Abstract:
Air pollution has been associated with poor health outcomes and continues to be a risk factor for respiratory health in children. While higher particulate matter (PM) levels are associated with increased frequency of symptoms, lower lung function, and increase airway inflammation from asthma, the precise composition of the particles that are more highly associated with poor health outcomes or healthcare utilization are not fully elucidated. PM is measured quantifiably by current air pollution monitoring systems. To better determine sources of PM and speciation of such sources, a particulate matter (PM) source apportionment study, the Cleveland Multiple Air Pollutant Study (CMAPS), was conducted in Cleveland, Ohio, in 2009–2010, which allowed more refined assessment of associations with health outcomes. This article presents an evaluation of short-term (daily) and long-term associations between motor vehicle and industrial air pollution components and pediatric asthma emergency department (ED) visits by evaluating two sets of air quality data with healthcare utilization for pediatric asthma. Exposure estimates were developed using land use regression models for long-term exposures for nitrogen dioxide (NO2) and coarse (i.e., with aerodynamic diameters between 2.5 and 10 μm) particulate matter (PM) and the US EPA Positive Matrix Factorization receptor model for short-term exposures to fine (<2.5 μm) and coarse PM components. Exposure metrics from these two approaches were used in asthma ED visit prevalence and time series analyses to investigate seasonal-averaged short- and long-term impacts of both motor vehicles and industry emissions. Increased pediatric asthma ED visits were found for LUR coarse PM and NO2 estimates, which were primarily contributed by motor vehicles. Consistent, statistically significant associations with pediatric asthma visits were observed, with short-term exposures to components of fine and coarse iron PM associated with steel production. Our study is the first to combine spatial and time series analysis of ED visits for asthma using the same periods and shows that PM related to motor vehicle emissions and iron/steel production are associated with increased pediatric asthma visits.

ABSTRACTHenry Wood Elliott (1846–1930), a U.S. Treasury official assigned to monitor the harvest of northern fur seals on the Pribilof Islands in the 1870s, became a self-taught expert on, and defender of, the species. His careful documentation of the seals’ breeding behaviour, and of their commercial harvest, complemented by hundreds of detailed and evocative watercolours, provides a unique record of this once abundant species and the lucrative industry that revolved around it. Elliott's outspoken lobbying on behalf of the seals’ protection is often credited with saving the species from extinction. His paintings of the seals, the seal harvest, and life on the Pribilof Islands in the second half of the nineteenth century constitute an unmatched historical record of this remote region.Elliott was able to witness two full breeding seasons (and harvesting) of the fur seals during his initial stay on the Pribilofs from April 1872 to October 1873. He returned to the islands to conduct a follow-up census of the seals, on behalf of the U.S. Government, in the summer of 1874. He traveled there unofficially and at his own expense in 1876. His fourth trip to the Pribilofs was in the spring of 1890 (again on behalf of the U.S. Department of the Treasury), in response to news of a dramatic decline of the seal populations. In April, 1891, because of his public revelation of mismanagement of the fur seal harvest, Elliott was fired by the Treasury. He continued his tireless lobbying on behalf of the fur seals as a private citizen for the rest of his life. He visited the Pribilofs for the last time on behalf of the House Committee on Expenditures in the Department of Commerce and Labor in the summer of 1913. Born in Cleveland Ohio on November 13, 1846, Elliott died in Seattle Washington on May 25, 1930.

In this article, we describe our comparison of the Cleveland Clinic surveillance definition of Clostridium difficile–associated diarrhe and the definition mandated by the Ohio Department of Health. We found the definitions to be concordant only 71% of the time; the Ohio Department of Health definition identified 278 of the 391 cases identified by the Cleveland Clinic definition. Surveillance definitions mandated by the Ohio Department of Health overrepresented the number of cases attributable to nosocomial transmission at our institution.

Context.— The Department of Anatomic Pathology is a division of the Pathology & Laboratory Medicine Institute at Cleveland Clinic Abu Dhabi. The hospital offers the same model of care as its US-based counterpart the Cleveland Clinic, established in 1921 in Cleveland, Ohio. Pathology services at Cleveland Clinic are internationally acclaimed: the endeavor for Cleveland Clinic Abu Dhabi was to create a parallel facility, with the same standards in a greenfield start-up environment. Objective.— To narrate how we addressed challenges customary in any laboratory start-up and issues distinctive to our setting with the aim to provide a model for others involved in a similar undertaking. Data Sources.— All information in this article is based on published literature obtained by search on internet-based search engines, Clinical and Laboratory Standards Institute, and the authors' firsthand experience. Conclusions.— Key considerations in establishing an anatomic pathology laboratory are careful planning and design, adherence to local and international regulatory standards, selection of equipment and supplies, appropriate staffing, development of a laboratory information system, and sound test validation. In addition to meeting our clinical needs, alliance with the US Cleveland Clinic had an integral role in establishing our laboratory and regional reputation.

AbstractThis essay examines the origin, permutations, potentials, challenges, and implications of two successive, collaborative public history research, teaching, and learning projects undertaken by the Department of History at Cleveland State University, Cleveland, Ohio, and the Department of History and Archeology at Maseno University, Kisumu, Kenya between 2014 and 2018. The two projects explored how opportunities created by the mobile revolution in Africa could be leveraged to generate new ways of acquiring historical information and knowledge between students and faculty in universities separated by enormous distances and by disparate social, economic, and political experiences. Specifically, the projects examined how the cellphone revolution could reshape the production and dissemination of knowledge about important sites, places, events, and people in modern Africa. The essay examines the conception and permutations of the two projects; identifies and explores their potentials and challenges; and proffers thoughts and suggestions that may guide similar future endeavors.

The Language Services Department in the Cleveland Heights-University Heights City School District is described. Its framework can serve as a model for the extension of language support services in the public schools. Traditional Language/Speech/Hearing Services are offered in accordance with PL 94-142 including Early Childhood Intervention. Bilingual Education and English as a Second Language (ESL) are other programs under the auspices of the department. It is the first program in the State of Ohio to include English as a Standard Dialect (ESD) as part of Language Services. The Bilingual Education, ESL, and ESD programs are comprehensively explained.

A case report of the 5-year treatment path of a female patient with Nemaline Rod Myopathy treated by the Department of Oral and Maxillofacial Surgery and the Department of Craniofacial Orthodontics at the Case Western Reserve University in Cleveland, Ohio is presented. Myopathic patients present with disease-specific facies that requires extensive surgical intervention. Additionally, these patients are medically complex, requiring extensive pre and post-surgical planning to avoid adverse events. Our multi-disciplinary and multi-staged treatment plan required no extended post-surgical hospital stays or emergent interventions and she was to reach her stated goal of closing her lips and speaking.

The Cleveland Clinic was established in 1921 under the direction of 4 experienced and iconic physicians: George Crile, Frank Bunts, William Lower, and John Phillips. The Clinic initially employed a staff of only 6 surgeons, 4 internists, 1 radiologist, and 1 biophysicist, but Crile was quick to realize the need for broadening its scope of practice. He asked his close friend, Harvey Cushing, for assistance in finding a suitable candidate to establish a department of neurosurgery at the Cleveland Clinic. With his full endorsement, Cushing recommended Dr. Charles Edward Locke Jr., a former student and burgeoning star in the field of neurosurgery. Unfortunately, Locke’s life and career both ended prematurely in the Cleveland Clinic fire of 1929, but not before he would leave a lasting legacy, both at the Cleveland Clinic and in the field of neurosurgery.
