Abstract:
Background: This study “clinico-pathological correlation of abdominal lymphadenopathy" intended to know the various etiological causes behind the abdominal lymphadenopathy and different modes of presentation which can help in treatment and prognosis in our setup which helps in better management of these cases thus helping to improve prognosis. Methods: The" Clinico-Pathological Correlation of Abdominal Lymphadenopathy is a clinical study of 250 consecutive cases of abdominal Lymphadenopathy which were found during intraoperative laparotomy in Hamidia Hospital (Gandhi Medical College), Bhopal. This is observational study will be carried out in the Department of Surgery, Gandhi Medical College. Total 250 consecutive cases will be included in this study having per-operative findings of lymphadenopathy, where laparotomy will be done for various regions. Prior to surgery thorough history will be taken and meticulous physical examination will be performed. Necessary laboratory and imaging studies shall do to establish the diagnosis. Results: Tuberculosis is one of the common causes of Abdomen lymphadenopathy. Age incidence more in males and in second and third decade of life is more common. Abdominal pain, loss of weight and appetite with bowel disturbances are the common clinical manifestation. It is obvious from this study that tuberculosis is a problem in our country. Regarding the abdominal tuberculosis, the diagnostic problem persists for those patients where pulmonary tuberculosis is not obvious. Most tubercular patients present with perforation peritonitis. Conclusions: This study support fact that abdominal lymphadenopathy is important indicator of underlying cause of pathogenesis from which we reach the diagnosis. In present study in 10% cases only lymph nodes are positive for tuberculosis without tissue diagnosis, which can be treated to prevent the further complications.

Abstract:
Introduction: Present study relates to the results of clinical examination of 10 patients with sickle hemoglobin. Material and Methods:All the patients belonged to TharuChaudhary community of Nepal. The patients were clinically examined. Sickling test was done using Sodium metasulphite. Hemoglobin agarose gel electrophoresis was done at alkaline pH. Total hemoglobin was estimated by cyanhemoglobin method. Results: Sickling test was positive in all the patients. In addition, electrophoresis revealed detection of HbS in all the patients. Four of 10 patients had homozygous sickle cell disease (βglobin genotype SS). Five other patients had heterozygous sickle cell trait (βglobin genotype AS). Another patient had double heterozygous sickle cell β+thalassemia; high level of fetal hemoglobin (16.4%) was also detected in this patient. Discussion: Capillary thromboses might have resulted in minor infarcts, resulting in pain in chest and abdomen. Conclusion:The patients belonged to mid-western region of Nepal; this region is infested with vivax malaria. High frequency of malaria in this region might have resulted in biological advantage for sickle hemoglobin patients.

Abstract:
INTRODUCTION:- Leprosy is one of the major health problems in Nepal, caused by Mycobacterium leprae. Histopathological examination is considered as important tool for proper classification of the disease. This study was carried out to study the histopathological features of leprosy in skin biopsies and classify based on microscopy, bacterial index to correlate with clinical presentations. MATERIAL AND METHODS:- A retrospective observational study was done at Department of Pathology of Universal College of Medical Sciences (UCMS) Bhairahawa for 2 years from 1st November 2015 to 31stOctober 2017 and total 68 cases were analysed. Cases in which histopathological diagnosis of leprosy was made or considered differential diagnosis were selected for study. Ridley and Jopling classification was used histologically to make diagnosis of leprosy. Copies of issued histopathology reports, that are preserved in the department routinely were used to obtain data pertaining to age, sex, clinical information and histopathological findings. RESULTS:- This study included 68 patients diagnosed histologically as leprosy. Histopathological examination of the skin biopsies of 68 patients revealed that, the maximum histopathological cases 28 (41.17%) are BT followed by 15 cases (22.05%) of IL (15) and 9 (13.23%) of TT. Complete parity between clinical diagnosis and histopathology was observed in 55.88 % and disparity was seen in 44.12%. CONCLUSION:- For proper classification of Leprosy, correlation of clinical and histopathological features along with bacterial index is more useful than considering any of the single parameter.

Abstract:
A prospective study was carried out on 52 patients with allergic rhinitis, during March 1993–January 1995. This was to analyse the occurrence of bronchial asthma among them and to correlate the symptoms, signs and pathological findings of allergic rhinitis with that of bronchial asthma in those patients with both the disorders. The therapeutic benefits with regard to the symptoms of bronchial asthma in these patients were assessed after treatment of their nasal allergy for a period of at least four months.A definite aetio-clinico-pathological correlation could be established between allergic rhinitis and bronchial asthma and it could be concluded from the study that prompt attention to nasal symptoms in cases with co-existent bronchial asthma helps to a large extent in curbing the symptoms of the latter.

Abstract:
Dengue infection is a major vector borne disease. The classical signs and symptoms include high fever, severe headache, chill, and rash. Atypical manifestations of dengue infection with hepatic involvement are frequently reported, ranging from mild elevations of liver enzymes to acute liver failure. Analysis of 110 serologically confirmed cases of dengue infection in Holy Family Red Crescent Medical College hospital a tertiary care hospital was done. Patients with normal aminotransferase levels were categorized into Grade A, patients with at least one of the enzymes raised above normal but less than 3 times were categorized into Grade B, those with at least one of the enzymes elevated more than 3 times but less than 10 times were graded as C, and those with elevations more than 10 times as Grade D. 83.7% patients had alterations of transaminase levels, with 45.5% categorized into grade b, 30.9% into grade C, and 7.3% into grade D or acute hepatitis (P value 0.224). Rise of aspartate aminotransferase (AS7) levels were higher compared to the levels of alanine aminotransferase (AL7). Liver damage with alteration of aminotransferases is a common manifestation of dengue infection and also valuable markers for monitoring these patients.

Abstract:
Background: Breast cancer (BC) is a common cause of cancer related death, yet the efforts to follow-up patients in developing countries are humble. This study aims to establish a primary percentage of the 7-year BC survivors in Duhok province and correlate it to the age and the main histopathological features at time of diagnosis; including the tumor size and the status of the lymph nodes (LNs). Material and method: A total of sixty BC patients were included in this follow-up study, all of them were diagnosed in the histopathological department of the central lab in Duhok before 2013. The survival rate was estimated by contacting the survived patients or the families of the late BC patients. Results: The study reported 7-year survival rate of 30%. Mortality was significantly higher in patients younger than 40 years and in patients equal or older than 50 years. Most of the patients (86.7%) had tumor size equal or greater than 2cm at time of diagnosis. Mortality was significantly higher when the tumor size was greater than 5 cm. About 78.3% of the patients had positive LNs. Mortality in these patients was only slightly higher (70.2%) than those presented with negative LNs (69.2%). Conclusion: The high percentage of mortality in the first 7-years and the high percentages of patients presented with large tumor size and positive LNs at time of first diagnosis require a special attention and further follow-up studies.

Abstract:
Background: Cancer is one of the risk factors with estimates suggesting annual incidence of venous thrombo-embolism (VTE) is 0.5% in cancer patients compared to 0.1% in the general population. VTE is the second most common cause of death from cancer. Cancer associated thrombosis (CAT) is the target of intense interest in recent medical literature, including its epidemiology, pathophysiology, challenges in diagnosis, prophylaxis and treatment. Cancer types, stages, treatment and comorbidities are among the risk factors for developing VTE. Methods: This was a retrospective study based on patient hospital records from May 2017 to Dec 2021. About 50 patient records reportedly having CAT were studied to meet research objectives. Data pertaining to patient age, gender and comorbidity was collected for study purpose. Patients having confirmed diagnosis for deep venous thrombosis (DVT) (Duplex scan) and malignancy (Biopsy) were considered to ensure data completeness for analysis and reporting. Results: Our study showed that there was no correlation between different types of cancer with deep venous thrombosis, neither with different approach of treatment had it showed any significant results nor with any comorbidity. But we found out a correlation between different cancer with various comorbidity. Conclusions: Our study aimed at exploring clinical and pathological correlation between DVT and malignancy. Our study was a sincere effort to study clinic-pathological correlation among cancer patients with known cases of venous thrombosis. Further studies are warranted with large sample and more extensive case record format, and preferably long-term follow-up to understand patient outcome.

Abstract:
Bowen’s disease is a type of squamous cell carcinoma in situ. It is most frequently diagnosed in the sixth to eighth decade of life. However, there have also been a few reports of pigmented Bowen’s disease. A single or multiple erythematous, round to irregular, scaly, keratotic, nodular, pigmented patches/plaques are first signs of the lesion. All cases of Bowen’s disease presented at a tertiary care centre over a period of seven years were included in this study. The patient details and clinical history were obtained from medical records. Histopathological slides of these cases were reviewed and analyzed. There were fourteen cases of Bowen’s disease with most patients belonging to 5-6th decade of life (80%). Males were more commonly affected (60%). Duration of the lesion ranged between 3-5 years in 60% of the patients. Most common site involved was lower back (80%). Most common presenting feature was hyperpigmented, erythematous plaques with scaling seen in 90% of the patients. Epidermal changes such as marked acanthosis with full thickness atypia were noted in 90 % and hyperkeratosis, parakeratosis was noted in 70% of the patients. Lichenoid infiltrates at dermo-epidermal junction was noted in 70% cases. The correct identification helps in appropriate diagnosis and treatment after biopsy confirmation. Adequate resection helps in preventing risk of invasive malignancy in these conditions.

Abstract:
Background: Alcohol is one of the leading causes of “preventable” morbidity and mortality worldwide. It is associated with liver damage. A gray area is temporal relation between clinico-biochemical severity and histological changes in liver, neither this issue has been widely studied. Methods: A hospital based cross sectional clinico-pathological pilot study was undertaken in a tertiary care hospital in West Bengal in patients with a history of alcohol intake who had been admitted in the inpatient department of medicine. Assessment of patients with history of alcohol intake with respect to clinical, biochemical and histopathological examination was performed. The correlation between clinico-biochemical severity and histopathological stages in cases of alcoholic liver disease was evaluated. Results: There was a significant correlation between clinico-biochemical severity and liver biopsy changes. The severity of histopathological changes of alcoholic liver disease was found to correlate significantly with the severity of abdominal parameters with Pearson correlation cofactor of 0.819.Conclusions: Both the clinic-biochemical severity and histological changes had no correlation with the duration of alcohol intake in contrast to earlier studies which had demonstrated a definite correlation of alcoholic liver disease (ALD) with both the amount and duration of alcohol intake. Larger studies will be required to substantiate the findings of this study.

This study was aimed to correlate between clinical and histopathological spectral correlation of newly diagnosed multibacillary leprosy cases for a period of 1 year.

Methods: The study was conducted in the Department of Dermatology and Venereology, Government Medical College, Thiruvananthapuram, for a period of 1 year. All newly diagnosed multibacillary leprosy cases were included.

Results: A total of 40 patients with clinically diagnosed multi bacillary leprosy were studied. Maximum numbers of leprosy patients studied were in the age group of 40-49 years. Maximum clinical and histopathological correlation was found in 75% (9/12) in lepromatous leprosy and least correlation of 44.44% was noted in borderline lepromatous (4/9).

Conclusions: In this study maximal clinico-histopathological spectral correlation was found in lepromatous spectrum. The percentage of correlation was considerably less in all the other groups.

Abstract:
Objective: To investigate the correlation between clinico-pathological features and lymph node metastasis of papillary thyroid microcarcinoma (PTMC). Methods: The clinico-pathological data of 142 cases of papillary thyroid microcarcinoma were collected. The relationship between sex, age, tumor location, multi-focality and cervical lymph node metastasis were analyzed. Results: There was increased rate of lymph node metastasis in male patients with PTMC. There was no significant correlation between age, multifocality and lymph node metastasis. Conclusion: The histological subtypes of PTMC have different clinicopathological characteristics and are important factors for cervical lymph node metastasis. Further evaluation of its histological classification is helpful for clinical treatment strategy.

Abstract:
BACKGROUND: HER2 protein expression in gastric carcinoma, in correlation with existing, acknowledged prognostic factors which include the parameters that determine the TNM stage of the disease, could become the basis for ongoing research in the field of molecular targeted and personalised therapy.AIM: To determine the expression of the HER2 protein in gastric carcinoma and to correlate the expression of a HER2 protein with clinicopathological characteristics of the disease.MATERIAL AND METHODS: The data of HER2 protein expression and the parameters of the TNM classification were obtained from the histopathological reports of the Institute of Pathology in Skopje, and for the clinical stage we used patient’s files from the University Clinic for Abdominal Surgery in Skopje.RESULTS: The analysis of the correlation of HER2 protein expression and TNM classification parameters pointed out a significant correlation between HER2 protein expression and intragastric localisation of gastric carcinoma (P = 0.005), and the tumour grade of differentiation (P = 0.034). There was also a positive correlation between HER2 protein expression pattern and positive lymph nodes in patients with gastric carcinoma (P = 0.03). The expression pattern of HER2 +++ was significantly more common registered in patients with positive lymph nodes (P = 0.03)CONCLUSION: The expression of HER2 protein could represent a biological marker with prognostic and predictive value in patients with gastric carcinoma. Considering the high mortality rate in patients with gastric carcinoma and lack of international standardised therapeutic approach, research of the role and significance of HER2 overexpression and Trastuzumab therapy may prove useful in the development of new therapeutic strategies.

Abstract:
Background: The histopathological examination of representative skin biopsy specimens is widely regarded as the gold standard for the resolution of dermatological diagnostic dilemmas, especially with good clinico-pathological correlation, and the skin biopsy is the dermatologists’ most requested investigation. Studies on the clinico-pathological consistency in the diagnosis of skin disorders from our part of the world are very rare. Objective: This study aimed at determining the consistency between the provisional clinical diagnosis and the histopathologiscal diagnosis of skin biopsy specimens. Materials and Methods: This study was carried out over a six-month period using specimens from the 58 patients who consented to skin biopsy during this time, and comparison was made between the provisional clinical diagnosis and the histopathological diagnosis. Results: Of the 58 subjects that participated in the study, 29 were males and 29 were females. Their ages ranged between 7 and 68 years, with a mean age of 37.3 years. The spectrum of histopathologically diagnosed disorders included skin tumours and tumour-like conditions (12 cases, 20.7%), psoriasiform disorders (10 cases, 17.2%), lichenoid disorders (9 cases, 15.5%), infectious disorders (8 cases, 13.8%), disorders of epidermal maturation and keratinization (7 cases, 12.1%), spongiotic disorders (7 cases, 12.1%), vesiculo-bullous disorders (3 cases, 5.2%), and others (3.4%). Overall, the histopathological diagnosis was consistent in 19 cases (32.8%), corroborative in 12 cases (20.7%), with clinico-pathological correlation, and inconsistent in 27 cases (46.6%), leading to the issuance of a completely new diagnosis. Conclusion: The observed degree of clinico-pathological inconsistency in the evaluation and diagnosis of skin disorders in our environment implies that skin biopsy for histopathological examination is highly valuable if patients are to be issued the appropriate diagnoses for the avoidance of inappropriate treatments. The contribution of histopathology to the accuracy of dermatological diagnosis is significantly enhanced by clinico-pathological corroboration.

Background: The present study aimed to know the common pathologies presenting as neck swellings in our region and their clinical, radiological, and cytopathological correlation in order to understand the nature of the lesion and accuracy of the diagnostic tools in finding a definitive diagnosis.

Methods: Patients presenting with neck swelling to the ENT department of a tertiary care centre in Mangalore between June 2013 to July 2018 were included in the study based on retrospective and prospective sampling. Appropriate cytolopathological and radiological investigations were done and analyzed.

Results: Out of 160 cases analyzed histopathology diagnosed 86.9% as benign and 13.1% as malignant. Radiologically 94.9% were diagnosed to be benign and 5.6% malignant whereas in cytological investigation 89.4% were diagnosed as benign and 6.9% as malignant while the remaining 3.8% cases were cytologically inconclusive. Intra operatively 6.9% cases had features of malignancy and rest 93% were benign. Thyroid was the most common benign neck swelling (73.2%) followed by cervical lymphadenopathy (6.9%) and thyroglossal cyst (5.6%).

Conclusions: Discrepancy in clinico-radio-cytopathological correlation was highest in cases of thyroid swelling (more so in case of solitary thyroid nodule). None of the pre-operative investigations for neck swellings is 100% specific or definitive in diagnosing a benign from a malignant lesion in the neck.

Abstract:
Three hundred and sixty-four out of 547 children with primary nephrotic syndrome (PNS) treated at the Department of Child Health, Medical School, University of Indonesia between January 1970 and December 1979, were biopsied. The clinical, laboratory, and pathological characteristics of these patients were evaluated and compared with those reported by the International Study of Kidney Disease in Children (ISKOC, 1978) comprising children of Western countries with PNS. Results of this study showed that the spectrum of the histopathological changes in the 2 study populations was slightly different. Clinical and laboratory characteristics of certain types of PNS also showed some differences compared with those of ISKDC report. Patients with non-minimal changes nephrotic syndrome (NMCNS) were more frequently found in this study, and they were older than those of ISKDC report, i.e., more children < 6 years of age were affected with this disease in Jakarta. Similarly, this study showed that patients with minimal changes nephrotic syndrome (MCNS) were older than those of ISKDC report; the peak age of Western patients was less than 6 years. Patients with focal segmental glomerulosclerosis (FSGS) and membranoproliferative glomerulonephritis (MPGN) reported by ISKDC were more severe than those of this study as far as clinical and laboratory characteristics were concerned. The causes of these clinical and histopathological discrepancies between the two studies need to be further elucidated.
