical experts.

The aim of this study was to examine adolescent compulsive internet use (CIU) across a one-year period, to examine associations with symptoms of depression, anxiety and dissociation, as well as to analyze the potential mediating effect of dissociation symptoms. Previous studies have indicated that dissociation might serve as a coping strategy and/or as a consequence of problematic internet use. Participating in the study were 80 adolescents (39 girls and 41 boys), ages from 12 to 18 (mean age 14.90 years) at the time of the first measurement. At Time 1 and one year later at Time 2 measurement points the participants completed the Trauma Symptom Checklist (Briere, 1995) and items from the Compulsive Internet Use Scale (Meerkert, VanDen Eijnden, Vermulst, & Garretsen, 2009). Correlation analyses showed associations between CIU and the adolescents’ ratings of anxiety, depression and dissociation at Time 1 and Time 2. Mediation analysis indicated partial mediation of dissociation symptoms between CIU at Time 1 and Time 2. A comparison of these relationships across time provides opportunity to discuss possible directions of causality and implications of bidirectionality between dissociative tendencies and adolescent prolonged, compulsive internet use.

The paper describes the biomechanical behavior of a cervical implanted unit (CIU) in two conditions: during the physiological and extreme loading. In order to reveal these behaviors, the anatomical structure composed by the C2 and C3 cervical vertebras was implanted using a plate-screws metal structure. The implant was design to perform dynamical, by allowing longitudinal, transversal and rotational movements. The physiological conditions were simulated by the pulsatory negative loading, while the extreme loading was simulated by the alternant symmetrical loading. The tests reveal two behaviors: the durability of the CIU in the physiological loading conditions and the failure of the structure under extreme load.

The Terra Nova Intrusive Complex (TNIC) in northern Victoria Land, Antarctica, results from widespread magmatism during the Early Paleozoic Ross Orogeny. According to field relationships, geochemistry, and geochronology data, the northern part of the TNIC comprises the Browning Intrusive Unit (BIU), which is associated with an arc crustal melting including migmatization of the Wilson Metamorphic Complex, and the later Campbell Intrusive Unit (CIU), which is attributed to the mantle and crustal melting processes. Zircon U-Pb ages suggest Late Neoproterozoic to Early Cambrian protolith with Late Cambrian metamorphism (502 ± 15 Ma) in the WMC, Late Cambrian formation (~500 Ma) of the BIU, and Early Ordovician formation (~480–470 Ma) of the CIU. Sr-Nd isotopic characteristics of the BIU indicate predominant crustal component (εNd(t) = −8.7 to −8.9), whereas those of the CIU reflect both mantle (εNd(t) = 1.8 to 1.6) and crustal (εNd(t) = −4.0 to −7.5) compositions. These results suggest that the northern TNIC magmatism occurring at ~500–470 Ma originated from partial melting of the mantle–mafic crust components and mixing with felsic crust components. By integrating the results with previous studies, the TNIC is considered to be formed by a combination of the mantle and mafic crust melting, crustal assimilation, felsic crust melting, and magma mixing during the Ross Orogeny.

A standardized rule-based scoring system, the Correct Information Unit (CIU) analysis, was used to evaluate the informativeness and efficiency of the connected speech of 20 non-brain-damaged adults and 20 adults with aphasia in response to 10 elicitation stimuli. The interjudge reliability of the scoring system proved to be high, as did the session-to-session stability of performance on measures. There was a significant difference between the non-brain-damaged and aphasic speakers on each of the five measures derived from CIU and word counts. However, the three calculated measures (words per minute, percent CIUs, and CIUs per minute) more dependably separated aphasic from non-brain-damaged speakers on an individual basis than the two counts (number of words and number of CIUs).

STUDY SELECTION/DATA EXTRACTION: Articles and abstracts pertaining to desloratadine were considered for inclusion, with emphasis on randomized, placebo-controlled, double-blind trials. DATA SYNTHESIS: Desloratadine is approved for the treatment of symptoms associated with seasonal allergic rhinitis (SAR), perennial allergic rhinitis (PAR), and chronic idiopathic urticaria (CIU) in patients aged ≥12 years. In placebo-controlled trials, desloratadine demonstrated superior efficacy as a once-daily treatment of SAR, PAR, and CIU. Data suggest that desloratadine has antiinflammatory and decongestant activity. CONCLUSIONS: Desloratadine appears to be a “me-too” agent, with no major differences compared with other second-generation antihistamines.

The expression of the following mediators was evaluated: adhesion molecules (ICAM-1, ELAM-1, VCAM-1); mast cell proteases (chymase and tryptase) and proinflammatory cytokines (IL-1β, IL-3, IL-6 and TNF-α). Of the 20 subjects enrolled, 3 dropped out of the study. Treatment proved successful in most cases (88.2%) (p <0.01) and a significant improvement of all symptoms was registered. Treatment was well-tolerated by all patients; adverse events, neither serious nor drug-related, occurred in any case. Immunochemistry revealed at the baseline a significant expression of ELAM-1, VCAM-1, tryptase, chymase, and TNF-α (p <0.05) in non-lesional skin of patients compared to normal controls. After treatment with fexofenadine, there was a significant decrease in the expression of ELAM-1 (p= 0.02), VCAM-1 (p= 0.04) and tryptase (p=0.04), whereas no relevant change was observed for the other parameters examined. This work confirms the efficacy and tolerability of fexofenadine HCl 180mg in CIU. These preliminary data show a trend towards a decrease in the expression of tryptase and some adhesion molecules after treatment, suggesting an anti-inflammatory activity of fexofenadine.

The aim of this study was to reveal most clinically effective and economically expedient anti-H 1-receptor antagonist cetirizine generation (original or generic), to carry out the analysis of pharmaceutical equivalence of cetirizine generation anti-H-receptor antagonists manufactured by different companies. Materials and methods. Study is executed in design of simple open comparative prospective randomized research in parallel groups. The preliminary analysis of volume and structure of H-antihistamine cetirizine generation products retails for the purpose of definition of the most widespread trademarks was carried out 117 patients with chronic idiopathic urticaria (CIU) treated with various trademarks cetirizine 10 mg/day within 4 weeks were included in the study. Clinical efficiency criteria: dynamics of urticaria symptoms, change of index severity, remission period, days free from CIU symptoms. The pharmaceutical analysis included comparative studying of rate of dissolution and quantitative identification of medicinal substance.
