Abstract:
This research seeks to investigate why some orphaned children drop out of school in Oshana region, Namibia, despite concerted efforts by the Government Republic of Namibia and other stake holders to keep them at school. The study aimed to identify the indicators preceding AIDS orphaned children dropping out of schools in the Oshana region-Namibia. A cross-sectional study, which adopted both quantitative and qualitative approaches, was used to gain a complete picture of the situation of orphans and school dropouts from the perspective of school teachers, learners, and orphans who have already dropped out of school. It was found that many orphans still face many challenges and to some, continuing with schooling is a luxury that is ill afforded. The total percentages of learners that dropped out of school are: 52.9% orphans in grades 7 and 8 in 2009 while 41.7% dropped out in 2010. Poor awareness of Government support initiatives, poor psychosocial support, leaving school to grieve the death of a parent and working to earn income, ranked amongst the highest reasons some orphans dropped out of school. Provision of material resources in addition to human torch as demonstrated by majority of life skill teachers towards the well-being of AIDS orphans have greatly ameliorated the self-worth and schooling of orphans.

Abstract:
Both Young’s modulus and specific membrane capacitance (SMC) are two important physical parameters for characterizing cell status. In this paper, we utilized a thin-neck-micropipette aspiration system to simultaneously quantify Young’s modulus and SMC value of six types of cell lines in different progression grades, which include four grades from the lowest metastatic potential G1 to the highest potential G4. We investigated how these two physical properties possess heterogeneities in bladder cancer cells with different grades and what roles they might play in grading bladder cancer. The characterization results of these cells of different cancer grades is linearly correlated with the cancer grades, showing that the Young’s modulus is negatively linearly correlated with bladder cancer grades, while SMC shows a positive linear correlation. Furthermore, the combination of these two physical properties on a scatter diagram clearly shows the cell groups with different cancer grades, which means that this combination could be a potential tumor grading marker to identify cancer cells with different metastatic potential.

Abstract This paper gives an overview of the main challenges and achievements faced by Windhoek's water management sector. The paper highlights pertinent issues arising from increased water demand, and also explores current and future water supply augmentation options. Water planners experience management challenges as a result of a combination of factors, mainly, lack of funds and staff, limited expertise, poor communication between stakeholders, and weak regulation and enforcement. In order to meet these challenges water managers need to develop more robust and resilient strategies, including greater focus on water demand management.

Abstract:
This study adapted and validated a questionnaire for assessing Namibian students’ scientific epistemic beliefs and explore their relation to achievement in science, gender, grade and socioeconomic status. The study was correlational, with a sample of 944 (45% male; 55% female) Grades 11 and 12 students with the mean age M=17.9, SD= 1.4. A Likert scale questionnaire comprised of four dimensions of beliefs: source; certainty; development; and justification of scientific knowledge. The adapted questionnaire had good reliability with the Cronbach’s alpha of subscales ranging from .80 to .83 and the overall reliability of .70. Model fit analysis yielded good statistical fit with Chi-square ratio to degrees of freedom= 2.71, RMSEA= 0.043, CFI= 0.95, TLI= 0.94, SRMR= .032. The overall regression model was significant F (4, 939) =8.218, p <.001, R2= .034. Two dimensions, certainty (b=.154, p<.001) and justification (b=.100, p<.05) statistically significantly predicted achievement in science. There was statistically significant difference in beliefs about source in terms of gender and grade and about certainty in terms of grade. The results show that the questionnaire works well with the Namibian sample used given the good model fit for the data and reliability. Results are discussed in terms of 21st century skills development.

Abstract:
In Namibia, natural science (NS) is one of the priority subjects in upper primary phases (Grades 4 to 7). However, in the Omusati Region of Namibia, there are increasing public concerns that many learners are not performing well in NS. Thus, this study surveyed the views of NS teachers on the challenges affecting upper primary learners’ performance in NS at three selected combined schools in the Omusati Region. Qualitative research design using face-to-face interview was used to gather the views of upper primary NS teachers in the schools. Seven teachers who are currently teaching NS at the selected schools were selected using the purposive sampling method to participate in the study. The results obtained revealed that the following challenges affect upper primary learners’ performance in NS in the selected schools: medium of instruction, lack of adequate teaching and learning resources, lack of laboratories, sensitive topics, overcrowded classroom, indiscipline among learners, absenteeism, unfavorable teaching environment, hunger, and lack of proper guidance from subject advisory teachers. The study recommended that the government should provide schools with feeding programs, build laboratories, renovate classes and provide teaching materials. The schools should evolve disciplinary measures to check the learners’ indiscipline.

Abstract:
The main purpose of this paper was to investigate the relationship between banks’ credit risk and profitability and liquidity shocks in Namibia for the period 2009 to 2018 using the SVAR model. In estimating the SVAR regression model, granger causality, impulse-response functions and forecast error variance decomposition were employed and evaluated. The sample consisted of Namibian commercial banks. By auditing liquidity data between 2009 and 2018, empirical results showed that liquidity risk is caused by a combination of structural shocks. The granger causality, impulse-response functions and forecast error variance decomposition documented that credit risk (non-performing loans) is key factor affecting liquidity conditions in Namibia in the medium to long run. In addition, the empirical results showed that quality earnings (ROA) have minimal impact on liquidity conditions in the short run. Reforming assets quality policies and earnings quality policies can be valuable policy tools to minimize liquidity shortages and avoid insolvent banks in Namibia.

Abstract:
Outbreaks of Rift Valley fever (RVF) occurred in Namibia in 2010 and 2011. Complete genome characterization was obtained from virus isolates collected during disease outbreaks in southern Namibia in 2010 and from wildlife in Etosha National Park in 2011, close to the area where RVF outbreaks occurred in domestic livestock. The virus strains were sequenced using Sanger sequencing (Namibia_2010) or next generation sequencing (Namibia_2011). A sequence-independent, single-primer amplification (SISPA) protocol was used in combination with the Illumina Next 500 sequencer. Phylogenetic analysis of the sequences of the small (S), medium (M), and large (L) genome segments of RVF virus (RVFV) provided evidence that two distinct RVFV strains circulated in the country. The strain collected in Namibia in 2010 is genetically similar to RVFV strains circulating in South Africa in 2009 and 2010, confirming that the outbreaks reported in the southern part of Namibia in 2010 were caused by possible dissemination of the infection from South Africa. Isolates collected in 2011 were close to RVFV isolates from 2010 collected in humans in Sudan and which belong to the large lineage containing RVFV strains that caused an outbreak in 2006–2008 in eastern Africa. This investigation showed that the RVFV strains circulating in Namibia in 2010 and 2011 were from two different introductions and that RVFV has the ability to move across regions. This supports the need for risk-based surveillance and monitoring.

Abstract:
This study investigated the removal of sulfur and iron from shungite rocks through different methods after fine grinding: flotation, magnetic separation, microwave treatment, and chemical leaching. In this work, first, a mineralogical study of shungite was conducted. The carbon, silica, iron, and sulfur compositions in the as-received shungite were 45.4%, 38.3%, 4.6%, and 2.4%, respectively. In flotation, a sulfur grade of 1.4% was obtained. In the wet high-gradient magnetic separation at a magnetic flux density of 1 tesla, the iron and sulfur grades in the nonmagnetic fraction were 2.8% and 1.9%, respectively. Furthermore, the sulfur reduced to 0.2% by the 9 min microwave irradiation. In addition, chemical leaching using chelating reagents and inorganic acids was utilized to remove iron and sulfur. Nitrilotriacetic acid (NTA) could reduce the iron and sulfur grades to 2.0% and 0.9%, respectively. For leaching using reverse aqua regia, the iron and sulfur grades were reduced to 0.9% and 0.23%, respectively. For leaching using a 6N HCl with H2O2 aqueous solution, the iron and sulfur grades were reduced to 0.8% and 0.34%, respectively. Overall, chemical leaching using HCl with H2O2 was the most effective for iron and sulfur removal from shungite.

Abstract:
Purpose Preclinical models suggest that the use of anti–vascular endothelial growth factor (anti-VEGF) therapy with antiestrogens may prevent or delay the development of endocrine therapy resistance. We therefore performed a feasibility study to evaluate the safety of letrozole plus bevacizumab in patients with hormone receptor–positive metastatic breast cancer (MBC). Methods Patients with locally advanced breast cancer or MBC were treated with the aromatase inhibitor (AI) letrozole (2.5 mg orally daily) and the anti-VEGF antibody bevacizumab (15 mg/kg intravenously every 3 weeks). The primary end point was safety, defined by grade 4 toxicity using the National Cancer Institute Common Toxicity Criteria, version 3.0. Secondary end points included response rate, clinical benefit rate, and progression-free survival (PFS). Prior nonsteroidal AIs (NSAIs) were permitted in the absence of progressive disease. Results Forty-three patients were treated. After a median of 13 cycles (range, 1 to 71 cycles), select treatment-related toxicities included hypertension (58%; grades 2 and 3 in 19% and 26%), proteinuria (67%; grades 2 and 3 in 14% and 19%), headache (51%; grades 2 and 3 in 16% and 7%), fatigue (74%; grades 2 and 3 in 19% and 2%), and joint pain (63%; grades 2 and 3 in 19% and 0%). Eighty-four percent of patients had at least stable disease on an NSAI, confounding efficacy results. Partial responses were seen in 9% of patients and stable disease ≥ 24 weeks was noted in 67%. Median PFS was 17.1 months. Conclusion Combination letrozole and bevacizumab was feasible with expected bevacizumab-related events of hypertension, headache, and proteinuria. Phase III proof-of-efficacy trials of endocrine therapy plus bevacizumab are in progress (Cancer and Leukemia Group B 40503).

Abstract:
The Namibian electricity sector has mainly relied on electricity imports from the Southern African Power Pool (SAPP) over the last decade. However, a growth in electricity demand and scarce import options could cause energy shortages. Therefore, new power plants ought to be commissioned in the near future to avoid the forecasted energy crisis. In this context, Concentrating Solar Power (CSP) generation is regarded as an appropriate alternative to conventional energy technologies, particularly for the excellent solar regime available in Namibia. The study presents a GIS analysis that identifies suitable areas for CSP establishment. A broad range of geographical parameters such as solar radiation, topography, hydrology or land use are examined. The calculations show that the CSP ceiling generation in Namibia is equivalent to 70% of the worldwide electricity production. Moreover, the study offers a scenario analysis where concrete CSP alternatives are compared to coal-fired plant projects developed by the national power utility. Meteonorm and System Advisor Model (SAM) are used to design CSP alternatives located in the area offering the best combination between high solar irradiation and short distances to the infrastructures. Despite the affordability concern which has to be addressed with sound financial instruments, CSP represents a seminal opportunity for the energy sector in

Abstract:
To establish whether clinical prognostic factor outcomes differed based on the initial severity of facial weakness and to determine the association between the initial severity of facial weakness and favorable outcomes. This retrospective cohort study analyzed all patients with Bell’s palsy who visited the outpatient clinic of our university hospital from 1 January 2005 through 31 January 2021. The primary outcome was the rate of recovery at 6 months, evaluated separately in patients with initial House–Brackmann (H-B) grades 3–4 and 5–6. Secondary outcomes included clinical factors associated with favorable outcomes stratified by the initial H-B grade. The rate of favorable recovery was higher in patients with initial H-B grades 3–4 than initial H-B grades 5–6 (82.9% vs. 68.2%, p < 0.001). Multivariable logistic regression analysis showed that age 19–65 years and good electromyography (EMG) results were prognostic of good outcomes in patients with initial H-B grades 3–4. In addition, good EMG results, controlled hypertension, and combination antiviral therapy were significantly prognostic of favorable outcomes in patients with initial H-B grades 5–6. Subgroup analysis interactions showed that combination antiviral therapy (OR: 3.06, 95% CI 1.62–5.78, p < 0.001) in initial H-B grades 5–6 were associated with more favorable outcomes at 6 months than with initial H-B grades 3–4. Our results showed that the proportion of patients who achieved favorable outcomes at 6 months and multiple clinical factors affecting favorable outcomes differed significantly among patients differing in initial severity of Bell’s palsy.

Abstract:
Introduction: Antibiotics are among the most commonly used therapeutic agents for humans globally, and their use has been associated with the development of resistance. The objective of this study was to identify sources for quantifying antibiotic usage patterns and to assess such use in ambulatory patients in the private health sector of Namibia. Methodology: A retrospective analysis of prescription claims data and sales data for the period 2008 to 2011 was conducted. Antibiotic use was expressed in the number of antibiotic-containing prescriptions and volume of units sold and then standardized using defined daily dose per 1,000 inhabitants per day. Results: Antibiotic usage was highest in females (53%), in people 18–45 years of age (41%), and in Windhoek (34%). Overall, wholesale data showed higher antibiotic use than prescription claims data. However, both sources showed similar patterns of antibiotic use. Penicillins were the most used pharmacological group, with amoxicillin/clavulanic acid combination being the most used of the agents. Conclusion: Antibiotic use in the private sector of Namibia is comparable to that of high-consuming European countries such as Italy. A trend observed in this study was the decrease in the use of narrow-spectrum antibiotics in favour of broad-spectrum and newer antibiotics. Since this was the first study to assess antibiotic use in the private sector of Namibia, it could serve as a starting point for continued monitoring of antibiotic use in the whole of Namibia in the context of the World Health Organization’s Global Action Plan to contain antibiotic resistance.

Abstract:
Introduction: Shiga toxin-producing Escherichia coli (STEC) are an important group of emerging zoonotic pathogens carried in the intestinal tracts of ruminants. They can cause mild diarrhea and fatal disease characterized by hemolytic uremic syndrome, especially in children, the elderly, and immune-compromised individuals. Methodology: The aim of this study was to determine if sheep harbor STEC. Sheep feces (n = 40), brisket wool (n = 40), and 150 meat samples were collected from the flank (n = 35), rump (n = 35), brisket (n = 20), shank (n = 25), diaphragm (n = 10), and neck (n = 25) of slaughter-age sheep at a high-throughput abattoir and tested for STEC using a combination of culture and real-time polymerase chain reaction techniques. Results: E. coli O103 (5/40) and O145 (5/40) strains were isolated from the feces and E. coli O157:H7 was isolated from brisket wool (10/40) and flank meat (5/35). The results of this study provide the first report of STEC infections in sheep in Namibia. Conclusions: The results of this study show that sheep, like cattle, can shed STEC strains in their feces, which can contaminate meat and expose humans to infections.

Abstract:
Aim: to study the antihypertensive efficacy of a fixed full-dose perindopril arginine-indapamide combination in patients with grades 2–3 arterial hypertension (AH) who do not receive antihypertensive therapy or those who do not achieve blood pressure (BP) control with other antihypertensive drugs. Subjects and methods. The trial enrolled 30 patients: 20 (66,6%) males and 10 (33,3%) females aged 30 to 60 years (mean age 50,5±7,1 years). Grades 2 and 3 AH was recorded in 28 (93,3%) and 2 (6,6%) patients, respectively. According to office measurements, the baseline BP (systolic BP (SBP)/diastolic BP (DBP) averaged 169±13,3/100,3±6,9 mm Hg in the group. Before included into the trial, 25 (83,4%) patients had taken antihypertensive agents, of them 10 (40%) and 2 (5%) examinees had combination therapy and fixed-dose combinations, respectively. Results. During therapy with a fixed-dosed combination of perindopril arginine 10 mg and indapamide 2,5 mg, there was generally a trend for SBP and DPB to lower at week 2 of therapy and there was a significant reduction in SBP by 42,4±11,2 mm Hg and in DBP by 20,1±9,3 mm Hg by the end of month 3. The goal BP (lower than 140/90 mm Hg) was achieved in 96,6% of the patients. Conclusion. The fixed full-dose perindopril arginine-indapamide combination allows therapeutic efficiency to be enhanced in grades 2–3 AH patients having no target BP values.

Abstract:
Background Physical fighting is particularly detrimental for young people, often affecting other areas of their developing lives, such as relationships with friends and family and participating in risky behaviors. We aim to quantify the amount of problematic physical fighting in Namibian adolescents and identify modifiable risk factors for intervention. Methods We used the Namibia 2013 Global School-based Student Health Survey (GSHS). This survey collects health-related information on school-attending adolescents in grades 7 to 12. We defined physical fighting as having participated in at least two physical fights in the 12 months prior to responding to the survey. Factors that may be associated with physical fighting were identified a prior based on the literature and included age, sex, anxiety, suicide planning, truancy, physical activity, bullying victimization, presence of supportive parental figures, presence of helpful peers, extent of social network, and food insecurity. Multivariable logistic regression models were created to identify factors associated with physical fighting. Results A total of 4,510 adolescents were included in the study. A total of 52.7% female. 16.9% of adolescents reported engaging in at least two physical fights in the previous year. Factors associated with an increased odds of physical fighting included having a suicide plan, anxiety, truancy, food deprivation and being bullied. Increased age and loneliness were associated with a decreased odds of physical fighting. Conclusion This study identifies problematic physical fighting among adolescents in Namibia. We recommend public health and school-based programming that simultaneously targets risk behaviours and conflict resolution to reduce rates of physical fighting.

Abstract Multivariate regression models were developed to predict green board-foot yields for the common factory-lumber grades processed from sugar maple and basswood factory grade logs. These models use the standard log measurements of grade, scaling diameter, log length, and proportion of scaling defect. Any combination of lumber grades (such as 1 Common and Better) or total yield can be predicted with these models. The coefficients presented here can be used in computer programs related to sawmill simulations, economic modeling, or log-yard inventory systems. North. J. Appl. For. 4:154-157, Sept. 1987.

Abstract:
Purpose Objective structured clinical examinations (OSCE) evaluate clinical reasoning, communication skills, and interpersonal behavior during medical education. In France, clinical training has long relied on bedside clinical practice in academic hospitals. The need for a simulated teaching environment has recently emerged, due to the increasing number of students admitted to medical schools, and the necessity of objectively evaluating practical skills. This study aimed at investigating the relationships between OSCE grades and current evaluation modalities. Methods Three-hundred seventy-nine 4th-year students of University-of-Paris Medical School participated to the first large-scale OSCE at this institution, consisting in three OSCE stations (OSCE#1–3). OSCE#1 and #2 focused on cardiovascular clinical skills and competence, whereas OSCE#3 focused on relational skills while providing explanations before planned cholecystectomy. We investigated correlations of OSCE grades with multiple choice (MCQ)-based written examinations and evaluations of clinical skills and behavior (during hospital traineeships); OSCE grade distribution; and the impact of integrating OSCE grades into the current evaluation in terms of student ranking. Results The competence-oriented OSCE#1 and OSCE#2 grades correlated only with MCQ grades (r = 0.19, P<0.001) or traineeship skill grades (r = 0.17, P = 0.001), respectively, and not with traineeship behavior grades (P>0.75). Conversely, the behavior-oriented OSCE#3 grades correlated with traineeship skill and behavior grades (r = 0.19, P<0.001, and r = 0.12, P = 0.032), but not with MCQ grades (P = 0.09). The dispersion of OSCE grades was wider than for MCQ examinations (P<0.001). When OSCE grades were integrated to the final fourth-year grade with an incremental 10%, 20% or 40% coefficient, an increasing proportion of the 379 students had a ranking variation by ±50 ranks (P<0.001). This ranking change mainly affected students among the mid-50% of ranking. Conclusion This large-scale French experience showed that OSCE designed to assess a combination of clinical competence and behavioral skills, increases the discriminatory capacity of current evaluations modalities in French medical schools.

Abstract:
The role of tachykinin neurons of the myenteric plexus in the regulation of the intestinal peristaltic reflex was examined in rat colonic segments that enable separate measurement of ascending contraction and descending relaxation. Release of immunoreactive substance P (SP) and substance K (SK) increased (68% for SP; 70% for SK) during ascending contraction and decreased (37% for SP; 13% for SK) during descending relaxation. Exogenous SP and SK augmented ascending contraction and inhibited descending relaxation. Neither SP antiserum nor SK antiserum had any effect on descending relaxation. The antisera had no effect also on ascending contraction induced by low grades of stretch (2 g), but they inhibited significantly contraction induced by high grades (6-10 g) of stretch (52 +/- 8% inhibition with SP antiserum and 48 +/- 8% with SK antiserum). A combination of both antisera abolished ascending contraction induced by high grades of stretch. Atropine inhibited only partially ascending contraction at high grades of stretch. The pattern of inhibition by both antisera and by atropine implied that ascending contraction induced by high grades of stretch was mediated by tachykinin neurons acting directly on smooth muscle cells as well as indirectly via release of acetylcholine from cholinergic motor neurons.

Abstract:
The purpose of this study was to survey assessment and grading practices employed by secondary music teachers throughout the southwestern region of the United States. Three main research questions guided the study: (1) What types of school district frameworks and classroom contexts are secondary music teachers operating within? (2) Which specific assessment and grading practices are employed most commonly by secondary music teachers? (3) Do any contextual or individual difference variables influence secondary music teachers’ assessment and grading practices? Two mailings yielded 352 total usable questionnaires (36%) with a sampling error of +/—5%. Participants reported that their school districts emphasized letter grades and that music course grades were equally weighted with other course grades in calculating student grade point averages and generating credit toward graduation. Yet, music teachers seldom received administrative guidance or altered assessment approaches due to standards-based curriculum adoption. Participants based grades on a combination of achievement and non-achievement criteria, with non-achievement criteria receiving greater weight in determining grades. Although instructional time, number of students taught, and number of concert performances prepared/ given had no substantive relationship with assessment decisions, grading practices were influenced by teaching level and teaching specialization.

Abstract:
The establishment of enclosed conservation areas are claimed to be the driving force for the long-term survival of wildlife populations. Whilst fencing provides an important tool in conservation, it simultaneously represents a controversial matter as it stops natural migration processes, which could ultimately lead to inbreeding, a decline in genetic diversity and local extinction if not managed correctly. Thus, wildlife residing in enclosed reserves requires effective conservation and management strategies, which are strongly reliant on robust population estimates. Here, we used camera traps combined with the relatively new class of spatially explicit capture-recaptured models (SECR) to produce the first reliable leopard population estimate for an enclosed reserve in Namibia. Leopard density was estimated at 14.51 leopards/100 km2, the highest recorded density in Namibia to date. A combination of high prey abundance, the absence of human persecution and a lack of top-down control are believed to be the main drivers of the recorded high leopard population. Our results add to the growing body of literature which suggests enclosed reserves have the potential to harbour high densities and highlight the importance of such reserves for the survival of threatened species in the future.

Abstract:
The paper uses a combination of theory and both quantitative and qualitative evidence to demonstrate the significance and challenges of agricultural development in Namibian green scheme projects. For quantitative, a structured questionnaire to produce descriptive statistics was administered to 135 small farmers while eight (8) project manager who were interviewed at the studied schemes as key informant served as source of qualitative information that pin pointed out challenges and opportunities, faced by the small farmers in these schemes. The evidence points to the fact that although there are myriad of challenges, such as challenges related to production, access to efficient and effective market and access to credit faced by farmers, production and access to efficient and effect market challenges emerged as the most stumbling blocks to the optimal production and sales of small farmers’ produce. Usually access to agricultural credit is seen as one of the major challenges of smallholder farmers in Africa. In this study access to agricultural credit was less seen as a major stumbling block to the smallholder farmers’ productivity. This is attributed to the current farmers’ agricultural credit support scheme in place between Agricultural Bank of Namibia (Agribank) and the government of Namibia.

Abstract:
In early 2004, 29% of Namibian Members of Parliament were women, putting Namibia fourth in continental Africa and seventeenth worldwide in terms of women's representation in a national legislature. This article sets out to determine how such a high percentage of women has been elected to the National Assembly in Namibia since independence. It suggests that electoral gains have been achieved through a combination of factors: the use of a closed list proportional representation electoral system and voluntary quotas on the part of political parties at the national level, sustained pressure over the past three to five years from a nascent women's movement influenced by the global women's movement, and the active participation of women inside and outside the country in a protracted and violent struggle for independence that was only attained in 1990. The first two factors confirm past experience and accumulated knowledge on the significance of choice of electoral system and use of quotas, and the importance of women's organisations to elected women's legislative agendas and success. The last factor deviates from experience, and from a literature that suggests that women's active participation in political struggles has not always translated into tangible gains for women.

AbstractWe report a single-center phase I/II trial exploring the combination of everolimus (EVE) and mycophenolate mofetil (MMF) as calcineurin inhibitor (CNI)-free GVHD prophylaxis for 24 patients with hematologic malignancies and indication for allogeneic HCT after a high dose or reduced-intensity ablative conditioning. The study was registered as EudraCT-2007-001892-12 and Clinicaltrials.gov as NCT00856505. All patients received PBSC grafts and no graft failure occurred. 7/24 patients (29%) developed acute grades III and IV GVHD (aGVHD), 16/19 evaluable patients (84%) developed chronic GVHD (cGVHD) of all grades, and 6/19 (31.6%) of higher grades. No severe toxicities related to study medication were observed. The median follow-up of all surviving patients is 2177 days. The 3-year OS was 45.2% (95% CI: 27.4–61.4%), and the 3-year PFS was 38.7% (95% CI: 22.0–55.1%). The cumulative incidence of relapse at 1 year and 3 year was 25% (95% CI: 12.5–50.0%), and 33.3% (95% CI: 18.9–58.7%), the cumulative incidence of NRM at 1 year and 3 years was 20.8% (95%CI: 9.6–45.5%), and 29.2% (95%CI: 15.6–54.4%), respectively. The utilization of CNI-free GVHD prophylaxis with EVE+MMF resulted in high rates of acute and chronic GVHD. Therefore, we do not recommend a CNI-free combination of mTOR inhibitor EVE with MMF as the sole GVHD prophylaxis. In subsequent studies, this combination should be modified, e.g., with further components like post-transplant cyclophosphamide (PTCy) or anti-thymocyte globulin (ATG).

Abstract:
In this paper, a multistage decision-making problem concerning uncertainty and ambiguity is discussed using Pythagorean fuzzy sets. Complement Pythagorean fuzzy membership grades and their properties are also considered. Using the definition of an alpha-level set, we introduce the multistage decision-making problems, where the possibility theory and satisfaction grades are declared with the help of Pythagorean membership grades. Pythagorean multistage decision-making is an uncertain theory, where decision-maker has only one opportunity to choose the scenario under the combination of Pythagorean possibility and satisfaction grades at each stage. According to the selection of criteria, a series of decision points are concluded. The payoff collaborates with these decision points at each stage. The multistage decision-making using Pythagorean fuzzy sets is the scenario-based theory in place of other theories like lottery-based theory etc. The results have been calculated using multistage Pythagorean fuzzy sets in which the decision-maker has only one chance to select the optimal solution. The TOPSIS technique has been applied and the comparison between these two techniques is highlighted.

Modern, cost-effective pipelines are moving beyond the API X70-X80 limits of the 1990s. Over the last few years, more interest has been placed on the X100-120 grades because they are potentially more economical to build and operate. To reach the impressive properties required by these new grades, the proper combination of alloy and rolling process design must be implemented, along with highly controlled interrupted accelerated cooling and hot leveling. This paper discusses some of the underlying physical metallurgy that is required and points out areas where further research and development would be useful.

This study examines male and female course persistence and choice of economics degree via a combination of student, instructor, and structural characteristics. We find that students of both genders who declare economics as their major are more likely to take additional economics courses than their non-major peers. Additionally, students' economics grades are a significant determinant of course persistence and degree selection, but men and women respond somewhat differently to their absolute and relative grades. Finally, men's economics degree selection is significantly correlated with their math abilities, while women's economics degree selection is correlated with both their math and verbal aptitudes.

Abstract:
Abstract. Soil degradation is a major threat for farmers of semi-arid north-central Namibia. Soil conservation practices can be promoted by the development of soil quality (SQ) evaluation toolboxes that provide ways to evaluate soil degradation. However, such toolboxes must be adapted to local conditions to reach farmers. Based on qualitative (interviews and soil descriptions) and quantitative (laboratory analyses) data, we developed a set of SQ indicators relevant for our study area that integrates farmers' field experiences (FFEs) and technical knowledge. We suggest using participatory mapping to delineate soil units (Oshikwanyama soil units, KwSUs) based on FFEs, which highlight mostly soil properties that integrate long-term productivity and soil hydrological characteristics (i.e. internal SQ). The actual SQ evaluation of a location depends on the KwSU described and is thereafter assessed by field soil texture (i.e. chemical fertility potential) and by soil colour shade (i.e. SOC status). This three-level information aims to reveal SQ improvement potential by comparing, for any location, (a) estimated clay content against median clay content (specific to KwSU) and (b) soil organic status against calculated optimal values (depends on clay content). The combination of farmers' and technical assessment cumulates advantages of both systems of knowledge, namely the integrated long-term knowledge of the farmers and a short- and medium-term SQ status assessment. The toolbox is a suggestion for evaluating SQ and aims to help farmers, rural development planners and researchers from all fields of studies understanding SQ issues in north-central Namibia. This suggested SQ toolbox is adapted to a restricted area of north-central Namibia, but similar tools could be developed in most areas where small-scale agriculture prevails.

In order to find a method to characterize the inclusion distribution of high-cleanliness materials, complementary techniques were tested, such as high-frequency (80 MHz) ultrasonic testing, X-ray tomography or Extreme Value Analysis (EVA). The micro-cleanliness was also characterized by standard methods based on observations of polished surfaces by light optical or scanning electronic microscopy. The combination of all these techniques, enhanced by metiS software, allows us to determine the complete 3D distribution of oxides or to estimate the probability of largest inclusion size by modelling virtual samples. At the end, fatigue testing was performed in order to try to link fatigue results to previous characterization outcomes.

The paper examined causality between Private Sector Credit Extension (PSCE) and Economic growth using quarterly data for the period 2000:Q1-2017:Q4, in Namibia. The variables employed were Gross Domestic Product (GDP), Private Sector Credit Extended, Broad Money Supply (M2) and lending rates. The study tested for stationarity in order to determine the order of integration. Furthermore, a co-integration test was conducted on different sets of variables to establish the long run relationship. Granger causality test was also conducted to establish the direction of the relationships between the variables. The results for the stationarity test showed a combination of different orders of integration. The co-integration test revealed a stable long-run relationship among the variables. The Granger causality test results revealed one-directional causality running from PSCE to GDP. Therefore, one can conclude that that change in private sector credit extended can help predict economic growth.

Abstract:
Twenty-four canine cutaneous nodules, diagnosed as mast cell tumors by fine-needle aspiration biopsy and confirmed by histopathologic analysis by staining with hematoxylin and eosin (HE) and toluidine blue, were analyzed by computerized nuclear morphometry on panoptic- and HE-stained cytopathology slides. Two hundred nuclei per lesion were examined. The morphometric parameters investigated were nuclear area, mean diameter, perimeter, regularity factor, and ellipticity factor. Lesions were graded as I (well differentiated), II (intermediate differentiation), or III (poorly differentiated) according to the following morphologic features: invasiveness, cellularity and cellular morphology, mitotic index, and stromal reaction. Nuclear morphometric results were then compared with histopathologic grades. Values of nuclear area, mean diameter, and perimeter increased with increase in histopathologic grade, but statistical analysis revealed significant differences only between grades II and III and between grades I and III when HE was used ( P, 0.01) and between grades I and III with panoptic stain ( P, 0.05). The ellipticity factor and regularity factor did not reveal significant differences between histopathologic grades. The results indicate that nuclear morphometric analysis, in combination with the rapid and inexpensive cytopathology technique, can help in mast cell tumor grading, thus contributing to the establishment of a more precise prognosis and treatment.

Abstract:
Lodging is one of the main factors affecting the quality and yield of crops. Timely and accurate determination of crop lodging grade is of great significance for the quantitative and objective evaluation of yield losses. The purpose of this study was to analyze the monitoring ability of a multispectral image obtained by an unmanned aerial vehicle (UAV) for determination of the maize lodging grade. A multispectral Parrot Sequoia camera is specially designed for agricultural applications and provides new information that is useful in agricultural decision-making. Indeed, a near-infrared image which cannot be seen with the naked eye can be used to make a highly precise diagnosis of the vegetation condition. The images obtained constitute a highly effective tool for analyzing plant health. Maize samples with different lodging grades were obtained by visual interpretation, and the spectral reflectance, texture feature parameters, and vegetation indices of the training samples were extracted. Different feature transformations were performed, texture features and vegetation indices were combined, and various feature images were classified by maximum likelihood classification (MLC) to extract four lodging grades. Classification accuracy was evaluated using a confusion matrix based on the verification samples, and the features suitable for monitoring the maize lodging grade were screened. The results showed that compared with a multispectral image, the principal components, texture features, and combination of texture features and vegetation indices were improved by varying degrees. The overall accuracy of the combination of texture features and vegetation indices is 86.61%, and the Kappa coefficient is 0.8327, which is higher than that of other features. Therefore, the classification result based on the feature combinations of the UAV multispectral image is useful for monitoring of maize lodging grades.

Abstract Japanese patients with leukemia who received bone marrow from human leukocyte antigen (HLA)-compatible siblings had a low incidence of acute graft-versus-host disease (GVHD). Twenty-five (21%) of 120 patients developed moderate (grade II) to severe (grades III to IV) acute GVHD. Severe GVHD was only seen in patients older than 20 years of age. It is also notable that only 2 (5%) of 39 patients who received the combination of methotrexate and cyclosporine (MTX/CSP) for the prevention of GVHD developed grade II acute GVHD, and none developed grades III to IV acute GVHD. Thirteen (30%) of 44 patients receiving MTX alone and 10 (27%) of 37 patients receiving CSP alone developed grades II to IV acute GVHD. Multivariate life-table analysis indicated that the prophylaxis by MTX/CSP was the risk factor for the low incidence of grades II to IV acute GVHD. Compared with the reported incidence of acute GVHD in the patients of the United States, lower incidence of acute GVHD in Japanese BMT patients might be attributable to a lesser degree of genetic diversity in histocompatibility antigens among Japanese.

This article discusses the problems of efficient removal of nitrogen in the vacuum tank degasser operating at the electric arc furnace shop of Uralskaya Stal (Ural Steel) OJSC in the course of manufacturing of high-quality low carbon steel grades by means of steel grades «2» and «T». In order to determine the reasonable and balanced treatment parameters that ensure the required level of nitrogen content in the above steel grades, an analysis of production data for the period of November-December 2016 has been carried out. This analysis is the basis for identifying the vacuum degassing parameters in compliance with the technological capabilities and well-balanced levels, which allow predicting the level of nitrogen content in steel. To assess the cumulative quantitative effect of the main parameters of vacuum degassing on nitrogen removal, there has been performed a regression analysis. As a result, there have been obtained multiple regression equations describing a rational combination of steel treatment parameters for achieving the required nitrogen removal level.

Abstract:
4579 Background: The combination chemotherapy of paclitaxel, ifosfamide, and nedaplatin (PIN regimen) has been shown to be effective in patients with advanced urothelial cancer. The aim of this presentation is to evaluate long-term followup outcomes in patients who had treated with standard cisplatin-based chemotherapy. Methods: The PIN regimen consisted of paclitaxel 175 mg/m2 on day 1, ifosfamide 4.5 g/m2 divided over days 1, 2 and 3, and nedaplatin 70 mg/m2 on day 1; PIN was given every 28 days. The doses of nedaplatin and ifosfamide were reduced according to the impairment of renal function (24h CCr) just before starting each cycle of PIN therapy Results: Thirty two patients who had previously undergone cisplatin-based chemotherapy were entered onto the study (median age, 66 years; median performance status, 1, median 24h CCr, 62ml/min [29–119ml/min] ). Metastatic sites at study entry included the lymph nodes (24), lung (6), bone (5), primary (5), soft tissue (3), and liver (2). Among the 32 assessable patients, there were 5 CRs (16%) and 19 PRs (59%) for an overall RR of 75 % (95% confidence interval [CI], 59–91 %). The median time to progression was 8 months (range, 0–50+ months) and the median survival was 22 months (range, 4–52+ months). Twelve patients received the full dose of PIN combination (median 5 cycles) and 20 patients required dose reduction (median 3 cycles). Nine of 12 (75%) patients who were treated with the full-dose PIN combination obtained a CR or PR, as did 15 of 20 (75%) patients who required dose reduction. Grades 3 and 4 neutropenia occurred in all patients. Febrile neutropenia occurred in 8 (25%) patients. Grades 3 and 4 thrombocytopenia occurred in 8 (25%) patients. Grades 3 and 4 anemia was observed in 6 (19%) patients. Grade 3 neuropathy occurred in 1 (3%) patient, who could not continue to receive additional cycles of PIN combination after 3 cycles. No patient experienced renal failure. There were no treatment-related deaths. Conclusions: The PIN combination was highly active and tolerable in patients who had previously treated with cisplatin-based chemotherapy, and might be suitable for those with mild to moderate renal insufficiency. No significant financial relationships to disclose.

"Quantification of dose dependence and frequency of checkpoint inhibitor immune-mediated adverse events: A Bayesian model-based meta-analysis." Journal of Clinical Oncology 38, no. 5_suppl (February 10, 2020): 83. http://dx.doi.org/10.1200/jco.2020.38.5_suppl.83.

Abstract:
83 Background: Immune checkpoint inhibitors (ICIs) are associated with immune-mediated adverse events (imAEs). The objective of this study was to use a Bayesian model-based meta-analysis to quantify dose dependence and compare imAE frequencies for PD-1, PD-L1 and CTLA-4 inhibitor monotherapies and their combinations. Methods: We searched PubMed, TrialTrove, ASCO and ESMO databases and retrieved relevant ICI safety data. In order to quantitatively compare safety across doses and drugs against a given target, we converted the various dose regimens used into drug exposures derived from pharmacokinetic models; we also normalized exposures by the corresponding drug potency. We performed a Bayesian meta-analysis for Grades 3&4 of treatment related (trAE) and immune-mediated (imAE) adverse events. Results: A total of 149 articles were identified, covering 35,559 patients in 197 dosing cohorts treated with ICI therapies. For PD-1 and PD-L1 inhibitor monotherapies, no dose dependence of AEs was found; Grades 3&4 trAE rates for anti PD-L1 vs. anti PD-1 were, respectively, 12% and 15%. The AE rates for the different ICI drugs and organ classes were estimated. Dose dependence was found for anti CTLA-4 monotherapies, for total trAEs of Grades 3/4, gastrointestinal and hepatic imAEs. Dose dependence was found and quantified for anti CTLA-4 in combination with anti PD-1 with respect to total trAEs of Grades 3/4 trAEs and imAEs per gastrointestinal and hepatic organ groups. We found that combination of anti PD-L1 agents with anti CTLA-4 exhibited lower AE rates, as compared to anti PD-1 combined with anti CTLA-4. Conclusions: We introduced a novel meta-analysis methodology and used it to quantify and compare AE rates across ICI agents. Significant AE rate dose dependencies were observed for CTLA-4 inhibitors, either as monotherapy or used in combinations. Patients naive to anti-cancer therapies exhibited higher AE rates vs. previously treated patients. AE rates for CTLA-4 + PD-1 inhibitor combination regimens were supra-additive vs. the respective monotherapies. AE rates for anti PD-L1 agents were lower vs. anti PD-1, both in monotherapy and combinations with CTLA-4.

Journal of Clinical Oncology 35, no. 15_suppl (May 20, 2017): e21674-e21674. http://dx.doi.org/10.1200/jco.2017.35.15_suppl.e21674.

Abstract:
e21674 Background: Carfilzomib (Carf)is a novel proteasome inhibitor (PI) that is approved for patients with relapsed multiple myeloma (RMM) who have failed ≥ 1 prior lines of therapy. The incidence and seriousness of Carf associated cardiotoxicity (CT) is not well defined. We hypothesize that CT is more frequent than that seen with other PIs. We performed systematic review of Carf literature with meta-analysis to determine its incidence and overall risk. Methods: Initial search of literature led to a total of 175 Carf related articles. However, we used 29 publicatons; phase I/II, phase II and phase III (n = 3) clinical trials in which Carf was used as monotherapy or in combination with other chemo agents. We excluded phase I studies and studies without CT data. Incidence rates and odds ratios (OR) were calculated with either fixed effect or random effect model based on the heterogeneity of included studies. Toxicity was reported according to CTCAE v4.0. Results: A total of 4560 patients with various hematological and solid malignancies were included. Incidence of all grades and high grades (≥ 3) CT (including arrhythmias, CHF with LVEF drop, and coronary syndrome) were 7.8% and 4.72%, respectively. When compared to control group taken from phase III clinical trials, the risk of developing CT due to Carf was significantly higher with OR of 1.90 and 2.03 (P < 0.01) for all grades and high grades, respectively. Moreover, incidence of CT was significantly higher in Carf combination therapy (9.85%) compared to Carf monotherapy (5.40%) (P = 0.01). Furthermore, incidence of high grade CT was 7.5% and 5% with and without concomitant immunomodulatory agent (IMiD), respectively (P = 0.004). There was no variation in the incidence of CT among newly diagnosed versus RMM (P = 0.6), and no Carf dose effect. Mortality rate associated with cardiotoxicity was 1.5%. Conclusions: Overall incidence of Carf related CT seems to be higher than that reported with other PIs. Although, the pathophysiology is poorly understood, this trend could potentially be secondary to irreversible nature of proteasome inhibition by Carf. There seems to be a significant increase in CT with combination of Carf and an IMiD. Prior therapies and higher Carf doses have no effect on CT incidence.

Abstract:
Difficulties in reading and math are more likely to occur simultaneously than difficulties in either area alone; however, the research on that comorbidity is relatively sparse. The purpose of this study was to examine the relation between early reading skills as predictors of math achievement. A group of 102 kindergarten and 65 first-grade students were assessed with curriculum-based measures of early reading and early math, and a measure of broad math achievement. The results of multiple regression analyses indicated that when early numeracy was controlled for, the measures of early reading did not explain unique variance in math achievement among students. Interestingly, screening with only measures of early reading skills yielded acceptable area under the curve (AUC) values but did not yield accurate identification of students at risk for math difficulties (MD) when misclassification and specificity were taken into account. Results suggest that early math measures are most accurate in identifying students at risk for MD in early grades. Findings provide further insight into the relation between math and reading skills at the start of formal schooling. Authors provide recommendations for a combination of reading and math screeners to predict broad math achievement.

Abstract:
Modern weldable high strength steel grades are typically based on low-carbon alloy concepts using microalloying for obtaining a good strength-toughness balance. Such steel grades having a yield strength in the range of 420 to 690 MPa are very commonly used in pipelines, heavy vehicles, shipbuilding and general structural applications. Thermomechanical processing during hot rolling combined with accelerated cooling is an established means of producing such steel grades. Considering the alloying concepts, the use of niobium and molybdenum, and in selected cases boron, is very efficient to achieve high strength and good toughness. However, all targeted applications of such high strength steels involve extensive welding. Thus, heat affected zone properties are of particular importance. The present paper investigates the effects of Nb, Mo and Ti on the heat affected zone properties. Variations of the Mn and Si contents are considered as well. Additionally, the influence of post-weld heat treatment in the coarse-grained heat-affected zone (HAZ) is considered. In this approach, HAZ subzones were generated using laboratory weld cycle simulations in combination with systematic variation of alloying elements to scrutinize and interpret their specific effects. The results indicate that Mo and Nb, when alloyed in the typical range, provide excellent HAZ toughness and guarantee sufficiently low ductile-to-brittle transition temperature. An alloy combination of Nb, Mo and Ti improves performance under hot deformation conditions and toughness after post-weld heat treatment.

Abstract:
Aim: To assess the correlation between syntactic structure analysis (SSA) features, revised dysplasia grade and the presence of high‐risk human papillomavirus DNA in cervical intraepithelial neoplasia (CIN). Materials and methods: HPV polymerase chain reaction (PCR) was assessed in 101 consecutive biopsies and consensus in CIN grade between the experts occurred in 88 cases (CIN1=16, CIN2=27, CIN3=45). SSA was performed in the diagnostic histological section of the CIN lesions in these patients and SSA features were compared with the blind review CIN grade, and presence/absence of high‐risk HPV DNA. Results: One of the SSA features (points from which the surrounding surfaces has 4 edges, PECO‐4) was significantly different between all three consensus CIN grades. Many more features revealed significant differences between CIN1 and CIN2 or between CIN2 and CIN3 cases. With stepwise discriminant analysis, the best multivariate combination of features to distinguish the different CIN grades were the Maximum MST Line Length (MML) and the Area Disorder. Crude overall classification of the consensus grades with these features was 69%. The MML and the Area Disorder is also the best combination to distinguish cases with and without high‐risk HPV DNA (77% correct classifications). Conclusions: SSA features are correlated with both CIN grade and presence of high‐risk HPV DNA, but the discrimination power is not good enough to be used as a routine method for quality control of subjective grade or as a surrogate marker for high‐risk HPV DNA presence.

Abstract:
Desertia gen. nov., which belongs to tribe Massonieae, is described from south western Namibia and north western South Africa. Desertia is at first sight related to Whiteheadia and Massonia, but it can be clearly differentiated by the papillose bracts, subcampanulate flowers with straight, suberect or slightly spreading free portion of tepals and the rugose seeds. This genus is based on Whiteheadia etesionamibensis, for which the combination in Desertia is presented. Furthermore, a second species in the genus, D. luteovirens, is here described based on distinct morphological and molecular characters. Furthermore, taxonomic revisions are presented for Namophila and Whiteheadia, including identification keys for the three genera and data on morphology, ecology, and distribution for all accepted species.

Abstract:
We attempted to establish an ultrasound (US) imaging-diagnostic system for histopathological grades of differentiation of hepatocellular carcinoma (HCC). We conducted a retrospective study of histopathologically confirmed 200 HCCs, classified as early (45 lesions), well- (31 lesions), moderately (68 lesions) or poorly differentiated (diff.) (56 lesions) HCCs. We performed grayscale US to estimate the presence/absence of halo and mosaic signs, Sonazoid contrast-enhanced US (CEUS) to determine vascularity (hypo/iso/hyper) of lesion in arterial and portal phase (PP), and echogenicity of lesion in post-vascular phase (PVP). All findings were of significance for the diagnosis of some (but not all) histological grades (p < 0.001–0.05). Combined findings with a relatively high diagnostic efficacy for early, poorly and moderately diff. HCC were a combination of absence of halo sign and isoechogenicity in PVP of CEUS (accuracy: 93.0%, AUC: 0.908), hypovascularity in PP (accuracy: 78.0%, area under the curve (AUC): 0.750), and a combination of isovascularity in PP and hypoechogenicity in PVP (accuracy: 75.0%, AUC: 0.739), respectively. On the other hand, neither any individual finding nor any combination of findings yielded an AUC of over 0.657 for the diagnosis of well-diff. HCC. Our study provides encouraging data on Sonazoid CEUS in the histological differential diagnosis of HCC, especially in early HCC, and the effectiveness of this imaging method should be further proved by prospective, large sample, multicenter studies.
