Full textSeveral studies have investigated peri-implantitis complication, but no consensus to date has yet been established. Purpose: The study aims to clarify our understanding of peri-implantitis and to determine the prevalence and causal factors associated with its presence in a cohort of patients at Laval University’s periodontics clinic. Methods: A cross-sectional study was carried out on 86 patients and 258 implants placed between 1999 and 2014. Each patient’s data was completed using their medical and dental records, a pre-established questionnaire, and a clinical and radiological recall examination. Results: The prevalence of patients and implants with peri-implantitis was 15.11% and 6.44%, respectively. The statistically significant factors for peri-implantitis were found to be the dental condition and joint prostheses. Conclusion: Our results show a lower peri-implantitis prevalence than those found in the literature. This confirms that our protocols are well implemented and executed.

Full textThe goal of this thesis is to investigate genetic and environmental factors, both initiating and influencing signaling centers that regulate tooth development and thus producing associated defects. Essentially, my research program utilizes patient-based rare disease phenotypes to create novel mouse models. This study also involved investigating the developmental effects of excess retinoic acid on enamel formation to gain understanding of the mechanisms by which environmental factors can alter enamel development. Other studies investigated enamel and dental anomalies in Ltbp3 and Smoc2 mutant mice. These results advance our understanding of tooth development, and may translate towards optimizing clinical diagnosis, and improving treatment strategies for several human rare diseases. An improved understanding of rare disease models and our testing of clinically relevant approaches using rodent models is a feasible approach to address bone degeneration problems

Full textIntroduction: Since first Bränemark dental implants were implanted, operative surgery and materials for implantation have evolved. Titanium was the gold standard for dental implant material. But others materials like Zirconium were studied in replacement of Titanium due to allergy, bio-integration end aesthetic reasons. Contributing risk factors of Titanium implant failure include oral hygiene, tabacco and early loading among others factors. Objective: The aim of this clinical study was to evaluate survival rate of Zircon implants after 2 years, to collect information for tissue integration of Zirconium implant and to identify risk factors for Zircon implant failure. Material and Methods: ZIR-ROC study, conducted in the Oral Surgery Departement in Montpellier University Hospital, used zircon monobloc (ZIR-ROC®) implants (Paris Implant Society®). 43 implants were required for statistical analysis. Drilling and implantation were conducted with 2ingis® surgical guide system. The Periotest® value was the first endpoint for the implant osseointegration analysis. Gingival and dental plaque indices as well as intraoral X-ray analysis were part of the 22 secondary variables. Data were collected just after implantation surgery, and then, 1 week, 4 weeks, 5 months, 1 year and 2 years after implantation. Kaplan-Meier method, Cox regression model and principal component analysis were used for statistical analysis. Results: All implants showed postoperative optimal primary stability (Periotest® values between -8 and +1; mean: -5,7). Implants failure rate reached 31 percent after the 5 initial months of osseointegration. Clinical mobility was observed for the failed implants. Risk factors published in the scientific literature were observed in the Cox regression model with significant statistical effect for tobacco and alcohol consumption but without significant statistical effect for osseous density or even temporary crown insertion. Furthermore, gingival inflammation and dental plaque presence were low on Zircon implant. Discussion and Conclusion: Zirconium remains a biocompatible material for dental implantation but there were probably many reasons for the observed early implant failures and this study could not demonstrate the only involvement of Zirconium based material. Operative surgery used in this clinical study and the design of the implants were studied for researching other risk factors. In addition, an in vitro study comparing Zirconium versus Titanium as gold standard, was conducted for testing affinity of dental stems cells on Titanium and Zirconium-based dental materials

Full textThree different rettressive forms of bone lesions have been studied. An osteogenesis imperfecta was found in a family with three loops of consanguinity. A young patient and bis father bave been suffering fromatypical osteogenesis imperfecta : fractures in childhood and lack ofpathological manifestations after the age of 6. Using molecular studies. We were able to elirninate any mutations in collagen I chains. An autosomic recessive inheritance could be likely in the family studied. So. We studied some genes implicated in some resembling osteogenesis imperfecta syndromes observed either in humans or in animal models of this disease. We eliminated b’ molecular studies the possible implicationof 3 different genes in the two members of the farnily concerned. By homozygosity mapping, we were able to localize the gene responsible for this ne disease in a short region of the chromosome 11(11 q23. 3 to Il q24. I region) containing 48 different genes. The Caffev disease (infantile cortical hyperostosis) is characterized b\ appearance in young childen (before the age of 3 years) of painful swelling of sub cutaneous tissues with bone lesions. The recovery is spontaneous and can be faster b using indomethacin. An heterozygous mutation in COLlAI gene was recentlv found responsible ofthe post natal form ofthe disease. Indeed. Prenatal lethal forms ofthe Caffev disease have been described. But, in the two cases studied. No mutation ofCOLIAI gene bas been previously found. We have observed in our patients that the COLlAlgene mutation could be absent in a typical post natal form and. On contrar\. That the mutation of COL I Al could be observed in a prenatal lethal form. Our observations indicate that there is a genetic heterogeneity ofthe Caffey disaese with implication of an unknown gene. Bisphosphonates are used as inhibitors of osteoclastic function in the therapy of osteogenesis imperfecta. Because a bisphosphonates-induced teething failure bas been previously described in rats we were interested in the effects ofthese drugs on teething ofoung osteogensis imperfecta children. We have been ableto demonstrate that bisphosphonates induce a delay in teething (mean 1. 67 years). This effect disappears when. Bisphosphonate therapy is stopped

Full textThe curve of Spee is situated in a para-sagittal plane. His study is done only at the mandibular teeth. It is not a "simple" plan of dental occlusion because this curve interests, also of bone, the mandibular condyle and particularly its front edge. The curve of Spee depends on the condyle location but also on the axis of each of the mandibular teeth which participate in its shape. Its curve aligns the directions of the constraints along major dental axes. From metrics and angular values measured on 3D images of scanned mandibles, the objectives were: - To propose a help in the reconstruction of incomplete and / or deformed mandible and absent or incomplete occlusal plane. - To identify simple geometric principles to optimize the positioning of the artificial roots (implants) in modern Man. Full textEndodontic treatment is a delicate surgical procedure, which can be explained by a number of factors, notably: the complexity of apical anatomy, the difficulty in determining the proper working length, and the noxious effects that current filling materials may have on periapical tissues. The purpose of this research was to develop an injectable bone substitute (IBS) capable of inducing apical healing. Previously developed within the Equipe Mixte INSERM 99-03, IBS is a composite material made up of biphasic calcium phosphate (BCP) granules graded to adapt to the root canal (40 and 80 urn), and a cellulosic polymer gel, hydroxy-propyl-methyl-cellulose (HPMC). First, animal experimentation allowed IBS to be tested: the formation of an apical barrier, depending on the material injection level, thus provided evidence of its healing power. Subsequently, in order to improve the material injectability properties in a region difficult to access, a rheological study made it possible to investigate in an original way the physicochemical properties of the mineral phase, then the organic, and finally of the combination of both. Tests are conducted using a controlled-stress rheometer in two test modes: flow and dynamics. A predominantly "viscous" or "elastic" behaviour, depending on polymer concentration and the mineral filling ratio, allows the material injectability limits to be approached. All these results suggest the possibility of using IBS as an injectable bioactive biomaterial enabling the formation of an apical barrier, that is to say, aided biological sealing of the apex by cementogenesis

Full textFor the past 60 years, dental resin composites have been constantly evolving, becoming increasingly functional and aesthetic materials. However, obtaining the ideal dental restorative material, with optimum mechanical and physicochemical properties, is still a significant challenge today. The nature and proportion of the monomers, the type, size and content of fillers, the quality of the bonds between the fillers and the matrix and the presence of porosities inside materials all emerge as aspects that can influence these properties. To improve our understanding of the impact of these various factors, a number of studies have been undertaken using three commercially-available resin composites with different viscosities: one high-viscosity resin (Filtek P60 – 3M ESPE), one moderate-viscosity resin (Grandio – Voco) and one low-viscosity resin (Filtek Supreme XTE – 3M ESPE). The influence of clinical handling of materials on the rate and volume of porosities has been evaluated by 3D X-ray computed tomography. Handling increases the porosity percentage and reduces the average porosity volume. Furthermore, the more flowable the material is, the more porosities it has within it. Two groups of samples were prepared in order to evaluate the influence of porosity on the mechanical and physicochemical behavior of materials: one group of samples consisted of materials simply extruded from syringes, while additional porosities were added artificially in the other samples. The polymerization stress was determined using a tensile testing machine, the absorption/solubility was measured in accordance with ISO 4049, the true tensile mechanical behavior was assessed using the VidéoTractionTM system, and the apparent compressive and flexural behaviors were studied using a compression test machine and a 3-point flexural test machine, respectively. The various organic and mineral components appear to be key elements in the physicochemical and mechanical behavior of materials. The initial porosity is also a predominant factor in terms of explaining this behavior. However, its influence is not proportional to its rate, since the addition of artificial porosities does not significantly affect the results

Full textObjective : Assess the effect of informed consent (IC) on the level of state anxiety (STAI-S) using two modes of communication (oral and written). Methods : 151 adult patients were randomized. To explain treatment, 76 patients received the current version of IC (oral) while 75 patients received a detailed form of IC (written). Results : No significant difference in the level of state anxiety was demonstrated between the two groups. The overall administration of IC slightly decreases patient anxiety (p=0.006). Very dentally anxious patients have a significant improvement in their level of state anxiety after obtaining the IC (p=0.0004). The degree of dental anxiety is positively correlated with state anxiety (p<0,0001). Women (p=0,02) and a previous unpleasant experience at the dentist (p=0,001) are predictors of dental anxiety. Conclusion : Using a IC should be part of the context of current clinical practice because it promotes a uniform and standardized discussion.

Full textCareing of persons in situation of handicap is a priority of public health since a certain number of years. A collective awareness begun through the societies history showing an evolution of the concept of the handicaped person. Indeed, from the vision centred on the medical deficiency, the society preferred the intégrative vision insisting on the residual capacities of these persons in situation of handicap. This evolution is also reflected in the terminology and in the classifications which are tools of study, evaluation and research for the specific needs in this domain. This work tends to highlight the long work of ethical reflections on the subject which actually inspired numerous legal texts in such a degree, that it remains difficult to distinguish clearly the border between both particularly in health matter. We shall see through this study the difficulties of application of these texts. More specifically, in the field of the odontostomatology, who remains the poor relation of the global medical care of the person in situation of handicap. Thus progresses have to be realized as well on the medical plan that on the legal plan for the futur considering the variety of the situations of handicap which we can meet

Full textInfection control practices have been mainly concentrated in the dental operatory whereas the articles transferred between the dental clinic and the dental laboratory as well as laboratory instruments have received less attention. This study attempts to document the practices of dental care professional implicated in the fabrication of dental prosthesis as to the asepsis of these items as well as their perceptions towards it. In June 2008 an auto-administrated and anonymous questionnaire was sent to a random sample of dentists, denturologists and dental laboratory directors licensed to practice in the province of Quebec (Canada). From the 1,100 questionnaires sent, 376 were returned filled-in. Almost two thirds of responders (72.1%) claim to disinfect or sterilize laboratory instruments and 74.9% to disinfect transferred articles, with percentages varying according to the group of articles (impressions, prosthesis, etc.). However, only 9.1% regularly identify disinfected work. More than half of the responders (51.4%) think it is difficult to apply a form of asepsis on transferred articles and 62.4% believe there is a lack of information towards it. This study is the first to have been addressed simultaneously to the three groups of professionals and the first to question them on their perceptions. It demonstrated that the application of asepsis measures to transferred articles and laboratory instruments by dental care professionals is not always complying with existing recommendations. There is a need to reinforce their application, especially of asepsis measures on transferred articles.

Full textStatement of problem: Patients’ satisfaction and oral health-related quality of life (OHRQoL) are two commonly used patient-reported outcomes. Convincing evidence is lacking to demonstrate the relation between these two outcomes in completely edentate patients who receive prosthodontic treatments. In addition, it is still unclear which elements of denture satisfaction share the best association with OHRQoL. Objectives: To evaluate the level of association between patients’ denture satisfaction and OHRQoL in edentulous patients, and to identify the determinants of satisfaction that best predict OHRQoL. Methods: Data from 255 edentulous elders who participated in a randomized clinical trial were used to analyze the relationship between OHRQoL and denture satisfaction. OHRQoL was evaluated with the Oral Health Impact Profile (OHIP-20) questionnaire. The McGill Denture Satisfaction Instrument was used to assess patients’ satisfaction with treatment. Outcomes were measured prior to treatment and 6 and 12 months after delivery of new dentures. Simple and multiple linear regressions analyses were performed to statistically assess the relationship between the variables. Results: A highly positive association exists between oral health-related quality of life and denture satisfaction ratings. Chewing ability (P = .005) and oral condition (P = .002) are the aspects of satisfaction presenting the best association with OHRQoL. These two variables explained 46.4% (P = .0001) of the variance in OHIP’s change score following treatment. This association varied with time, but not with type of treatment or socio-demographic variables. Conclusions: Within the limitations of this study, a highly positive association exists between OHRQoL and denture satisfaction. Chewing ability and oral condition are the determinant of denture satisfaction best associated with OHRQoL, predicting 46.4% of its improvement following a treatment.

The aim of this study was to determine the magnitude of the effect of IODs and CDs on the OHRQoL and assess the stability of the treatment over time taking into account the level of SOC. METHODOLOGY: Edentulous participants (n = 172, mean age 71, SD = 4.5) wearing CDs or IODs were followed up for two years. The OHRQoL was assessed at baseline and both follow-ups using the « Oral Health Impact Profile (OHIP-20) » questionnaire. The SOC was only assessed at both follow-ups using « The Orientation to Life (SOC-13) » questionnaire. Statistical analyses were conducted to evaluate between and within treatment differences (descriptive, bivariate and multivariate statistical analyses). RESULTS: A significant pre-/post-treatment improvement in the OHRQoL for both treatment groups was found (Wilks’s Lambda = 0.473, F (1,151) = 157.31, p < 0.0001). The magnitude of the effect in the IOD group was 1.5 times larger than that in the CD group. These outcomes were stable over the two-year study period and the SOC did not influence any of these results. CONCLUSION: IODs provide better long-term OHRQoL than CDs regardless of SOC level. The results are clinically meaningful and confirm the superiority of IODs over CDs.
