Abstract:
Background:Individual surgeon and institutional performance are usually assessed by morbidity and mortality rates, which can be calculated using peri-operative metrics, such as POSSUM (Physiological and Operative Severity Score for the enUmeration of mortality and morbidity). Post-operative risk can be estimated using the surgical Apgar outcome score. However, pre-operative co-morbidity may contribute to case risk diversity and affect immediate peri-operative metrics and short- and long-term morbidity and mortality. We estimated the correlation between pre-operative co-morbidity or risk assessment indices and peri-operative metrics in urological patients. Material and Methods:The study included 100 consecutive patients (80.8% males, mean age ± SD 66.3 ± 10.7 years, range 30 - 88 years) undergoing major open urological procedures (39 nephrectomies, 43 radical prostatectomies, 18 radical cystectomies). Pre-operative co-morbidity was assessed using Charlson Comorbidity Index (CCI), age-adjusted CCI (AA-CCI), Cumulative Illness Rating Scale (CIRS), and Index of Co-Existent Diseases (ICED). Pre-operative risk was assessed with the American Society of Anesthesiologists index (ASA). Functional status was quantified based on estimation of the metabolic equivalent (MET). Peri-operative metrics included POSSUM and surgical Apgar scores. Results: All pre-operative indices significantly correlated with POSSUM, but none correlated with the surgical Apgar score. Conclusions:In patients undergoing major open urological procedures, risk stratification in the post-operative setting using the surgical Apgar score is independent of pre-operative co-morbidity status. In contrast, pre-operative co-morbidity and risk assessment correlated with peri-operative metrics used to calculate morbidity and mortality risk. Reports of death and complication rates do not take into account case diversity and, therefore, should be adjusted for co-morbidity status.

Am J Sports Med. 2014;42(11):2769–2776. Clinical Questions: In pediatric patients, does early operative treatment of an anterior cruciate ligament (ACL) injury result in decreased knee instability compared with delayed or nonoperative treatment? Data Sources: This review focused on the PubMed/MEDLINE and EMBASE databases. The following query searches were used: ACL or anterior cruciate ligament and young or child or children or pediatric or immature. Dates searched were not specified. A separate search was also conducted of abstracts published between 2009 and 2011 from the American Academy of Orthopaedic Surgeons; American Orthopaedic Society for Sports Medicine; International Society of Arthroscopy, Knee Surgery, and Orthopaedic Sports Medicine; European Society of Sports Traumatology, Knee Surgery, and Arthroscopy; American Orthopaedic Association; Arthroscopy Association of North America; Pediatric Orthopaedic Society of North America; and American Academy of Pediatrics conferences. Study Selection: Available studies were included only if they were written in English; were of level 1, 2, or 3 evidence (grading taxonomy not stated); were cohort designs that compared nonoperative and operative treatments; involved an early versus delayed ACL reconstruction that could be prospective or retrospective; and reported primary outcome interest measures. Animal studies, basic science studies, case series, reviews, commentaries, and editorials were excluded from the review. Data Extraction: A systematic assessment tool, Guide to Community Preventive Services: Systematic Reviews and Evidence-Based Recommendations, was used by 2 of the authors to independently grade the quality of each study that met the inclusion criteria. The tool focused on 6 areas: intervention and study description, sampling, measurement, analysis, interpretation of results, and other execution factors. This tool helped to ensure consistency, reduce bias, and improve the validity and reliability of preventive health care studies. Eleven studies met the inclusion criteria. Six studies compared nonoperative with operative treatment, and 5 studies compared early reconstruction (open physes) with delayed reconstruction (closed physes). Studies in this meta-analysis consisted of the following: four level-3 prospective studies, four level-3 retrospective studies, one level-2 retrospective study, one level-3 case-control study, and one level-3 study with both prospective and retrospective data collection. All of the studies included data related to patient demographics, treatment interventions, follow-up duration, presence of any meniscal symptoms, time to return to sport participation, patient-reported outcomes (International Knee Documentation Committee [IKDC], Lysholm, or Tegner scores), the need for a second surgical procedure, and any posttreatment problems. Main Results: Of those who chose the nonoperative route, 75% reported instability, whereas only 13.6% of those who had surgery reported instability. These data also showed that nonoperative or delayed-operative patients were 33.7 times more likely to report instability than the early operative group. Those who chose the nonoperative route had a 12 times greater risk (odds ratio = 12.2, 95% confidence interval = 1.55, 96.3) of developing a meniscal tear after the initial injury. Three studies included in the meta-analysis reported return to sport status, but only 2 studies provided adequate data for both operative and nonoperative patients. In 1 study, 92% of operative patients were able to return to sport, but only 43.75% of nonoperative patients were able to do so. The second study reported that all operative and nonoperative patients were able to return to the same level of sport after injury. Of those in the early operative group, 6% required a repeat surgical intervention for either an ACL rerupture or a meniscal tear, and 19% of those who initially chose nonoperative treatment eventually needed surgery to repair the ACL or meniscus. Findings favor the early operative group over the delayed operative and nonoperative groups based on IKDC scores. One study reported a significant difference in operative patients, with an IKDC mean score of 95 compared with 87 in the nonoperative group. Similarly, a different study reported a mean score of 94.6 in the early operative group compared with 82.4 in the delayed operative group and was stated to have met the minimal clinically important difference (MCID). The MCID was not met for the Lysholm and Tegner scores between operative and nonoperative patients. Conclusions: The results of this meta-analysis favor early operative treatment for pediatric patients with ACL tears over delayed or nonoperative treatment. Early operative treatment is initiated shortly after the injury, while the patient is still skeletally immature and the growth plates are open. Current evidence suggests that early ACL reconstruction will result in less knee instability and a more likely return to the preinjury activity level without affecting the growth plates or causing growth disturbances.

Full text
Abstract:
This thesis uses Hong Kong’s empirical data to refute “Olson’s conjecture,” which stated that it is harder for a smaller group than a larger group to disintegrate. It investigates the rise and fall of the civil servants’ co-operative building society, which was a special form of housing arrangement for civil servants in Hong Kong that began in the 1950s. The focus is on the voluntary dissolution of civil servants’ co-operative building societies. Members of a society have a choice to maintain the status quo or opt for dissolution to potentially increase the value of their properties. Olson’s group theory (Olson 1965) and neo-classical economics factors were adopted to evaluate the decision time (i.e., from 1985 until the dissolution date) of the co-operative building society. Group size, land value, the real option value of redevelopment, and the overall housing market sentiment were significant factors that affected the timing of the dissolution of each co-operative building society.

Anbrine, Shama. "The Co-operative Model Town Society : history, planning, architecture and social character of an indigenous garden suburb in colonial Lahore." Thesis, University of Liverpool, 2014. http://livrepository.liverpool.ac.uk/2010879/.

Full text
Abstract:
This thesis investigates the Co-operative Model Town Society Lahore; a town covering an area of around 2000 acres developed in the 1920’s in the (then) suburbs of Lahore, capital of Punjab province in British India. The Town is a remarkable interpretation of Ebenezer Howard’s Garden City ideals and the co-operative principles. However, the real significance of the town is it being a unique example of a co-operative garden town built by the local Indian bourgeois, majority of whom were forced to migrate due to the disturbances that followed after the declaration of independence to India and Pakistan in 1947. Despite the admiration and significance of the Town in the realm of pre and post-Independence Lahore, very little has been documented about it. Its formal documented history is non-existent while its original built environment, an excellent example to illustrate late-colonial architecture in the region, is diminishing rapidly due to negligence and reconstructions. The aim of this research is the documentation of history, urban form, social character and architecture of the pre-independence Model Town. Consequently, through an analysis of its built environment this study intends to develop an insight into the colonizer-colonized cultural transfers, in particular, to the transformation of British town planning ideas in the colonies due to their interpretation by the local Indians under the influence of prevalent religious, cultural and social practices. The research was conducted by the process of historical construction, whereby evidences from the documents and the built environment have been used collectively to develop the historiography of the town. The selection of primary material has been based on its availability as the documentary evidences are scattered across Pakistan, India and the UK. The built environment has been documented using the official drawings as well as onsite surveys and measurements where the original drawings are unavailable. The Town has been analysed in a comparative setting with respect to contemporary urban, architectural and social trends and practices prevalent locally ( both by the colonial rulers and the colonized natives) as well as global movements, especially Western ideologies and perspectives and their retrospect local adaptations. The documentation and analysis were used as a ground for four interpretative conclusions. The first conclusion elaborates on the need of urban historiography in Pakistan. The second conclusion interprets the Model Town in the realm of twentieth century urban planning in the region. The third conclusion elaborates on the colonial architectural styles. The fourth conclusion gives an insight into the Model Town with reference to western styled ‘Indian’ nationalism.

Abstract:

Background:  The  co-operative  building  society  is  getting  more  attractive  on  the  market;  its purpose  is  to provide  its members a  living worth  its price. The monthly  fees  from  the building society contributes  to cover all  the expenses for  the co-operative building society, which means that  a members  living  expenses depends on how  these  costs  are handled. This  thesis  examines how the co-operative building is managed to fulfill its member’s economic interest.

Purpose:  The  purpose  of  this  thesis  is  to  examine  how  the  members’  economic  interest  is fulfilled in a co-operative building society, with focus on the revenues and costs which influence the fee to the members.

Methodology: The thesis is written with a qualitative approach, existing theories have been used to  examine  the  collected  empirical  material.  The  empirical  material  is  collected  from  a  co-operative building society, where internal documents and financial reports have been used.

Results:  The  result  of  the  thesis  is  that  the members’  economic  interest  in  the  examined  co-operative  building  society  is  not  as  well  fulfilled  as  in  other  compared  co-operative  building societies.  Another  result  is  that  there  is  generally  no  connection  between  the  price  for management and the operating costs.

Abstract:
The present work presents a research that had for objective to analyze the motivations and justifications of Serra Gaucho's fruit growing to associate to organizations. They were used as foundation, two theoretical bases: the beginnings, objectives and characteristics of the cooperative organizations and the paradigm paraeconômico according to Guerreiro Ramos propositions. The field research was accomplished in qualitative character, through contact with associates, assets and no assets and or no associates, with interest and without interest in associating to agricultural cooperative organizations, that work with storage, classification and commercialization of fruits in natura. The contact with those Area 4’ fruit growing of OCERGS Organization of the Cooperatives of the State of Rio Grande do Sul, denominated Serra was accomplished through the technique ball-of-snow and they were constituted then in the public-objective of this research. The data collection instruments were constituted of semi-structured interviews, document retrieval and direct observation. The method of content analysis was adopted for analysis of the data. The data analysis happened taking into account the external aspect of the social system in agreement with the dimensions proposed in the law of the appropriate requirements and, for a group of categories of values, that configure the internal aspect of the social system. At the end the reasons that take the fruit growing to associate or abandon the cooperative organizations in this area were exposed, identifying what rationality type guides their decisions.

Abstract:
The need for a robust model for predicting the value of condominiums and houses are becoming more apparent as further evidence of systematic errors in existing models are presented. Traditional valuation methods fail to produce good predictions of condominium sales prices and systematic patterns in the errors linked to for example the repeat sales methodology and the hedonic pricing model have been pointed out by papers referenced in this thesis. This inability can lead to monetary problems for individuals and in worst-case economic crises for whole societies. In this master thesis paper we present how a predictive model constructed from a multilayer perceptron can predict the price of a condominium in the centre of Stockholm using objective data from sources publicly available. The value produced by the model is enriched with a predictive interval using the Inductive Conformal Prediction algorithm to give a clear view of the quality of the prediction. In addition, the Multilayer Perceptron is compared with the commonly used Support Vector Regression algorithm to underline the hallmark of neural networks handling of a broad spectrum of features. The features used to construct the Multilayer Perceptron model are gathered from multiple “Open Data” sources and includes data as: 5,990 apartment sales prices from 2011- 2013, interest rates for condominium loans from two major banks, national election results from 2010, geographic information and nineteen local features. Several well-known techniques of improving performance of Multilayer Perceptrons are applied and evaluated. A Genetic Algorithm is deployed to facilitate the process of determine appropriate parameters used by the backpropagation algorithm. Finally, we conclude that the model created as a Multilayer Perceptron using backpropagation can produce good predictions and outperforms the results from the Support Vector Regression models and the studies in the referenced papers.

Abstract:
This thesis contributes to closing a gap in the field of science, technology and innovation (STI) policy research: despite many theoretical advances in the field, we still do not know why some urgent societal issues (or ‘challenges') remain unaddressed, notwithstanding the technological advances that could potentially address them. In particular, radical technological innovations – innovations that depart from the established technological trajectory – would offer greatest potential to address societal challenges. While the source of radical innovations is often new entrepreneurial firms, established firms (‘incumbents') are likely to play an important role in developing them because of the vast resources and complementary assets they possess. Incumbents however, face few immediate incentives to develop radical innovations in response to societal challenges. The analytical puzzle of this thesis is thus to explain how, when, and why industries change (or not) their strategies (in particular, their technological strategy) in order to address a societal problem. This puzzle is disentangled into interrelated research questions: A) How do societal issue‐related pressures (on the incumbent industry) from different domains (namely, civil society, science, political arena, economy) evolve? B) How does the incumbent industry respond to changing pressures around societal issues, in terms of technological, political, cultural and economic strategies? C) In particular, when and why do industry actors decide to develop substantive technological responses? To answer these questions, the thesis develops a new analytical perspective that combines insights from (a) issue life‐cycle and issue attention cycle theories (from the Business & Society field) with (b) the so‐called ‘Triple Embeddedness Framework' and (c) concepts from business strategies, innovation management, corporate political strategies, and technology policy. This novel perspective represents an ideal‐typical model of issue evolution (‘issue life ‐cycle'). The model, which I call the Dialectic Issue Life‐Cycle (DILC) model, is applied to three case studies of the American automobile industry's responses to various societal problems (local air pollution, auto and highway safety, and climate change). Combining qualitative and quantitative research methods in an original way, the case studies aim not only to investigate the validity of the framework, which also provides conceptual answers to the research questions, but also to further refine it and nuance the conceptual answers. By explaining how incumbent industry actors respond to societal challenges, this thesis ultimately contributes to the practical policy debate of how incumbents can be stimulated to develop radical innovations that help address societal challenges.

Full text
Abstract:
In the decade after the Civil Rights Act of 1964, African Americans made historic gains in accessing employment opportunities in racially integrated workplaces in U.S. business firms and government agencies. In the previous working papers in this series, we have shown that in the 1960s and 1970s, Blacks without college degrees were gaining access to the American middle class by moving into well-paid unionized jobs in capital-intensive mass production industries. At that time, major U.S. companies paid these blue-collar workers middle-class wages, offered stable employment, and provided employees with health and retirement benefits. Of particular importance to Blacks was the opening up to them of unionized semiskilled operative and skilled craft jobs, for which in a number of industries, and particularly those in the automobile and electronic manufacturing sectors, there was strong demand. In addition, by the end of the 1970s, buoyed by affirmative action and the growth of public-service employment, Blacks were experiencing upward mobility through employment in government agencies at local, state, and federal levels as well as in civil-society organizations, largely funded by government, to operate social and community development programs aimed at urban areas where Blacks lived. By the end of the 1970s, there was an emergent blue-collar Black middle class in the United States. Most of these workers had no more than high-school educations but had sufficient earnings and benefits to provide their families with economic security, including realistic expectations that their children would have the opportunity to move up the economic ladder to join the ranks of the college-educated white-collar middle class. That is what had happened for whites in the post-World War II decades, and given the momentum provided by the dominant position of the United States in global manufacturing and the nation’s equal employment opportunity legislation, there was every reason to believe that Blacks would experience intergenerational upward mobility along a similar education-and-employment career path. That did not happen. Overall, the 1980s and 1990s were decades of economic growth in the United States. For the emerging blue-collar Black middle class, however, the experience was of job loss, economic insecurity, and downward mobility. As the twentieth century ended and the twenty-first century began, moreover, it became apparent that this downward spiral was not confined to Blacks. Whites with only high-school educations also saw their blue-collar employment opportunities disappear, accompanied by lower wages, fewer benefits, and less security for those who continued to find employment in these jobs. The distress experienced by white Americans with the decline of the blue-collar middle class follows the downward trajectory that has adversely affected the socioeconomic positions of the much more vulnerable blue-collar Black middle class from the early 1980s. In this paper, we document when, how, and why the unmaking of the blue-collar Black middle class occurred and intergenerational upward mobility of Blacks to the college-educated middle class was stifled. We focus on blue-collar layoffs and manufacturing-plant closings in an important sector for Black employment, the automobile industry from the early 1980s. We then document the adverse impact on Blacks that has occurred in government-sector employment in a financialized economy in which the dominant ideology is that concentration of income among the richest households promotes productive investment, with government spending only impeding that objective. Reduction of taxes primarily on the wealthy and the corporate sector, the ascendancy of political and economic beliefs that celebrate the efficiency and dynamism of “free market” business enterprise, and the denigration of the idea that government can solve social problems all combined to shrink government budgets, diminish regulatory enforcement, and scuttle initiatives that previously provided greater opportunity for African Americans in the government and civil-society sectors.
