This article explores the emergence of transcranial magnetic stimulation (TMS) as a new therapeutic approach and the implications of this technology for the study and treatment of neuropsychiatric disorders, with a focus on major depression. Relapse, chronicity, and varying degrees of treatment resistance characterize major depression. A substantial number of patients are not effectively treated with pharmacology or medications alone. It is proposed that TMS, along with other device-based therapies emerging in psychiatry, may define a potential new treatment platform, with existing therapeutics for major depression. Through the example of a study, this article describes the methodological considerations in the development of TMS for the treatment of major depression. Device-based approaches to therapeutic neuromodulation hold the promise of significant clinical advantages compared to existing treatments for major depression, but evidence in well designed and properly blinded multicenter trials is still lacking, hence, research in this area is ongoing.

The practice of clinical child and adolescent psychology is then explored in diverse settings, including school, community mental health, outpatient, inpatient, forensic, and private practice settings. Implementation of evidence-based practices in these settings may help close the oft-cited gap between research and clinical practice. Finally, timely topics are presented that will likely influence mental health care and research with children and adolescents over the next couple of decades, namely, the neurobiological mechanisms underlying psychopathology and treatment, electronic and telehealth delivery developments, cognitive training methods and technology, transdiagnostic approaches, and pediatric psychopharmacology. The volume concludes with special attention to the dissemination of evidence-based assessment and treatment practices.

The inclusion of a pharmacist in the palliative care team is becoming appreciated and widespread. Effective medicines management can improve patient outcomes and have financial benefits for organizations. Most patients prefer to spend as much time as possible at home so pharmacist input in the community is especially crucial in ensuring they receive their medicines and have information to support them in taking them. Palliative care normally involves using strong opioids and the legal requirements surrounding their use can be complex. Pharmacists have detailed knowledge in this area so can advise services how to use them safely and legally. It is common practice to use medicines outside their licence in palliative care, for other indications and routes than originally designed and also when mixing them in syringes for continuous infusion. Information on these practices can be best provided by pharmacists.
