Full text-- The present investigation work has been carried out keeping in mind the Regulation of Grades and Holding of the School of Pre Grade of the Ability of Medicine of the National University bigger than San Marcos, with the purpose of obtaining Medical Surgeon's title. Objective: The investigation was guided The investigation was conducted to identify complicaciones laparoscopic cholecystectomy in patients who were treated in the Surgery Department 6-II in the Hospital National Archbishop Loayza in the period understood between January 01 of the 2007 to December 31 of the 2009. Sample: The selected sample was 289 patients. Instruments: Were the used instruments conformed by a record of gathering of data meetly elaborated for the study ends, was the association of the variables analyzed through the test of square Chi, was the association force with OR, considered also: 0,05. Type of Study: Observational. Investigation design: Descriptive traverse. Result: We found that 78.5% of patients were female and the average age of the total was 46.2 ± 14.6 years. Complications occurred in 8.3% of cases, being the most frequent gallbladder bed bleeding (6.55%) and vascular injury (1.05%). Acute cholecystitis was associated significantly (p less than 0.001) with gallbladder bed bleeding (OR: 8.5, CI 2.2-24.9) Conclusions: Complications of laparoscopic cholecystectomy in the Surgery Departament 6-II in the Hospital National Archbishop Loayza were of 8.3% which is below the international figures from different series. The study concludes that there is a statistically significant association between acute cholecystitis and gallbladder bed bleeding

Full text-- Introduction: The technological advances regarding surgery so far have allowed us to make non-invasive surgery a reality. In light of this, it is of great significance that the golden standard of this technological achievement has been accomplished in the fields of gall bladder and biliary ductule surgery. Nevertheless, despite all the advantages showcased by laparoscopic surgery, there are times when complications arise, during and after the procedure. Due to this fact, our main goal was to describe the most frequent complications on laparoscopic cholecystectomy seen at Hospital Nacional Sergio E. Bernales during the January 2009 to August 2010 period. Methods: Types of research: non-experimental, observational, descriptive, transversal, and retrospective. The reasearch design applied is transversal descriptive. The total number of studied patients was 2200. The target population comprised 356 cases that were operated laparoscopically within the period of the study and met the criteria of inclusion, some of whose variables include age, gender, pre-surgery US findings on the vesicular wall, presurgery US findings on the diameter of the bile duct, surgical technique, duration of surgery, and change to open surgery. Laparoscopic cholecystectomy complications were considered an independent variable. A data sheet was used to collect information, and SPSS statistical analysis software for Windows, ver. 19.0 was used for descriptive and inferential statistical analysis. Results: The average age was 36,03 years old with a standard deviation of 5,7 years. The lowest age was 19 and the greatest was 62. The greatest number of complications appeared in the female group, with a number of 288 patients (80,9 % of the total number of patients). 306 patients (86% of the total number) --59 male and 247 female-- were operated using closed technique (with a Veress needle). The surgery time ranged from 1 to 2 hours in 229 cases (64,3%). As regards complications, 120 patients were reported to have infected wounds (33,7%); 78 patients reported bleeding (21,9%); biliary ductule injury was reported in 55 patients (15,4%); there was subcutaneus emphysema in 49 patients (13,8%), postsurgery jaundrice in 30 patients (8,4%), and hollow viscus in 24 patients (6,7%). Conclusion: In this study, the predominant group is the female population with 288 patients. The age group most affected by laparoscopic complications was between 20 and 40 years of age. The closed surgery technique with a Veress needle was applied the in the most cases (306; 86%). The average laparoscopic cholecystectomy time was between 1 and 2 hours (229; 64,3%). Only 65 patients required a shift in surgical technique (18,3%). The earliest complication reported was bleeding, and the complication that appeared the latest was wound infection, the latter being the more frequent. -- Key words: Complications, laparoscopic cholecystectomy

Full textEl objetivo del presente trabajo fue determinar las complicaciones intraoperatorias post extubación en la anestesia general balanceada con remifentanilo comparada con fentanilo para colecistectomías laparoscópicas. Se diseñó un estudio de doble cohorte. Fueron 63 pacientes post-colecistectomizados con anestesia general balanceada con fentanilo y 48 pacientes con anestesia general balanceada con remifentanilo. Toda la información fue analizada usando el programa estadístico SPSS 15. La estadística descriptiva expresó a los datos cuantitativos en media ± desviación estándar porque la prueba de Shapiro Wills indicó la normalidad de los datos. Los datos cualitativos se mencionaron como frecuencias y porcentajes. Las variables cuantitativas se analizaron con la T grado de libertad y t. Las variables cualitativas se analizaron con la Chi cuadrada. El riesgo relativo se obtuvo de Tablas de contingencia en las variables dicotómicas. Se llegó a la conclusión de que existe mayor probabilidad de presentar complicaciones con la anestesia general balanceada con fentanilo que con la anestesia general balanceada con remifentanilo. Las complicaciones más frecuentes en el primer grupo fue la tos y laringoespasmo en cambio en el segundo grupo fue la hipertensión.
