Abstract:
The nucleosome is the fundamental unit of chromatin and its highresolution Xray crystal struc ture was solved in 19971. This remarkable achievement showed the elegant and highly ordered packing of histones within 1.75 turns of DNA and it hinted at ways in which histones may be involved in regulating higherorder chromatin structure. Perhaps most striking is the way in which histone Nterminal tails protrude from their own nucleosome and, in some cases, make contact with adjacent nucleosomes. Simple extrapolation from this observation has implicated these interactions in higherorder chromatin regulation and many considered it likely that PTM (posttranslational modification) of the tails would affect internucleosomal stability and hence DNA processes such as transcription. Histone tails actually support multiple PTMs, including acetylation (see the article by Colyn CraneRobinson in this issue), phosphorylation and methylation. In this brief overview, I will consider histone methylation and discuss what we understand concerning its role.

The urgent establishment of an effectively protected area in the Cordillera de Colán is strongly recommended in order to help secure the future for this poorly known primate and other mammal and bird species of conservation importance.

Background: Endoscopic polypectomy is an accepted procedure for prevention of colorectal cancer. Yet, large and difficult polyps present a technical and medical challenge to correct treatment. Objective: The aim of this study was to report and analyze the experience in the treatment of large colon polyps. Material and methods: We analyzed the endoscopic resection of large colon polyps performed between January 2006 and March 2018. Results: Of 3397 lower gastrointestinal endoscopies, 22 patients underwent resection of 25 large polyps (0.64%). There were no complications or deaths in the series. Conclusions: Endoscopic resection of large colon polyps is feasible and safe in lesions > 20 mm in diameter. The procedure serves both as biopsy or definite treatment and is an additional tool for the surgeon who has the capability of dealing with the complications or completing with surgery.

Background: In patients with colorectal cancer with synchronous liver metastases (CLM), complete resection of the primary tumor with the metastases is the only option for curative treatment. Several case series have been reported but no randomized controlled trials have been published. Objective: The aim was to evaluate if the simultaneous laparoscopic resection of the primary colon tumor and liver metastases is feasible and safe. Material and methods: A retrospective study was conducted with patients with suspected CLM sche- duled for simultaneous laparoscopic resection of the primary tumor of the colon and liver metastases. The preoperative and operative variables, short- and long-term outcomes and pathological variables were analyzed. Results: A total of 89 laparoscopic liver resections (LLR) were performed during the study period. In 28 patients, LLR was simultaneous with other procedures, 21 of which corresponded to laparoscopic colon resection. Mean surgical time for both procedures was 407 minutes. Mean hospital length of stay was 8 days. None of the patients died and overall morbidity rate was 71% with only one major complication. Overall survival and relapse-free survival at three years was 55.2% and 16.3%, respectively. Conclusion: This is the first publication analyzing this approach in our country. In well selected cases, the simultaneous laparoscopic approach is feasible, with low morbidity and mortality and acceptable oncological results.

Objetivos: Determinar la histología vegetal y la influencia del extracto etanólico de la planta entera de Bidens pilosa L. sobre el cáncer de colón inducido en ratas. Diseño: Estudio experimental. Lugar: Facultades de Medicina, de Farmacia y Bioquímica y de Ciencias Biológicas, Universidad Nacional Mayor de San Marcos, Lima, Perú. Material biológico: Ratas Holzmann. Intervenciones: Se agrupó 48 ratas Holtzmann, de 2 meses de edad, con pesos 100 g a 130 g, en seis grupos de ocho cada uno, y se indujo cáncer de colon con 1,2-dimetilhidrazina. Los grupos estuvieron constituidos por control normal, grupo con patología y grupos con patología y tratamientos. Principales medidas de resultados: Nivel de óxido nítrico, estrés oxidativo y cambios en el patrón celular del colon. Resultados: Se encontró incremento de los niveles de óxido nítrico y lipoperoxidación en los animales con 1,2-dimetilhidracina (DMH) y disminución en los que recibieron el tóxico más extracto de la planta. Al estudio histopatológico, con la DMH se evidenció desorganización celular, adenocarcinoma indiferenciado e invasivo; en tanto que, con los tratamientos se observó citoprotección, no dependiente de la dosis, siendo mayor a 50 mg/kg. Los hallazgos probablemente se expliquen porque los flavonoides y los compuestos fenólicos presentes en el extracto cumplen un rol importante en la inhibición del estrés oxidativo y también como anticancerígenos, inhibiendo el crecimiento de tumores. Se encontró detalles histológicos que servirían como caracteres diagnósticos, ayudando a verificar la identidad de la especie vegetal en polvo. Conclusiones: En condiciones experimentales, el extracto etanólico de la planta entera de Bidens pilosa L. presentó efecto quimioprotector sobre el cáncer de colon.

Resumen Alrededor del 13-20% de los pacientes con tumores del estroma gastrointestinal (GIST) se han descrito un tumor sincrónico o metacrónico, afectando la mayoría de veces al tracto gastrointestinal. Describimos el caso de un hombre de 81 años derivado a nuestra consulta con clínica de rectorragia y proctalgia, diagnosticado mediante colonoscopia y biopsia informado de adenocarcinoma sigmoide y GIST rectal. El tratamiento que llevamos a cabo combinó resección anterior baja junto a resección de la lesión rectal por vía endoanal. La importancia de este caso reside en la infrecuencia de los GIST de localización rectal (7-11%) cuya asociación sincrónica a adenocarcinoma colorrectal, los hace aún más inusuales.

Resumen Caso clínico: Mujer de 74 años que consulta a Urgencias por absceso glúteo derecho recidivante y que tras hacerle body-TAC, por el hallazgo incidental preoperatorio de nódulos pulmonares bilaterales, se diagnóstica de neoplasia de ciego perforada de manera contenida en forma de colección retroperitoneal que fistuliza a tejido cutáneo. Discusión: El cáncer de colo-rectal es el de mayor incidencia en España. Los signos y síntomas de presentación más comunes incluyen rectorragia, dolor abdominal, anemia inexplicable y / o un cambio en el hábito intestinal. Una parte importante de los pacientes puede presentar características clínicas sugestivas de enfermedad metastásica debido a diseminación linfática y hematógena y en raras ocasiones se puede manifestar como absceso en regiones adyacentes por perforación del propio tumor. Conclusiones: El carcinoma de colon perforado siempre debe considerarse como un diagnóstico diferencial para los abscesos de los miembros inferiores recidivantes y con mal control con manejo habitual.

Abstract:
Acute as well as chronic and recurrent colic episodes were caused by right dorsal colon (RDC) impaction in 7.98% of all colic patients. Significantly higher statistical evidence was figured out in hucul horses, mares and 7-year-old animals. Diagnosis of RDC impaction was established on the basis of rectal palpation (68.75%) or laparotomy (31.25%). Yield of rectal palpation was limited by a difficult accessability of RDC, small body frame of the animal, severe abdominal pain and by further physiological or pathological changes in abdominal cavity. Simple RDC impaction was diagnosed in 16 (50%) of 32 cases, in 9 horses the impaction associated with the large colon displacement or torsion. In 4 horses also small colon impaction together with RDC impaction and in 3 horses impaction of another GIT part were found. Clinical signs of disease were influenced by these pathological findings. In two cases the diagnosis sand impaction was established during laparotomy. RDC impaction was treated conservatively in some patients (28.13%), however in most patients surgery was needed (65.63%). Conservative therapy was successful in the horses with short duration of disease and in cases, where the RDC impaction was the only pathological finding without associated complications. As indicators for surgery were chronic course of disease, high degree of general health alteration, severe abdominal pain with no reaction to analgesics, cases where the strangulation could not be excluded by rectal palpation or another lesions that cannot be treated conservatively and/or unsuccessful conservative therapy. The surgery was used in 21 cases (65.63%). 17 patients recovered from anestesia, 1 horse was destroyed in 14 days due to the repeated impaction. Complications, that were associated with surgery in the group of patients with RDC impaction, occur also during surgical treatment in all type of colic patients. In 2 mares the abortion occurred, in 3 horses the convalescence time have been complicated by diarrhoea, and in 1 horse by acute laminitis. Surgical wound infection was a cause of ventral hernia in 1 horse. 25 horses from all 32 cases hospitalised and treated either conservatively or surgically were passed to home care (78.12%). On the basic of obtained results there is possible to evaluate the prognosis in patients with acute and simple impaction as favourable and in patients in which the disease was complicated by further pathology as obscured. Chronic recurrent impaction was diagnosed repeatedly (3×) in one horse, that did not respond to the conservative treatment and surgery provided the only short time solution. Therefore the prognosis in this case was evaluated as not advanced.
