This study examined the degree to which an individual's propensity to experience emotional distress and restraint are predictive of grief intensity in a sample of older widow(er)s ( N = 51). Characteristic distress was found to be a significant predictor of present grief intensity, whereas repressive defensive-ness was found to predict past grief, as recalled from the time of loss. Grieving often begins at the time of cancer diagnosis, continuing as the patient faces numerous role and functional losses, surging when death is anticipated, and typically manifesting most intensely for the relatives after the patient’s death. This chapter focuses on how providers may support those who are grieving, describing common grief reactions, risk factors for poor bereavement outcomes, and interventions that have garnered empirical support in the growing grief intervention evidence base. Supportive psychotherapy normalizes grief, educates about adaptive reactions to loss and change, explores any maladaptive coping, and assesses for complicated grief. Psycho-oncologists can continue family-centered care through bereavement by providing empirically supported grief interventions or making appropriate referrals when there is a need for more specialized grief support. The strongest intervention effects are likely to be seen when targeting high-risk or symptomatic individuals at least six months postloss. Given the substantial variability in the quality of grief intervention research, future clinical trials should utilize rigorous methods, while remaining mindful of ethical research practices with vulnerable bereft individuals. Notably, there has been limited research focusing specifically on those bereaved by cancer, who often have traumatic experiences and vulnerabilities. In addition, more resources are needed to implement evidence-based bereavement care in oncology settings.

The human experience of loss and grief will touch us all. Grief is a deeply personal experience that will be influenced by culture, spiritual belief, the nature of the loss, gender, and the relationship with the deceased, whether it be as an individual or within a family setting. Grief has been of particular focus in the literature, given the increasing incidence of terrorism and subsequent loss via violence and trauma. Research in this area has a long history and is still evolving, with recent focus on the pathology of ongoing grief and its inclusion within the Diagnostic and Statistical Manual of Mental Disorders, fifth edition (DSM-5). The majority of individuals demonstrate resilience and do not require intervention. This is less so when suffering and distress are acute and ongoing, and there are significant health impacts or functional impairment. This chapter discusses the phenomenology of ‘normal grief’ and prolonged or complicated grief, risk and protective factors that may influence the course and outcome such as trauma and the nature of the loss, and possible physical and mental health consequences in the context of current assessment and models of management.

In the past there was an impression that they appeared expensive, which resulted in a lack of enthusiasm to develop them by funding bodies and commissioners. However, the evidence demonstrating the long-term cost-effectiveness of rehabilitation is robust. Many people with long-term neurological conditions will live for many years after the onset of the condition and investment in their physical and psychological functioning early on will, over that person’s lifetime, will result in substantial savings. Nevertheless, calculating economic evaluations can be complicated and the correct measure must be chosen to identify the change produced by the rehabilitation intervention. These data must then be handled appropriately, and any ancillary costs included. The economic impact of the rehabilitation programme is wider than a purely healthcare intervention and will include potential earnings and reduced costs to social care. The economic analyses will also include housing, education, and vocational outcomes, and the effect of the long-term condition on family members who may have a caring role.
