Abstract:
Background Ethical conflicts complicate clinical practice and often compromise communication and teamwork among patients, families, and clinicians. As ethical conflicts escalate, patient and family distress and dissatisfaction with care increase and trust in clinicians erodes, reducing care quality and patient safety. Objective To investigate the effectiveness of a proactive, team-based ethics protocol used routinely to discuss ethics-related concerns, goals of care, and additional supports for patients and families. Methods In a pre-post intervention study in 6 intensive care units (ICUs) at 3 academic medical centers, the electronic medical records of 1649 patients representing 1712 ICU admissions were studied. Number and timing of family conferences, code discussions with the patient or surrogate, and ethics consultations; palliative care, social work, and chaplain referrals; and ICU length of stay were measured. Preintervention outcomes were compared with outcomes 3 and 6 months after the intervention via multivariate logistic regression controlled for patient variables. Results The odds of receiving a family conference and a chaplain visit were significantly higher after the intervention than at baseline. The number of palliative care consultations and code discussions increased slightly at 3 and 6 months. Social work consultations increased only at 6 months. Ethics consultations increased at both postintervention time points. Length of ICU stay did not change. Conclusions When health care teams were encouraged to communicate routinely about goals of care, more patients received needed support and communication barriers were reduced.

Abstract:
Imaging buried geological boundaries is one of a major objective during the interpretation of magnetic field data in Geophysics. Therefore, edge detection and edge enhancement techniques assist a crucial role on this aim. Most of the existing edge detector methods require to obtain special points such as in general the maxima of the resulting image. One of the useful tools in estimating edges from magnetic data is the tilt angle of the analytical signal amplitude due to its value slightly dependence on the direction of magnetization. In this study, the maxima of the tilt angle of analytical signal amplitudes of the magnetic data was determined by a curvature-based method. The technique is based on fitting a quadratic surface over a 3×3 windows of the grid for locating any appropriate critical point that is near the centre of the window. The algorithm is built in Matlab environment. The feasibility of the algorithm is demonstrated in two cases of synthetic data as well as on real magnetic data from Tu Chinh-Vung May area. Abstract:
In June 2016, four large atlases comprising two hundred and fifty anatomical drawings which were made around 1654-1660 in Leiden (Netherlands) were identified at the Bibliothèque interuniversitaire de santé. Made by the painter Marten Sagemolen (ca. 1620-1669) under the direction of the anatomist Johannes van Horne (1621-1670), they systematically describe the muscles of man. This extensive collection had been lost since the mid-18th century. The albums entered the collections of the Ecole de santé in 1796, along with the magnificent drawings by the painter Gerard de Lairesse (1641-1711) for the anatomy of Govard Bidloo (1649-1713), and they have remained in the medical library at 12, rue de l'Ecole-de-médecine, today called BIU Santé médecine, which now belongs to Université Paris Cité. Purchased as an anonymous lot accompanying Lairesse's masterpiece, the albums had not been successfully examined until 2016. The great rarity of this collection and the surprise of its identification aroused curiosity. After initial documentation and digitisation work by the BIU Santé's Health History Department, a restoration and study project was carried out with the Bibliothèque nationale de France and the Centre de recherche et de restauration des musées de France. It was followed by an international conference on 18 and 19 June 2021, of which this volume provides the proceedings, and an exhibition of the atlases at the Musée d'histoire de la médecine d'Université Paris Cité, from 15 November 2021 to Saturday 15 January 2022.

Abstract:
In the past three years, the Federal Railroad Administration (FRA) has issued two significant new regulations covering the qualification and operation of passenger railroad equipment. The 1998 Track Safety Standards and 1999 Passenger Equipment Safety Standards provide comprehensive requirements for the qualification and operation of passenger equipment. Among many safety issues, these standards address the response of rolling stock at speed to track deviations, and the potential for derailment and/or injury to passengers. The principles contained in the Safety Standards are also used by FRA to evaluate safety performance of vehicles for operation at high cant deficiency for speeds less than 90 mph as part of FRA’s waiver process. This paper describes the types of static and dynamic tests and equipment that are required with respect to vehicle-track interaction to demonstrate compliance of new equipment with the Standards, and the technical fundamentals behind the requirements. Examples include the National Railroad Passenger Corporation’s (Amtrak’s) experiences in applying for waivers and qualifying new equipment under these regulations, particularly the high cant deficiency testing of the Talgo train in the Pacific Northwest, and the high-speed and high cant deficiency testing of Maryland Rail Commuter’s (MARC’s) MARC-III bi-level cars, Acela Express trainsets and Amtrak’s high-horsepower electric locomotives in the Northeast Corridor. In addition, technical data and procedures necessary to have existing equipment approved for operation at higher speed or cant deficiency are also described. This paper highlights nuances of the new regulations and common misinterpretations.
