Purpose This article addresses the ethical conduct of client-centered telepractice, with the following purposes: to define ethical communication, to describe state licensure requirements for practicing clinicians, and to provide examples of lawful and unlawful telepractice. Conclusions The hypothetical scenarios presented in this article employed examples from telespeech; however, the guiding ethical principles are similarly relevant to tele-audiology service delivery. The driving force for telepractice is to improve the quality of life of clients with communication disorders.

Although mental health service providers have focused on the effects of trauma and related interventions for decades, little is known about pervasive and historic trauma, particularly for socially marginalized individuals. Thus, clinical issues associated with sociopolitical oppression have been under-investigated. Coupled with the lack of sufficient cultural competence when working with diverse clients, mainstream clinicians frequently lack adequate case conceptualization skills and culturally sensitive interventions to assist clients from diverse backgrounds. Using traumatic stress as a framework for exploring evidence-based interventions to address long-term, pervasive marginalization and its psychological effects, the authors propose that mindfulness techniques are particularly beneficial to this client population. The authors reviewed culture-centered interventions to address traumatic stress for marginalized client populations, focusing on the mindfulness practice of Falun Gong. Recommendations for practice include the inclusion of traumatic stress theory and techniques in pre-service training, professional development training for practitioners focusing on mindfulness techniques with clients assessed with historical trauma, and Web-based training for clinical faculty to enhance their knowledge about traumatic stress, historical trauma, and associated interventions for clients from marginalized communities. The authors offer recommendations for future research that focuses on studies exploring the usefulness of Falun Gong in working with clients with traumatic stress.

Mental and medical health-care providers often serve as first responders and longer-term therapeutic support for transgender intimate partner violence (T-IPV) survivors. Mental health-care providers in particular are positioned to offer important, client-centered assistance in dealing with the intricacies of addressing IPV, and research finds they are among the most trusted and utilized sources of help by survivors. Likewise, due in part to the emergency medical needs of survivors of physical IPV, medical health-care providers can play a vital role in recognizing IPV victimization and guiding patients toward needed resources. Drawing on both the T-IPV and broader lesbian, gay, bisexual, transgender, and queer IPV literatures, this chapter provides a template for best practices in a multisystem approach with regard to developing culturally appropriate mental and medical health-care provider training programs, screening protocols, treatment modalities, and auxiliary service referrals when working with T-IPV survivor populations.

This chapter is concerned with the impact of practitioner biases on the experience of a meaningful life for individuals who live with serious mental illness (SMI). Professional biases, systemic biases that originate in societal fear and lack of knowledge, and internalized stigma taken on by the consumer affect life decisions. Following a history of treatment initiatives experienced by consumers as abusive, it is important to understand how a system envisioned to protect and treat was often experienced as harmful. In the 1980s a movement emerged to transform the nature of mental health treatment to a client-centered, recovery-oriented model. In 1999, the Surgeon General proclaimed that all agencies serving this population should be recovery oriented. Yet, the shift to this approach to understanding people with SMI has not been complete. While there are many explanations why practitioners may not fully embrace this perspective, this chapter introduces the concept of “schemas” from cognitive behavioral theory as a way of examining professional biases in the field of SMI.
