Full textO presente relatório de trabalho de projeto foi realizado no âmbito do curso de Mestrado em Enfermagem Médico-Cirúrgica (EMC), e contempla o Projeto de Desenvolvimento Académico (PDA) e o Projeto de Aprendizagem Clínica (PAC). O estágio decorreu num Serviço de Urgência (SU), de um hospital na margem sul. No PDA procuramos dar resposta a uma nossa inquietação, acerca do papel de enfermeiro especialista em EMC como promotor da aprendizagem. Realizamos uma Revisão integrativa da literatura (RIL) com o objetivo de identificar quais os aspetos considerados pelos estudantes de enfermagem (EE) e pelos enfermeiros orientadores (EO), como influenciadores da aprendizagem em contexto de prática clínica (CPC). Emergiram 6 categorias de aspetos que influenciam a aprendizagem: Acolhimento, Organização do ensino clínico (EC), CPC, Processo de ensino-aprendizagem, EO e EE. O enfermeiro especialista, pelas competências comuns especializadas que possui, pode ter um papel facilitador do processo de ensino-aprendizagem em CPC dos EE do CLE, pela sua intervenção junto às equipas de enfermagem, nomeadamente do âmbito de: Preparação e organização do EC em CPC; Acolhimento e integração do estudante no CPC; Acompanhamento e monitorização do EC em CPC. Durante este percurso conseguimos desenvolver competências cientificas, técnicas e humanas para prestar cuidados especializados na área da EMC. O enfermeiro mestre e especialista em EMC presta cuidados de enfermagem ao doente em situação critica e com doença crónica, incapacitante e terminal, realiza investigação, gere os cuidados e tem capacidade de tomada de decisão em situações complexas, utilizando os seus conhecimentos e competências especializadas e promovendo a qualidade na prestação de cuidados.
This project report report was carried out under the Master's course in Medical-Surgical Nursing (MSN), and includes the Academic Development Project (ADP) and the Clinical Learning Project (CLP). The Internship took place in the emergency room (ER),of an hospital on the south of Portugal. The ADP seek to address our concern about the role of nurse specialist in MSN as a learning promoter. We performed an integrative literature review (ILR) in order to identify the aspects considered by nursing students (NS) and the guiding nurses (GN) as learning influencers in the context of clinical practice (CCP). Emerged six categories of aspects that influence learning: Reception, organization of clinical teaching (CT), CCP, teaching-learning process, GN and NS. The nurse specialist, by common expertise they have, may have a facilitating role in the teaching-learning process in CCP for NS, for their intervention in the nursing teams, in particular under: Preparation and organization of the CT in CCP ; Reception and integration of the NS in the CCP; Tracking and monitoring of CT in CCP. During this way we have developed scientific, technical and human skills to provide specialized care in the MSN. The nurse master and specialist in MSN provides nursing care to the patient in critical situation and in chronic, debilitating or terminal illness, conducts research, manages care and has decision-making capacity in complex situations, using their knowledge and expertise skills, to promote quality in the nursing care.

A Teoria das Transições da Afaf Meleis serviu de linha orientadora a este trabalho. Sendo o seu principal objetivo expor todo o processo de desenvolvimento de competências específicas e comuns do enfermeiro especialista e as competências de mestre. Realizando-se uma análise e reflexão acerca do desenvolvimento da aquisição de competências do enfermeiro especialista. O grau de mestre em enfermagem carece dum conhecimento aprofundado no domínio da sua área de especialização. Durante estágio II comtemplou-se a realização dum projeto de intervenção em serviço, segundo a metodologia de projeto, de forma a responder a um problema identificado num contexto de estágio. Com este pretende-se a implementação duma norma – Cuidados de Enfermagem a pessoa com TET, de modo a uniformizar a prática na UCI. O projeto de intervenção contribuiu para o desenvolvimento de competências do enfermeiro especialista, nomeadamente no domínio da gestão de cuidados e da melhoria contínua da qualidade.

Being your main goal to expose the entire process of developing specific and common skills of specialist nurse and master skills. An analysis and reflection about the development of skills acquisition of the specialist nurse. The master's degree in nursing requires in-depth knowledge in the field of its area of expertise. During stage II, a project of intervention in service according to the project methodology was contemplated, in order to respond to a problem identified in an internship context. With this it is intended the implementation of a standard - Nursing Care the person with TET, in order to standardize the practice in the ICU. The intervention project contributed to the development of skills of the specialist nurse, namely in the field of care management and continuous quality improvement.

Full textEste relatório de estágio surge como etapa final da conclusão do ciclo de estudos, referente ao curso de Mestrado em Enfermagem na Área de especialização em Enfermagem Médico-Cirúrgica: A Pessoa em Situação Crítica. Tem como principal objetivo refletir e analisar criticamente as atividades desenvolvidas no processo de aquisição de competências comuns e específicas do enfermeiro especialista em Enfermagem Médico-Cirúrgica: A Pessoa em Situação Crítica, assim como as competências de Mestre em Enfermagem. No presente relatório de estágio, encontra-se descrito todo o processo relativo ao Projeto de Intervenção em Serviço, levado a cabo no estágio Final do curso de Mestrado, que decorreu numa Unidade de Cuidados Intensivos Coronários, onde se implementou a metodologia ISBAR nos momentos de transição de cuidados do doente crítico, assumindo-se como um papel importante na melhoria da segurança do doente, assim como um elemento importante na continuidade dos cuidados, evitando possíveis eventos adversos e visando uma transição segura de cuidados do doente crítico. A análise reflexiva efetuada constituiu por si só um importante elemento de crescimento profissional e pessoal, considerando-se adquiridas as competências que conferem o título de enfermeiro especialista em Enfermagem Médico-Cirúrgica: A Pessoa em Situação Crítica, assim como as competências inerentes à atribuição do grau de Mestre em Enfermagem.
This internship report appears as the final step of the conclusion of the cycle of studies, referring to the Master's degree in Nursing in the area of specialization in Medical-Surgical Nursing: The Person in a Critical Situation. Its main objective is to reflect and analyse critically the activities developed in the process of acquiring common and specific competencies of the specialist nurse in Medical-Surgical Nursing: The Person in Critical Situation, as well as the competencies of Master in Nursing. In the present report of the internship, all the process related to the Intervention Project in Service, carried out in the final stage of the Master's course, has been described, which took place in a Coronary Intensive Care Unit, where the ISBAR methodology was implemented in the moments of care transition of critical patient, assuming itself as an important role in improving patient safety as well as an important element in the continuity of care, avoiding possible adverse events and aiming for a safe care transition of critical patient. The reflexive analysis carried out was an important element of professional and personal growth, considering the acquired competencies that confer the title of specialist nurse in Medical-Surgical Nursing: The Person in Critical Situation, as well as the competencies inherent in the attribution of the Master's Degree in Nursing.

Full textO presente relatório surge no âmbito do 3º Curso de Mestrado em Enfermagem Médico-Cirúrgica, lecionado na Escola Superior de Saúde do Instituto Politécnico de Setúbal. Através deste, e com recurso a uma metodologia reflexiva, pretendemos apresentar todo o processo de aprendizagens decorridas ao longo do Curso, que contribuíram para o desenvolvimento de competências de Enfermeiro Especialista (EE), comuns e específicas, e de Mestre, na área da Enfermagem Médico-Cirúrgica. Aquando do desenvolvimento destas competências, ocorreram simultaneamente o percurso em contexto letivo e os Estágios, sendo os últimos entendidos como oportunidades privilegiadas de formação experiencial, onde para além do exercício clínico, concebemos um Projeto de Intervenção em Serviço, com recurso à Metodologia de Trabalho de Projeto, e um Projeto de Aprendizagem Clínica. O primeiro projeto teve por objetivo major promover a melhoria da qualidade dos cuidados de enfermagem prestados às pessoas internadas num Serviço de Cirurgia Geral, no âmbito da prevenção das Úlceras por Pressão, através da avaliação do risco de desenvolvimento, após confirmarmos a inexistência de uniformização de procedimentos relativamente à aplicação da Escala de Braden (apesar da existência de uma norma de orientação clínica sobre a Prevenção de úlceras por pressão a nível hospitalar). Quanto ao segundo projeto, mais dirigido ao desenvolvimento das competências específicas do EE, envolveu a realização de várias atividades, tanto na área da Pessoa em Situação Crítica, como da Pessoa em Situação Crónica e Paliativa, por ambas se inscreverem no âmbito da Enfermagem Médico-Cirúrgica, dando-se como exemplo a produção e consequente divulgação de Posters, bem como Ações de Sensibilização dirigidas às visitas das pessoas internadas. A referir ainda a ancoragem do nosso percurso de aprendizagens numa teoria de médio alcance, Teoria do Conforto de Kolcaba, por promover o cuidado centrado na satisfação das necessidades de conforto das pessoas, onde revemos a nossa praxis.

Full textNo presente relatório pretendemos demonstrar, num processo de análise e reflexão, todo o percurso académico realizado no âmbito do Curso de Mestrado em Enfermagem Médico- Cirúrgica, durante o qual realizamos estágios numa Unidade de Cuidados Intensivos Coronários (UCICOR), desenvolvemos um Projeto de Intervenção em Serviço (PIS), pela metodologia de trabalho de projeto, e um Projeto de Aprendizagem Clínica (PAC). O PIS intitulado “A Segurança passa pela Prevenção” aborda a problemática de ocorrência de quedas do cliente hospitalizado no Serviço de Cardiologia/UCICOR. Estes incidentes são uma realidade das instituições de saúde e um indicador de qualidade dos cuidados de enfermagem. A realização deste projeto possibilitou-nos intervir na componente da segurança do cliente, através da adoção de um conjunto de estratégias pela equipa de enfermagem, nas quais evidenciamos a avaliação do risco de queda dos clientes e o envolvimento dos mesmos na prevenção e redução da ocorrência de quedas. A sua fundamentação teórica assenta na Tipologia dos Vinte e Um Problemas de Enfermagem de Faye Glenn Abdellah. O PAC constituiu-se como essencial na promoção de boas práticas profissionais, que visam a qualidade dos cuidados, dando resposta ao desenvolvimento de Competências Comuns e Específicas do Enfermeiro Especialista em Enfermagem Médico-Cirúrgica, desenvolvidas no estágio III. Na Pessoa em Situação Crítica, ao nível da prevenção e controlo da infeção implementamos o procedimento “Boas Práticas no Despejo do Saco de Drenagem de Urina”. Na Pessoa em Situação Crónica e Paliativa direcionamos a nossa intervenção para o cuidador do cliente com Insuficiência Cardíaca. Assim, neste relatório, procuramos evidenciar como os aportes teóricos lecionados e os projetos desenvolvidos em contexto de estágios, na procura da excelência no exercício profissional, promoveram a aquisição e desenvolvimento das Competências Comuns do Enfermeiro Especialista, das Competências Específicas do Enfermeiro Especialista e Mestre em Enfermagem Médico-Cirúrgica.
Abstract: In this report we intend to demonstrate, in a process of analysis and reflection, the entire academic course conducted in the context of a master’s degree in Medical-Surgical Nursing. For this purpose we did various internships at the Coronary Intensive Care Unit (CICU) as well as developed, using a project work methodology, an Intervention Project in Service (IPS) and a Clinical Learning Project (CLP). The IPS, entitled "Security is achieved through Prevention" concerns the occurrence of falls in patients admitted into the Cardiovascular Service /CICU. These incidents are a fact in health institutions and an indicator of nursing care quality. The achievement of this project had enabled us to participate in the field of patient security through the adoption of a set of strategies by the nursing team, in which we emphasized the assessment of the patient fall risk and the involvement of the patient himself in the prevention and reduction of falls. Its theoretical foundation is based on the Faye Glenn Abdellah’s Typology of the Twenty-One Nursing Problems. The CLP was established as essential in promoting good professional practice, that aim the quality of care and responds to the development of both Common and Specific Skills of the Nurse Specialist in Medical-Surgical Nursing. These skills were developed during the internship III. In a Critical Situation, at a level of prevention and control of infection, the procedure implemented is "Good Practices in the Disposal of the Urine Drainage Bag". In a Chronic and Palliative Condition we direct our intervention to the caregiver of the patient with Heart Failure. In short, this report seeks to show how the theoretical contributions and projects, developed in the context of internships, as well as the search for excellence in professional practice, promoted the acquisition and development of both Common and Specific skills of the Specialist Nurse and the Master in Medical-Surgical Nursing.

Full textO presente Relatório de Trabalho de Projeto surge no âmbito do 3º Curso de Mestrado em Enfermagem Médico-Cirúrgica da Escola Superior de Saúde do Instituto Politécnico de Setúbal e a sua construção, apresentação e discussão pública visam a obtenção do grau de Mestre nesta área científica. Os objetivos deste relatório passam por apresentar o PIS – Projeto de Intervenção em Serviço e o PAC – Projeto de Aprendizagem Clínica, e descrever e analisar o desenvolvimento das competências adquiridas como Enfermeira Especialista e Mestre em Enfermagem Médico-Cirúrgica. No âmbito dos estágios realizados no Bloco Operatório do Centro Hospitalar X foram desenvolvidos, segundo a metodologia de trabalho de projeto, dois projetos: um Projeto de Intervenção em Serviço sobre “Integração de Enfermeiros no Bloco Operatório – O Primeiro Passo para Cuidados de Excelência”, cuja realização nos permitiu criar e implementar um programa de integração de novos enfermeiros no Bloco Operatório, com o objetivo de melhorar a qualidade e a segurança das integrações, sendo que foi um contributo para uma eficaz integração, facilitadora e potenciadora de uma prestação de cuidados de excelência; e um Projeto de Aprendizagem Clínica, centrado no desenvolvimento dos domínios nas áreas da prevenção e controlo de infeção, da resposta na catástrofe e emergência multivítima e do cuidar da pessoa em situação crítica e em situação crónica e paliativa. Ambos os projetos contribuíram para o desenvolvimento das competências comuns e específicas do Enfermeiro Especialista em Enfermagem Médico- Cirúrgica, designadamente em Enfermagem em Pessoa em Situação Crítica e em Pessoa em Situação Crónica e Paliativa, bem como as competências de Mestre em Enfermagem Médico-Cirúrgica. Ao longo do Relatório, descrevemos o percurso efetuado no Curso e respetivos estágios, analisando os seus aportes no desenvolvimento de competências especializadas e de mestre em enfermagem, contribuindo para a melhoria contínua da qualidade dos cuidados prestados, dentro do alvo desejável da excelência na profissão de Enfermagem.
Abstract: This Project Work Report comes out as part of the 3rd edition of the Medical- Surgical Nursing Master’s degree in the School of Health, Polytechnic Institute of Setúbal. Its construction, presentation and public discussion aims the achievement of a Master’s degree in this scientific area. The objectives of this report are to present the IPS – Intervention Project in Service – and the CLP – Clinic Learning Project – and describe and analyze the development of skills acquired as Specialist Nurse and Master in Medical-Surgical Nursing. Two projects were carried out during the internship: an Intervention Project in Service entitled as "Integration – The First Step to Excellence Care", which allowed us to create and implement a program for the integration of new nurses in the operating room, which has been a contribution to an effective integration, promoting and enabling the provision of a care of excellence; and a Clinic Learning Project, aimed at developing the skills of a Specialist Nurse regarding the prevention and control of infections, the answer in disaster situations and multi victim emergencies, and the care of someone in critical and in chronic and palliative situations. Both projects contributed to the development of the common and specific skills of the Specialist Nurse in Medical-Surgical Nursing, namely in the care to the Patient in Critical Situation or in Chronic and Palliative Situation, as well as the development of the skills of Master in Medical-Surgical Nursing. Throughout the Report, we will describe the route followed during the Course and respective internships, analyzing their contributions to the development of expertise and mastership in Nursing, and to the continuous improvement of the quality of care provided within the desirable goal of excellence in the Nursing profession.

Full textO presente Relatório de Trabalho de Projeto surge como um documento ilustrativo das aprendizagens adquiridas durante os três estágios realizados no Serviço de Urgência e no estágio de opção na Unidade de Cuidados Intensivos do Hospital Regional, no âmbito do 3º Mestrado em Enfermagem Médico-Cirúrgica da Escola Superior de Saúde do Instituto Politécnico de Setúbal. Visa apresentar uma reflexão crítica do desenvolvimento das competências comuns do Enfermeiro Especialista, das competências específicas do Enfermeiro Especialista em Enfermagem em Pessoa em Situação Crítica e do Enfermeiro Especialista em Enfermagem em Pessoa em Situação Crónica e Paliativa; e ainda das competências de Mestre em Enfermagem Médico-Cirúrgica. Ao longo dos estágios, seguindo a Metodologia de Projeto, desenvolveu-se um Projeto de Intervenção em Serviço e um Projeto de Aprendizagem Clínica. A problemática identificada no âmbito do Projeto de Intervenção em Serviço incidiu sobre a gestão de risco. Foram produzidas uma Proposta de Política de Risco, três Normas de Procedimento relativas à Identificação e Avaliação de Riscos, à Auditoria sobre Risco e Segurança e ao Relato de Incidentes aplicáveis a toda a instituição e formou-se a equipa de enfermagem do Serviço de Urgência através da criação de um dossier temático disponível na intranet. No âmbito do Projeto de Aprendizagem Clínica, com o objetivo de dar resposta às competências específicas do Enfermeiro Especialista em Enfermagem em Pessoa em Situação Crítica e do Enfermeiro Especialista em Pessoa em Situação Crónica e Paliativa, foi elaborado um Registo de Paragens Cardiorrespiratórias, com a finalidade de produzir mais um elemento com implicância na segurança dos cuidados, que foi anexado a um manual sobre reanimação que se encontrava a ser desenvolvido no Serviço de Urgência. Ao nos apercebermos das dificuldades da equipa de enfermagem do Serviço de Urgência na punção e manutenção do Cateter Venoso Central Totalmente Implantado com Reservatório Subcutâneo elaborámos um instrumento de formação em formato poster para transmitir as informações mais pertinentes, que implicam o conforto e a qualidade de vida do cliente, dando ênfase também às questões relacionadas com o controlo da infeção. A realização deste trabalho permitiu adquirir e desenvolver as competências inicialmente propostas.

Full textEste relatório surge no âmbito do Curso de Mestrado em enfermagem Médico-Cirúrgica (EMC) e tem como finalidade apresentar reflexivamente o percurso conducente ao desenvolvimento de competências de enfermeiro especialista (comuns e específicas) e de Mestre. Para isso, foi utilizada a metodologia de trabalho de projeto, integrando dois projetos distintos: projeto de intervenção em serviço (PIS) e projeto de aprendizagem clínica (PAC), desenvolvidos ao longo dos estágios realizados num serviço de cirurgia geral (SCG). O PIS, formulado sob a premissa de que uma eficaz gestão dos cuidados está numa das bases da qualidade dos mesmos, incidiu sobre a necessidade de uniformizar procedimentos relativamente à avaliação funcional das pessoas internadas no SCG. Este, veio permitir, através de formação e criação de documentos normativos e orientadores desta prática, promover a qualidade dos cuidados de enfermagem através da otimização da intervenção dos enfermeiros no domínio da avaliação funcional da pessoa internada no SCG. Foi nosso referencial teórico a Teoria do Défice do Auto-Cuidado de Orem, com enfoque na importância de direcionar os cuidados prestados às necessidades de cada pessoa, partindo de uma avaliação funcional, dotada de objetividade através do uso de escalas validadas, como a escala modificada de Barthel. Por sua vez, no PAC projetaram-se atividades específicas, das quais se destacam: participação num exercício de simulação de inundação numa unidade de cuidados intermédios e realização de respetivo poster de divulgação; realização de ação de sensibilização aos familiares das pessoas internadas no SCG, sobre importância da higienização das mãos na visita; e realização de um poster sobre hipodermoclise, classificado em 2º lugar num encontro de enfermagem. Neste documento analisamos crítica e reflexivamente ambos os projetos, e os contributos dos mesmos no desenvolvimento das competências supra-citadas.
This report is set within the context of the Medical Surgical Nursing (MSN) Master’s Degree, in order to reflexively present the route which led us to the development of specialist (either common or MSN specific) and Master nurse skills. Therefore, we used the project methodology, integrating two distinct projects: intervention project in service (IPS) and clinical learning project (CLP), both developed throughout the internship in a general surgery service (GSS). The IPS was formulated under the premise that effective care management is one of the bases of its quality, and focused on the need to standardize procedures for the functional evaluation of people admitted to the GSS. Through training and creation of normative and guideline documents, the IPS has promoted the quality of nursing care by optimizing the nurses’ involvement in the functional evaluation of the hospitalized patient in GSS. Our theoretical framework was the Orem’s Self-Care Deficit Theory, focusing on the importance of care directed to the needs of each person, and based on a functional evaluation, endowed with objectivity through the use of validated scales such as the modified Barthel index. In turn, on the CAP we designed specific activities, among which the following are highlighted: participation in an exercise of flood simulation in an intermediate care unit and the development and disclosure of the respective poster; performing of an awareness session to the relatives of those admitted to the GSS, about the importance of hand hygiene in the visit; and creating a poster on hypodermoclysis, that was ranked 2nd in a nursing meeting. In this document we analyze critically and reflexively both projects, and their contributions to the development of the above-mentioned skills.

Apresentamos uma apreciação crítica e reflexiva do caminho percorrido, sendo que a sua génese teve como base o trabalho realizado utilizando a metodologia de trabalho de projeto, durante os Estágios I, II e III, inseridos no plano de estudos. Todos os estágios foram realizados numa Unidade de Cuidados Intensivos Cardiológicos e a temática abordada é a Infeção Associada aos Cuidados de Saúde (IACS) em Utentes a aguardar Implante de Pacemaker Definitivo/Cardiodisfibrilhador Implantável. Foi realizado um Projeto de Intervenção em Serviço (PIS) visando a temática acima referida de modo a possibilitar a aquisição das competências de Enfermeiro Especialista em Enfermagem Médico-Cirúrgica e as competências de Mestre em Enfermagem Médico-Cirúrgica, bem como a operacionalização das mesmas. Durante a elaboração deste foi possível identificar práticas incorretas relacionadas com a higienização das mãos e utilização de equipamentos de proteção individual tendo sido elaboradas sessões de formação de modo a reciclar conhecimentos sobre estas temáticas. Foi também realizado um Projeto de Aquisição de Competências (PAC) de modo a realizar uma reflexão crítica acerca das competências adquiridas, à luz do Modelo de Sinergia da American Association of Critical Care Nurses (AACN).
This report is the result of an academic path made with the objective of securing common and specific Medical- Surgical Specialized competences as well as competences in Medical-Surgical Masters, by attending the 4th Masters in Medical- Surgical Nursing in Escola Superior de Saúde of the Instituto Politécnico de Setúbal. In this report we can find a critic and reflexive appreciation of the path taken which was based on the work done using the Work Project methodology during the Internships I, II and III, part of the study plan. All the internships were developed in a Cardiac Intensive Care Unit and the theme was Health Care Related Infections in Clients Waiting for Pacemaker Device Implantation. An Intervention Project was created about the thematic referred above to make it possible for the student to acquire all the Medical-Surgical Specialized Nursing competences as well as the Masters competences in Medical-Surgical Nursing and their application. While building this project it was possible to identify incorrect practices related to hand hygiene and individual protection equipment. Training sessions were held to review concepts about these themes. For the critic reflexion about the acquired competences a Competence Acquiring Project was produced in the light of the American Association of Critical Care Nurses Synergy Model.

Full textThis report expresses the experiences during the travelled route in the several places of stage under the Nursing Master Course with specialization in Medical Nursing – Institute of Health Sciences – Universidade Católica Portuguesa, Porto. It also lists the great learning, focusing on the main activities and developed skills, thoughts and reflections, respectively, on the module I: Emergency Service and module III: Infection Control Services, performed in Centro Hospitalar Cova da Beira. The acquisition of skills, technical skill, planning and decision taking before complex and unpredictable situations, in emergency context and in the control and prevention of hospital infection, was a constant and a contribution, as well as integrate the difficulties, the clashes that a specialist nurse faces under an ethical, moral, social and deontological perspective, based on the assumptions that guide the nursing profession, in addressing to the critical patient. The developed work reflects the deepening of the aspects and contributions considered relevant, as the realization of a training action on the Emergency Service entitled "Invasive Monitoring" as well as in the Commission Service Infection Control, about the theme "SIRS, SEPSIS, MODS and Blood Cultures". It was still performed a flyer on "Simple Measures Save Lives" and a work based on "Analysis of results of the inquires about surveillance of the Five Moments in the hand hygiene" in partnership with the teams, allowing a participatory role in the building this path and in the promotion of the development of future practices which boost efficiency and the improvement of the quality of the provided care. These behaviours contributed to the acquisition of a greater autonomy, according to an advanced perspective of the care and a greater social visibility, essential for the development of the profession. The complexity of the diseases and the size of the resources imposed with singleness the differentiation of the professionals, who trained to be able to monetize and prioritize the use of each of these knowledge. The report is structured according to the stages carried out, which contains the outlined objectives, the undertaken activities and acquired skills, in the two modules and it has as goal to serve as a tool for certification of technical, human, relational and communication competences, as a specialist in medical nursing -surgical and obtain a master's degree. The methodology is based on the analytical method through reflection of the activities and the reasoning based on pedagogical knowledge.

Full textThis report will describe the path taken and the skills acquired in the Internship inserted in the Curricular Unit - Final Internship and Report conducted within the Masters Course in Nursing with a specialization in Medical-Surgical Nursing taught in the “Instituto de Ciências da Saúde-Porto”, part of “ Universidade Católica Portuguesa”. The internship went from February 4th to March 29th, 2019, and from April 3rd to June 6th 2019, in a hospital center, in the local coordination group of control and prevention of infections and antimicrobial resistance (GCLPPCIRA), and in the multipurpose intensive care unit (UCIP), respectively. The methodology used is descriptive and analytical, through reflection on the developed activities and acquired skills. About the internship practiced at GCLPPCIRA, the main activities were the analysis and review of clinical files of some patients and also their epidemiological surveillance. There was also participation in various procedures related to infection control, with a close up in the preventing the infection of the surgical site, that being made through the analysis of the knowledge of some professionals in a surgical service and subsequent intervention as improvement of care provided. In regard to the internship performed at the UCIP, the main activities consisted of providing specialized nursing care to critically ill patients, participating in various procedures from invasive monitoring, hemodialysis, placing the central venous catheter, enteral and parenteral feeding, among others. Besides that, there was also a case study, with a close up on the theme of mechanical ventilation, more specific, the process of ventilatory weaning. All these phases led to the personal and professional development allowing better providing of nursing specialized care when approaching the critically ill patient, always keeping the focus on the minimization of infections associated with health care. It is up to the specialist nurse, on one hand, to act having his focus on the reduction of the rates of healthcare associated infections (IACS) and, on the other hand, to provide excellence in the care of critically ill patients, adopting a holistic approach, always involving family and / or other significant people in the disease process.

The Master's degree in Medical-Surgical Nursing is assigned to the holders of in-depth knowledge and expertise in their field of intervention, who demonstrate high levels of clinical judgment and decision making in a particular area of intervention (Nurses' Order, 2010). Thus, with the present report, was intended to describe the course of acquisition and development of Master's competences in Medical-Surgical Nursing, starting from the skills of Specialist Nurse in this area, combining them with professional experience and the academic / more recent scientific evidence, as well with the references of the Nursing profession. The description of this course, based on reflection and reasoned defense, was guided by Decree-Law 65/2018 of August 16, namely by the units of competence described and related to the academic degree of Master. Those reflection contributed to the consolidation of the skills acquired through post-masters and masters courses and, at the same time, to the awareness of the importance of continuous investment in personal and professional training. It was also emphasized the importance of project based learning as major driver in the acquisition of Master's skills in Medical-Surgical Nursing, at neurotrauma area, trough the project developed at this Master's Course , entitled "Promotion of the Safety of Patients Victims of Traumatic Brain Injury".

Full textThe clinical practice settings are by excellence places where the nurse mobilizes, integrates and combines multiple and heterogeneous knowledge. This ability enables the development a set of skills, which in case of clinical nurse specialists, are structured in four common domains: (i) professional, ethical and legal responsibility; (ii) continuous quality improvement; (iii) healthcare management and (iv) professional learning development. These were the domains that guided the critical and reflexive appraisal on the learning experiences developed during the traineeships, carried out in the Emergency Department and Intensive Care Polyvalent Unit of a Hospital in Northern Portugal, which served as basis for the creation of this report, which falls within the scope of the Curriculum Unit – Final Stage and Report, included in the 13th Master's Course in Nursing with a Specialization in Medical Surgical Nursing from the Critical Care Nursing field, at Universidade Católica Portuguesa. The objectives of this report are to present the skills development pathway of the clinical nurse specialists in Medical Surgical Nursing, as well the recognition of competencies in the area of Pre-Hospital Nursing, through the conceptual framing of the professional expertise of clinical nurse specialist, the reflexive analysis of the learnings developed on the training grounds and, finally, the evidence of competencies recognition in Pre-Hospital Nursing. In this regard, I resorted to bibliographic research and critical reflexive analysis of the formative journey undertaken in both training contexts. Having completed the journey, I consider that the reflexive analysis of the activities developed on the training grounds will allow future decision making close to the level of expertise that is required of the Clinical Nurse Specialist in Medical Surgical Nursing from the Critical Care Nursing field.

Full textWith this report, the author aims to point out the evolutionary input of each internship module throughout the Master Degree in Medical Surgical Nursing carried out at ESESJCluny. It also predetermined to provide prominence to the development of specific skills in the care of critical patients. The clinical teaching was incorporated into Internship I – Emergency Room (ER); Internship II – Intensive Care Unit (ICU) and Internship III – Optional. Regarding the Internship on the Emergency Room, I intend to highlight the acquisition of competencies in relation to the person in critical situation, according to the triage system, stabilization and resuscitation. Always taking into account the priority setting in the approach to the victim, considering the holistic care of both patient and family. In relation to the Internship on the Intensive Care Unit, the objective is to emphasize the development of competencies in providing care to the critical patient with imminent or established organ failure, without disregarding the relational aspect with the family. Concerning the optional Internship, I attempt to explain the implicit process that I went through to acquire the core competencies needed for the care of the patient with intestinal elimination ostomy, in preoperative and postoperative phases of surgery. Bearing in mind the prevention of complications and the preparation for discharge from the hospital setting priorities related to the specific needs of the ostomized person, thereby contribute to improve a healthier transition and adaptation of the person and the family to their new condition. This report is an illustrative essay in my career growth as a result of the addition of new skills and specialized knowledge in nursing, in consonance with the method and with the expected and achieved results. Attained specially through the direct contact with the patients and their families, the participation in healthcare and the interaction with different teams of healthcare practitioners. Thus showing the irreplaceable importance of internships. The report was fulfilled with a critical reflection on the common and also specific skills of the advanced practice nurse in Medical-Surgical Nursing, associated with the description of activities based on scientific and technical expertise. Supported by ethical and deontological values of Nursing, as well as Bolonha descriptors for the second cycle of studies which have clarified the key points of the author’s evolution, heading the Master approval in Medical-Surgical Nursing specialization.

Customer safety has become very important in recent years. The process of improving customer safety is complex and challenging (Galvão, 2009). As a perioperative nurse, the importance of safety in the delivery of nursing care is understood, especially to the person in critical situation. The care of the person in critical situation requires expertise, it is expected that the nurse specialist in Medical-Surgical nursing holds a set of specific skills that allow him to provide a differentiated, humanized, continuous and quality care, aiming at full well-being of the customer. It is on this premise that the objective of the above-mentioned report is based on the description and reflexive analysis of the course carried out and the acquisition of common and specific competences in the area of specialty mentioned. Such description is reported through the experiences experienced in the three clinical teaching, intensive care, urgency service and operative room. All this process was completed in the mobilization of specialized knowledge that will served as a guide to a conscious pratice, ethical, professional and quality,through behaviors requierd to a safe practice. The complexity inherent in clinical contexts requires, increasingly, a specialized practice based on the skills recommended for the nurse specialist and master in Medical-Surgical Nursing.

Full textThe present report, named “By the track of the expertise in medical-surgical nursing: from the telephone triage process and prehospital counseling to intensive care unit”, comes up from the Third Master Course in Medical-Surgical Nursing, of the College of Nursing of São José de Cluny, in the school year of 2018/2019. This report aims to demonstrate the development of common and specific skills in the medical-surgical area in urgent, intensive care and prehospital context; highlight the development of reflexive-critical analysis skills, arising from professional practice in my area of expertise, and its implications for essence of care, as well as expose the acquisition of Master’s skills through the presentation and discussion of this report. The Internship I ran in the Multipurpose Emergency Service of the Dr. Nélio Mendonça Hospital and the Internship II – Intensive Care was credited to me by the Technical-Scientific Council, considering the skills I acquired in the 12 years of professional practice at the Intensive Care Unit. The Internship III – Option was focused in the prehospital context: at the Emergency Line of the Azores Regional Civil Protection and Fire Service; in the Immediate Life Support Terceira of the Santo Espírito Hospital; and in the Emergency and Rapid Intervention Team of the Madeira Civil Protection Regional Service. The Option was based in my interest in deepening the emergent patient approach from the prehospital to Intensive Care Unit, as well as obtain a full view of the Emergency Medical System of the two Autonomous Regions. Organized into three chapters, this report emphasizes the acquisition and development of common and specific skills of the specialist nurse in medical-surgical nursing in the area of caring for the person in critical situation, towards the acquisition of Master’s in Medical-Surgical Nursing skills. I used the descriptive, analytical and reflexive methodology, in order to report the lived experiences, the setbacks found and the proposed solutions.

Full textThe person in critical condition and/or organ failure and in a chronic and palliative situation constitutes the focus of nursing care specialist (Nurses' Order, 2011). Thus, in response to a legal requirement set forth in order n.º. 8046/2015 of July 22, this report aims to give visibility to my course of building specific skills in this area of specialty, from a critical-reflexive analysis of the process which resulted in the acquisition of a specialized knowledge and consequently in the recognition as Specialist Nurse. Thus, the skills inherent to specialized care in Medical-Surgical Nursing were the guiding thread for the analysis of my professional development, based on the moments of learning lived in context of clinical teaching of the Master Course in Medical-Surgical Nursing of the São José de Cluny nursing school, namely in stage I (Emergency Service), stage II (Multi-purpose Intensive Care Unit) and stage III of Option (Pneumology Service). Performing a critical reflexive analysis on the same, i show my learning in the scope of the common and specific competences of the Specialist Nurse in Medical-Surgical Nursing. At the same time I analyze these same learning in a comparative relation with the profile of competencies required for the attribution of the Master's degree in Medical-Surgical Nursing, stressing the importance of a practice based on the most pertinent and current knowledge, based on the latest scientific evidence, in order to allow the resolution of complex problems in the specialized nursing care in a sustained way and in the light of the values of the nursing profession.

Full textSkills development enriches the ability of the nurse to intervene and promotes quality of care. Nurses, in the quest to enhance their skill set integrate learning processes and continuous training to enable them to meet the needs of the individual, its family and other significant individuals. The specialist nurse holds a repertoire of skills that are articulated, namely: instrumental skills; interpersonal skills and systemic skills that allow him to see the individual as a global being in interaction. This skill set is the basis for elaborating the theories developed during internship period. This report presents the nursing practices directed at the needs of the critically ill patients and their families, in the operating room and in an intensive care unit. From what was observed during the nursing practices, one could highlight a greater collaboration and involvement of the nursing team for family contribution during surgery. The clinical practices, because of their specificity, allowed the implementation of two different theories: in the operating room there was the need to use Peplau’s Interpersonal Relationships Theory as guidance when receiving patients; and Kolcaba’s Theory was used to analyze the therapeutic bath in the Burn Unit in order to conduct activities in the balneotherapy room. Applying these two nursing theories enriched and broadened the field of view regarding the intervention of nurses in these contexts. The nurse specialized in the medical-surgical critically ill patient has a prominent role in promoting and implementing a quality of care practice, either by using his specialized knowledge in patient’s illness or by specific skills that he has and that allow him to be aware of the needs of the individual.

Full textThis report arises in the scope of the Master´s Degree in Nursing with a specialization in Medical-Surgical Nursing, in the field of care of the Critically Ill Patient Nursing, at Institute of Health Sciences–Porto, of the Universidade Católica Portuguesa and the aim is to make a report of performed activities and developed skills during the 2nd year, in the setting of Health Care Associated Infection and Intensive Care internships between 02/11/2020 and 27/02/2021, making use of my previous first year Emergency Department experience. In the first part of this document I approached the common skills of the specialized nurse according to Ordem dos Enfermeiros, with all the COVID-19 related challenges in the background. About ethical accountability, I talked about possible changes in the focus of care and potencial dilemmas. In the subject of quality of care, I focused on the role of management and leadership. I also delve into aspects like resiliency, self-care and team capacitation towards the pandemics. In the second part of this work I approached the specific skills having the first year Emergency Department internship as a starting point, developing skills on management and emergency situations, as well as with literature review about oxygen therapy and air humidifier utilization. Next, I mentioned the development of skills in the setting of the Local Coordination Council-an optional field -, in several interventions like literature review on reusable vs disposable surgical gowns, which pointed to a strong recommendation towards the former, and in an improvement opportunity with a poster about transesophageal endoscopes decontamination. Concerning Intensive Care, it was important to center my attention in the aspects related with communication skills, sedation management, delirium, pain and infection control. After the path done one can conclude that the specialized Nurse has to have the ability to caring of individuals in critical health status and organ failure in a global and holistic fashion, knowing how to handle with complex therapeutic protocols and differentiated techniques, always with humanization awareness. This course allowed me to recognize and develop communication, leadership, management, teaching, clinical literature searching and critical thinking skills.

Full textThis report was written based on the criteria presented on the study plan of the 13 th Master in Nursing with Specialization in Medical-Surgical Nursing, at the Escola de Enfermagem (Porto), Instituto de Ciências da Saúde da Universidade Católica Portuguesa, representing the course unit “Final Stage and Report”, that happen between a period of twenty weeks, from 07/09/2020 to 08/02/2021 of the current academic year. Before each placement, I have structured a project, and considering this, the purpose of this essay is to document the experiences and skills acquired in different areas – a general intensive care unit, located in the north of the country, and a Pre-Hospital Assistance at the National Institute of Medical Emergency. It was used an analytic and descriptive methodology but, mainly, a critical-reflexive one, to give emphasis to the entire training and learning processes, towards nursing specialization, with all the inherent skills, in particular, problem resolution, management, training, research, guidance, and differentiated care based on current evidence for critical patients. Structurally, this report is organized according to the common domains of the nurse specialist recommended by the Portuguese Nursing Council in 2019: Professional responsibility, ethics and law; development of professional skills. In conjunction with this, another chapter was created regarding specific and important skills of the Nurse Specialist in Medical-Surgical Nursing due to the enhancement that they give to my profile as a specialized care professional. Cross-sectional to the entire placement, this report shows my proactivity in search for knowledge, and the ongoing improvement of my clinical assessment, critical thinking and judgment, which is essential to the formation of diagnosis and problem resolution. Care was provided to the critical patient and his family that were experiencing acute health disease transitions in several urgent and emergent situations that were vital to the acquisition of specific skills. I will also highlight the attainment of training moments through the identification of education needs about current and relevant subjects for the improvement of practice and excellence of care that is provided, offering in this way my positive contribution to the multidisciplinary team and to the work placements. In this context, this is a report created on the basis of an introspective and retrospective reflection constituting itself in a learning tool. I consider that the initial objectives were achieved, and the conclusion portrays, in a parallel form to the all document, the culmination of a difficult but productive path with constant improvement and acquisition of skills, abilities, knowledge and competencies related to the care of the person in a critical situation, in the sphere of care in Medical-Surgical Nursing, with expertise and mastery.

Full textUncertainty in disease, intrinsic to the very process of illness, is assumed to be a major disorder of human homeostasis. The critical, unexpected, sudden and life-threatening disease assumes this role, leading by itself to the phenomenon of Uncertainty in a more perennial way. In addition to the disease per se, the clear impact of this reality in the client’s life project, its decision-making and coping capacity justifies an in-depth study in this field. Such investigation is essential to all those seeking a rigorous care planning of their differentiated interventions in the field of nurse specialty. The expected outcomes lead to effective health gains for the client and/or their families. A scoping review identifying the determinant factors of Uncertainty, as well as the facilitators of the management of Uncertainty in critical family members admitted to intensive care unit, is the central topic of the research presented in this report. Its conception starts exactly from an evolutionary perspective at the level of the “to be with”. In function of the person in critical situation, we present, reflect and analyse the practice and skills (developed or acquired) in the line of “to know”, “to know how to do” and “to know how to be”; in order to take full care of the Human Being expressed into a set of responses bio, psycho, socio, cultural and spiritual. In this process, we systematically support ourselves in the Relation of Help, without which techniques are merely procedures without a therapeutic dimension.

Full textThis report fulfilled within the Master's Degree in Nursing with a specialization in Medical-Surgical Nursing, from the Instituto de Ciências da Saúde, Universidade Católica Portuguesa, illustrates the path taken during the internship period, in two intensive care units in one hospital in the center of the country. This document aims to describe in detail the experiences acquired, the obstacles that emerged and the interventions provided. Integrated in the provision of care, the humanization emerges as a constant challenge for nurses because it projects the holistic view of the patient in their professional practice and, therefore, encompassing in their interventions the totality of the individual's dimensions: bio-psycho-social- cultural and spiritual. Adding to the provision of care, based on a holistic view of the patient, we must add the recognition and respect for the fundamental rights of the human being, which is the case of the imperative principle of human dignity transversal to each and every individual and, therefore, to all the sick. My path was dedicated to the acquisition of technical, scientific, human and relational skills, where the moments dedicated to reflection with bibliographic research were crucial points to deepen and improve the theme of care delivery to patients in critical condition. For the preparation of this report, a descriptive, analytical-reflective methodology was adopted, directed to practice in the different clinical teaching environments. With this report I intend to entrust readers with a spectrum that demonstrates, a set of skills acquired and developed by a nursing specialist in the medical-surgical area and whose pillars are: autonomy, initiative, sense of responsibility and, fundamentally, a critical-reflective spirit. The nursing specialist in medical-surgical nursing must be attentive and audacious to the different dimensions of care for people in critical situations and their families.

Full textMaster's Degree in Nursing with Specialization in Medical-Surgical Nursing aims the acquisition of skills for advanced nursing care for the adult or the old patient with severe and critical illness. For this, it were done three internship modules, explained throughout this report. The report was prepared based on the descriptive and reflective method and aims to present objectively: the activities developed; reflection on the attitudes taken during the probationary period; as well as the competences that resulted from this learning process and the competencies to obtain the master's degree in Nursing with Specialization in Medical-Surgical Nursing. The internship was held in three different services: Emergency Service, Multipurpose Intensive Care Unit e Regional Coordination Located of the Program for Prevention and Control of Infections and Resistance to Antimicrobials. In the Emergency Service, I contacted with urgent / emergent cases, which it was necessary to identify the problems in an useful time and to establish priorities in the specialized nursing actions. The Multipurpose Intensive Care Unit refers a place of high complexity that forces to the strict supervision and continuous monitoring of critical patients. These both modules allowed me to acquire competencies related to the care of the person to experience complex processes of critical illness and / or organ failure. The internship in the Regional or Located coordination Group of the Prevention and Control of Infections and Resistance to Antimicrobials allowed me The development of the subject of infection control. In this module, i also checked for compliance with standards and i developed skills to maximize the intervention in the prevention and control of infection in the face of critical and / or organ failure. The professional learning results from the diverse situations identified during the internship. So, for the Module I, I created a poster for the activation of the Sepsis “Green Way”. In the module II, I run the project for the implementation of the Nursing Activities Score. in the both modules and after the creation of evaluation grids for the hand hygiene procedure, insertion and utilization of Vesical Catheter and insertion and utilization of Peripheral Venous Catheter, and lastly having carried the statistical analysis. For the last module, I developed the booklet flyer prevention of Hospital infection: Hospitalized patients and visits and the change to manual infection control. This internship provided a very interesting experience and it was possible to demonstrate the active role that the Specialist Nurse Practitioner in Medical-Surgical Nursing, in order to foster good practice, contributing to improve the quality and efficiency of care provided, taking into account a practice based in the evidence.

Full textThe present Report illustrates the path taken along the Probation of the second Master Degree in Nursing, and has referential integrity for the conclusion of the Specialization in medical-surgical Nursing. It fits in the Curricular Unit Report, and it constitutes a critical and reflective written testimony of the developed activities to acquire and to expand specific abilities in that area. The Probation consisted of a total of 750 hours divided in three Modules so that the developed abilities had contributions of distinct realities, such as the services of Surgery (Unit 2) and Emergency of the Hospital of Santo António, included in the Centro Hospitalar do Porto, and the Unit of Special Care/Burns of the Hospital of Prelada. The main goal of this document is to describe the activities developed in each module of Probation and demonstrate the acquisition of specific abilities in the area of medical-surgical Nursing. In order to achieve it this Report is divided in two distinct parts. In the first one it is described the process of development of abilities, through the exposition of the developed activities and its integration in the Project of Probation traced initially. In the second part it is proven the reflective capacity through the presentation of an innovative perspective on the role of the nurse specialist in medical-surgical Nursing: it will be the professional who not only steps in to attenuate the critical situations but, above all, to prevent them. On the other hand it evokes the plan of health of the critical patient as an ethical-deontological referential. The document culminates with the presentation of health indicators sensitive to the care of the nurse specialist in medical-surgical Nursing. Full textThis report was created within the scope of the curricular unit of the Master's Degree in Nursing, integrated in the Master's Degree in Nursing, with Specialization in Medical- Surgical Nursing, of the Universidade Católica do Porto do Instituto de Ciências da Saúde, after completing the internship in the Service of Urgency, Unit of Intensive Care of Cardiothoracic Surgery and in the service of Palliative Care in two hospitals of the northern zone of Portugal. It translates the development, acquisition and integration of knowledge for the provision of specialized care in the area of Medical-Surgical Nursing to the adult and elderly person with severe or critical illness, chronic with incapacitating and palliative disease. This report aims to: describe the activities developed throughout the internship modules; demonstrate the acquired skills, as well as the identification of health problems and the development of effective solutions; reflect on acquired skills, identify health problems and develop effective solutions; to obtain the Master's Degree in Nursing with Specialization in Medical-Surgical Nursing. It is structured considering the four Competency Domains Common to Specialist Nurses. Was used the descriptive method, always considering a critical and reflective analysis of the activities developed based on the scientific basis. The improvement opportunities found in the internship modules were translated into the work "Care of the Thoracic Drain" in order to standardize the care in the Emergency Department. It was considered relevant to understand coping strategies and difficulties faced by nurses towards death in the different modules, having applied the questionnaire "The nurse towards death”. Throughout my course I wrote a book entitled "The Experiencing of Death", where I present a theoretical framework of this subject and then the processing of data resulting from the analysis of the questionnaire. Finally, and based on this work, I wrote the article entitled "Coping strategies used by nurses to deal with the dying process". This article has been submitted to the magazine Palliative Care and was accepted for publication. With the preparation of this report, I hope to be able to clearly convey my experience at this stage, demonstrating the sharing of knowledge.

Full textThis report has a double purpose, first to describe the route taken in the context of traineeship modules recommended by the curriculum, “Professional Nursing Master’s degree in Medical- Surgery Specialization”, by the Institute of Health Sciences, Portuguese Catholic University and second to evidence the skills development of Nurse Specialist from this route, which will be mobilized for my professional praxis. The traineeship featured three separate modules, which were performed at the Centro Hospitalar de Setúbal, EPE, by the following chronological order: Module III (Geriatric Nursing – optional nature), held in the context of professional practice Orthopedic I of Hospital Ortopédico Sant'iago do Outão; Module I (Emergency Service) and Module II (Intensive care/Intermediate) both performed at Hospital de São Bernardo, in the Emergency Service and Intensive Care Service, respectively. During the course of the three modules of traineeship, I chose to direct my learning towards the family / informal caregivers of elderly hospitalized clients. This choice was based on the new challenges in healthcare, particularly due to the phenomenon of population aging, the contingency and social changes. In Module III - Geriatric Nursing, the activities developed focused on planning and implementing the project “Caring to a household - a tripartite partnership” promoting Education in Health through the interaction elder client/ informal caregivers /nurse. Besides this project, I drew up a training activity and a poster regarding “Pressure Ulcer Treatment”, directed towards the uniformisation of nursing procedures. On the other hand, Module I - Emergency Service, the development of this traineeship was mainly under the care of the family / informal caregivers of elderly clients in urgent / emergent situations. In regard to the service’s needs, I organized the training activity “Approach to politraumatized client: techniques in trauma”, as well as a manual of procedures concerning the politraumatized client in emergency. My aim was to make nursing professionals aware of these good practice guidelines. Finally, in Module II, I promoted the development of skills related to “Communication of Bad News”, minding how limited we are as professionals in these issues. I also established a procedure guideline “Regarding welcoming the family in Intensive Care Service”, with the purpose of optimizing the process by involving all the intervenients. The accomplished projects, in the course of the traineeship modules, has allowed the development of skills in approaching the client/family, taking into account it’s complex needs, as well as implementing projects capable of contributing to better Nursing healthcare

Full textThis paper aims to describe the training course held in the Stroke Unit at the Hospital do Espírito Santo de Évora, EPE under pedagogical guidance of Professor Raul Cordeiro, which took place in the period of 1st November 2015 to 30th April 2016. The methodology used for the completion of the report was the descriptive, analytical and reflective methodology that allowed me, through a narrative character, to objectively describe the experiences, the problem-situations encountered, the solutions proposed and the difficulties during the implementation stage. The objectives of this document is to report and describe the experiences in the practice of care, the activities and the contexts in which they were committed, analyze them and reflect how they contributed to the development of knowledge and expertise in Medical-Surgical Nursing in care to the person in critical condition with a diagnosis of stroke in the area of Health Units Management. Full textThis report describes the course through the Internships in a General Intensive Care Unit and a specialized Intensive Care Unit. It is organized according to the common competency domains of specialist nurses intersecting with the specific competencies of the Specialist Nurse in Medical- Surgical Nursing, more specifically in assisting the person in critical condition. A critical-reflexive methodology was used in the analysis of the activities performed, based on the actions of the Specialist Nurse, addressing the legal, moral and deontological framework, the issues of differentiation, value added and excellence. The focus was given to the organizational aspects to support care management and continuous improvement of quality, with the reflection, in an article, of the use of the Nursing Activities Score Scale in Intensive Care clinical practice; proposals for improving the usability of Nursing information systems; planning of a project to implement a patient 's diary in intensive care, execution of a case study in the context of oncologic intensive care on a patient undergoing esophagectomy and participation in continuous quality improvement activities such as audits in the scope of the Stop Infeção project. We believe that in the short and medium term, those activities will impact beyond the period of the specialty internship, obtaining the benefits of participating in training activities, thus the clinical settings taking advantage of the trainees' contribution. The potential for greater use of the skills, competencies and information produced by specialist nurses, their impact on the quality of patient care, innovation and critical thinking can contribute to a broader recognition of the specialty in Medical- Surgical Nursing, toward excellence in care.

Full textNursing can be seen as a science, discipline and profession seeking to respond to the challenges of a society in constant change and progress, by developing knowledge and providing training to appropriate and timely responses to new requests. The rising care complexity implies nurse’s continuous learning; to enable you to make more and more demanding decisions about the care delivered to the person, family and community. Nursing is increasingly demanding and complex. The radical change in the health system, the severe shortage of nurses in recent years, the aging population and people to make use of health services increasingly in critical condition and for longer periods will have implications in nursing. The nurse responds to people more demanding and nurses must take a more active role in decision-making, greater accountability and autonomy (Patrick in Howard, 2011). In the social role that the nurse holds and exert, in this case for Nurse Specialist in Medical-Surgical, constant updating of their knowledge and professional development is required. Based on a logical recognition of a set of skills, professional experience based on reflective practice and formal moments of knowledge acquisition sustained in evidence-based practice can thus assure the quality of nursing care. This report presents, in the 1st half, a systematic literature review and, in the 2nd half, a reflective analysis of the course curriculum and skills developed in response to the needs of the person and family in critical conditions in the context of an emergency service center and in an outpatient palliative care. The theoretical pillars that supported my interventions were based in the Transition Model by Afaf Meleis (2000) and the Theory of Comfort by Kolkaba (1994). The medical-surgical nurse specialist assists people in transition, especially in critical illness situations. Throughout the internship, the person / family was valued in a unique and singular way, providing comfort care at all moments, influenced by the great currents of thought and by a changing world (Paradigm Transformation)

Full textThe present report was developed within the Final Internship of the 13th Master's Degree Course in Medical and Surgical Nursing, which was focused on Critical Care and took place in Oporto’s Institute of Health Sciences of the Universidade Católica Portuguesa. It intends to show the development and acquisition of new skills that provide the consolidation of knowledge and know-how of a Specialist Nurse. The report was organized into four chapters: the chapter on the development of competences, was structured according to the four competence domains, based on the descriptive, analytical and critical-reflective methodology. The first one, corresponding to the introduction, represents the framework and objectives of the attended internships as well as of this report. In the second chapter, are describe the characteristics of the different internships was made, since only knowing the placement in which we are practising it is possible to provide quality nursing care. The third chapter concerned both skills development and the objectives set for the different internships. Being the fourth and final chapter, the conclusion includes a retrospective of the entire learning process. Regarding the professional, ethical and legal responsibility domain, the emphasis was placed on the professional practice, which must always be ruled by the code of professional ethics, as well as by the ethical principles of autonomy, justice, beneficence and non-maleficence. The domain of continuous quality improvement was based on one of the specific skills of a Specialist Nurse in Critical Care; in this area, it is necessary to act effectively and efficiently to achieve quality and safety of both nursing care and therapeutic environment. It was possible to work on questions that allow us to reflect on the proactive role that the Specialist Nurse must have, incorporating in their practice new knowledge based on scientific evidence, aiming the improvement of care. As to the care management domain, the main interest was to understand the management behind a care unit, as well as the importance that the Specialist Nurse has in this particular area. Concerning the domain of development of professional learning, problem situations inevitably arose during the specific internship here mentioned. These guided the performance during the internship, as they incited the research of new knowledge and the systematization of the previously acquired, being both fundamental contributes to decision making. Caring is a dynamic process that implies attention, dedication and learning, as well as a symbiosis between the one who cares and the one who is cared for. It is from this need for Naring that nursing emerges. Only a Specialist Nurse in Critical Care, with their adjacent skills, will be able to give an adequate response to the person and the person’s family experiencing complex medical and/or surgical processes. The Specialist Nurse plays a decisive role in providing specialized care to critically ill patient.

Full textThe present report, elaborated as part of the Master's Course in Nursing with Specialization in Medical-Surgical Nursing in the area of Nursing for People in Critical Situation, from the Institute of Health Sciences - Porto, Universidade Católica Portuguesa, reflects the path taken during the internship in two contexts: urgent care and extra-hospital emergency, from November 2020 to February2021. This document objective is to describe the experiences, difficulties and interventions implemented based on the skills of a specialist nurse in medical surgical nursing. This report was written following a descriptive, analytical and reflective methodology directed to practice. The approach to patients in critical condition in intensive care enriched me as a future specialist nurse, contributing to the development of my skills. Humanization, being transversal to different contexts, emerged as a constant challenge in daily and professional nursing practice, therefore a literature review has been performed on the topic. A more humane perspective needs to be considered for everyone involved in the health care process, from the sick person to the health professional. There are several challenging areas that require change in order to achieve a more humane process, like: waiting time management; overcrowding services and their organization; health care professionals valorization. An early approach to this theme in schools of higher education could promote these changes and allow for better care. My path allowed me to acquire technical and scientific skills as well as humane and relational skills through moments of reflection. This report outlines the objectives that I set out to achieve and what I actually achieved during my formative journey and that presented significant importance.T he fact that I focused on the humanization process became one of the key points of my journey, perceiving the role of the specialist nurse in medical-surgical nursing to improve the care provided to people in critical situations.

Full textCardio-respiratory arrest is one of the most relevant situations in the context of urgency / emergency, whose multidisciplinary action is preponderant. The specialist nurse, as a facilitator of learning in the work context, has a solid base of knowledge and specialized skills, which allow him to contribute effectively to the promotion of quality and safety in health. The Intervention Project followed the project methodology and come from the training need evidenced in the Training Plan for a Basic Emergency Service, validated by a systematic review about the formation and training of Basic Life Support, for health professionals. This is an area of knowledge, in which the training content must be adequate to the expected action of each professional group (clinical and non-clinical), as a tool for the acquisition of both cognitive and social skills. The Intervention Project materialized in a training session, based on the simulated practice of Basic Life Support, addressed to the Operational Assistants of that Service. The formation and training of Basic Life Support improves the performance of health professionals in the event of cardio-respiratory arrest, on the one hand, by improving skills, on the other hand, it favors the motivation for performing resuscitation maneuvers. This document reflects in a critical and reflective way the path of acquisition and development of skills of a specialist nurse in Medical-Surgical Nursing to the Person in Critical Situation and Master of Nursing skills, fundamental to the attribution of title and degree, anchored in the theoretical and of clinical practice

Full textThe present report intends to describe the journey made during the internship period, seeking to highlight the learning process and the development of skills regarding the specialist nurse, along with a continuous reflection exercise about the developed activities, the experience of the clinical practice and the experienced difficulties, developing a critical vision of the events and having the comforting communication process as guiding theme, which I tried to make transversal to the three modules and having as main aim, the specialized healthcare to the patient and his/her family. The healthcare provided to the person in critical condition and its family in different learning contexts, allowed the development of specialized healthcare skills as well as it raised the team’s awareness for the importance of communication and promoted the comfort of the patient in critical condition through a dynamic intervention. The specialized healthcare provided to the person with chronical pain and its family, promoting the osteoarticular and cancer pain in the Chronic Pain Unit, led to the elaboration of a guiding handbook about comfort postures regarding the spinal pathology, which has been recognized as a comforting instrument. Finally, the importance of communication as a comforting strategy has been strengthened in relation to the General Emergency Service, through the elaboration and application of a communication skills evaluation checklist of the nurses with the patient / family and by doing a training session to present the achieved results. This report is the result of the learning, personal and professional growing processes, promoting the experience based on evidence, directed to the quality, humanization and security of the healthcare provided to the patients and their families in the different contexts.
