Abstract:
Widows and four groups of professionals (clergy, physicians, counselors, and funeral directors) were compared in regards to their perceptions of conjugal bereavement. Results suggested consistent differences between widows' self-perceptions and professionals' views regarding the impact of bereavement and the necessity for adaptative coping mechanisms to deal with loss. Professionals saw bereavement as having a more negative impact and requiring the use of coping skills to a greater extent than did widows. Such differences remained when age at bereavement or length of widowhood were taken into account. Professionals' perceptions of conjugal bereavement were similar to one another. Help-seeking behaviors, age at bereavement, and length of bereavement differentiated widows, while experience in counseling widows failed to separate professionals. Both age-related experiential differences with loss and gender differences in coping with loss may explain these findings. These data underscore the importance of efforts to bring widows and professionals into direct contact with one another in order to lessen misperceptions of conjugal bereavement.

Abstract:
A reductive reading of Humanae vitae seeks to limit its appeal to a ban on contraception. In truth, however, it offers a vision of human sexuality and conjugal love with broad and enduring relevance. In setting forth the intrinsic complementarity and irreducibility of the unitive and procreative dimensions of the conjugal act, Paul VI has given us a hermeneutical key for assessing many contemporary ethical dilemmas in human reproductive medicine. From this perspective, this article seeks to apply the logic of Humanae vitae to several real-life scenarios confronted by medical practitioners, educators, and ethicists working in the field of fertility and reproductive health. These include a consideration of the ethics of prescribing hormonal contraceptives, the possibilities of investigating male infertility, issues of cooperation in counseling and assisting conception in same-sex relationships, the ethics pertaining to assisted reproductive technology (ART), the contested case of prenatal adoption, and the application of double-effect reasoning. Summary: On the occasion of the fiftieth anniversary of the promulgation of Pope Paul VI’s encyclical Humanae vitae, this article seeks to defend its enduring relevance to modern-day society, through application of its reasoning to contemporary dilemmas in reproductive medicine. It considers real cases of the ethics of prescribing hormonal contraceptives, of investigating male infertility, of cooperating in counseling and assisting conception in same-sex relationships, of ART, of prenatal adoption, and the application of double-effect reasoning.

Abstract:
The book Mediation of family conflicts published by Pro Universitaria Publishing House in December 2020 is the latest publication by the authors Apostu Iulian and Voinea Maria. The paper develops a theoretical approach on the evolution of conjugal structures in the Romanian space, highlighting the resources that define these changes and their effects in terms of functional changes in the family. The originality of the work derives from the new vision that the book brings in this dilemma - mediation or separation – thus synthesizing the sociological and psychological elements about mediation, counseling and therapy. The book is an excellent landmark for those who want to understand the idea of mediation, an action viewed from a threefold perspective: sociological, psychological and of intervention in social work.

Abstract:
Objetivo: analisar a frequência de sífilis entre os usuários do Centro de Testagem e Aconselhamento de Montes Claros, Minas Gerais, e os fatores associados à infecção. Método: trata-se de um estudo transversal, com componentes descritivos e analíticos, realizado com usuários atendidos no serviço entre 2014 e 2019. Os dados foram coletados de fonte secundária, por amostragem aleatória e sistemática. Resultados: a amostra foi composta por 957 formulários de usuários e a frequência de casos de testes rápidos reagentes para sífilis foi de 11,3%, com distribuição semelhante entre os sexos. O diagnóstico da sífilis se associou de forma significativa às variáveis: situação conjugal, idade, escolaridade, quantidade de parcerias sexuais, orientação sexual e uso de drogas no último ano. Conclusão: programas de aconselhamento e testagem rápida devem ser incentivados para prevenção e diminuição das infecções sexualmente transmissíveis em Montes Claros e em todo país.

Durant la deuxième moitié du xxe siècle, l’entretien (counseling) centré sur la personne, associant une écoute dite active respectant le patient/usagers a fait ses preuves dans la relation aidant /aidé. Ces techniques ont pu s’adapter en fonction des cultures, des contextes et des différents topiques. Elles ont été utilisées dans de très larges spectres (conseil conjugal, accompagnement à la mort, révélation de maladies graves…). Le début du xxie siècle a permis de décrire de nouveaux outils permettant des entretiens plus rapides et tout aussi efficaces dans certains domaines tels que les entretiens pour améliorer l’adhésion aux thérapeutiques ou pour trouver ses propres ressources pour faire face à ses pratiques à risques. Ces dernières années de nouveaux outils sont utilisés pour améliorer l’efficacité sur le changement des comportements à risque et avoir une approche plus globale de l’individu et de son environnement dans la vraie vie. Ces outils sont principalement utilisés dans le cadre du model IMB ou « Intervention, Motivation and Behavior » (comportement). La brève communication est un processus de communication qui peut être utilisé par tous les professionnels de santé, formé à des techniques d’écoute brève, elle se réalise en moins d’une demi-heure, souvent en quelques minutes après avoir entendu la demande du patient, et avoir intégré ses déterminants environnementaux.

Abstract:
This paper looks at the journey of eleven counsellors in marital counselling centres in French-speaking Belgium, from the creation of the centres in 1953, to the 1970s, when contraception became legal, and abortion became a public issue. At the time of Humanae Vitae, groups of volunteers, working within Catholic organisations where counselling took place, began to structure their activity around Carl Rogers’s ethics of client-centred therapy, placing their religious ideology in a secondary position to focus on the problems experienced by the couples and women they were receiving in the centres. These were often challenges they were experiencing themselves in their own lives. The reiteration of the Catholic orthodox view on contraception through Humanae Vitae marked a gap between the counsellors and the Church. This contribution questions the identity-related tension of Catholics working in conjugal counselling centres and the type of commitments they made to both the conjugal centres and the Church in a moment where family planning was debated both in the Church and politically.

Abstract:
(1) Background: Guidelines on vaginal breech delivery require birth weight restrictions and neglect the impact of pelvic measurements despite contradicting evidence. There is a great need for more evidence on delivery outcome predicting factors for patients counselling. (2) Methods: We performed a prospective cohort study on 748 primiparous women intending vaginal breech birth and analyzed combined influence of fetal birth weight (BW) and the obstetric conjugate (conjugate vera obstetrica, CVO) on delivery outcome. (3) Results: We generated a BW/CVO ratio and devided our study cohort at median (257.8 g/cm) into a low ratio group (LR, with low birth weight and wide obstetric conjugate) and a high ratio group (HR, high birth weight and narrow obstetric conjugate). Cesarean section (CS) rate was significantly higher in HR (50.3%) as compared to LR (28.3%, p < 0.0001). Fetal morbidity was not different. In vaginally completed deliveries duration of birth was significantly longer in vHR (557 min) as in vLR (414 min, p < 0.001). Manual assistance to deliver the arms (‘Louwen maneuver’) positively correlated with birth weight (r2 = 0.215; p = 0.005) and the BW/CVO ratio (r2 = 0.0147; p = 0.02). (4) Conclusions: A high fetal birth weight combined with a tiny CVO predicts higher cesarean section probability, longer birth duration and the necessity to perform arm delivery assistance. Birth weight and pelvic measurements should be topics of great importance in patients counselling.

RESUMEN El estudio se sitúa en el ámbito de la orientación educativa y se centra en indagar cómo los asesores psicopedagógicos intervienen en el desarrollo y la formación permanente de los profesionales de la educación para mejorar la educación inclusiva. La nueva organización de la red de Centros de Formación, Innovación y Recursos (CEFIRE) de la Comunidad Valenciana exige repensar los modelos de asesoramiento psicopedagógico y la práctica orientadora desde las funciones otorgadas y las asumidas. Se presentan los resultados de un estudio empírico cuyo objetivo es profundizar en los roles desempeñados y las estrategias utilizadas por los orientadores-asesores para impulsar el avance de la educación inclusiva en los centros educativos. Metodológicamente se adopta un enfoque cualitativo que permite realizar un estudio descriptivo a través de grupos focales en los que participan 22 asesores de educación inclusiva de todos los CEFIREs del territorio. Como resultado se obtiene que acompañar, dinamizar, o gestionar recursos, son algunos de los roles desempeñados y que la creación de redes de recursos, el impulso de las metodologías inclusivas o las visitas pedagógicas se identifican como las estrategias más efectivas. En las conclusiones se destaca la importancia de conjugar diversas estrategias para favorecer la movilización del conocimiento y el avance de las prácticas inclusivas. Finalmente se plantean los retos a los que se enfrentan los asesores para desarrollar un modelo de acompañamiento colaborativo más acorde con el enfoque de la inclusión y con las consiguientes implicaciones en el campo de la orientación y la psicopedagogía.ABSTRACT The study is situated in the field of educational counselling and focuses on investigating how educational psychologists are involved in the development and in-service training of educational professionals to improve inclusive education. The new organisation of the network of Training, Innovation and Resource Centres (CEFIRE) in the Valencian Community requires rethinking the models of educational psychology counselling and counselling practice from the perspective of the functions provided and those assumed. We present the results of an empirical study that aims to deepen the roles played and the strategies used by counsellors-advisers to promote the advancement of inclusive education in educational centres. Methodologically, a qualitative approach is adopted that allows a descriptive study to be carried out through focus groups in which 22 inclusive education advisers from all the CEFIRE centres in the territory participate. As a result, some of the roles played include accompanying, energizing and managing resources. The creation of resource networks, the promotion of inclusive methodologies or pedagogical visits are identified as the most effective strategies. The conclusions highlight the importance of combining various strategies to promote the mobilization of knowledge and the advancement of inclusive practices. Finally, the challenges faced by advisers in developing a collaborative accompaniment model more in line with the approach to inclusion and with the consequent implications in the field of counseling and psychopedagogy are presented.

ObjectiveTo develop and evaluate interventions to improve quality of care in 4 priority areas in an urban safety net adult rheumatology clinic serving a racially/ethnically and socioeconomically diverse patient population.MethodsThe Institute for Healthcare Improvement’s Model for Improvement was used to redesign clinical processes to achieve prespecified benchmarks in the following areas from 2015 to 2017: 13-valent pneumococcal conjugate vaccine (PCV13) administration among immunocompromised patients; disease activity monitoring with the Clinical Disease Activity Index (CDAI) for patients with rheumatoid arthritis; latent tuberculosis infection (LTBI) screening for new biologic users with RA; and reproductive health counseling among women receiving potentially teratogenic medications. We measured performance for each using standardized metrics, defined as the proportion of eligible patients receiving recommended care.ResultsThere were 1205 patients seen in the clinic between 2015 and 2017. Regarding demographics, 71% were women, 88% identified as racial/ethnic minorities, and 45% were eligible for at least 1 of the quality measures. Shewart charts for the PCV13 and CDAI measures showed evidence of improved healthcare delivery over time. Benchmarks were achieved for the CDAI and LTBI measures with 93% and 91% performance, respectively. Performance for the PCV13 and reproductive health counseling measures was 78% and 46%, respectively, but did not meet prespecified improvement targets.ConclusionThrough an interprofessional approach, we were able to achieve durable improvements in key rheumatology quality measures largely by enhancing workflow, engaging nonphysician providers, and managing practice variation.

Abstract:
Horizontal gaze palsy with progressive scoliosis (HGPPS) is a rare, inherited disorder characterized by a congenital absence of conjugate horizontal eye movements with progressive scoliosis developing in childhood and adolescence. Mutations in the Roundabout (ROBO3) gene located on chromosome 11q23–25 are responsible for the development of horizontal gaze palsy and progressive scoliosis. However, some studies redefined the locus responsible for this pathology to a 9-cM region. This study carried out a systematic review in which 25 documents were analyzed, following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) standards. The search was made in the following electronic databases from January 1995 to October 2019: PubMed, Scopus, Web of Science, PEDRO, SPORT Discus, and CINAHL. HGPPS requires a multidisciplinary diagnostic approach, in which magnetic resonance imaging might be the first technique to suggest the diagnosis, which should be verified by an analysis of the ROBO3 gene. This is important to allow for adequate ocular follow up, apply supportive therapies to prevent the rapid progression of scoliosis, and lead to appropriate genetic counseling.

Background: Most of the couples suffering from infertility report it to be the most stressful and depressing period of their life, more so if it is a primary infertility. Studies regarding the prevalence and role of infertility-specific stress especially in eastern part of India is very limited. The objective of the present study was to estimate the prevalence of infertility-specific stress and its role in marital adjustment in women diagnosed with infertility.Methods: It was a cross-sectional study done on 80 married couple diagnosed with infertility (both primary and secondary) over 1 year from July 2016 to June,2017. Dyadic Adjustment Scale (DAS), "semi-structured questionnaire" compiled by the authors and "ICD-10 Classification of Mental and Behavioral Disorders (Clinical Descriptions and Diagnostic Guidelines)" were used for the evaluation. The analysis was done using SPSS (version 16) and Chi-square test.Results: Around 86% infertile women and 21% infertile men were found to suffer from mental stress. Infertility related stress were more in patients with primary infertility than in secondary one. Women mostly (56.5%) coped with stress by self-blaming whereas men (58.2%) by blaming the partner.Conclusions: Mental stress was significantly associated with infertility. In fact, maladjustment in marital relation caused by the stress adversely affected the conjugal life and thus also the fertility. Proper counselling of both partners might be helpful to solve this problem.

Knowing the determinants of couple adjustment is a challenge, both for predicting this adjustment and for helping couples in therapy in the best possible way. We based this study on the Person’s Social Value Theory (Beauvois, J.-L. [1976]. The topic of social conduct evaluation. Connexions, 19, 7-30) which postulates that two dimensions – social utility and social desirability – support self- and other- descriptions. This study aimed to evaluate the way the evaluation of own social value within couple and the evaluation of social value within couple of the partner influence the dyadic adjustment of the spouses. In addition, we took into account the duration of the couples and the emotional competences of the spouses (using the PEC). Participants were the spouses of 152 voluntary heterosexual couples who completed a booklet of questionnaires. The results showed that the two dimensions of person’s social value within couple influence partners' dyadic adjustment but in a different way for men and women and according to the duration of the couples’ relationship. Furthermore, the effect of social value within couple seems to cover partially the classic effect of emotional competences on couple experience and satisfaction. The discussion underlines the relevance and interest of using the social value within the couple in the study of conjugal relationships as well as in counselling couples.

The study focused on the conduct of integrated health services in real hard-to-reach/under-served communities to address maternal and child health problems from 2016–2018, riding on the Global Vaccine Action Plan that was conducted in Seventeen (17) Local Government Areas at risk of sustaining polio transmission in Borno State from July 2014 to December 2015, it was further expanded to 1712 for both real Hard-to-reach and Under-served communities in 25 accessible Local Government areas for integrated Health services using two strategies, Real Hard-to-reach areas and under-served communities which targeted newly liberated communities including internally displaced persons between 2016 to 2018. The findings from the study show treatment of minor ailment, 628731 clients were seen, and 9482 were referred to next health facilities for proper diagnosis and management, 88421 pregnant mothers received intermittent preventive therapy, sensitization on exclusive breast feeding, complementary feeding, focused antenatal care, Water sanitation, and hygiene. Furthermore, the findings show that oral polio, measles, hepatitis, yellow fever, tetanus toxoid, and pneumococcal conjugate vaccination was carried out for children from 0-11 months, while tetanus vaccination was carried out for pregnant women. Based on the findings of the study, it was concluded that the mobile health teams had improved the health status of Children, women of childbearing age, pregnant mothers, and other age groups in Real Hard-to-reach and under-served communities like Antenatal/post-natal, treatment of minor ailments, vaccination, nutritional screening, HIV testing counselling and referrals, Gender-based violence identification counselling and referrals and de-worming.

This article is an ethnographic exploration of a women’s sharia court in Mumbai, a part of a network of such courts run by women qazi (Islamic judges) established across India by members of an Islamic feminist movement called the Bharatiya Muslim Mahila Andolan (Indian Muslim Women’s Movement). Building upon observations of adjudication, counselling, and mediation offered in cases of divorce and maintenance by the woman qazi (judge), and the claims made by women litigants on the court, this article explores the imaginaries of the heterosexual family and gendered kinship roles that constitute the everyday social life of Islamic feminism. I show how the heterosexual family is conceptualised as a fragile and violent institution, and divorce is considered an escape route from the same. I also trace how gendered kinship roles in the heterosexual conjugal family are overturned as men fail in their conventional roles as providers and women become breadwinners in the family. In tracing the range of negotiations around the gendered family, I argue that the social life of Islamic feminism eludes the discourses and categories of statist legal reform. I contribute to existing scholarship on Islamic feminism by exploring the tension between the institutionalist and everyday aspects of Islamic feminist movements, and by exploring the range of kinship negotiations around the gendered family that take place in the shadow of the rhetoric of ‘law reform’ for Muslim communities in India.

Background: Streptococcus pneumoniae is the main bacterial pathogen of community-acquired pneumonia in Korea. This study aimed to enhance the vaccination rate by evaluating the effectiveness of current methods of recommendation.Methods: A retrospective analysis of the medical records of 143 patients with high risk of pneumonia among first-visit outpatients was conducted. High-risk patients for pneumonia are defined by the U.S. Advisory Committee on Immunization Practices. One hundred and twenty-seven patients, excluding 16 with pneumococcal 13-valent conjugate vaccination (PCV13) history prior to the first visit, were divided into three groups according to the method of vaccination recommendation: banner (B) group; banner+brochure (B+Br) group; banner+brochure+doctor's recommendation (B+Br+DR) group. The vaccination rates among patients in each group were compared.Results: Patients with high risk of pneumonia showed 11.2% rate for PCV13 vaccination upon their first visit;however, this rate increased to 39.2% following counseling (P<0.001). Variation among the three groups was as follows: 9.0% in the B group, 20.0% in the B+Br group, and 75.0% in the B+Br+DR group (P<0.001). After adjusting for age, sex, high-risk group, occupation, and residence, a regression analysis was carried out. The odds ratio for the PCV13 vaccination rate compared to the B group was 2.49 (95% confidence interval [CI], 0.55-11.34) for the B+Br group and 43.72 (95% CI, 11.52-165.96) for the B+Br+DR group.Conclusions: Among the available methods, we are thought to be more effective to add doctor's verbal recommendation in a clinical setting.

Abstract Study question Comparing the sexual function, marital adjustment, and quality of life of long-term infertile and parenting couples after a history of ART Summary answer Long time infertility alters the sexual function in women but does not impact negatively the quality of life and the marital adjustment of the couples What is known already The short-term impact of the diagnosis of infertility and its treatments on heterosexual couples has been investigated. But very little literature exists regarding the evolution of the quality of life, marital agreement, and the sexual function of long-term infertile couples. After ART, parenthood is considered to impact the quality of life and thus the conjugal life of the couples in a short time. But there are very few studies comparing the quality of life of couples who underwent ART in regard to the success or not of their parenthood project in the long term. Study design, size, duration This is a prospective, cohort-type analytical study where each patient is their own control over time. Patients were recruited at the Fertility Treatment Unit of our department, over a 10-year period (2009-2018). An interview was held with each partner at the 1st consultation and 10 years later. The recruitment phase took place from January 1st, 2009, and May 31, 2009. Participants/materials, setting, methods We included all heterosexual married couples with women under 40 and men under 45. The evaluation of the quality of life was standardized by the Medical Outcomes Study Short Form 36 (SF-36) and the Fertility Quality Of Life (FertiQol). The female sexual function was evaluated with the Female Sexual Function Index (FSFI). The male sexual function was evaluated through the International Index of Erectile Function (IIEF). The marital adjustment Test (MAT) evaluated the conjugal life. Main results and the role of chance We included 100 couples (100 women and 100 men) of whom only 44 were still together after 10 years:18 were still childless, 8 had adopted, and 18 had live births. The divorce rate in our study population was 56%. There were no significant differences in the socio-demographic features of the parenting and childless couples. Men's sexual function was not different between fathers and childless men. Childless women's sexual function was impaired compared with mothers either regarding the global function (p = 0.003) or all its components (Desire (p = 0,025), excitation (p = 0,001), lubrification (p = 0,007), orgasm (p = 0,017), pain (p = 0,008), and satisfaction (p = 0,006)). There is no difference in terms of marital adjustment (p = 0.479) and in terms of quality of life when applying the SF36. In the 18 long-term infertile women, the core FertiQol emotional scores and the FertiQol treatment tolerance scores were lower than in men (respectively, p = 0.02 and 0.003). Limitations, reasons for caution The small size of the sample of our study is the main limitation of the study. It is mainly related to the high rate of divorce in this specific population and the long period of the study. Wider implications of the findings Health care professionals need to emphasize the information of the couples of the potential effects of long-term infertility on the couple's marital life, the sexual life, and the quality of life, especially among women. They also need to help the patients seek the help and counseling they need. Trial registration number not applicable
