Full textA possible pathogenic mutation in the β3-adrenergic receptor gene (Trp64Arg) has been reported to be associated with Diabetes type 2, Non-insulin-dependent diabetes mellitus (NIDDM), in different group ethnic subjects. Since that, several mutations of candidate genes have been reported associated with Diabetes type 2 in Peruvians and subjects from other nationalities. It was investigated the frequency of polymorphism Trp64Arg in the β3-adrenergic receptor gene, biochemical and anthropometric measurements of 72 type 2 diabetic subjects, and 21 as control group, both genders, aged between 30 and 70 years all of them of the province Sullana (North of Perú) as this city as high prevalence of Type 2 Diabetes in the country. The laboratory study was conducted in: Peruvian Institute of Molecular Biology (IPBM), the Laboratory of Molecular Biology of Faculty of Veterinary Medicine and Faculty of Pharmacy and Biochemistry – Universidad Nacional Mayor de San Marcos. The analysis of the results was performed using the SPSS 15 version found that the frequencies of the genotypes of β3-adrenergic receptor gene in the study population was: 61.1% (44) Trp64Trp, 27.8% (20) Trp64Arg and 11.1% (8) Arg64Arg for diabetics, and: 57.1% (12) Trp64Trp, 14.3% (3) Trp64Arg and 28.6% (6) Arg64Arg to controls. Furthermore, we found the following allelic frequencies in the β3-adrenergic receptor gene in diabetic patients and controls for the Trp allele, 75% and 64.3% for Arg: 25.0% and 35.7% respectively, which suggests that the frequency allelic to the mutation (Arg) was slightly lower but not statistically significant (p same to 0.088). Diabetics were found with bad glycemic metabolic control and hyperglycemia. In applying the Student t test in diabetics and controls was observed that only the glycosylated Hemoglobin (Hb-G) had statistical significance (p same to 0.039), which could be associated with the Trp64Arg polymorphism. In the analysis of genotypes versus BMI, dyslipidemia and glycemic metabolic control in subject controls was significant difference for the BMI parameter in the different genotypes, but not in type 2 Diabetes. Of risk analysis found that Odds ratios in the total study subjects (diabetic controls more) Trp64Arg genotype achieved a degree of risk factor for overweight, dyslipidemia, and bad glycemic metabolic control, and that Arg64Arg genotype is a risk factor related with overweight and diabetes, but none had significant statistical difference. We conclude that the polymorphism under this study is important, but neither the only, nor the most decisive factor to develop type 2 Diabetes.

Full textTo evaluate the clinical and epidemiological characteristics of TB in patients with SLE, we design a casecontrol study in the Rheumatology service of the Hospital Nacional Guillermo Almenara. For this purpose, we reviewed the clinical charts of 387 patients with SLE who were hospitalized between 1990 and 2002. During this time we found 14 cases of TB matched wíth 28 patients with LES but without TB according to sex and age. Patients with TB had a higher time of SLE and received a higher accumulate dosis of steroidá. We did not found differences between the clinical presentation of SLE of thís groups. Seven cases (50%) correspond to patients with extrapulmonary TB. Respectively were included cases of osteoarticular, meningeal, pleural, endobronchíal, renal, pericardical and cutaneous TB. People with pulmonary TB had a higher time of SLE and patients with extrapulmonary TB had a higher frequency of TB history. The diagnoses of TB were made by study of acid fast bacilli in 617 cases of pulmonary TB and 417 cases of extrapulmonary TB. Conclusion: We found 14 cases (3.61 %) of TB in SLE patients. 50% correspond to extrapulmonary forms. Patients with TB had a higher time of SLE and received a higher dosage of steroids. We recommend strategies for an early diagnoses of TB in SLE patients and Isoniazid usage in all of them.

Full textDiversos estudios han evaluado las complicaciones derivadas de ovario-histerectomía en perras. Sin embargo, las reacciones adversas derivadas de estos procedimientos realizados en programas a gran escala, que se caracterizan por ser rápidas y mínimamente invasivas, aún no habían sido descritas. El objetivo de este estudio fue identificar las complicaciones intraoperatorias y postoperatorias en perras esterilizadas en programas masivos de control reproductivo, a través de un estudio longitudinal descriptivo. Se registró la incidencia de efectos adversos asociados a esta cirugía en 384 hembras caninas que asistieron a programas de control de natalidad en 5 comunas de las regiones de Valparaíso, Bío Bío y Metropolitana. Los datos se registraron en fichas individuales que incluían información general de los pacientes, así como de su estado sanitario y reproductivo y datos asociados al procedimiento quirúrgico, tales como tiempos quirúrgicos y tamaño de incisión. Las complicaciones fueron registradas durante los tiempos intraoperatorio, postoperatorio inmediato y postoperatorio a corto plazo y fueron descritas según el grado de compromiso (mayor y menor compromiso) y según tipo (técnica quirúrgica, incisión quirúrgica u otras complicaciones) y se establecieron relaciones entre la presentación de complicaciones y características de las pacientes y características de la cirugía. La media de la edad de las pacientes intervenidas fue de 31,81 +/- 30,93 meses, siendo un 63,11% de los pacientes de edad adulta (mayores a los 12 meses de edad y menores de 96 meses). El promedio de tiempo de realización de la cirugía fue de 16,7 +/- 10,43 minutos con un tamaño de incisión promedio de 2,8 +/- 0,87 cms. El total de pacientes con complicaciones fue 90 (23,44%) y el total de complicaciones fue 138. La mayor frecuencia de complicaciones fue durante el tiempo postoperatorio a corto plazo, y fueron de menor complejidad (63,04%); del total de complicaciones, la mayor cantidad fueron asociadas a la incisión quirúrgica durante el período postoperatorio a corto plazo (62,32%). No hubo muertes asociadas a la técnica quirúrgica en estudio. El mayor riesgo de presentar complicaciones se asoció a la existencia de uno o más partos previos y no existió relación entre la presencia de complicaciones y tiempo quirúrgico ni tamaño de incisión. Las características de este tipo de cirugía permiten clasificarla como una técnica con tiempos quirúrgicos reducidos y mínimamente invasiva, con una frecuencia y distribución de complicaciones semejante a los reportados por otros autores para cirugías tradicionales; en consecuencia, la técnica se presenta como segura para los programas masivos de esterilización

Full textIntroduction: Intensive therapy improves glycaemic control and reduces long-term diabetes complications. The Diabetes Control and Complications Trial (DCCT) and its extension, the Epidemiology of Diabetes Interventions and Complications Trial (EDIC), further demonstrated that optimization of glycaemic control generates a “metabolic memory” or “legacy effect” that retains these beneficial effects on micro- and macrovascular complications beyond the intervention period. Hypothesis: Establishing intensive therapy from the onset of type 1 diabetes and progressively adopting the technical improvements available leads to adequate control from the early stages of the disease and maintains this control over time. Early and continued optimization of glycaemic control modifies the natural history of late complications in type 1 diabetes. Methods: We analyzed a cohort of 138 consecutive, newly diagnosed type 1 diabetes patients included in an intensive therapy programme in the Endocrinology Department at Hospital de la Santa Creu i Sant Pau from 1985 to 1994. The age at onset was 24 ± 10 years and 60% were male. During the follow-up period of 20.1 ± 3.5 years, we evaluated the mean glycaemic control (mean annual HbA1C from the first year), associated cardiovascular risk factors, and microvascular and macrovascular complications developed during follow-up. Results: The mean HbA1C during follow-up was 7.27 ± 1%, being <7% in 43.5% of patients and <8% in 75.4%. Mean HbA1C in the early years correlated with subsequent long-term values, and mean HbA1C from the third year correlated with all the subsequent values. BMI increased over the course of the study, and the prevalence of overweight-obesity was similar to that in the general population in Spain. At study end, 12.9% of patients had metabolic syndrome. Overweight or obesity in the early years was associated with poor glycaemic control in later years. Overweight or obese patients in the 3rd or 4th year had worse control throughout the 20 years of the study ((HbA1C 7.6±1.1% and 7.5±1.1% vs 7.1±0.9% and 7.0±0.8%, respectively; p <0.05). Only 29% (40/138) of patients had chronic complications. Twenty-three patients (16.4%) had retinopathy (4 proliferative retinopathy), 14 (10.3%) had nephropathy (1 case of clinical albuminuria and renal failure), 16 (11.6%) had peripheral polyneuropathy, and 4 (2.9%) had macroangiopathy. Patients with complications had worse overall glycaemic control (7.7 ± 1.1 vs 7.0 ± 0.9; p <0.001) and had already presented worse glycaemic control in the first 5 years of diabetes (6.9 ± 1.5% vs 6.3 ± 1.2%; p <0.05). They also had a higher prevalence of overweight or obesity in the first years of the disease (31.3 vs 10.1% at 2 years; p <0.05) and a higher prevalence of metabolic syndrome at study end. Conclusion: In type 1 diabetes, adopting an intensive treatment programme at disease onset and progressively incorporating advances in technology and insulin preparations helps to achieve and maintain adequate long-term glycaemic control in most patients. This control is associated with a low prevalence of late complications, changing the natural history of the disease. Overweight or obesity and poor glycaemic control in early stages are markers of poor long-term control and future development of complications.
