Recently, new long-acting formulations of racemic methylphenidate (MPH: Ritalin® LA, Metadate® CD and Concerta®) and amphetamine (AMP: Adderall XR®) were developed and are now approved by the Food and Drug Administration (FDA). In addition, dexmethylphenidate (Focalin ™), the pharmacologically active d-threo enantiomer of MPH, also was approved by the FDA. In the initial phases of development, prototypes of these five new formulations were evaluated using the University of California, Irvine (UCI) Laboratory School Protocol (LSP), in which surrogate measures of efficacy are collected in highly controlled settings rather than clinical measures of effectiveness in the less-controlled, natural environments of home or school. The LSP studies were followed by large effectiveness and safety studies required for gaining FDA approval. These initial efficacy and side effect studies in the LSP provided missing information about the basic pharmacokinetic (PK) and pharmacodynamic (PD) properties of MPH and AMP and produced some new discoveries (i.e., acute tolerance) that were used to help design the final products. The final once-a-day formulations used different drug delivery systems to achieve long-acting efficacy (Ritalin® LA, Metadate® CD, Concerta®, Adderall XR®). All four drug delivery systems were based on two processes: first, a bolus delivery (BD) process to achieve rapid onset of efficacy (mg), and second, a controlled delivery (CD) process to achieve rates of delivery (mg/hr) or a delayed bolus (mg) to maintain efficacy. A theoretical approach was used to compare and contrast the new once-a day formulations of MPH by selecting total daily doses (mg/d) that would equate drug delivery by the first process (mg of the Initial bolus) and the second process (mg/hr over specified time period). In addition to efficacy, applications of the LSP to measure common side effects related to eating and sleeping were described and discussed.

As demandas judiciais por medicamentos não dispensados pelo Sistema Única de Saúde (SUS) geram consequências orçamentárias preocupantes, pois os recursos são limitados e seu gerenciamento deve ser executado segundo as normas das políticas de saúde. Um dos medicamentos mais solicitados judicialmente para a população infanto-juvenil diagnostica com o Transtorno de Déficit de Atenção e Hiperatividade (TDAH) é o metilfenidato, princípio ativo da Ritalina e do Concerta. Neste contexto, o objetivo geral do trabalho foi investigar a relação entre diagnóstico, judicialização do TDAH e possível oneração do erário público com a prescrição desse medicamento. Para alcançar o objetivo proposto, a pesquisa envolveu investigação quali-quantitativa. Os achados evidenciaram a oneração dos cofres públicos com a judicialização do TDAH e a presença de diagnósticos falso-positivos na amostra.

O uso de medicamentos, por pessoas saudáveis, para melhorar o funcionamento do cérebro e aprimorar o desempenho cognitivo pode ser chamado de aprimoramento cognitivo farmacológico ou de doping intelectual. Um dos medicamentos mais utilizados no Brasil com esta finalidade é o Cloridrato de Metilfenidato, comercializado com o nome de Ritalina ou Concerta. O presente artigo objetivou compreender, por meio de uma revisão de literatura, como o cloridrato metilfenidato tem sido utilizado no aprimoramento cognitivo farmacológico e a relação deste fenômeno com a medicalização social. Sabe-se que estes medicamentos tem como efeito imediato o aumento da concentração, mas a longo prazo podem causar supressão do crescimento, aumento da pressão sanguínea, episódios psicóticos, entre outros. Ainda assim, eles tem sido utilizados por estudantes universitários com a finalidade de aumentar sua capacidade produtiva e cumprir prazos e metas. Como esta prática tem ganhado cada vez mais adeptos e o acesso a este medicamento é facilitado, é necessário aprofundar o entendimento deste fenômeno.

Abstract:
The article highlights the methods of working on a Ukrainian piano concert with students-pianists in the lessons on "Instrumental Performance (Piano)" in higher education institutions. The stages of the formation and development of the Ukrainian piano concert are presented: the end of the 20s-30s of the XX century – the origin and formation, the 40-60s – the development period, the 80-90s – the heyday of the concert genre. The best examples of this genre in the works of domestic composers of the past and the present are analyzed, namely, piano concerts by L. Revutsky, M. Skorik, V. Ptushkin. It has been proved that the positive qualities of the piano concerto No. 2 of L. Revutsky consist in the organic development of Ukrainian classical traditions, in the deep embodiment of elevated, cheerful moods, lyrical-excited pathetics, and strong-willed images. The piano concerto No. 1 by M. Skorik is characterized by an artistic-figurative emotionality combined with brilliant virtuosity and is designed for vivid external effects. Marked by neoclassical style, M. Skorik’s concert is built on the free competition of a virtuoso pianist with an orchestra and refers to the type of concerts with relatively equal parts of solo and orchestra. When performing the interpretation of the first part of the piano concert of V. Ptushkin, the main attention is paid to the nature of the performance of the main themes, namely the contrast and dialogue between them. It was found out that acquaintance of students-pianists in the instrumental performance classes with the genre of the Ukrainian piano concerto expands the knowledge of future music art teachers in developing this genre in our country. Processing the best examples of piano concerts by composers of the past and the present develops a sense of ensemble playing among students, influences timbre hearing, introduces composers' innovations in the fields of style, musical language, means of shaping, as well as a wide emotional palette of musical images. The concerts focused on the functions of a soloist and orchestra, the artistic content of works, shaping, composer innovations in the field of musical language.

Objective: There are no clear guidelines for managing the dental treatment of children with attention deficit hyperactivity disorder (ADHD). The use of sedation in combination with chronic ADHD medication use is also not well defined. This study surveyed the prevalence of ADHD children, management techniques and knowledge of pharmacologic therapies of these children among Israeli dentists. Study Design: A specially designed questionnaire was distributed to all Israeli dentists attending a national conference in 2016. Results: Of the 160 dentists who attended the conference, 96 completed the survey (60% response rate), and they included 46 (51%) pediatric dental specialists and 50 (49%) general dental practitioners. The medications Ritalin and Concerta were most familiar to the respondents (98.9%). Eighty-seven (91.1%) of the practitioners responded that their ADHD patients take their usual doses of any drug for treating ADHD symptoms, regardless of whether or not the dentists intended to use sedatives. The practitioners invented their own behavior management techniques with varying degrees of success. Conclusions: There are no specific guidelines for the most effective pharmacologic protocol (co-administration of ADHD drugs and dental sedatives) or behavior management techniques for the provision of optimal dental care to children with ADHD.

Aims and MethodTo investigate the effectiveness of switching children with attention-deficit hyperactivity disorder (ADHD) from immediate- to sustained-release psychostimulants (Concerta XL, a novel methylphenidate hydro-chloride) and to examine factors associated with treatment success or failure. This was a retrospective study of all such children known to four clinicians in Lincolnshire, over a 2-year period. The initial response to treatment and the response to slow-release psychostimulant as judged by the clinicians were recorded. Data were analysed using the Statistical Package for the Social Sciences version 12.ResultsOf the children who were switched (n=97) and on whom clinical judgement was available (n=92), a statistically significant number (32%) responded poorly (P<0.001). In 26 out of the 97 patients, the switching was considered as a treatment failure and they were switched back to the original immediate-release stimulants. There was no significant-difference with possible confounding variables between children who responded well and those who responded poorly after switching to sustained-release drug.Clinical ImplicationsIn a real-life clinical situation there is a significant failure rate when a child with ADHD is switched from an immediate- to sustained-release psychostimulant. There is evidence of association between bruxism and the increasingly common central nervous system stimulants prescribed for attention deficit hyperactivity disorder (ADHD), as well as the selective serotonin reuptake inhibitors (SSRIs) often prescribed for depression or anxiety. However, the evidence is not clear on whether these medications inducing bruxism are directly associated with temporomandibular joint disorder (TMD). The aim of this work is to evaluate whether these medications are associated with TMD symptoms.Methods. Medical history and participant data were obtained for 469 patients from the University of Pittsburgh School of Dental Medicine, Dental Registry and DNA Repository, dating back to 2006. The chi-square test was used to determine any statistically significant associations.Results. There were no statistically significant associations between ADHD stimulant medications or SSRIs and reported TMD symptoms. However, there were significant differences seen between specific brands of medications and reported TMD symptoms. Individuals prescribed methylphenidate (Concerta) were less likely to report temporomandibular joint discomfort (p=0.01). Conversely, individuals prescribed citalopram (Celexa) were more likely to report temporomandibular joint discomfort (p=0.04).Conclusion. Signs and symptoms of temporomandibular joint dysfunction may be influenced by the use of certain medications prescribed for depression or attention deficit hyperactive disorder.

Abstract:
Objetivo: Observar o perfil de uso de metilfenidato e correlatos (MFC) entre estudantes de medicina e avaliar variáveis relacionadas a qualidade do sono e prática de atividade física. Métodos: Foi realizado um estudo transversal com 180 estudantes de medicina da PUC Goiás via formulário online em maio de 2020. Para análise dos dados foram empregados os testes do X2 e teste exato de Fisher, utilizando o software Past versão 4.03. Resultados: A prevalência do uso de MFC foi de 23,3%, sendo 38,9% (n=16) do sexo masculino e 61,9% (n=26) do sexo feminino. Observou-se que 57,1% (n=24) tiveram diagnóstico médico com equivalente prescrição de algum dos fármacos, enquanto 42,9% (n=18) fizeram uso não prescrito. Os medicamentos comerciais citados foram: Ritalina®, Venvanse®, Concerta® e Adderal®. Os principais efeitos adversos relatados foram insônia (62,1%), taquicardia (58,6%), ansiedade (51,7%), alteração do apetite (51,7%), estresse (41,4%), tremores (41,4%), boca seca (34,5%) e abstinência (17,2%). Conclusão: Conclui-se que o uso de psicoestimulantes cerebrais sem prescrição médica foi elevado. Devido as reações adversas desses fármacos, existe risco à saúde dos estudantes que fazem seu uso.

Our interest in the position and significance of the concert music of Ivan Jevtic in the development of this genre in the Serbian music of the second half of the 20th century is based on several facts. Judging from the number of concerts and the variety of their stylistic, aesthetic, technical, expressive and historical qualities, Jevtic comes across as a composer who was the pioneer of several particular and general tendencies in the development of Serbian concert music, especially when we have the following in mind: his relationship to the musical heritage; his aspirations to master new contemporary tendencies; the time of general stylistic turmoil; compositional techniques etc. The comparative analysis of three concerts: Concerto for Tuba, Cello Symphony and Concerto for Viola points to the basic elements of traditional heritage, his reshaping of this heritage, as well as the elements of modern musical expression in the works of this composer. The analyzed paths of a part of his entire oeuvre do not exemplify the complete development of the concert genre in the Serbian music of the second half of the 20th century but, alongside with other important aspects such as historical, aesthetic, technical etc. they assist us in a better understanding of the tendencies in the development of this genre in Serbia, but also of the tendencies in Serbian music after 1945.

Zusammenfassung: Fragestellung: Untersuchung der Wirksamkeit und Verträglichkeit einer Umstellung auf OROS®-Methylphenidat (OROS®-MPH, Concerta®) bei Kindern und Jugendlichen mit ADHS. Methodik: Prospektive, multizentrische, offene Studie über 3 Wochen mit 213 Kinder mit ADHS im Alter von 6 - 16 Jahren, die von einem kurzwirksamen MPH-Präparat auf OROS®-MPH einmal täglich in flexibler Dosierung umgestellt wurden. Primäre Studienendpunkte waren Veränderungen der klinischen Symptome, erhoben mittels der Subskala Unaufmerksamkeit/Hyperaktivität der IOWA Conners Rating Scale und die Beurteilung der globalen Wirksamkeit durch Eltern und Lehrer. Sicherheit und Verträglichkeit wurden durch Erfassung von unerwünschten Ereignissen ermittelt. Die Entwicklung von Gewicht, sowie Veränderungen von Schlafqualität und Appetit wurden beobachtet. Ergebnisse: Die Umstellung auf OROS®-MPH einmal täglich führte zu einer signifikant besseren Kontrolle der ADHS-Symptome in der Elterneinschätzung. Dagegen zeigten sich nach Beurteilung der Lehrer keine signifikanten Veränderungen zur vorherigen Therapie. Die globale Wirksamkeit wurde von den Lehrern in 55%, von den Eltern in 79% und von den Prüfärzten in 77% der Fälle als «gut» oder «hervorragend» angesehen. OROS®-MPH wurde gut vertragen. Häufigste unerwünschte Ereignisse (UE) waren Kopfschmerzen (8,9%) und Rhinopharyngitis (7,0%). Unerwartete UE wurden nicht beobachtet. Schlussfolgerungen: Die Umstellung von ADHS-Patienten, die zuvor stabil auf IR-MPH eingestellt waren, auf OROS®-MPH einmal täglich führte zu einer signifikanten Verbesserung der ADHS-Symptome nach Einschätzung der Eltern. In der Schule zeigte OROS®-MPH eine vergleichbare Wirkung wie die vorher durchgeführte Therapie.

Several studies have shown that ADHD has a significant negative impact on multiple domains of quality of life of children and adolescents, particularly in terms of behavioral, emotional and psychosocial quality of life. The psychopedagogical intervention can be therapeutic, preventive and promote educational inclusion. Moreover, it aims to address the learning process, how it is developed and how the individual relates himself to learning, both in the cognitive, social and emotional domains. Our purpose was to understand whether the psychopedagogical intervention influences the quality of life of adolescents with ADHD considering their pharmacological plans. The aim of this study was to evaluate the impact of a psychopedagogical intervention on quality of life of a sample of adolescents with ADHD taking into account their pharmacological plans (rubifen, ritalin, and concerta). We used quantitative and qualitative assessment measures: the KIDSCREEN-10, to measure the quality of life of these adolescents, and a semi-structured interview to assess the impact of a psychopedagogical intervention in the lives of these adolescents and their parents, respectively. The results showed that psychopedagogical intervention has a positive impact on the quality of life of adolescents with ADHD, as well as in the family dynamics. Due to the demands of school work (homework) and their negative impact on the family dynamics and consequently in the quality of life, the results corroborate the relevance of a psychopedagogical intervention to improve these areas.
