Full textCyclophosphamide is an alkylating agent widely used for the treatment of malignant dis eases. It also has been used to treat systemic vasculitides. This immunosuppressive therapy, given in association with corticosteroids, has markedly improved the prognosis of these diseases. Although the pharmacokinetics of cyclophosphamide in cancer patients have been well documented, no information has been reported on cyclophosphamide pharmacokinetics in patients with systemic vasculitis and connectivitis. The aim of this study was to evaluate the concentrations and pharmacokinetic parameters of cyclophosphamide and its key circulating intermediate metabolite, 4-hydroxycyclophosphamide/aldophosphamide, in these patients and to define a relationships between pharmacokinetic parameters and toxic effects. The results are consistent with those obtained for cancer patients, in spite of a wide inter-patient variability of cyclophosphamide and 4-hydroxycyclophosphamide/aldophosphamide concentrations in our study [. . . )

Full textIn recent years, a number of studies have reported associations between periodontal diseases and systemic conditions such as diabetes, cardiovascular diseases and adverse pregnancy outcomes. It appears that no published study has examined the knowledge and practice behaviors of canadian physicians about periodontal diseases. The aim of the study was to evaluate periodontal knowledge and practice behaviors of internal medicine trainees in Canada. A 15 question electronic survey was sent to seventeen canadian internal medicine programs. Six program directors accepted to participate to the study and a total of 126 students completed the survey (response rate for the students = 24 %). Knowledge about periodontal diseases of the internal medicine students was limited (64,9 % of good answers). Students generally under-estimated the prevalence of periodontal diseases and most of them were not aware of the systemic conditions associated with periodontal diseases. Furthermore, their practice behaviors did not follow the recommendations published in the literature and 71,4 % reported having not received training about periodontal diseases in medical school. No link was observed between the knowledge results and the number of years completed in internal medicine and 84,9 % were interested in getting more training about periodontal diseases and their associations with general health. According to the results, education in periodontal health is needed in canadian medical programs in order to improve internal medicine physician’s knowledge, promote best practices and improve interprofessional collaboration.

Full textThe metalloproteinases (MMPs) play a key role in the migration of haematopoietic stem cells. The production of these enzymes is regulated by different components of the extracellular matrix (ECM). In this study, we have shown that in normal CD34+ cells hyaluronic acid (HA) as well as SDF-1, induces MMPs secretion. This stimulatory effect is associated with an increase of cytosolic calcium. In contrast to HA induced-effect, SDF-1-induced MMPs secretion appears to be dose-dependent. We have also shown that CD34+ cells express syndecan-4, a selective membrane receptor of SDF-1. Besides, we have studied the secretory activity of pre-B acute lymphoblastic (ALL) cells derived from bone marrow (BM) or peripheral blood (PB) of leukaemic children. The MMPs secretion has been evaluated at basal and stimulated (by SDF-1, GM-CSF, b-FGF and VEGF) levels. A large heterogeneity has been observed among the studied patients. VEGF and b-FGF are found to be able to provoke predominantly MMP-2 secretion while GM-CSF mainly induces MMP-9 production, more particularly in SP cells. The molecular mechanisms involved in these responses have been investigated by using specific inhibitors of NFB, MEK1 and PI3K pathways. The expression of MMP-9 has appeared to be mediated through MEK1/PI3K pathways. It has been shown that all stimulatory effects observed in the present work are calcium-dependent. In addition, b-FGF favours the leukaemia proliferation by strengthening cell survival. Thereafter, we have investigated whether the pre-ALL cells themselves could produce through an autocrine loop these stimulating cytokines. The presence of mRNA of VEGF in pre-B ALL cells has been detected in most of studied patients, while the presence of mRNA of VEGF-R1 has been observed in all patients. Altogether, the present findings strongly suggest that different ECM components studied in this work (HA, SDF-1, GM-CSF, b-FGF and VEGF) would play a crucial role in the migration of leukaemic and normal haematopoietic stem cells by regulating the MMPs secretion. These data constitute a basis to consider new strategies of therapies

Full textAccording to classical conception of the anatomo-functional organization of language, there are two main cortical areas: a frontal area (Broca) and a temporal one (Wernicke) respectively involved in language production and comprehension. Functional magnetic resonance Imaging (fMRI) reveals cortical areas of activation and diffusion tensor imaging-based tractography (DTI) makes feasible the visualization of white-matter tracts in the human brain. On the basis of these techniques, a new conception of language cortical and sub-cortical organization arose, supporting the hypothesis that language processing network is dissociated in an dorso-ventral way. Dorsal areas of the brain being devoted to phonologic processing and its ventral areas to semantic processing of speech. Different techniques such as fMRI, DTI, intraoperative cortical and subcortical mapping made possible to describe two pathways involved in the language network: a dorsal stream and a ventral stream respectively involved in phonologic and semantic processing. As the dorsal route is composed of a unique pathway, the superior longitudinal fasciculus, the ventral stream appears to be composed of two different pathways. First a direct pathway, the inferior occipitofrontal fasciculus, connecting the posterior temporal areas to the orbitofrontal region. The second one, an indirect pathway, the inferior longitudinal fasciculus links the posterior occipitotemporal to the temporal pole, then relayed by the uncinate fasciculus connecting the temporal pole to the basifrontal areas. According to these observations the aim of our work is In the present work, we aim (1) to confirm that the MdLF is constantly found in control subjects and that it can be delineated from the other fiber tracts that constitute language pathways, (2) to characterize the reorganization of language network's functional connectivity follmowing surgical removal of left hemisphere low grade gliomas. In this study, we confirmed that the MdLF is constantly found in healthy volunteers and we clearly delineate the MdLF from the other fascicles that constitute language pathways, especially the ventral pathway. Considering language plasticity, our findings suggest that in the postoperative period, brain plasticity occurs with an ipsilateral recrutment and increased fonctional connectivity in the left hemisphere. Finally, we report a collaborative work observing that intraoperative electrostimulations of the white matter underlying the left posterior cingulate, while performing a naming task, systematically induced an unresponsive state for few seconds in relationship with a dream-like state. This result provides direct evidence that connectivity underlying the posterior node of the default mode network permits maintained consciousness of the external world
