Abstract:
Working with goals in counselling and psychotherapy provides a detailed guide to using goals in clinical practice, and the empirical and theoretical foundations for this work. The book is aimed at psychologists, psychotherapists, and counsellors of all orientations—both in training and in practice—who work with adults and/or with children and young people. The introduction to the book defines goals, looks at their development, and discusses the rationale for, and challenges of, goal-oriented practice. Chapter 2 explores philosophical perspectives on goals, critically examining the relevance of these ideas to therapeutic practice. Chapter 3 extends this by looking at the psychological evidence on goals and goal-setting, examining its relationship to emotions and wellbeing, and the dimensions along which goals can vary. The following chapter, written by service users, presents their perspective on working with goals: why they may find it helpful, what they want from it, and what they see as the challenges. Chapter 5 reviews the evidence on goal consensus and therapeutic outcomes; and this is followed by a review of the different measures that can be used for goal monitoring and feedback (Chapter 6). Chapters 7 and 8 focus specifically on clinical practice: identifying effective strategies for goal-setting; and for working with goals across the therapeutic encounter. Goal-oriented practices are then considered in relation to the principle therapeutic orientations (Chapter 9). The book concludes with the analogy of therapy as a ‘journey of discovery’ (Chapter 10), with the client’s individual goals setting the direction for travel.

Abstract:
Cardiac genetic counselling is an emerging but important subspecialty. The qualifications of cardiac genetic counsellors depend on the country of practice, but at a minimum they are Master’s-level trained health professionals with expertise in genetics, and are integral members of the multidisciplinary inherited cardiovascular disease clinic. Though the framework is diverse in different countries, key roles include investigation and confirmation of family history details, discussion of inheritance risks and facilitation of cardiac genetic testing, communication with at-risk relatives, and increasingly, curation of genetic test results. The use of next-generation sequencing technologies has seen a recent shift in the uptake of genetic testing, due to greater availability and lowered costs. As these gene tests become more comprehensive, including large panels of genes and even whole exome or whole genome sequencing, the need for cardiac genetic counsellors to provide informed consent, appropriate pre- and post-test genetic counselling, and ongoing curation of the variants identified is evident. Finally, given the improved understanding of the psychological implications of living with a cardiovascular genetic disease, cardiac genetic counsellors are integral in delivering psychosocial care and identifying patients requiring intervention with a clinical psychologist.

Lifestyle is usually based on longstanding patterns and is highly determined by social environment and socioeconomic status. Additional factors such as chronic stress, cognitive impairment, and negative emotions (e.g. depression, anxiety) further impede the ability to adopt a healthy lifestyle, as does complex or confusing advice by medical caregivers. In clinical practice, increased awareness of these factors will facilitate empathetic counselling and the provision of simple and explicit advice. Established cognitive-behavioural strategies are important tools to help with behaviour change and medication adherence. Specialized healthcare professionals (e.g. nurses, dieticians, psychologists) should be involved whenever necessary and feasible. Reducing dosage demands to the lowest applicable level is the single most effective means for enhancing adherence to medication.
