<p>The aim of the present Structural Engineering Document, a state-of-the-art report, is to review the progress made worldwide in the use of fibre rein­forced polymers as structural components in bridges until the end of the year 2000.<p> Due to their advantageous material properties such as high specific strength, a large tolerance for frost and de-icing salts and, furthermore, short installation times with minimum traffic interference, fibre reinforced polymers have matured to become valuable alternative building materials for bridge structures. Today, fibre reinforced polymers are manufactured industrially to semi-finished products and ccimplete structural components, which can be easily and quickly installed or erected on site.<p> Examples of semi-finished products and structural components available are flexible tension elements, profiles stiff in bending and sandwich panels. As tension elements, especially for the purpose of strengthening, strips and sheets are available, as weil as reinforcing bars for concrete reinforcement and prestressing members for internal prestressing or external use. Profiles are available for beams and columns, and sandwich constructions especially for bridge decks. During the manufacture of the structural components fibre-optic sensors for continuous monitoring can be integrated in the materials. Adhesives are being used more and more for joining com­ponents.<p> Fibre reinforced polymers have been used in bridge construction since the mid-1980s, mostly for the strengthening of existing structures, and increas­ingly since the mid-1990s as pilot projects for new structures. In the case of new structures, three basic types of applications can be distinguished: concrete reinforcement, new hybrid structures in combination with traditional construction materials, and all-composite applications, in which the new materials are used exclusively.<p> This Structural Engineering Document also includes application and research recommendations with particular reference to Switzerland.<p> This book is aimed at both students and practising engineers, working in the field of fibre reinforced polymers, bridge design, construction, repair and strengthening.

The present chapter focuses on fluent speech segmentation abilities in early language development. We first review studies exploring the early use of major prosodic boundary cues which allow infants to cut full utterances into smaller-sized sequences like clauses or phrases. We then summarize studies showing that word segmentation abilities emerge around 8 months, and rely on infants’ processing of various bottom-up word boundary cues and top-down known word recognition cues. Given that most of these cues are specific to the language infants are acquiring, we emphasize how the development of these abilities varies cross-linguistically, and explore their developmental origin. In particular, we focus on two cues that might allow bootstrapping of these abilities: transitional probabilities and rhythmic units.

Since the mid-twentieth century, the DSM has guided psychiatrists in categorizing disorders. Revisions have taken years, with work groups considering changes to the entire manual. A more timely and efficient approach to updating the DSM would involve continuous improvement of particular diagnostic categories, when and if supported by advances in the field. The aim is to avoid the delays in the incorporation of new knowledge that are inherent in updating at intervals of a decade or more. The American Psychiatric Association has therefore established a structure by which evidence-based proposals for changes to the DSM can be considered and adopted on an ongoing basis. This chapter describes how proposals will be considered and the standards to be applied to proposed modifications. Challenges include calibrating optimal rigor of the review process, pruning diagnoses that lack validity or clinical utility, and subjecting the process itself to iterative improvement.

This chapter examines continuous sedation as a way to relieve unbearable suffering in patients at the end of life. After considering consensus and guidelines on continuous sedation, it looks at the debate over terminology and definition. It then discusses the practice of continuous sedation in various countries and how it is performed, along with the importance of patient consent and autonomy in all sedation guidelines. The chapter goes on to analyze some of the commonly invoked justifications for continuous sedation, including the doctrine of double effect, last resort and refractory suffering, autonomy and patient consent, and proportionality. It also reviews contentious issues raised by continuous sedation, such as whether it should be restricted to patients with a very short life expectancy, artificial nutrition and hydration, and existential or psychological suffering.

Individuals with obsessive-compulsive disorder (OCD) often have good insight into the irrational nature of their obsessions and the excessive character of their compulsions, but insight exists along a continuum and is markedly poor in some patients. This chapter reviews the assessment and phenomenological correlates of variable insight in OCD in both pediatric and adult populations. It reviews the definition of insight and its relationship to the evolution of diagnostic criteria for obsessive-compulsive disorder, as well as the major assessment tools used to measure and quantify insight for clinical and research purposes. The relationships between insight and clinical characteristics of OCD, including symptom severity, comorbidity, and treatment response are reviewed, followed by a review of neurobiological correlates of insight and the relationship between poor insight and schizophrenia spectrum disorders.

The idea of studying peace—rather than studying war, genocide, and political violence and then inferring about peace—has gained considerable traction in the past few years, after languishing in the shadows of conflict studies for decades. But how should peace be studied? The book offers a parallax view of how we think about peace and the complexities that surround the concept—that is, the book explores the topic from different positions at the same time. Toward this end, the authors review existing literature and provide insights into how peace should be conceptualized—particularly as something more than the absence of conflict. They provide an approach that can help scholars overcome what the authors see as the initial shock of unpacking the “zero” in the war–peace model of conflict studies. Additionally, they provide a framework for understanding how peace and conflict have and have not been related to one another in the literature. Finally, they put forward three alternative ways that peace can be studied, thereby avoiding any attempt to control the emerging peace research agenda and, rather, assisting in and encouraging thinking about a topic we all have some opinions on but that has yet to be measured and analyzed in a way comparable to that of political conflict and violence.

A syringe driver, or pump, represents a simple and cost-effective method of delivering a continuous subcutaneous infusion (CSCI). A CSCI provides a safe and effective way of drug administration and can be used to maintain symptom control in patients who are no longer able to take oral medication. There have been several developments in this field since the third edition of this highly successful book. The text in this edition has been completely revised, including a new chapter describing the compatibility and stability of drugs in addition to incorporating new treatment options and an extensive list of new compatibility data. This book serves as a valuable reference source, providing a comprehensive review of syringe pump use and administration of drugs via CSCI. The first chapter provides an overview of syringe pumps and CSCIs, including a useful array of frequently asked questions and reference to needlestick injuries. The new second chapter discusses the reasons why drugs in solution are at risk of compatibility and stability issues. The third chapter incorporates revised and referenced information relating to most drugs likely to be administered via a CSCI using a syringe pump. We begin by organizing different social-cognitive and self-regulatory perspectives along the temporal continuum of job loss and job search, and discuss the experience of job loss and its impact on the individual during subsequent job search. This chapter discusses key components of cardiovascular physiology applicable to clinical practice in the field of anaesthesiology. From theory development to ground-breaking innovations, the history of cardiac and vascular anatomy, as well as physiology, is presented. Utilizing knowledge of structure and function, parameters created have allowed adequate patient clinical assessment and guided interventions. A review of concepts reveals the impact of multiple physiological variables on a patient’s haemodynamic state and the need for more accurate and efficient measurements. In particular, it is noted that a more reliable index of ventricular contractility is the end-systolic elastance rather than the ejection fraction. Constant direct preload assessment has not yet been achieved but continues to be determined through surrogate variables, and continuous cardiac output monitoring for oxygen delivery, although advancing, has limitations. Considering the effect of compound factors perioperatively, especially heart failure, modifies the goals and interventions of anaesthetists to achieve improved outcomes. Therefore, medical management prior to surgery and complete assessment through history, physical examination, and diagnostic tests are a priority. This chapter also details the expectations following volume expansion to augment haemodynamics during surgery, the concept of functional haemodynamic monitoring, and limitations to the parameters applied in assessing fluid responsiveness. Challenging the accuracy of conventional indices to predict volume status led to the use of goal-directed therapy, reducing morbidity and minimizing length of hospital stay. The mainstay of this chapter is to reinforce the relevance of advances in haemodynamic monitoring and homeostasis optimization by anaesthetists during surgery, using fundamental concepts of cardiovascular physiology.

This chapter comments on Article 112 of the Rome Statute of the International Criminal Court. Article 112 establishes the Assembly of States Parties. The Assembly plays an important role in the operations of the Court, acting in its own right and through its subsidiary bodies such as the Committee on Budget and Finance. The Assembly meets once a year for about two weeks. Its responsibilities are assured on a continuous basis through its permanent Secretariat and its Bureau. In addition to what are essentially administrative functions, the Assembly of States Parties also has a legislative role. Through adoption and amendment of the Elements of Crimes and the Rules of Procedure and Evidence it exercises a significant influence upon the law applicable before the Court. It is also, together with the review conferences, the forum for amendments to the Rome Statute itself.

The relatively recent advent of magnetic resonance imaging has given us an invaluable ‘window’ into the brain in ADHD. Here we review the literature on the structural neuroimaging of ADHD throughout the lifespan. Meta-analyses and large individual studies converge to find anomalies in the basal ganglia in ADHD; some appear developmentally stable, while others are progressive. Compromise of the cerebral cortex and cerebellum are also commonly reported, and developmental trajectories of these structures have been linked with the highly variable clinical course of the disorder. ADHD can be considered dimensionally, lying at the extreme end of a continuous distribution of symptoms and underlying cognitive processes. Some studies find such dimensionality is also present in ADHD-related neuroanatomical change. Pilot studies have examined how variation in some candidate genes is tied to neuroanatomy in the disorder. Studies at the level of the entire genome await much larger cohorts.

The conclusion of the discursive history outlines that in contrast to studies that argue for a broad decline of Christian narratives of deathbed experiences in the early modern centuries, there is ample evidence of a continuous stream of Christian reports. These reports are, from the 18th century onward, seconded by a broad current of Spiritualist–Occult and Gnostic–Esoteric reports. Transmitters of these reports were mostly religiously interested individuals—preeminently spokespersons of non-mainstream churches and denominations such as Pietists, Theosophists, Occultists, and Spiritualists, joined, in the early 20th century, by parapsychologists. Most common are descriptions of a paradisiacal realm, tranquility, quietness, and feelings of peace or mental clarity. Handed down in religious contexts were descriptions of a border, God, or angels. However, other elements such as the life review (absent in premodern narratives), autoscopic out-of-body experiences, or the “tunnel” clearly developed over time and assumed new meanings.

Abstract:
This chapter provides a review and critique of empirical research on perceived discrimination and health. The patterns of racial disparities in health suggest that there are multiple ways by which racism can affect health. Perceived discrimination is one such pathway, and this chapter reviews the published research on discrimination and health. This recent research continues to document an inverse association between discrimination and health. This pattern is now evident in a wider range of contexts and for a broader array of outcomes. Advancing our understanding of the relationship between perceived discrimination and health will require more attention to situating discrimination within the context of other health-relevant aspects of racism, measuring it comprehensively and accurately, assessing its stressful dimensions, and identifying the mechanisms that link discrimination to health.

While infections have always played an important role in the history of mankind, advances in science and technology as well as rapid globalization have resulted in an unprecedented wave of new and old infections thrust into the limelight. The recent pandemic of H1N1 influenza virus infection demonstrates the recurrent theme of emerging and reemerging pathogens that continue to impact public health and patient care areas. Drug resistance among various organisms (not limited to bacteria) has unfortunately become the expectation and, not infrequently, we have been left with few or no efficacious treatment options, an experience not witnessed in more than 7 decades. Human immunodeficiency virus infection continues to challenge our abilities to provide the desired level of care in most areas of the world. Novel syndromes of infection continue to be defined as newer forms of immunosuppression and the development of unique medical devices become standard practice in all areas of medicine and surgery. For trainees and practitioners in the field of infectious diseases today, these factors mandate intense study to establish an expertise in the field that is required to provide best practices now and beyond. This board review will be pivotal in that education. This book is designed and intended primarily for infectious diseases trainees and practitioners preparing for the infectious disease subspecialty examination of the American Board of Internal Medicine. We believe that this book will also be useful to infectious diseases practitioners as well as general internists and other clinicians who desire a comprehensive but practical overview of contemporary infectious diseases topics.
