This thesis describes investigations conducted to study factors influencing the outcome of antifungal treatment of denture stomatitis. Clinical, mycolognic, haematologic and prosthodontic factors were evaluated. The humoral and cellular immune responses were also studied. Two antifungal agents were compared: systematically administered fluconazole and topically administered amphotericin. A total of 29 patients were randomised to receive fluconazole 50mg daily for 14 days; 30 were randomised to receive amphotericin lozenges and cream for 28 days. A whole gut lavage study performed in pouch patients showed that there is gut protein loss in ileoanal pouches similar to that in IBD. This protein loss relates to disease activity and may be used as an objective indicator of disease activity in ileoanal pouches. The protein loss is evident in the first few months of pouch function and may settle at a later stage. The whole gut lavage method allowed measurements of haemoglobin in the lavage specimen. This bioactive antibody causes thyrotoxicosis and it may contribute to extra-thyroidal manifestations, particularly Graves’ ophthalmopathy and pre-tibial myxoedema. Charcot-Marie-Tooth disease type 2C (CMT2C) is an autosomal dominant neuropathy characterized by limb, diaphragm, and laryngeal muscle weakness. We studied five unrelated families with CMT2C, of which two showed significant linkage to chromosome 12q24.11. Linkage analysis excluded this locus in one of the remaining families, suggesting genetic heterogeneity within this CMT subtype. Poly(ADP-ribose)polymerase (PARP) is a nuclear enzyme involved in the repair of DNA single strand breaks via the Base Excision Repair pathway (BER). Temozolomide, a DNA alkylating agent recently licensed for the treatment of gliomas and melanoma, produces DNA lesions which are targets for BER. Preclinical studies demonstrate that PARP inhibitors increase the cytotoxicity and antitumour activity of temozolomide suggesting that PARP inhibitors may have a clinical role as chemopotentiating agents. Chronic cough is a common presenting symptom to both general practice and respiratory clinics and in up to 20% of patients, the cough remains unexplained after extensive investigation and treatment trials. In this thesis, we have shown that patients with idiopathic chronic cough are predominantly female, have an onset of cough in middle age and have a high prevalence of organ specific autoimmune disease, particularly hypothyroidism.
