Abstract:

Abstract The effects of filling the core box cavity and sand compaction in processes of core production by blowing methods (blowing, shooting) depend on several main factors. The most important are: geometrical parameters of cavity and complexity of its shape, number, distribution and shape of blowing holes feeding sands as well as the venting of a technological cavity. Values of individual parameters are selected according to various criteria, but mostly they should be adjusted to properties of the applied core sand. Various methods developed by several researchers, including the authors own attempts, allow to assess core sands properties on the basis of special technological tests projecting the process into a laboratory scale. The developed criteria defining a degree or a filling ability factor provide a better possibility of assessing the core sand behavior during flowing and core box filling, which indicate the value and structure of the obtained compacting decisive - after hardening - for strength and permeability. The mentioned above aspects are analyzed - on the basis of authors’ own examinations - in the hereby paper.

Objectives:The aim of the Consensus on Health Economic Criteria (CHEC) project is to develop a criteria list for assessment of the methodological quality of economic evaluations in systematic reviews. The criteria list resulting from this CHEC project should be regarded as a minimum standard.Methods:The criteria list has been developed using a Delphi method. Three Delphi rounds were needed to reach consensus. Twenty-three international experts participated in the Delphi panel.Results:The Delphi panel achieved consensus over a generic core set of items for the quality assessment of economic evaluations. Each item of the CHEC-list was formulated as a question that can be answered by yes or no. To standardize the interpretation of the list and facilitate its use, the project team also provided an operationalization of the criteria list items.Conclusions:There was consensus among a group of international experts regarding a core set of items that can be used to assess the quality of economic evaluations in systematic reviews. Using this checklist will make future systematic reviews of economic evaluations more transparent, informative, and comparable. Abstract:

A valid method to diagnose depression in palliative care has not been established. In this study, we aim to determine the prevalence of depression and the discriminant validity of the items of four sets of diagnostic criteria in palliative care. This is a cross-sectional study on 240 palliative care patients where the presence of depression was based on the Diagnostic and Statistical Manual of Mental Disorders, DSM–IV Criteria, Modified DSM–IV Criteria, Cavanaugh Criteria, and Endicott’s Criteria’s. Anxiety, depression, and distress were measured with Hospital Anxiety and Depression Scale and Distress Thermometer. The prevalence of depression among the palliative care patients was highest based on the Modified DSM–IV Criteria (23.3%), followed by the Endicott’s Criteria (13.8%), DSM–IV Criteria (9.2%), and Cavanaugh Criteria (5%). There were significant differences (p < 0.05) in the depressive symptoms showed by DSM–IV item 1 (dysphoric mood), item 2 (loss of interest or pleasure), and Endicott’s criteria item 8 (brooding, self-pity, or pessimism) among the palliative patients, even after adjustment for the anxiety symptoms and distress level. We found that dysphoric mood, loss of interest, and pessimism are the main features of depression in palliative patients. These symptoms should be given more attention in identifying depression in palliative care patients.

Abstract:

This paper presents the core design and performance characteristics of a 300 MWt small modular reactor (SMR) with fuel assemblies of the AP1000 reactor. Numerical calculations have been performed to evaluate a proper active core size and core loading pattern using the SRAC code system with the JENDL-4.0 data library and the CORBRA-EN code. The calculated temperature coefficients including fuel temperature, coolant temperature, and isothermal temperature coefficient provide adequate negative reactivity feedbacks. The thermal-hydraulic analysis reveals acceptable radial and axial fuel element temperature profiles with significant safety margin of fuel and clad surface temperature. A safety analysis using the CORBRA-EN code shows that the core will remain covered during the entire transient procedure of the fast transient of remarkably increasing power that would be caused by the ejection of control rod. The analysis results indicate that the core with a cycle length of 2.22 years is achievable while satisfying the operation and safety-related design criteria with sufficient margins.

Summary The data generated from special-core-analysis (SCAL) tests have a significant impact on the development of reservoir engineering models. This paper describes some of the criteria and tests required for the selection of representative samples for use in SCAL tests. The proposed technique ensures that high-quality core plugs are chosen to represent appropriate flow compartments or facies within the reservoir. Visual inspection and, sometimes, computerized tomography (CT) images are the main tools used for assessing and selecting the core plugs for SCAL studies. Although it is possible to measure the brine permeability (kb), there is no direct method for determining the porosity (f) of SCAL plugs without compromising their wettability. Other selection methods involve using the conventional-core-analysis data (k and f) on "sister plugs" as a general indicator of the properties of the SCAL samples. A selective technique ideally suited for preserved or "native-state" samples has been developed to identify reservoir intervals with similar porosity/permeability relationships. It uses a combination of wireline log, gamma scan, quantitative CT, and preserved-state brine-permeability data. The technique uses these data to calculate appropriate depth-shifted reservoir-quality index (RQI) and flow-zone indicator (FZI) data, which are then used to select representative plug samples from each reservoir compartment. As an example application, approximately 400 SCAL plugs from an Upper Jurassic carbonate reservoir in the Middle East were tested using the selection criteria. This paper describes the step-by-step procedure to select representative plugs and criteria for combining the plugs for meaningful SCAL tests. Introduction The main goal of coring is to retrieve core samples from a well to get the maximum amount of information about the reservoir. Core samples collected provide important petrophysical, petrographic, paleontological, sedimentological, and diagenetic information. From a petrophysical point of view, the whole-core and plug samples typically undergo the following tests: CT scan, gamma scan, conventional tests, SCAL tests, rock mechanics, and other special tests. The data are combined to get information on heterogeneity, depth shift between core and log data, whole-core and plug porosity and permeability, porosity/permeability relationship, fluid content (Dean-Stark), RQI, FZI, wettability, relative permeability, capillary pressure, stress/strain relationship, and compressibility. The petrophysical data generated in this way play important roles in reservoir characterization and modeling, log calibration, reservoir simulation, and overall field production and development planning. Among all the petrophysical tests, the SCAL tests (which include wettability, capillary pressure, and relative permeability determination) are critical and time-consuming. A reservoir-condition relative permeability test can sometimes run for several months when mimicking the actual flow mechanisms taking place in the field. Therefore, it is very important to design these tests properly and, in particular, to select the samples that ensure meaningful results. In short, the samples must be "representative samples," which can capture the overall variability within the reservoir in a more scientific way. Unfortunately, the most important aspect of all SCAL procedures, the sample selection, is one of those least discussed. According to Corbett et al. (2001), API's RP40 (Recommended Practices for Core Analysis) makes very little reference to sampling; similarly, textbooks on petrophysics do not have sections on sampling. The Corbett et al. paper reviewed the statistical, petrophysical, and geological issues for sampling and proposed a series of considerations. This has led to the development of a method (Mohammed and Corbett 2002) using hydraulic units in a relatively simple clastic reservoir. In this paper, some issues related to sample-selection criteria (with special focus on carbonate reservoirs) will be discussed. A large data set of conventional, whole-core, and special-core analyses on a well in an Upper Jurassic carbonate reservoir was used to characterize representative samples for SCAL tests.

Objective Evidence for the management of acute otitis externa (AOE) is limited, with unclear diagnostic criteria and variably reported outcome measures that may not reflect key stakeholder priorities. We aimed to develop 1) a definition, 2) diagnostic criteria and 3) a core outcome set (COS) for AOE. Study design COS development according to Core Outcome Measures in Effectiveness Trials (COMET) methodology and parallel consensus selection of diagnostic criteria/definition. Setting Stakeholders from the United Kingdom. Subjects and methods Comprehensive literature review identified candidate items for the COS, definition and diagnostic criteria. Nine individuals with past AOE generated further patient-centred candidate items. Candidate items were rated for importance by patient and professional (ENT doctors, general practitioners, microbiologists, nurses, audiologists) stakeholders in a three-round online Delphi exercise. Consensus items were grouped to form the COS, diagnostic criteria, and definition. Results Candidate COS items from patients (n = 28) and literature (n = 25) were deduplicated and amalgamated to a final candidate list (n = 46). Patients emphasised quality-of-life and the impact on daily activities/work. Via the Delphi process, stakeholders agreed on 31 candidate items. The final COS covered six outcomes: pain; disease severity; impact on quality-of-life and daily activities; patient satisfaction; treatment-related outcome; and microbiology. 14 candidate diagnostic criteria were identified, 8 reaching inclusion consensus. The final definition for AOE was ‘diffuse inflammation of the ear canal skin of less than 6 weeks duration’. Conclusion The development and adoption of a consensus definition, diagnostic criteria and a COS will help to standardise future research in AOE, facilitating meta-analysis. Consulting former patients throughout development highlighted deficiencies in the outcomes adopted previously, in particular concerning the impact of AOE on daily life.

The pressure hull is the primary element of submarine, which withstands diving pressure and provides essential capacity for electronic systems and buoyancy. This study presents a numerical analysis and design optimization of sandwich composite deep submarine pressure hull using finite element modeling technique. This study aims to minimize buoyancy factor and maximize deck area and buckling strength factors. The collapse depth is taken as a base in the pressure hull design. The pressure hull has been analyzed using two composite materials, T700/Epoxy and B(4)5505/Epoxy, to form the upper and lower faces of the sandwich composite deep submarine pressure hull. The laminated control surface is optimized for the first ply failure index (FI) considering both Tsai–Wu and maximum stress failure criteria. The results obtained emphasize an important fact that the presence of core layer in sandwich composite pressure hull is not always more efficient. The use of sandwich in the design of composite deep submarine pressure hull at extreme depths is not a safe option. Additionally, the core thickness plays a minor role in the design of composite deep submarine pressure hull. The outcome of an optimization at extreme depths illustrates that the upper and lower faces become thicker and the core thickness becomes thinner. However, at shallow-to-moderate depths, it is recommended to use sandwich composite with a thick core to resist the shell buckling of composite submarine pressure hull.

Despite the prevalence of work spouses, scarce empirical research has focused on the communication occurring within these relationships, leaving managers with little understanding as to whether organizations can or should communicate support for employees forming these relationships and how privacy is navigated among work spouses. Building on McBride and Bergen’s conceptualization of the work-spouse relationship, we used Communication Privacy Management theory (CPM) to understand what, if any, privacy rule decision criteria individuals used as they negotiated disclosures within their work-spouse relationships. Analysis of interviews with 41 people in work-spouse relationships suggests that participants recognized both their own core privacy rule decision criteria and when these criteria were similar to or different from the criteria influencing their work spouse’s. Furthermore, work-spouse relationships formed despite organizational efforts to keep them at bay. Theoretically, the findings add to CPM theory, such that they establish the need to examine catalyst criteria as current and previous, as well as argue for the addition of confirming criteria to account for situations in which catalysts reinforce routinized privacy rules. Overall, the findings from this study advance the literature on communication in the work-spouse relationship and CPM theory and highlight the role that workplaces play in fostering these types of relationships.

Abstract:

The article presents the use of .NET Core in web applications development. The analysis covers tests performed on a test application based on the .NET Core framework. The following benchmarking criteria were selected for the framework analysis: pattern search in text, counting parsing time, rendering operations on graphical files, adding files to archives, CRUD operation speed. These criteria were compared on two different operating systems - Windows, Linux. Additionally, the paper presents results obtained after the tests and their analysis. The hypothesis that tests will be executed faster on Windows than on Linux has only been partially confirmed.

This article looks at how a comprehensive list of one category of idioms, that of ‘core idioms’, was established. When the criteria to define a core idiom were strictly applied to a dictionary of idioms, the result was that the large number of ‘idioms’ was reduced to a small number of ‘core idioms’. The original list from the first source dictionary was added to by applying the same criteria to other idiom dictionaries, and other sources of idioms. Once the list was complete, a corpus search of the final total of 104 ‘core idioms’ was carried out in the British National Corpus (BNC). The search revealed that none of the 104 core idioms occurs frequently enough to merit inclusion in the 5,000 most frequent words of English.

Abstract:

This study aims to produce teaching materials in the form of CORE-based modules on geometry transformation materials and to determine the responses of students, educators to the modules developed and to determine the effectiveness of CORE-based modules on geometry transformation material. This research method is Research and Development (R&D) using the ADDIE model which consists of the stages Analysis, Design, Development, Implementation, Evaluation. Based on the result of the validation of the material expert and the module media are stated with the “proper” criteria. Student responses on a small scale and large scale (field), as well as educator responses to modules with criteria of attractiviness aspects “very interesting”. While the responses of educators to the modules developed obtained an average score of 4,42 with the criteria of attractiveness aspects “very interesting”. The results of the assessment of the effectiveness test obtained an N-Gain score of 0,694 with “moderate” qualifications. So teaching material in the form of CORE-based mathematics learning modules (Connecting, Organizing, Reflecting, Extending) on geometry transformation material is declared feasible, interesting, and effective to be used as teaching material in the learning process.

Of 106 patients attending a gender-identity clinic, 73% satisfied DSM–III criteria for transsexualism. These DSM–III positives had a significantly younger age of onset and were less likely to experience sexual arousal with cross-dressing than those who did not satisfy DSM–III criteria. They were also significantly more likely to fulfil a definition of ‘core transsexualism’. ‘Core transsexualism’ may represent a subgroup within DSM–III criteria for transsexualism and its defining features in this study were an early age of onset, low sexual activity, lack of sexual arousal with cross-dressing and homosexual orientation. Of the whole sample, 23% were referred for gender reassignment surgery, of whom 100% were DSM–III positive and 60% were ‘core transsexuals'.

Objective: Previous studies comparing atomoxetine and methylphenidate to treat ADHD symptoms have been equivocal. This noninferiority meta-analysis compared core ADHD symptom response between atomoxetine and methylphenidate in children and adolescents. Method: Selection criteria included randomized, controlled design; duration 6 weeks; and assessment of ADHD Rating Scale–IV–Parent Version: Investigator Administered and Scored (ADHDRS) scores. Six-week response rates, defined as ≥40% reduction in ADHDRS total score, were compared using a noninferiority margin of −15%. Results: Seven studies met inclusion criteria ( N = 1,368). After 6 weeks, 53.6% (95% confidence interval [CI] 48.6%-58.4%) of atomoxetine-treated patients ( n = 811) had responded compared with 54.4% (47.6%-61.1%) for methylphenidate ( n = 557), with atomoxetine demonstrating noninferiority to methylphenidate (absolute difference −0.9%, 95% CI −9.2%-7.5%). Conclusion: After 6 weeks of treatment atomoxetine and methylphenidate had comparable efficacy in reducing core ADHD symptoms in children and adolescents.

Abstract:

This paper presents the conceptual design of a 300 MWt small modular reactor (SMR)using fuel assemblies of the AP1000 reactor. Numerical calculations have been performed to evaluate a proper active core size and core loading pattern using the SRAC code system and the JENDL-4.0 data library. The analysis showed that Doppler, moderator temperature, void, and power reactivity coefficients are all negative over the core lifetime. Semi-analytical thermal hydraulics analysis reveals acceptable radial and axial fuel element temperature profiles with significant safety margin of fuel andclad surface temperature. The minimum departure from nucleate boiling ratio (MDNBR) is also calculated. The results indicate that a cycle length of 2.22 years is achievable while satisfying the operation and safety-related design criteria with sufficient margins.

Accurate time and budget is an essential estimate for planning software projects correctly. Quite often, the software projects fall into unrealistic estimates and the core reason generally owes to problems with the requirement analysis. For investigating such problems, risk has to identified and assessed at the requirement engineering phase only so that defects do not seep down to other software development phases. This article proposes a multi-criteria risk assessment model to compute risk at a requirement level by computing cumulative risk score based on a weighted score assigned to each criterion. The result of comparison with other approaches and experimentation shows that using this model it is possible to predict the risk at the early phase of software development life cycle with high accuracy.

Objectives: To identify adults with intellectual disability (ID) with a depressive disorder referred to a tertiary consultation clinic for psychiatric assessment; to investigate common presenting features of depression in adults with ID; to assess the utility of visual analogue scale (VAS) measures of emotion/behaviour, the CORE measure of psychomotor disturbance, and substitutive diagnostic criteria in the assessment of depressive disorders in this patient group. Method: Over a 6-month period 47 patients were seen for psychiatric evaluation. Patients in whom a diagnosis of depression was made were further assessed using: VAS measures of depression, irritability, verbal aggression, physical aggression, temper outbursts, regressed behaviour; CORE measure of psychomotor disturbance; and substitutive diagnostic criteria designed by the authors. Results: Ten patients were found to have a depressive disorder. Substitutive criteria resulted in a greater rate of diagnosis than standard DSM-IV criteria. The VAS measure of irritability was highly scored for all 10 depressed patients. All 10 depressed patients were assigned to the melancholic subgroup according to CORE score. Conclusions: Standard assessment measures and diagnostic criteria may require modification to enhance their utility in this patient group. Melancholic features require further investigation.

Core overtopping in embankment dams is an important phenomenon that may lead to contact erosion along the core–filter interface. This paper presents new experimental results regarding erosion mechanisms at the core–filter interface during core overtopping. The experimental results were obtained using a reduced-scale model with a variable upstream water level. Digital image correlation (DIC), microcomputed tomography (μCT), and sediment collection at the outlet were used to quantify erosion. Four experimental runs were conducted with a till core and different filters. Only one of the four filters satisfied the filter criteria that were applied. No contact erosion occurred during this test. For filters that did not respect the filter criteria, piping occurred within the core along the downstream slope when the water level reached the top of the core. As a result of the self-healing process within the core material, the erosion rate decayed with time as the hydraulic gradient increased. Results for DIC mainly reflected settlements within the filter due to erosion and a soil arching effect. The magnitude of the displacement vector obtained with DIC is directly proportional to the volume of till eroded. μCT showed that contact erosion occurred continuously.

No national guideline for mass casualty triage exists in the United States. The lack of a national guideline has resulted in variability in triage processes, tags, and nomenclature. This variability has the potential to inject confusion and miscommunication into the disaster incident, particularly when multiple jurisdictions are involved. The Model Uniform Core Criteria for Mass Casualty Triage were developed to be a national guideline for mass casualty triage to ensure interoperability and standardization when responding to a mass casualty incident. The Core Criteria consist of 4 categories: general considerations, global sorting, lifesaving interventions, and individual assessment of triage category. The criteria within each of these categories were developed by a workgroup of experts representing national stakeholder organizations who used the best available science and, when necessary, consensus opinion. Purpose Core needle biopsies (CNBs) are widely used to determine human epidermal growth factor receptor 2 (HER2) status in breast cancer. Recent publications reported up to 20% false-positive results on CNBs if immunohistochemistry (IHC) is compared with fluorescent in situ hybridization (FISH). To clarify, if confirmation of IHC positivity by FISH is generally required, we analyzed the reliability of IHC positivity on CNBs versus surgical specimens in a multi-institutional study. Patients and Methods Five pathologic laboratories contributed to this study by performing IHC on 500 CNBs and the corresponding surgical specimens overall. If IHC revealed score 2+ or 3+, HER2 status was confirmed by FISH in a central laboratory. We compared evaluation according to US Food and Drug Administration–approved scoring criteria and recently published American Society of Clinical Oncology (ASCO)–College of American Pathologists (CAP) guidelines. Results CNBs scored 3+ revealed five false-positive results if scoring followed the US Food and Drug Administration criteria (five of 40; 12.5%) and two false-positives in terms of the ASCO-CAP criteria (two of 33; 6.1%). IHC was false negative in one CNB only. By contrast, IHC on surgical specimens revealed five false-negative results, but only one false-positive result (one of 35; 2.9%) if scored following US Food and Drug Administration–approved criteria. With the aid of the ASCO-CAP criteria, false-positive IHC results were obtained in only one of the five participating institutions. Conclusion IHC 3+ scores on CNBs proved to be reliable in four of the five participating institutions if scoring followed the ASCO-CAP criteria. Therefore, accurate determination of HER2 status in breast cancer is possible on CNB using the common strategy to screen all cases by IHC and retest only 2+ scores by FISH. Prerequisites are quality assurance and the application of the new ASCO-CAP criteria.

Based on classical dynamic cylindrical cavity expansion theory, a nine-step penetration and perforation process of aluminum foam sandwich targets by truncated cone-nosed projectiles are developed theoretically. In the theoretical model, the friction, shear strength, and the force for tearing the cells in the core at the periphery of the projectile are considered, and the resistance force and instantaneous velocity are achieved from this process. On this basis, the effects of the geometry of projectiles, core thickness, and impact velocities of projectiles on absorbed energy are also analyzed. Simple composite failure criteria will be applied in the fracture and perforation of the face sheet, core, and back sheet. It is shown that the diameter of projectile and core thickness have significant influence on the ballistic velocity of the projectile, which is important for the impact response and absorbed energy of the sandwich. Numerical simulation at various impact velocities is also performed, and there is a good agreement between the numerical predictions and the analytical measurements.

AbstractLong-scale preformed plasmas are generated inside the cone by the pre-pulse of the heating laser in the cone-guided fast ignition scheme and it is found that coupling efficiency from the heating laser to fast electrons especially suitable for core heating is drastically reduced by the preformed plasmas. To mitigate this serious problem, an extremely thin film is suggested to cover the entrance of the cone. This method, however, introduces long rarefied plasmas around the entrance of the cone and the main pulse must propagate through these plasmas. Therefore, fast electron characteristics produced by the main pulse could be affected, and effects of long rarefied plasmas on fast electron generation are investigated. It is found that the electron beam intensity becomes larger than that without the rarefied plasma, but the energy coupling rate from the heating laser to the core decreases due to lack of appropriate electrons for core heating. To achieve less than 10% degradation of the core electron temperature, the thin film must be expanded by irradiation of the pre-pulse so that the length and the density of rarefied plasmas become less than 500 µm and one-tenth of the critical density. A thickness of the thin film can be determined by these criteria and the intensity of the pre-pulse.

Here we presented the classification contribution function (CCF), a new concept based on rough sets theory, which is regarded as the criteria for choosing attributes in the core of attributes. The basic idea of CCF is using of discernibility matrix to determine attribute core (if there is no core, using its reduction). Then through the classification contribution function to determine core classification contribution value and employ the big value as node. Next employing the selected attribute ways to divide decision-making system and each value attribute can produce a subset. The experiments show that, being compared with the entropy based C4.5 and weighted mean roughness, our method can get simpler decision tree and improve the efficiency of classification.

179 Background: The safe delivery of chemotherapy is of utmost importance to patients, health care providers and systems administrators. Cancer Care Ontario and the Program for Evidence-Based Care published two guidelines addressing issues of chemotherapy safety: Safe Handling of Parenteral Cytotoxics (Safe Handling) in 2007 and Key Components of Chemotherapy Labeling (Safe Labeling) in 2009. Concordance with the two documents was evaluated across systemic treatment hospitals in Ontario in order to direct quality improvement initiatives. Methods: Since 2009, concordance with Safe Handling has been assessed annually by self-reported questionnaire as part of the regular provincial performance review process. In 2011, concordance with Safe Labeling was assessed by submission and scoring of five standardized chemotherapy labels demonstrating core and additional criteria. Core criteria included patient and drug information, administration information, as well as adherence to other guidelines related to abbreviations and units of measurement. Individualized regional scorecards were compiled highlighting areas of discordance and then used to develop improvement plans. Results: As of October 2011, 98% of Ontario hospitals providing chemotherapy had updated policies and procedures in place for the safe handling of cytotoxics, significantly improved compared to a baseline of 77% in 2009. Baseline evaluation of Safe Labeling performed in 2011 showed a provincial average of 64% concordance for core criteria and 63% for additional criteria. Regionally, concordance scores ranged from 55% to 80% for core criteria and 38% to 75% for additional criteria. Conclusions: Concordance with safety practice guidelines has the potential to improve significantly with continued measurement and performance management. A baseline assessment serves to identify areas for quality and process improvements. Continued evaluation of guideline concordance is necessary in order to achieve the desired target rate for the province.

The present work studies the effect of introducing MOX fuel on Westinghouse AP1000 neutronic parameters. The neutronic calculations were performed by using the MCNP6 code with the ENDF/B-VII.1 library and the new release of the ENDF/B-VIII, the AP1000 core with three 235U enrichment zones (2.35 %, 3.40 %, and 4.45 %). The obtained results showed that the simulated model for the AP1000 core satisfies the optimization criteria as a Westing- house reference. The results which included: effective multiplication factor, keff, delayed neutron fraction, beff, excess reactivity, rex, shutdown margin, temperature reactivity coefficients, whole core depletion, neutron flux, power peaking factor and core power density, were calculated and compared with the available published results. The keff in the cold zero power was found to be 1.20495 and 1.20247 with the ENDF/B-VII.1 and the ENDF/B-VIII libraries, respectively, which matches the value of 1.205 presented in the AP1000 Design Control Document for the UO2 fuel core. On the other hand, keff in the cold zero power was found to be 1.19988 and 1.19860 for MOX core with the ENDF/B-VII.1 and the ENDF/B-VIII libraries, respectively, which show good reception and confirm the safety of the design and efficient modeling of AP1000 reactor core.

Abstract:

115 Background: Active surveillance (AS) is an established option for men with prostate cancer. Studies have shown that multiparametric-MRI along with MRI-TRUS fusion-guided biopsy (FB) may better assess risk in patients eligible for AS, compared to 12-core biopsy, due to improved detection of clinically significant cancers. The objective is to evaluate the performance of expanded criteria eligibility in men on AS being monitored with MRI-TRUS guided biopsy. Methods: Men on AS were included if they had mp-MRI and pathology data for 2 or more FB sessions. FB procedures consisted of targeted biopsies and random 12 core biopsies. Men participated in AS with low and intermediate risk prostate cancer, Gleason score ≤ 3+4=7 with no restriction on percent core involvement or number of cores positive. Progression was defined by patients with initial Gleason 3+3=6 to any Gleason 4, and Gleason 3+4=7 disease progressing to a primary Gleason 4 or higher. Results: 124 men on AS met study criteria. Low risk men had a mean age of 61.3 years versus intermediate risk men with a mean age of 65.5 years (p=0.0062). Mean PSA levels of the low and intermediate risk groups were 5.8 and 5.76 ng/ml (p=0.95), respectively. The mean length of follow-up was 22.56 months (range: 3.6 – 74.4 mo). Rates of pathologic progression in the intermediate and low risk patients were, 38.5% vs. 28.5% (p=0.33). Intermediate risk men had a mean progression-free survival (PFS) of 2.8 years compared to low risk men of 3.9 years (p=0.27). Patients were stratified according to established AS criteria (Epstein, Toronto, PRIAS) and rates of progression are summarized in the Table. 69% of patients met Epstein criteria for AS of which 29.4% (20/68) progressed compared to 28.5% for the low risk cohort overall. Conclusions: Men in our cohort who met strict criteria for AS had the same rate of progression as the entire expaned criteria low risk cohort, 29.4% vs 28.5%, respectively. Our data suggests that with accurate initial Gleason classification other AS criteria such as percent core or number of cores positive have no added benefit in predicting which men may have reclassification or progression of disease. To develop response criteria for adult dermatomyositis (DM) and polymyositis (PM). Expert surveys, logistic regression, and conjoint analysis were used to develop 287 definitions using core set measures. Myositis experts rated greater improvement among multiple pairwise scenarios in conjoint analysis surveys, where different levels of improvement in 2 core set measures were presented. The PAPRIKA (Potentially All Pairwise Rankings of All Possible Alternatives) method determined the relative weights of core set measures and conjoint analysis definitions. The performance characteristics of the definitions were evaluated on patient profiles using expert consensus (gold standard) and were validated using data from a clinical trial. The nominal group technique was used to reach consensus. Consensus was reached for a conjoint analysis-based continuous model using absolute per cent change in core set measures (physician, patient, and extramuscular global activity, muscle strength, Health Assessment Questionnaire, and muscle enzyme levels). A total improvement score (range 0–100), determined by summing scores for each core set measure, was based on improvement in and relative weight of each core set measure. Thresholds for minimal, moderate, and major improvement were ≥20, ≥40, and ≥60 points in the total improvement score. The same criteria were chosen for juvenile DM, with different improvement thresholds. Sensitivity and specificity in DM/PM patient cohorts were 85% and 92%, 90% and 96%, and 92% and 98% for minimal, moderate, and major improvement, respectively. Definitions were validated in the clinical trial analysis for differentiating the physician rating of improvement (p<0.001). The response criteria for adult DM/PM consisted of the conjoint analysis model based on absolute per cent change in 6 core set measures, with thresholds for minimal, moderate, and major improvement.

Abstract:

Objectives:This study describes the health technology assessment (HTA) framework introduced by Regione Lombardia to regulate the introduction of new technologies. The study outlines the process and dimensions adopted to prioritize, assess and appraise the requests of new technologies.Methods:The HTA framework incorporates and adapts elements from the EUnetHTA Core Model and the EVIDEM framework. It includes dimensions, topics, and issues provided by EUnetHTA Core Model to collect data and process the assessment. Decision making is instead supported by the criteria and Multi-Criteria Decision Analysis technique from the EVIDEM consortium.Results:The HTA framework moves along three process stages: (i) prioritization of requests, (ii) assessment of prioritized technology, (iii) appraisal of technology in support of decision making. Requests received by Regione Lombardia are first prioritized according to their relevance along eight dimensions (e.g., costs, efficiency and efficacy, organizational impact, safety). Evidence about the impacts of the prioritized technologies is then collected following the issues and topics provided by EUnetHTA Core Model. Finally, the Multi-Criteria Decision Analysis technique is used to appraise the novel technology and support Regione Lombardia decision making.Conclusions:The VTS (Valutazione delle Tecnologie Sanitarie) framework has been successfully implemented at the end of 2011. From its inception, twenty-six technologies have been processed.

Abstract:

To develop response criteria for juvenile dermatomyositis (DM). We analysed the performance of 312 definitions that used core set measures from either the International Myositis Assessment and Clinical Studies Group (IMACS) or the Paediatric Rheumatology International Trials Organisation (PRINTO) and were derived from natural history data and a conjoint analysis survey. They were further validated using data from the PRINTO trial of prednisone alone compared to prednisone with methotrexate or cyclosporine and the Rituximab in Myositis (RIM) trial. At a consensus conference, experts considered 14 top candidate criteria based on their performance characteristics and clinical face validity, using nominal group technique. Consensus was reached for a conjoint analysis–based continuous model with a total improvement score of 0–100, using absolute per cent change in core set measures of minimal (≥30), moderate (≥45), and major (≥70) improvement. The same criteria were chosen for adult DM/polymyositis, with differing thresholds for improvement. The sensitivity and specificity were 89% and 91–98% for minimal improvement, 92–94% and 94–99% for moderate improvement, and 91–98% and 85–86% for major improvement, respectively, in juvenile DM patient cohorts using the IMACS and PRINTO core set measures. These criteria were validated in the PRINTO trial for differentiating between treatment arms for minimal and moderate improvement (p=0.009–0.057) and in the RIM trial for significantly differentiating the physician's rating for improvement (p<0.006). The response criteria for juvenile DM consisted of a conjoint analysis–based model using a continuous improvement score based on absolute per cent change in core set measures, with thresholds for minimal, moderate, and major improvement.
