Introduction: The atopic march consists of atopic dermatitis (AD), allergic rhinitis or sinusitis (AR), allergic conjunctivitis (AC) and bronchial asthma (BA). The influence of concomitant atopic diseases on AD is under-explored. We determined the relationship between personal and family history of atopic diseases with AD severity and persistence. Methods: A cross sectional study involving AD patients was performed at dermatology clinics of two tertiary hospitals. Inclusion criterion was all patients diagnosed with AD. Demographic data, personal and family history of atopy (BA, AR and AC) were obtained by face-to-face interview. AD severity was evaluated using Investigator’s Global Assessment (IGA). AD was considered persistent if continued beyond age 12 years. Results: Sixty patients aged 21.6±17.2 participated. There were 25(41.7%) with concomitant AR, 16(26.7%) BA and 12(20%) AC. Forty seven (78.3%) patients had family history of atopy, 25(41.7%) had eczema, 24(40%) AR, 19(31.7%) BA and 2(3.3%) AC. Patients with BA [OR=3.8, 95%CI 1.04 to 14.4)], BA and AR [OR=9.74, 95%CI 1.13 to 83.67) and family history of BA [OR= 4.00, 95%CI 1.20 to 13.27) were more likely to exhibit moderate-severe AD. Personal AR, AC and family history of AD and BA were significantly more prevalent in patients with persistent AD. Conclusions: AR was the most common atopic disease associated with AD. Family history of eczema and AR were common. Personal BA, BA with AR and family history of BA were predictors of severe AD. Personal AR, AC and family history of AD and BA were associated with AD persistence.

AbstractObjective:The aim of this study was to determine the correlation between tuberculous meningitis and tuberculous otomastoiditis.Materials and methods:Meningeal involvement sites were investigated by magnetic resonance imaging in 32 patients (21 males, 11 females) who had previously been diagnosed with tuberculous meningitis. Clinical and laboratory findings and responses to anti-tuberculous treatment were evaluated, and the presence of concomitant tuberculous otomastoiditis was also investigated.Results:The meningeal involvement site was unilateral (in the sylvian fissure and the perimesencephalic cistern) in 28 patients (87.5 per cent), and bilateral and widespread in four patients (12.5 per cent). Tuberculous otomastoiditis was found in 11 of the patients with tuberculous meningitis (34.3 per cent). Otomastoiditis was on the same side as the meningeal involvement in nine of these 11 patients. Bilateral otomastoiditis with meningeal involvement was observed in two patients.Conclusions:Tuberculous meningitis is frequently accompanied by otomastoiditis, although the exact causal relationship between the two conditions is unclear. Since meningitis is a serious clinical condition, concomitant otomastoiditis generally remains unrecognised. Tuberculosis should be considered in the differential diagnosis of patients with otitis or otomastoiditis who do not respond to antibiotic therapy.

Background: Neuroticism is associated with inflated somatic symptom reporting. Worry and rumination are a cognitive concomitant of neuroticism and potentially mediate the neuroticism–somatic complaint relationship. Aims: The present study examined the degree to which worry and rumination mediated the relationship between neuroticism and somatic complaints. Method: A sample of 170 volunteers, recruited via convenience sampling, took part. Participants completed a series of self-report measures: the Eysenck Personality Questionnaire Revised-Short Form, Penn State Worry Questionnaire, the Ruminative Response Scale and the Somatic Symptom Scale-8. Results: Analysis revealed significant positive correlations between neuroticism, rumination and worry. Neuroticism, rumination and worry also correlated positively with somatic complaints. Using structural equation modelling, a mediational model indicated that rumination fully mediated the relationship between neuroticism and somatic complaints. Conclusions: Findings are consistent with the symptom perception hypothesis and have implications for healthcare in terms of managing individuals who present with multiple somatic complaints. Future research would benefit from adopting a longitudinal approach to test how rumination interacts with neuroticism and somatic complaints over time.

The most frequently reported neuropsychological deficits in schizophrenia are those of attention, executive function, and verbal memory. Whereas the former appear to be related to negative symptoms of schizophrenia, there is little agreement about which clinical symptoms are related to the verbal memory deficit. The aim of the present study was to delineate further the pattern of neuropsychological deficits in schizophrenia—especially those of verbal memory—and their relationship to clinical symptoms. One hundred patients with chronic schizophrenia and 62 healthy control subjects took part in the study. Assessments of patients took place within the first 3 weeks after admission to hospital. Nine neuropsychological tests, mainly measuring executive and memory function and attention, were administered to all subjects, and clinical symptoms, such as psychotic and negative symptoms and conceptual disorganization, were assessed in patients by means of the Positive and Negative Syndrome Scale (PANSS). Patients showed widespread cognitive deficits with verbal memory impairment best discriminating patients and controls. Conceptual disorganization was partly accounted for by poor verbal memory and a low IQ estimate, and negative symptoms by deficient word fluency; positive symptoms were not significantly related to cognitive deficits. The results indicate that there is a specific relationship between neuropsychological deficits and the more chronic of the clinical symptoms.

Abstract:
The objective of this study was to determine the prevalence of road traffic crashes (RTC)–related maxillofacial injuries, the concomitant injuries occurring with them, and to assess the relationship between the severity of maxillofacial and concomitant injuries. This was a prospective study involving 201 victims of RTC seen at the Accident and Emergency Department of the University College Hospital, Ibadan with maxillofacial injuries during the study period. Demographic data of the patients, the types of maxillofacial injuries, and concomitant injuries sustained were recorded. Severity of maxillofacial injury was determined using the maxillofacial injury severity scale (MFISS), while the severity of concomitant injuries was based on the ISS. Correlations between types and severity of maxillofacial injury and types and severity of concomitant injury were conducted to determine the predictability of concomitant injuries based on maxillofacial injury severity. Data were processed using SPSS Statistical software (SPSS, version 20.0 for windows, IBM SPSS Inc, Chicago, IL). Maxillofacial injuries constituted 25.4% of RTC-related admission by the Accident and Emergency Department. A total of 151 (75.1%) patients who presented with concomitant injuries participated in the study. Eighty-one (53.6%) sustained injuries to more than one body region. Head injury was the commonest (99, 65.6%) concomitant injury, followed by orthopedic injury (69, 45.7%). Increasing severity of maxillofacial injury showed a positive correlation with increasing ISS. Also, positive correlation was noted with increasing severity of maxillofacial injury and presence of polytrauma ( p = 0.01), traumatic brain injury ( p = 0.034), and eye injuries ( p = 0.034). There was a high prevalence of maxillofacial injuries in victims of RTC. There was a high incidence of concomitant injuries noted with these maxillofacial injuries. Significantly, this study showed a direct relationship between the severity of maxillofacial injury and head, ocular and polytrauma. This study further emphasizes the need for thorough examination of patients presenting with RTC-related maxillofacial injuries.

Abstract Abstract 5005 Chronic myelogenous leukemia (CML) and multiple myeloma (MM) are hematological neoplasms with involvement of distinct hematopoietic cell lineages. The simultaneous occurrence of CML and MM is an extremely rare event. We report the case of a 46-year-old asymptomatic patient who was diagnosed with CML after an abnormal white blood cell count (90×109 u/L) was found on a routine complete blood cell count. Physical examination showed only splenomegaly. At diagnosis, a monoclonal protein peak was observed on electrophoresis (0.82g/dL). Bone marrow (BM) biopsy was characterized by granulocyte hyperplasia without clonal plasmacytosis. Karyotype showed t(9;22)(q34.1;q11.2) and a BCR/ABL mRNA transcript was detected by RT-PCR. Cytoreduction was initiated with hydroxyurea for 20 days until imatinib 400 mg/day was initiated. Patient achieved a major molecular response (MMR) after 7 months of treatment and remains in MMR. After 12 months from diagnosis, the patient persisted with monoclonal peak, and a new BM biopsy showed 15% monoclonal plasma cells, compatible with IgG lambda Myeloma. No end-organ damage was found, and there was no indication for therapy. Karyotype was 46, XX and a FISH analysis was positive for IgH rearrangement and for del 13q. At the time of this report the patient persists in MMR for CML with a monoclonal peak of 1.2 g/dL. No evidence of MM progression has been observed. These findings indicate coexistence of CML in chronic phase and monoclonal gamopathy of uncertain significance (MGUS), progressing to smoldering MM IgG lambda during IM treatment. At this moment, the association of CML and MM has been considered coincidental, but it is possible that imatinib may induce changes in phenotype of plasma cells which might lead to progression of multiple myeloma. PurposeTo measure marketing performance in a holistic sense.Design/methodology/approachTo augment the prevailing customer relationship marketing paradigm, a holistic stakeholder relationship marketing paradigm is proposed in which holistic marketing performance is reflected in the delivery of long‐term economic, social, and environmental value to customer, employee, supplier, community, and shareholder stakeholders of a business in order to enhance sustainable financial performance. Present stakeholder attitudes are measured in a stakeholder performance appraisal within a stakeholder relationship marketing model, as timely, early warning signals of future stakeholder behaviour and concomitant future business performance.FindingsStakeholder performance appraisal results to date indicate that a holistic stakeholder relationship marketing orientation that incorporates triple bottom line philosophy significantly enhances business financial performance beyond that achieved by a customer relationship marketing orientation.Research limitations/implicationsThe stakeholder performance appraisal has been applied to only 33 businesses to date providing scope for wider application of this measurement system to demonstrate its practical usefulness in measuring holistic marketing performance and future financial performance.Practical implicationsThe stakeholder performance appraisal provides a perceptual overview of holistic marketing performance and concomitant business financial performance from stakeholders in terms of quantitative ratings of economic, social and environmental performance, and qualitative strengths, weaknesses, opportunities and threats. These data enable a business to plan stakeholder relationship marketing strategies to enhance performance and to predict future financial performance.Originality/valueThe stakeholder relationship marketing model and the stakeholder performance appraisal are new, unique, managerially useful additions to existing stakeholder models and metrics.

e18564 Background: Thrombosis and bleeding are the most important prognostic factors in myeloproliferative neoplasm (MPN). But there is not a prognostic factor for predicting survival at diagnosis. The determination of the intensity of the cancer incidence in brief- and family histories of patients may help to make a comment on the nature of the MPN. We investigated the effects of the concomitant diseases on survival and cancer intensity in brief- and family histories of the patients with MPN. Methods: Thirty-five (19 female, mean age 59 ±11 years) patients with MPN were enrolled to the study. The diagnosis of the patients were essential thrombocythemia in 19, polycythemia vera in six and primary myelofibrosis in six, and chronic myelocytic leukemia in four. By using Charlson’s concomitant disease index, concomitant disease weight incidence, concomitant state-age relationship, and probability of 10 years survey were investigated. Results: In brief history of the patients, only one (3%) patient had skin cancer. However there were some cancers in family history of 9 patients (26%). These were hematological malignancies in four patients (2 acute myelocytic leukemia and 2 non-Hodgkin’s lymphoma) and solid tumors in five patients (2 breast, 2 colon cancer, one skin and lung cancers). Mean Charlson’s concomitant disease weight index was 1.4±1.5 (0-6), concomitant state-age relationship was 3.2±2 (0-8), and probability of 10 years survey was 63±35% (0-98%). Conclusions: Although our study group is small, Charlson’s concomitant disease index may be used for the determining of life expectancy in MPN. Moreover cancer cluster in family history of these patients was notable.

Abstract:
The present study inspects the comparative effects of inside shareholding on firm value for group-affiliated firms and non-group-affiliated firms in Pakistan. The research uses the excess-value as a performance measure by employing a chop-shop approach in treatment effects regression model (TERM). The results reveal that lower level and higher level inside shareholding negatively associated with firms’ value. However, it positively relates at a moderate level. Also, inside shareholding-value relationship differs significantly for group-affiliated firms and non-group-affiliated firms. Firms with inside shareholding at lower level appear to more visible in business groups, and these firms show adverse firms’ value consistent with divergence of interest effect. Nevertheless, inside ownership firms at a higher level, namely standalone firms, negatively affect firm value with entrenchment effect. The findings also confirm non-linearity in inside shareholding-firm value relationships.

Abstract:
This article presents a critical review of the literature concerning concomitant speech and language disorders in stuttering children. Studies published since the 1920s that examined language onset and disorders of articulation, syntax and morphology, semantics, and word finding are analyzed. Collectively, the studies present a mixed impression of stutterers, not only because of methodological variations, but also because of the tremendous variability that exists among children who stutter. Although the evidence is not convincing that stutterers as a group are more likely than nonstutterers to have deficits in any of these areas, it is clear that some stutterers do have concomitant speech and language problems that may bear some relationship to their stuttering. The message from this body of research is that individual differences among stuttering children should not be ignored during clinical or research activities.

To evaluate the effect of fusion of the first metatarsophalangeal joint on the first intermetatarsal angle, a series of 62 consecutive first metatarsophalangeal fusions was reviewed. Of these, 47 had sufficient data to be included in this study. The results of this study showed that the change in the first intermetatarsal angle following a first metatarsophalangeal joint arthrodesis is directly proportional to the preoperative first intermetatarsal angle. Therefore, when a first metatarsophalangeal joint arthrodesis is performed on a patient with a wide intermetatarsal angle, a concomitant proximal first metatarsal osteotomy is usually not indicated.

Context:Deficiencies in scapular upward rotation and periscapular strength have been associated with various shoulder pathologies and decreased athletic performance. Therefore, proper periscapular strength and concomitant scapular upward rotation are important factors among overhead athletes, such as baseball players.Objective:To assess the relationships between lower trapezius and serratus anterior strength and the quantity of scapular upward rotation.Design:Descriptive study.Setting:Laboratory.Participants:24 professional baseball pitchers.Measures:Scapular upward rotation was measured at 0°, 60°, 90°, and 120° of humeral elevation. The maximum isometric strength of the lower trapezius and serratus anterior were measured.Results:There was a moderate-good positive relationship between lower trapezius strength and scapular upward rotation at 90° (r2 = .56, P = .001) and 120° (r2 = .53, P = .001). The relationships between scapular upward rotation and serratus anterior strength were all poor.Conclusion:A moderate-good relationship existed between lower trapezius strength and scapular upward rotation.

Over the last 3 decades, scientific evidence advocates an association between traumatic brain injury (TBI) and accelerated fracture healing. Multiple clinical and preclinical studies have shown an enhanced callus formation and an increased callus volume in patients, respectively, rats with concomitant TBI. Over time, different substances (cytokines, hormones, etc.) were in focus to elucidate the relationship between TBI and fracture healing. Until now, the mechanism behind this relationship is not fully clarified and a consensus on which substance plays the key role could not be attained in the literature. In this review, we will give an overview of current concepts and opinions on this topic published in the last decade and both clinical and pathophysiological theories will be discussed.

While the constructs of isolation and disengagement of elderly persons from society are not new, they are generally considered as independent and competing views. In this research was studied the relationship between isolation and disengagement of 365 urban elderly persons, using measures developed for survey research applications. The findings suggest isolation does occur among elderly persons but that the concomitant disengagement is voluntary rather than involuntary.

Abstract:
Progression of diabetic retinopathy is associated with a large number of risk factors, and hyperlipidemia is one of the most common. The work is focused on peculiarities of the progression of juvenile diabetic retinopathy (JDR), depending on the presence of concomitant distyroidism in patients with juvenile diabetes mellitus (JDM), the state of lipid metabolism and melatonin production. The aim. To determine the features of the JDR progression depending on the type of concomitant dysthyroidism, the state of lipid metabolism and production of the hormone melatonin. Materials and methods. The examination of three groups of patients was carried out: group 1 (152 patients) included patients in whom JDM proceeded without thyropathy; group 2 (99 patients) included patients with JDM in combination with autoimmune thyroiditis (AIT); group 3 (111 patients) included patients in whom JDM was accompanied by an increased level of thyroid-stimulating hormone (TSH). Results. It was found that the frequency of proliferative diabetic retinopathy (PDR) in the group with JCD and elevated TSH (21.6%) was 2.7 times higher than the frequency of PDR in the group with JDM and AIT (8.1%) and 3.7 times exceeded the frequency of PDD in JDM without distyroidism (5.9%). The greatest violations of all links of lipid metabolism were found in patients with JDM with an increased level of TSH, which contributes to a more pronounced progression of JDR than in patients without thyropathy or concomitant AIT. The average daily excretion of the hormone melatonin (M) among the studied groups was the lowest in patients with PDD with JCD in combination with increased TSH (38.4 ± 2.7 nmol/day) compared with patients with PDD with JCD without thyropathy (48.3 ± 3.8 nmol/day; p <0.01) and with AIT (42.5 ± 5.6 nmol/day; p <0.01), and compared with the control indicator (52.7 ± 5.8 nmol/day; p <0.001). Conclusions. Based on the results obtained it can be concluded that the combination of type 1 JDM with elevated TSH is accompanied by significant disorders of lipid metabolism and melatonin production and this is a risk factor for accelerated progression of JDR. OBJECTIVE: To evaluate concomitant prescribing of tranquilizers and hypnotics in antidepressant treatment with particular focus on the relationship to drug class in patients prescribed antidepressant treatment for depressive disorders. DESIGN AND METHODS: Repeated cross-sectional surveys of Swedish physicians in ambulatory care from 1991 to 1996, including specialty practices. The participation rate was 65–70%. RESULTS: Tranquilizers and hypnotics were prescribed in 36.1% of the visits in which the intention was to treat depression. Concomitant prescribing increased with the patient's age for all antidepressant drug classes. Women received more concomitant prescriptions than men when treated with nonselective monoamine-reuptake inhibitors (NSMRI), mainly tricyclic compounds. Psychiatrists prescribed more concomitant tranquilizers and hypnotics than other physicians, and general practitioners fewer, when treating depression with selective serotonin-reuptake inhibitors (SSRIs), mainly citalopram. A logistic regression model showed that the risk for concomitant prescribing was higher when an NSMRI was prescribed than with other antidepressants. CONCLUSIONS: Concomitant prescribing of tranquilizers and hypnotics was common among antidepressant-treated patients. The most plausible reasons for the high rate of concomitant prescribing were the symptoms of the depression itself and/or the common comorbidity with anxiety disorders seen in this group of patients. The results of this study indicate that the concern about increased tranquilizer and hypnotic use among patients treated with SSRIs suggested in other studies seems to be unfounded in Sweden.

Imbalance of pro-inflammatory and anti-inflammatory cytokines was detected in 104 examined the patients with chronic hepatitis C. These changes were the most significant in patients with concomitant diabetes mellitus (DM) type II. Achievement of sustained viral response (SVR) under the influence of antiviral therapy depends on the state of cytokines system. This was evidenced by the detected correlation relationship between the levels of viral load (VL) in patients with concomitant diabetes mellitus type II, namely, direct weak one between IL-4 and VL (r=0.21) and reverse weak one between the level of IL-2 and VL (r=-0.04). Virus elimination rate and frequency influenced by anti-viral therapy (AVT) depends on the balance of pro-inflammatory and anti-inflammatory cytokines. This was shown in the research by detection of direct medium correlation relationship between VL and IL-4 (r= 0.31) and reverse medium correlation relationship between VL and IL-2 (r= -0.45).

Two distinct perspectives have emerged in the literature to explain the relationship between perceived organizational support (POS) and affective commitment (AC): a social exchange perspective and, more recently, a social identity perspective. However, these views have never been considered together. Filling this gap, our study aims to examine the conjoint role of felt obligation (i.e., the social exchange perspective) and organizational identification (i.e., the social identity perspective) in the relationship between POS and AC. Based on two different samples, our results indicate that both felt obligation and organizational identification partially mediate the relationship between POS and AC. In sum, this research shows that the two mechanisms play a concomitant role in the link between POS and AC.

The effect of carbohydrate, protein and fat ingestion on simple as well as complex cognitive functions and the relationship between the respective postprandial metabolic changes and changes in cognitive performance were studied in fifteen healthy male students. Subjects were tested in three sessions, separated by 1 week, for short-term changes in blood variables, indirect calorimetry, subjective performance and different objective performance tasks using a repeated-measures counterbalanced cross-over design. Measurements were made after an overnight fast before and hourly during 3 h after test meal ingestion. Test meals consisted of either pure carbohydrates, protein or fat and were served as isoenergetic (1670 kJ) spoonable creams with similar sensory properties. Most aspects of subjective performance did not differ between test meals. For all objective tasks, however, postprandial cognitive performance was best after fat ingestion concomitant with an almost constant glucose metabolism and constant metabolic activation state measured by glucagon:insulin (G:I). In contrast, carbohydrate as well as protein ingestion resulted in lower overall cognitive performance, both together with partly marked changes (P<0.001) in glucose metabolism and metabolic activation. They also differently affected specific cognitive functions (P<0.05) in relation to their specific effect on metabolism. Carbohydrate ingestion resulted in relatively better short-term memory and accuracy of tasks concomitant with low metabolic activation, whereas protein ingestion resulted in better attention and efficiency of tasks concomitant with higher metabolic activation. Our findings support the concept that good and stable cognitive performance is related to a balanced glucose metabolism and metabolic activation state.

Elucidating signaling driven by lemur tyrosine kinase 3 (LMTK3) could help drug development. Here, we solve the crystal structure of LMTK3 kinase domain to 2.1Å resolution, determine its consensus motif and phosphoproteome, unveiling in vitro and in vivo LMTK3 substrates. Via high-throughput homogeneous time-resolved fluorescence screen coupled with biochemical, cellular, and biophysical assays, we identify a potent LMTK3 small-molecule inhibitor (C28). Functional and mechanistic studies reveal LMTK3 is a heat shock protein 90 (HSP90) client protein, requiring HSP90 for folding and stability, while C28 promotes proteasome-mediated degradation of LMTK3. Pharmacologic inhibition of LMTK3 decreases proliferation of cancer cell lines in the NCI-60 panel, with a concomitant increase in apoptosis in breast cancer cells, recapitulating effects of LMTK3 gene silencing. Furthermore, LMTK3 inhibition reduces growth of xenograft and transgenic breast cancer mouse models without displaying systemic toxicity at effective doses. Our data reinforce LMTK3 as a druggable target for cancer therapy.

The purpose of this article is to revisit the relationship between growth and poverty which was widely discussed during the sixties. In view of the renewed reliance on this mechanism during the eighties by the Reagan administration and a concomitant increase in the incidence of poverty, a revisit of the topic is relevant. A regression model similar to the previous studies, but expanded to capture the emerging issues during the eighties is estimated for the period 1964–1987. A simple variable coefficient model to capture the secular decline in the effectiveness of growth to reduce poverty is also estimated. It is found that over the sample period, growth and transfer payments had significant effects on poverty; but their effects have been diminished by changes in income distribution, family composition, and the decline of the manufacturing sector during the seventies and eighties. Another interesting finding is that the impact of growth on the incidence of poverty is race neutral.

We analyzed 60 patients aged from 30 up to 75 years with any of low-lumbar herniated intervertebral disks. Pain intensity was compared with the concomitant presence of chronic circulatory failure in the vertebral-basilar basin as a result of clinically significant abnormalities of blood vessels in the form of hypoplasia of vertebral artery and its tortuosity. We investigated dynamics of patient's pain under the influence of low-dose treatment by Cortexin (neurocytoprotector). In patients with chronic brainstem ischemia the herniated discs cause more intense and prolonged pain (radicular and local). More effective in these patients is a treatment with inclusion of Cortexin 20 mg intramuscularly within 10 days.

The article presents result of analysis of indicator of polymorbidity in sampling of 9871 patients of therapeutic clinic of the research institute of experimental and clinical medicine (Novosibirsk)during 2003-2011. The study was carried out using archive method with statistical analysis of all nosologic forms, groups and classes of ICD-10 irrespective of particular diagnosis being main or concomitant. The gender differences of indicator trans-systemic polymorbidity during 2003-2011consisted in that in males its increasing was more dynamic than in females. The trans-systemic polymorbidity is correlated with social economic indicators considering as stressors having significant importance for its occurrence and development. These regularities are more specific for males due to direct correlation between trans-systemic polymorbidity and level of payroll salary in contrast with females. Hence, necessity to take into account impact of social economic factors on development of polymorbid pathology for improving system of secondary and tertiary prevention, diagnostic, treatment and rehabilitation of concomitant nosologies in patients. The development of new medical economic standards of treatment of patients with polymorbid pathology to implement principles of personified medicine is required.

The purpose of this study was to compare complication rates after total ankle replacement in 2 groups of patients based on polyethylene insert size. The total cohort was divided into 2 groups based on insert size. Group 1 included patients with polyethylene insert size less than 10 mm in thickness. Group 2 included patients with polyethylene insert sizes 10 mm and larger. Available charts were reviewed for patients who underwent primary total ankle arthroplasty by one surgeon. Patient demographics, polyethylene insert size, implant used, concomitant procedures, postoperative complications, and patient-reported outcome scores were recorded. One hundred patients were available for follow-up and were included in this study, which ranged from March 2012 to July 2017. The average follow-up was 31.3 months (range = 10-60 months). Forty-eight females and 52 males were included in this study. There were a total of 63 patients in group 1 and 47 patients in group 2. The total complication rate for patients in group 1 was 11.1% (7/63), and in group 2 it was 16.2% (6/32). There was no statistical significance in complication rates when comparing the 2 groups (P = 0.5427). All patients underwent at least one concomitant procedure at the time of initial ankle replacement. Our findings show that total ankle replacement complication rates are equal when comparing large polyethylene inserts commonly utilized to correct deformities, versus small polyethylene inserts commonly utilized in primary resurfacing. Abstract To test the hypothesis that increments in plasma cyclic AMP during beta-adrenergic stimulation reflect integrated second messenger function of the tissues activated by the angonist, graded adrenaline infusion resulting in plasma adrenaline concentrations within the physiological range was performed in 8 healthy subjects with and without concomitant beta-adrenoceptor blockade by iv propranolol. A significant correlation was found between increments in plasma adrenaline and plasma cyclic AMP in the experiments without beta-blockade; during concomitant beta-blockade the increase in plasma cyclic AMP concentrations at low adrenaline infusion rates was prevented, whereas a small increase in cyclic AMP was found at high adrenaline infusion rates, probably owing to incomplete beta-receptor blockade. Likewise, the adrenaline-induced increments in blood substrates (glucose, lactate, glycerol and betahydroxy butyric acid) were significantly reduced but not completely prevented by beta-blockade. We conclude that an altered relationship between beta-agonist concentrations and plasma cyclic AMP may provide evidence for the existence of differences in beta-adrenergic sensitivity in man.

Purpose The connection between employees’ well-being and performance, although widely studied in organizational psychology, has received much less attention from HRM scholars. The purpose of this paper is to extend the literature by examining the impacts of the multidimensional structure of well-being consisting of psychological, social and health dimensions on employees’ task and contextual performance. Design/methodology/approach The authors collected data from 281 employees from the New Zealand service sector using a questionnaire survey. Factor analysis was used to determine items that form various facets of well-being and performance constructs. Hierarchical regression analyses were conducted to test the well-being – performance relationship. Findings The findings show that different facets of well-being differentially contribute to employees’ task and contextual performance. Specifically, the facets of happiness and trust were positively associated with both task and contextual performance, while the effects of life satisfaction and work life balance on task and contextual performance were insignificant. Moreover, work intensification was only associated with task performance, in contrast, job satisfaction and over commitment were only related to contextual performance. Practical implications The implications of these findings are two-fold. For researchers, a review and overhaul of the conceptualization and operationalization of well-being in HRM studies is long overdue. For managers, improvements to employees’ job performance and the organization’s health can result from simultaneously enhancing multiple dimensions of employees’ well-being. Originality/value This study provides new insights into the complex relationship between well-being and performance by incorporating a multidimensional and multifaceted perspective of well-being and highlighting the distinctive effects of various facets of well-being on different types of employees’ performance.

Introduction: Chronic obstructive pulmonary disease (COPD) with hypertension occupy a leading position in morbidity and mortality in the world. The question of studying a single pathological way of their development, the search for diagnostic markers and therapeutic targets in this comorbid pathology remains relevant. The aim of the study was to study cardiopulmonary parameters in patients with chronic obstructive pulmonary disease with concomitant hypertension, depending on the level of vasoactive intestinal peptide (VIP). Materials and methods: 99 patients with COPD GOLD 2 were examined, 54 of whom had concomitant hypertension II stage, in which the dependence of lipid metabolism, spirometry and hemodynamic parameters, depending on the level of VIP in blood serum. Results and conclusions: It was established that the smallest values of VIP and the greatest changes in cardiopulmonary parameters, lipid metabolism were found in the cohort of persons with concomitant hypertension. There was a significant decrease in spirometry values and an increase in hemodynamic parameters, respectively, a decrease in VIP levels in patients with COPD in combination with hypertension, which may indicate its role in the formation of these pathologies due to a decrease in its protective function, both in relation to apoptosis of alveolar cells and in relation to progression atherosclerosis and high blood pressure. It was also noted that in patients with the lowest VIP serum levels, a more rapid formation of COPD was observed. The data obtained make it possible to consider VIP as a diagnostic marker and a potential therapeutic target for the comorbid pathology examined.

Oxidative stress and inflammation promote renal injury via damage to molecular components of the kidney. Unfortunately, relationships between inflammation and oxidative stress are cyclical in that the inflammatory processes that exist to repair radical-mediated damage may be a source of additional free radicals, resulting in further damage to renal tissue. Oxidative stress and inflammation also have the ability to become systemic, serving to injure tissues distal to the site of original insult. This review describes select mediators in the exacerbatory relationship between oxidative stress, inflammation, and CKD. This review also discusses oxidative stress, inflammation, and CKD as they pertain to the development and progression of common CKD-associated comorbidities. Lastly, the utility of several widely accessible and cost-effective lifestyle interventions and their ability to reduce oxidative stress and inflammation are discussed and recommendations for future research are provided.

This study investigated the relationship between self-image and creativity in late adolescence. The multidimensional Offer Self-image Questionnaire was used to assess self-image while the How Do You Think Inventory was used to measure creativity. The results were similar to investigations indicating superior socioemotional functioning among gifted adolescents. More creatively scoring subjects ( n = 65) exhibited better psychosocial functioning on six of the eleven self-image dimensions measured than the Jess creatively scoring subjects ( n = 69). Abstract Sarcoidlike granulomas may occur in association with Hodgkin lymphoma and non-Hodgkin lymphoma. The granulomas may be concomitant and so extensive that they obscure the malignant process. In addition, a sarcoidosis-lymphoma syndrome has been described in which there appears to be a relationship between sarcoidosis and the development of a lymphoproliferative disorder. We report a case of a low-grade B-cell lymphoma with concomitant extensive sarcoidlike granulomas. The patient had no diagnostic clinical evidence of sarcoidosis, although she had an elevated serum calcium level and increased serum angiotensin converting enzyme activity. Increased serum calcium and serum angiotensin-converting enzyme activity have been associated with clinical sarcoidosis but have also occasionally been described in association with Hodgkin lymphoma and non-Hodgkin lymphoma without evidence of sarcoidosis. We describe our findings and illustrate the usefulness of immunoperoxidase immunophenotyping techniques in such a case.

OBJECTIVE: To report 2 cases of serotonin toxicity (ST) associated with concomitant use of linezolid and serotonergic drugs and review previously published case reports. CASE SUMMARIES: Case 1. A 38-year-old white female with cystic fibrosis treated with venlafaxine 300 mg/day for one year was prescribed linezolid 600 mg intravenously every 12 hours for treatment of methicillin-resistant Staphylococcus aureus (MRSA) pulmonary infection. She displayed symptoms of ST 8 days after the introduction of linezolid. The venlafaxine dosage was decreased to 150 mg/day, and symptoms gradually abated over 36 hours. Case 2. A 37-year-old male with multiple myeloma received citalopram 40 mg/day and trazodone 150 mg/day for anxiety-related disorders. Linezolid treatment with 600 mg orally twice daily was instituted for MRSA cellulitis. The following day, the patient developed anxiety, panic attacks, tremors, tachycardia, and hypertension that persisted throughout linezolid treatment. Symptoms finally waned 5 days after linezolid treatment was stopped. DISCUSSION: The symptoms observed in our patients were consistent with Sternbach's criteria for ST. A review of published case reports showed a short time to onset of symptoms following the introduction of linezolid, generally within 1–3 days. Also of note is the use of relatively high dosages of serotonergic drugs. Use of the Naranjo probability scale indicated a possible relationship between the use of linezolid and the occurrence of ST in both cases. CONCLUSIONS: Clinicians should pay special attention to patients treated with serotonergic drugs, especially those receiving dosages in the higher end of the normal range who are prescribed linezolid, and consider tapering or reducing the dosage of serotonergic drugs for the duration of antibiotic therapy.

Relevance. Till now there is no clear clinical trial understanding of the relationship between hypertension (AH) and the patient's cognitive function. Objective: the investigation of changes in cognitive function in patients with hypertension with concomitant type 2 diabetes mellitus (DM2). Materials and methods. 215 people were examined: Group I (n = 131) – stage II hypertensive disease (HD); Group II (n = 46) – HD, combined with diabetes mellitus2; Group III, control (n = 38) – without a diagnosis of HD and diabetes mellitus2. All examined blood pressure (BP), echocardiogram. Clinical complaints, anamnestic data, and neuropsychiatric testing data were used to diagnose moderate cognitive impairment (MCI). Used modified diagnostic criteria J. Touchon, R. Petersen. To assess cognitive function used: Short scale of assessment of mental state (Mini-Mental State Examination - MMSE); Frontal Assessment Battery (FAB); clock drawing test; test for literal and categorical associations; Schulte tables; the Global Deterioration Scale (GDS) and the Clinical Dementia Rating (CDR). Result. In patients with HD, the most significant risk factors for moderate cognitive impairment are hypercholesterolemia and overweight (with OR 1.8), obesity (OR 1.6), the presence of concomitant diabetes mellitus2, which, especially in overweight, significantly impairs cognitive function ( OR 2.56). Deterioration of cognitive function correlates with the duration of HD, cholesterol levels. Concomitant diabetes mellitus2 in patients with HD creates a statistically significant additional negative effect on the results of cognitive function. In patients with HD with a distorted daily blood pressure profile, the neurodynamic component of cognitive function is first of all affected - conceptualization, repetition, the reaction of choice. The relationship between cognitive function and daily monitoring of blood pressure often have a U-shaped relationship, where the maximum indicators of cognitive function are at the level of the optimal recommended blood pressure figures (130-139 mm Hg for SBP, and 75-85 mm Hg for DBP, and the daily index within 10%). Conclusions. There is a dependence on changes in cognitive function on the presence of risk factors and signs of subclinical damage to target organs. The nonlinear U-shaped character of the relationship between the average daily, average night, and average daily blood pressure and the results of screening tests and scales for assessing cognitive function are shown.

Background. Esophageal intramural pseudodiverticulosis (EIPD) is an idiopathic benign chronic disease characterized by flask-like outpouchings of the esophageal wall. It is unknown whether there is a genuine association between EIPD and eosinophilic esophagitis (EoE).Aims. To investigate a possible relationship between EIPD and EoE.Methods. Patients with radiographic or endoscopic evidence of pseudodiverticulosis were identified from the database at a single academic center. Cases were analyzed in three areas: clinical information, endoscopic findings, and course.Results. Sixteen cases of esophageal pseudodiverticulosis were identified. Five patients had histologic evidence of eosinophilic esophagitis. Patients with EoE had pseudodiverticula in the mid-to-distal esophagus while those with EIPD had pseudodiverticula predominantly in the proximal esophagus (p<0.001). EoE with pseudodiverticulosis occurred in younger patients (p<0.019). Food bolus obstructions were more common in patients with EoE and pseudodiverticulosis than in EIPD (p<0.034).Conclusions. This is the first case series supporting a potential association between EoE and pseudodiverticulosis. We also identify characteristic features of pseudodiverticulosis that may raise clinical suspicion of underlying eosinophilic esophagitis.

Drawing from emergent areas of sociological research and theorization, the authors consider the environmental impacts of militaries from a comparative-international perspective. The article begins with an overview of treadmill of production and treadmill of destruction theories, the latter of which highlights the expansionary tendencies and concomitant environmental consequences of militarization. This theoretical overview is followed by a narrative assessment of military growth and energy consumption, with a particular focus on the US military over the past century. Next, the authors detail the various environmental impacts associated with the growth and structure of national militaries, briefly discuss potential future research directions, and conclude by calling for scholars in future studies on society/nature relationships to seriously consider the environmental and ecological impacts of the world's militaries.
