Abstract:
Abstract Background and Purpose. The long-term management following an hemicorporectomy (HCP) is not well documented in the scientific literature. The purpose of this case report is to describe the 25-year history of a man with a spinal cord injury who experienced severe musculoskeletal deterioration and hemicorporectomy. Case Description. The client sustained T10 complete paraplegia at age 18 years, developed severe decubitus ulcers, and required an HCP as a life-saving measure 13 years later. The authors describe the chronology of several rehabilitation and prosthetic strategies and speculate on factors that may have contributed to their successes and failures. Outcomes. The client survived 12 years after the HCP and returned to independent mobility, self-care, and schooling despite complications with continued skin breakdown. Over the 12 years following discharge from the hospital after the spinal cord injury, he spent 749 days in the hospital. During the 12 years he lived after discharge from the hospital following the HCP, he was hospitalized 190 days. Discussion. The authors discuss factors contributing to the client's musculoskeletal deterioration including chronic wounds, postural deviations, and incomplete adherence to pressure-relief recommendations and raise considerations for physical therapists who treat patients after HCP.

Abstract:
Previous research has consistently found self-assessment bias (an overly positive assessment of personal performance) to be present in a wide variety of work situations. The present investigation extended this area of research with a multi-disciplinary sample of mental health professionals. Respondents were asked to: (a) compare their own overall clinical skills and performance to others in their profession, and (b) indicate the percentage of their clients who improved, remained the same, or deteriorated as a result of treatment with them. Results indicated that 25% of mental health professionals viewed their skill to be at the 90th percentile when compared to their peers, and none viewed themselves as below average. Further, when compared to the published literature, clinicians tended to overestimate their rates of client improvement and underestimate their rates of client deterioration. The implications of this self-assessment bias for improvement of psychotherapy outcomes are discussed.

Purpose The purpose of this study is to examine whether the timing of auditor terminations signals the riskiness of client firms. Design/methodology/approach This empirical study uses a sample of auditor switches during 2003-2014 to conduct univariate tests and multivariate regression analyses. Auditor switches occurring after the audit report date but before the shareholders’ meeting are classified as “planned” terminations and auditor switches that occur outside of this window are classified as “abrupt” terminations. Findings First, abrupt terminations are more strongly related to client risk factors than planned terminations. Second, relative to planned terminations, abrupt terminations are more likely to result from an auditor resignation rather than a client dismissal. Third, abrupt termination firms are more likely to have internal control weaknesses and experience delistings in the following year. Future operating performance is also worse after an abrupt termination. Finally, auditors and investors view abrupt terminations as riskier than planned terminations. Practical implications As the timing of the auditor termination is publicly available information, it can provide an important signal of deteriorating financial performance to shareholders and potential investors. Abrupt terminations could be costly to shareholders because those firms likely have lower quality financial reporting (due to internal control weakness) and deterioration of future operating performance. Originality/value While concurrent studies investigate the relation between the timing of new auditor appointment and audit quality, this is the first study to document the relation between the timing of auditor termination and the riskiness of client firms.

Abstract:
Purpose The purpose of this paper is to contextualise the architect–client relationship and evaluate the factors responsible for its deterioration, and then define the impact of these factors on the future needs of architects and clients, including how such knowledge can help emerging architects to develop an understanding of the profession at an early stage. It will attempt to reveal new insights and build consensus around issues, such as functionality and aesthetics, per cent-based fee structure, conflict of interest amongst architects, contractors and clients. Design/methodology/approach A combination of qualitative online survey, semi-structured interviews and online focus group discussions under the comprehensive umbrella of the case study method has been used to construct a pragmatic framework. The data collection was focused on revealed preferences rather than stated preferences, in terms of likes and dislikes, in a standard survey. Findings Overall, this paper strengthens the idea that the predicament of the profession and the marginalisation of architects is due to their detachment from clients. The findings suggest that the fee structure might be a major source of discontent and there is an urgent need for alternative routes of procurement, particularly for private residential clients. While most clients prefer functionality over aesthetics and want architects to be affordable, they are more willing to invest their trust in architects who can deliver from concept to completion. Research limitations/implications The arguments contested in this paper attempt to demystify the dynamics that are at play during the construction stage. It looks at power sharing, responsibilities and silent hierarchies that transpire between architects, clients and contractors, particularly in private residential projects. Originality/value The main recommendation of this paper is that to secure the future of the architecture profession emerging architects need to be trained more in client-centric skills than design-centric aptitude.

Abstract:
Negative change occurring during psychotherapy is relatively underresearched and current theories take little account of social context. Eight clients and four therapists were interviewed about their therapy experience when reliable score deterioration on an outcome measure was observed, with the aim of generating a Grounded Theory model of negative change. The emerging model identified three major themes: a context of adversity, the therapeutic experience, and help withdrawn (within the context of positive outcomes). Difficulties included wanting therapy to provide more advice, talking about distressing issues, relationship difficulties, ambivalence, and a wish for support instead of change. Client context merits increased attention. Changes in outcome measures used, more diverse supervision models, training to use outcome measures constructively, and sensitive, routine analysis of negative change are indicated. Increased availability of therapies able to address clients’ interpersonal and social context and a wider choice of therapy type would allow interventions to better fit the individual in context.

Abstract:
The purpose of this study was to investigate one-year changes and differences in changes in activities of daily living (ADL), usability, a history of falls, concerns about falling, and self-rated health across five housing adaptation (HA) client profiles identified previously using a cluster analysis approach: older adults with low level of disability (n = 59); older adults with medium/high level of disability (n = 26); adults with low level of disability (n = 10); adults with high level of disability (n = 8); and older adults with medium level of disability including at least moderate cognitive impairment (n = 5). Comparisons between the five profiles include secondary analyses aggregating those with low level of disability and those with medium/high level of disability. Changes within the client profiles demonstrate a complex pattern of improvements and declines, depending on outcome, with no profile showing consistent improvement or decline across all outcomes. The risks of deterioration over one year were the highest among those with cognitive impairments at baseline, but no recommendation of prioritization decisions based on baseline profiles can be made. Instead, it seems that all HA clients, independently of baseline profile, are at risk of increasing disability over time and require follow-up evaluations regularly.

Background: The process of caring for a schizophrenic client creates a stressful situation; caregiver's unpreparedness in dealing with this problem has an impact on the deterioration of the quality of life. The decline in quality of life of caregiver is influenced by several factors such as caregiver’s burden and characteristic of caregiver include age, gender, sex, marital status, education, income, and relationship with client. This study aimed to determine the relationship of burden and characteristic with the quality of life on the caregiver.Methods: The design of this study was descriptive analytic with cross sectional study approach on 186 caregiver with convenient technique sampling. Data were collected by the Zarit Burden Interview questionnaire and the World Health Organization Quality of Life (WHOQOL-BREF).Results: The results showed 99 (53.2%) caregivers had a low quality of life, 92 (49.5%) caregivers felt heavy burden. Chi-square statistical test proved a significant relationship between burden (0.001) and all characteristics caregiver include age (0.033), gender (0.033), sex (0.000) marital status (0.001), education (0.001), income (0.002), and relationship with client (0.000) with quality of life.Conclusions: The result showed that test proved a significant relationship between burden and characteristics caregiver with quality of life of multivariate analysis showed that marital status was the most correlated with quality of life of caregiver. Suggestions for Tampan Psychiatric Hospital Pekanbaru are to improve mental nursing service comprehensively to client and family like health education program about how to improve quality of life.

Ethics have a verifiable effect on the validity and economic supportability of quantity studying practices and additionally influencing the individual security of the profession. There is a developing consent inside and outside the construction industry that unscrupulous practices are endemic in the construction industry especially quantity surveying practice in Nigeria. This paper thusly, x-rayed the outcomes of unethical practices by Quantity Surveyors in the construction industry in Nigeria, and likewise displays more scholarly and practical approach in dealing with moral appraisal of Quantity Surveyors in the construction industry and ways to improve moral standard among them. Very much organized polls (questionnaire) were self-administered to stakeholders and professionals in construction organizations. Literature review also revealed that unethical practices result to dissatisfied clients, late compensation due to delays, low productivity/efficiency of project team, deterioration in professionalism, poor workmanship, high maintenance, upward review of contract cost, poor project coordination, poor quality infrastructure development, stunted growth of the industry, reduction in the life span of building, loss of public trust, conflicts between client and construction team, poor quality of infrastructural development and loss of public trust. Data were collected and analyzed using Mean Item Score (MIS) and Standard Deviation (SD). The results shows that dissatisfaction of clients, High maintenance cost and Poor workmanship were perceived to be the major ethical misconduct by Quantity Surveyors in the construction industry. The study recommended that Quantity Surveyors should not always make financial benefits, other advantages as their real concern and gain cash unethically by all means. Others professionals in the industry ought to dependably be caution and endeavor not to keep any untrustworthy conduct among themselves. Major client in Nigeria construction industry, which is the government need to be fair to the Quantity Surveyors and other professionals in the construction industry.

Abstract:
Patient-focused research systems have been developed to monitor and inform therapists of patients' treatment progress in psychotherapy as a method to enhance patient outcome. The current study examined the effects of providing treatment progress information and problem-solving tools to both patients and therapists during the course of psychotherapy. Three hundred seventy patients at a hospital-based outpatient psychotherapy clinic were randomly assigned to one of two treatment groups: treatment-as-usual, or an experimental condition based on the use of patient/therapist feedback and clinical decision-support tools. Patients in the feedback condition were significantly more improved at termination than the patients in the treatment as usual condition. These findings are consistent with past research on these approaches although the effect size was smaller in this study. Treatment effects were not a consequence of different amounts of psychotherapy received by experimental and control clients. Not all therapists were aided by the feedback intervention.

Abstract:
Researchers have found evidence that when clinicians use an evidence-based feedback system that uses Clinical Support Tools (CST) for not-on-track clients, deterioration rates fall and success rates improve (Shimokawa et al., 2010). Despite multiple studies finding evidence in support of using the CST, there has been a discrepancy between effect sizes (i.e., d = 0.5; Simon et al., 2012). As such, further replicate of these past studies is needed to discover if small effect sizes still persist and if so, what possible variables may contribute to inconsistent findings. For the current study, it was predicted that the use of the CST would result in significantly lower OQ-45 scores at treatment termination after controlling for the intake OQ-45 score. Additionally, previous research indicated that the combined intervention of the progress feedback plus CST would significantly reduce deterioration rates with those NOT. Out of 1,122 participants, 172 were randomly assigned to one of two conditions: The CST feedback group (n = 71) and the no CST feedback group (n = 101). There was not a significant difference in the mean OQ-45 scores for the CST feedback group (M = 2.39, SD = 20.95) and the no CST feedback group (M = 4.17, SD = 19.74). The results of this study raise questions about how regularly the therapists were monitoring their clients' progress feedback and whether the CST are effective. Additionally, the author evaluates the timing of when the CST were administered to clients and when therapists reviewed the feedback.

A recent study highlighted discrepancies between qualitative client self-reports of outcome and OQ-45 reports. Specifically, only 8.8% of clients who deteriorated during a course of therapy based on the OQ-45 perceived that they had deteriorated, while 50% of these clients perceived that they had improved in therapy (Top et al., 2018). This phenomenon, where different means of tracking outcomes yield divergent results, has been called “paradoxical outcome.” The trend suggests that the most advanced forms of tracking psychotherapy outcomes might not detect important facets of outcome from the perspective of psychotherapy clients. The current study is a qualitative investigation of the experience of psychotherapy clients who reported improvements in therapy despite meeting criteria for deterioration per the Outcome Questionnaire-45.2 (OQ-45; Lambert et al., 1996). We used a consensual qualitative research (CQR) protocol (Hill, 2012). CQR uses group consensus to detect themes in participant interviews. Common themes included attributing negative changes to factors outside of therapy, endorsing complicated circumstances, and reporting positive outcomes that were not well detected by the OQ-45. More results and their implications are discussed.

Objectivo: Cerca de 5% a 10% das pessoas que recorrem aos serviços de psicologia/ psicoterapia, se encontram pior no final do processo psicoterapêutico. A psicoterapia apesar de comprovadamente eficaz ainda apresenta lacunas neste sentido. Como tal, mais estudos são necessários acerca dos motivos que estão na base desta realidade. Assim sendo, este estudo teve como objectivo perceber do ponto de vista do cliente, que características do terapeuta podem estar associadas a eventos indesejáveis em psicoterapia. Método: Foi construído um questionário constituído por seis perguntas de questionamento aberto como forma de apreender com maior rigor a realidade dos eventos indesejáveis. Este questionário foi, por sua vez, foi disponibilizado em plataformas online sendo que o estudo contou com a participação de 45 participantes. De seguida, os dados foram analisados com recurso à análise de dados qualitativa grounded theory Resultados: Verificou-se que características do terapeuta estão associadas aos eventos indesejáveis, sendo que estas relacionam-se por um lado com factores relacionais e por outro lado com competências profissionais. Ademais, os eventos indesejáveis tiveram impacto a nível da percepção dos clientes da psicoterapia e como forma de lidar com os mesmos, os participantes utilizaram estratégias de afastamento como o dropout. Conclusões: Mais estudos são necessários na investigação dos eventos indesejáveis em psicoterapia sendo o terapeuta uma variável importante neste fenómeno.
Aim: About 5% to 10% of people who resort to psychology/psychotherapy services find themselves worse at the end of the psychotherapeutic process. Psychotherapy though efficient, continues to present gaps. As such, more studies are needed to understand the reasons underlying this reality. Thus, this study aimed to detect from the client's point of view, which may be the therapist characteristics associated with undesirable events in psychotherapy. Method: An online questionnaire, constituted of six open questions, was made as a way of understanding in more detail the reality of undesirable events in psychotherapy. This study counted with the participation of 45 people. Data were analyzed through grounded theory qualitative method of analysis. Results: Therapist characteristics associated with undesirable events are related with both relationship factors and professional skills. Undesirable events had impact on the way participants perceive psychotherapy and as a way to deal with this phenomenon, participant utilized distance strategies as dropout. Conclusions: More studies are necessary related with undesirable events in psychotherapy considering the therapist an important variable.

This meta-analysis examines the impact of measuring, monitoring, and feeding back information on client progress to clinicians while they deliver psychotherapy. It considers the effects of the two most frequently studied routine outcome monitoring practices: the Partners for Change Outcome System and the Outcome Questionnaire System. Meta-analyses of 24 studies produced effect sizes from small to moderate. Feedback practices reduced deterioration rates and nearly doubled clinically significant/reliable change rates in clients who were predicted to have a poor outcome. Clinical examples, diversity considerations, and therapeutic advances are provided.

The social worker’s first task with clients is to develop a positive relationship, but as the intervention progresses he or she faces the ongoing challenge of sustaining that relationship so that the client can experience a positive environment for working toward his or her goals. As with all relationships, however, despite the practitioner’s best efforts, conflicts and misunderstandings may develop between the two parties that can threaten their bond and possibly undermine the work being done. The term relationship rupture refers to any event in direct practice that contributes to a deterioration in the social worker/client relationship once it has been positively established. The purpose of this chapter is to help social workers to become more skillful in identifying and repairing ruptures so that the relationship can resume in a productive direction.

This chapter makes a case for the value of tracking mental health functioning over the course of psychotherapy. The need for progress monitoring in psychotherapy arises from the fact that about 8% of clients worsen during treatment, and clinicians have a difficult time identifying such treatment failures. A tracking measure (Outcome Questionnaire-45 [OQ-45]) consisting of 45 self-report items used to rate symptoms, interpersonal problems, and social role dysfunction is described and recommended for use in private practice with adults. The consequences for clients identified as at-risk cases in several clinical trials are presented. The data show that by using these outcome measures, deterioration rates can be substantially reduced and positive outcomes are doubled.

Abstract:
This chapter explores the link between the health outcomes of spinocerebellar ataxia and diet and nutrition as well as overall quality of life and well-being that is achieved as a result of nutritional support and nutritional profile. Spinocerebellar ataxia is a hereditary condition characterized by degenerative changes to parts of the brain, extending to the spinal cord, that affects mobility and voluntary actions. Due to the deteriorating impact of this neurological disorder, the management of health and wellness of the individual is imperative in stemming physiological decline and morbidity. The connections between dietary intake, quality of life and well-being are important components of the health response in providing optimum health outcomes for clients diagnosed with spinocerebellar ataxia. Consequently, an examination of factors that impede, promote and generally affect dietary intake, nutritional status and profile is essential towards improving disease related quality of life and morbidity and mortality risk. The cyclical impact of the neurological condition on nutritional status and its corresponding impact on disease progression is an important exploratory point. Finally, recommendations and standardized guidance are crucial to expanding the health care approach and the overall wellness of individuals with spinocerebellar ataxia.

Abstract:
<p>The original 100 m long Myrtle Avenue Viaduct was constructed in 1913 and carried two critical New York City Transit (NYCT) subway tracks in Brooklyn. The geometric configuration of the viaduct weaves through a narrow corridor on a NYCT-owned private right-of-way with multiple adjacent properties, including occupied houses straddling the viaduct on both the east and west sides. Many of these residents and businesses had to be relocated during the 10-month replacement process. Constant operation and exposure to the elements resulted in severe deterioration of the original viaduct leaving it in a state of disrepair. Factoring risk, cost and existing conditions of the structure, the engineer’s assessment led to a proposed design alternative beyond the client’s expectations, which originally called for replacement of the superstructure only. The designer recommended an entirely new structure and promised completion within the same time frame as the originally planned partial replacement.</p>

Healthcare is the world’s largest industries today. Incorrect decisions which have been taken in any industrial enterprise would impact year-end balance sheet or in worst case scenario, temporary economic downturns. However, mistakes which are made in the management of health industries would end up lowering quality of human life which could give rise to deterioration of the welfare society. Management philosophy of the health care institutions has a direct impact to solution of health issues. Therefore, their approach of management play an important role in the development of quality of life. Knowledge and individual skills of those involved in the management of health managers at different levels of organization is crucial for the future of the company and consequentially for welfare of nation. In this study; Aesthetic Medicine which is one of the most exclusive and ever-developing areas of health institutions has been analyzed. These institutions aim to enhance their social perception by improving their appearance with noninvasive aesthetic treatments and by providing preventive treatments to maintain their youthful appearance. The frequency of the applied treatments in aesthetic medical institutions varies according to their clients' economic welfare, socio-cultural evolution of their society, sex, age range and many other sociological parameters. In order to meet the expectations in health institutions, it is crucial to determine the correct customer profile by utilizing present opportunities of the sector. This study is prepared to present an approach to create sectoral innovation by drawing attention to self-renewing business growth methods for aesthetic medicine management.
