Abstract:
Conjoined twinning is one of the most fascinating human malformations. The incidence of conjoined is 1:50,000 to 1:100,000 live births, with 40–60% being stillborn and about 35% of the live births not surviving beyond 24 hours. The exact incidence of conjoint twins in Nigeria is not known as there are instances of under-reporting, largely due to the relatively poor prognosis and stigmatisation associated with conjoined twinning.
 We are reporting a case of thoraco-omphalophagus conjoint twins and a malformed third fetus diagnosed by ultrasound in the first trimester of pregnancy.
 A set of female conjoined twins and a malformed triplet, delivered by 25-year old primigravida via emergency Caesarean Section on the 22nd May 2021 on account of PROM and fetal distress at 34/52 GA, that were admitted in our level one neonatal unit. The twins were joined at the thoraco-abdominal region. Echocardiography showed a moderate- sized Atrial Septal Defect in twin 1 and a large Ventricular Septal Defect in twin 2. MRI showed a thoraco-omphalophagus set of conjoined twins with fused xiphisternum, myocardium, pericardium, liver and small bowel.

Abstract:
Conjoined twins, sometimes known as Siamese twins, are a rare kind of congenital human malformation. Conjoined twins are an extremely rare and rarely encountered impediment to monozygotic twinning, occurring in 1 in 100,000 to 1 in 250,000 live births. Conjoined twins share the same genes and are of the same sex. Reports have indicated a 3:1 female predominance. A 17-year-old woman at 23 weeks of gestation was referred at her second trimester of pregnancy from a rural area. Ultrasonographic evaluation revealed the presence of a monochorionic, monoamniotic twin pregnancy. The twins conjoined with a single face, single thorax and abdomen with separate upper limb and lower limb. It was finally diagnosed as a case of cranio-thoraco-omphalopagus conjoined twins of about 23±1 weeks with mild polyhydramnios. A conjoined twin is a well-known rare entity. It is very important to make a diagnosis by ultrasound in time to prevent future complications. Thoraco-omphalopagus type is known to present several cardiac anomalies. Thus, early diagnosis of conjoint twins, details of organ sharing by imaging modalities will help to predict prognosis, aid in patient counseling and further management. Abstract:
Conjoined twins have rights that must be fulfilled like other humans, namely the right to marry. However, Islamic Law scholars still debate the validity of conjoined twin marriages. This research discusses the debate about the marriage of conjoined twins in Islamic law. The research method uses normative juridical with an Islamic legal approach. The results show that conjoined twins are generally divided into two groups, conjoined twins who can be separated by surgery and conjoined twins who cannot be separated by surgery. Based on the organs or body parts that are connected or united, conjoined twins are divided into several types, such as cephalopagus, thoracopagus, omphalopagus, ischiopagus, craniopagus, heteropagus, epigastric heteropagus, phygopagus, thoraco-omphalopagus, prosopo-thoracopagus, rachipagus, parapagus and dicephalic parapagus. Islamic law does not specifically explain the type of marriage of conjoined twins. However, the law of conjoined twin marriage can be determined by classifying whether each conjoined twin has the consciousness of one person or two people. Besides that, it can also be determined by organising it into one person from below the navel and two people united to form two bodies above. The legal determination of twins on different bases means that the validity of conjoined twin marriages is still debated in Islamic law.

Objective: We present a case report of conjoined twins. Case report: Secundigravida nullipara, 28-years old, admitted for profuse bleeding at 13 weeks of gestation. Ultrasound confirmed vital pregnancy of conjoined twins – thoracopagus. After prenatal diagnostic consultation the patient decided for termination of pregnancy. Molecular analysis confirmed a female fetus without any chromosomal anomalies. Conclusion: The occurrancce of conjoined twins is very rare. Early prenatal ultrasound diagnosis plays an important role. Presented case report describes conjoined twins with poor prognosis because of one shared malformed heart. Abstract:
Conjoined twins or Siamese twins are identical twins that are joined in utero. The original Siamese twins were born in Siam (now Thailand) in 1811. They were males and lived for about 62 years. Incidence being 1 in 49,000 births to 1 in 1,89,000 births, this phenomenon is very rare. Approximate half are stillborn and one- third die within 24 hours of birth. Five types of conjoint twins are classically described, thoracopagus, omphalopagus, pygopagus, ischiopagus and craniopagus. With a reported incidence of 74-75% of all conjoint twins, Thoracopagus is the most common type. Omphalopagus with an incidence of 0.5% is the least common. Here we are reporting a case of thoraco-omphalopagus twin pregnancy with intra uterine death delivered by hysterotomy.

HIGHLIGHTS Conjoined twins have captivated mankind for centuries due to the rarity of this birth type; nonetheless, physicians have constantly encountered difficulties in dealing with conjoined twins. Early diagnosis of conjoined twins during prenatal examination is critical for ascertaining the prognosis of the fetus, guiding parental counseling over appropriate courses of action, and potentially enabling the termination of the pregnancy. First-trimester ultrasonography and MRI are complementary modalities in determining the diagnosis and prognosis of conjoined twins in early pregnancy. ABSTRACT Objectives: To discuss the crucial early diagnosis of conjoined twins to determine the type and prognosis. Case Report: A 27-year-old woman was referred to the type A referral hospital with suspicion of congenital abnormalities at 17 weeks of pregnancy. Ultrasound results showed intrauterine monochorionic monoamniotic twins with babies fused on their heads to the urogenital part. The MRI showed a craniopagus, suspected meningomyelocele, and severe bilateral hydronephrosis in the second baby. Due to non-separable cases and a bad prognosis for the fetus, the pregnancy was terminated using misoprostol induction and inserting a balloon catheter. The fetus is born weighing 400 g and 20 cm in length. Conjoined twins obtained the rostral type: a fused head with two faces, a fused thorax until the lower abdomen with one umbilicus, and two pairs of hands and feet. The diagnosis of conjoint twins becomes a problem in early pregnancy, mostly in developing countries. Early diagnosis of conjoined twins during prenatal examination is critical for ascertaining the prognosis of the fetus, guiding parental counseling over appropriate courses of action, and potentially enabling the termination of the pregnancy to prevent maternal stress and complications. Conclusion: Conjoined twins should be identified as soon as feasible to establish the best course of management for both mother and fetus. Ultrasonography and MRI are modalities for determining the diagnosis and prognosis of conjoined twins.

Heteropagus or parasitic conjoined twins represent an extremely rare anomaly, occurring in approximately 1 in 1 million cases. This condition is characterized by the presence of a parasitic twin with significant congenital abnormalities attached to an otherwise typically healthy fetus. The well-developed twin is known as the “autosite” or “host,” while the severely affected fetus is termed the “parasite.” Survival of the defective twin depends on the cardiovascular system of the second, relatively normal fetus. We present the case of a 27-year-old primigravida in her 14th week of pregnancy with ultrasound findings indicating parasitic conjoined twins, specifically omphalopagus. SituationD and M are conjoined twins born without an antenatal diagnosis and assessed as not suitable for separation. At the time of admission they were 21 months old with a combined weight of 17.1 Kg. D presented unwell with a raised heart rate and respiratory rate. A working diagnosis of sepsis (possibly urinary tract infection) was made. Advice was sought from pharmacy on the doses of ceftriaxone and paracetamol. Peripheral intravenous (IV) access was only available in twin M.BackgroundThe twins are joined side by side from the upper chest to the pelvis. They have separate heads, three arms and 2 legs. They have 2 hearts with a fused aorta, a shared liver, 2 gallbladders, 2 stomachs, 3 kidneys and a single bladder. D has a complex congenital heart condition and a poor prognosis. On admission, D was receiving propranolol, but M was not. The dose was based on the combined weight of the twins divided by 2. Conjoined twins are a rare phenomenon, occurring 1 in 50,000 to 100,000 births.1 Around 60% of these are stillborn or die shortly after birth. There are many different types of join with differences in shared organs and limbs. Consequently each twin pair is almost unique and consideration must be given as to how medication is dosed according to pharmacokinetic principles.OutcomeOpinion of the multidisciplinary team was that the twins have relatively separate circulations, although some cross-circulation would be expected. On admission, saturations in the right arm (twin D) were 75%. On the left side (twin M) this was 95%. Ceftriaxone is a highly protein bound, hydrophilic antibiotic,2 The degree of cross circulation (how much blood volume is shared between the twins) would affect the volume of distribution and hypoalbuminaemia was likely to increase the apparent volume of distribution. Based on this, ceftriaxone dosing was advised on the combined weight of the twins and given at 50 mg/Kg to M only. Ceftriaxone is excreted mainly unchanged in the urine and bile with little renal clearance or hepatic metabolism so this was not a concern. After 2 days, Ds CRP had reduced and the twins were switched to oral amoxicillin. Dosing was based on the combined weight of the twins and each was given half the dose. As each twin has a separate stomach, it was assumed relatively individual enteral absorption occurs. Ds CRP continued to drop and the twins were discharged home on day 4 with a further 3 days of oral amoxicillin. Paracetamol dosing was advised at 15 mg/kg based on the combined weight and half given to each twin. As required use was agreed, as there was uncertainty over the amount of hepatic metabolism that would occur by the twins shared liver.Lessons learntConjoined twins are a complex yet interesting challenge in terms of medication dosage and administration.
