Abstract:
Propósito: realizar una revisión de la evidencia acerca del tratamiento del cáncer de pulmón de célula no pequeña (CPCNP). Fuente de los datos: la información se obtuvo a partir de búsquedas practicadas en MEDLINE, CCT R, BIOSIS , EMBASE , LIL ACS y CIN HAL. También se recopilaron las referencias más representativas presentadas durante los últimos cinco años en los congresos ASCO, ESMO y de la IASLC. Extracción de los datos: los datos fueron extraídos por miembros asociados al ONCOLGroup. La recopilación de la información no siguió una estrategia uniforme. Resultados de la síntesis de datos: la terapia que se utiliza para tratar el carcinoma de pulmón de células no pequeñas (CPCNP) mejora la supervivencia global y la calidad de vida; no obstante, la mayoría de los pacientes mueren por la enfermedad antes del segundo año del diagnóstico, evento que ha favorecido la generación de nuevas estrategias que permitirán optimizar este desenlace. En la actualidad, el tratamiento estándar de primera línea implica varias combinaciones con base en algún platino que incrementan la supervivencia en comparación con la monoterapia y el mejor soporte paliativo. Estos regímenes son comparables respecto de su eficacia, pero difieren en el perfil de seguridad. Nuevas alternativas de tratamiento dirigidas contra blancos moleculares benefician a poblaciones específicas, cuando se administran solas o con otros agentes con los que presentan sinergismo. Esta revisión no realizó una evaluación sistemática de la evidencia. Conclusión: la terapia médica utilizada en el CPCNP modifica positivamente los desenlaces principales, incluyendo la calidad de vida

OBJETIVO: A quimioterapia adjuvante é recomendada na maioria dos casos de câncer de pulmão de células não pequenas (CPCNP) ressecados em pacientes nos estádios II ou IIIA. No entanto, diferentes esquemas quimioterápicos contendo cisplatina foram utilizados em estudos de fase III, e a melhor escolha permanece obscura. O objetivo deste estudo foi descrever a experiência do Instituto Nacional de Câncer (INCA), localizado na cidade do Rio de Janeiro (RJ), com o uso da combinação de cisplatina e etoposídeo nessa situação, com especial foco para os dados de sobrevida. MÉTODOS: Foram avaliados retrospectivamente os prontuários dos pacientes com diagnóstico de CPCNP que receberam terapia adjuvante no INCA entre 2004 e 2008. RESULTADOS: Foram incluídos 51 pacientes, e todos foram tratados com a combinação de cisplatina e etoposídeo. A mediana de tempo de seguimento foi de 31 meses de seguimento, e a mediana de sobrevida global foi de 57 meses. Na análise univariada, a sobrevida foi inferior nos pacientes submetidos a radioterapia + quimioterapia do que aqueles somente submetidos a quimioterapia (mediana de 19 vs. 57 meses; p < 0,001), e houve uma tendência a menor sobrevida nos pacientes em estádio III em relação àqueles em estádios I-II (mediana de 34 vs. 57 meses, respectivamente; p = 0,22). Não houve associações significativas entre a sobrevida global e gênero (p = 0,70), padrão histológico (p = 0,33) ou dose de cisplatina (p = 0,13). CONCLUSÕES: Nossos resultados corroboram a utilização da quimioterapia adjuvante, e os resultados de sobrevida se aproximam daqueles descritos nos principais ensaios clínicos randomizados. Contudo, é importante o acompanhamento a longo prazo nessa população

Los rearreglos del gen con actividad quinasa del linfoma anaplásico (ALK) ocurren infrecuentemente en el cáncer de pulmón de célula no pequeña (CPCNP) y constituyen un paradigma para la terapia dirigida contra oncogenes conductores en esta enfermedad. El crizotinib, un inhibidor del ALK disponible por vía oral, ofrece ventajas significativas para los pacientes positivos con toxicidad leve y beneficio clínico, motivos por los que fue aprobado para uso clínico en un subgrupo del cáncer de pulmón definido molecularmente. En la actualidad, se están desarrollando múltiples inhibidores del ALK dirigidos a maximizar la respuesta en presencia de menos efectos adversos, al igual que a controlar los mecanismos de resistencia subyacentes. Esta información permitirá optimizar en el futuro el tratamiento de los pacientes con cáncer de pulmón ALK positivo.

Abstract. The expansion-contraction model of Dungey cell plasma convection has two different convection sources, i.e. reconnections at the magnetopause and in the magnetotail. The spatial-temporal structure of the nightside source is not yet well understood. In this study we shall identify temporal variations in the winter polar cap convection structure during substorm activity under steady interplanetary conditions. Substorm activity (electrojets and particle precipitations) is monitored by excellent ground-satellite DMSP F15 conjunctions in the dusk-premidnight sector. We take advantage of the wide latitudinal coverage of the IMAGE chain of ground magnetometers in Svalbard – Scandinavia – Russia for the purpose of monitoring magnetic deflections associated with polar cap convection and substorm electrojets. These are augmented by direct observations of polar cap convection derived from SuperDARN radars and cross-track ion drift observations during traversals of polar cap along the dusk-dawn meridian by spacecraft DMSP F13. The interval we study is characterized by moderate, stable forcing of the magnetosphere-ionosphere system (EKL = 4.0–4.5 mV m−1; cross polar cap potential (CPCP), Φ (Boyle) = 115 kV) during Earth passage of an interplanetary CME (ICME), choosing an 4-h interval where the magnetic field pointed continuously south-west (Bz < 0; By < 0). The combination of continuous monitoring of ground magnetic deflections and the F13 cross-track ion drift observations in the polar cap allows us to infer the temporal CPCP structure on time scales less than the ~10 min duration of F13 polar cap transits. We arrived at the following estimates of the dayside and nightside contributions to the CPCP (CPCP = CPCP/day + CPCP/night) under two intervals of substorm activity: CPCP/day ~110 kV; CPCP/night ~50 kV (45% CPCP increase during substorms). The temporal CPCP structure during one of the substorm cases resulted in a dawn-dusk convection asymmetry measured by DMSP F13 which is opposite to that expected from the prevailing negative By polarity of the ICME magnetic field, a clear indication of a nightside source.

Abstract A Certificate of Public Convenience and Necessity (CPCN) is a common requirement for a new business owner in services such as utilities, communications, healthcare, and transportation. However, studies have called into question the effectiveness and purpose of these laws when applied in certain industries. This study chronicles the development of and justifications provided for the enactment of CPCN laws. It then empirically tests the effectiveness of these laws in the taxi industry. This paper finds no evidence that the CPCN laws in the taxi industry are meeting their objectives. Some evidence suggests that CPCN laws achieve the opposite of their intention.

The implementation, utilization and influence of the CPNP programme were examined by programme impact pathway (PIP) analysis.DesignFive CPNP programme components were evaluated: (i) degree of implementation; (ii) participants’ perception of the nutrition sessions; (iii) participants’ message recall; (iv) utilization of feeding and hygiene practices at early programme stage; and (v) participants’ engagement in other programmes.SettingHabro and Melka Bello districts, Ethiopia.SubjectsRecords of 372 nutrition sessions, as part of a cluster-randomized trial, among mothers (n876 in intervention area,n914 in control area) from a household survey and CPNP participants (n197) from a recall survey.ResultsOverall, most activities related to nutrition sessions were successfully operated with high fidelity (>90 %), but a few elements of the protocol were only moderately achieved. The recall survey among participants showed a positive perception of the sessions (~90 %) and a moderate level of message recall (~65 %). The household survey found that the CPNP participants had higher minimum dietary diversity at the early stage (34·0v. 19·9 %,P=0·01) and a higher involvement in the Essential Nutrition Action (ENA) programme over a year of follow-up (28·2v. 18·3 %;P<0·0001) compared with non-participants within the intervention area.ConclusionsOur PIP analysis suggests that CPNP was feasibly implemented, promoted a sustained utilization of proper feeding behaviours, and enhanced participation in the existing ENA programme. These findings provide a possible explanation to understanding CPNP’s effectiveness.

Abstract Purpose The design, implementation, and assessment of a comprehensive pharmaceutical care program (CPCP) for hepatitis C virus (HCV)-infected patients treated with direct-acting antivirals (DAA) are described. Summary The advent of DAA regimens has caused the evolution of the role of hospital pharmacists, leading to the development of more specialized models of pharmaceutical care. Three clinical pharmacists were incorporated into the pharmacy department of a general tertiary teaching hospital in Madrid, Spain, with the aim of developing and implementing a CPCP for HCV-infected patients. Pharmacists were responsible for proposing standards and local guidelines to physicians, monitoring adherence to guidelines, managing drug interactions and adverse drug events (ADEs), providing patient education, and evaluating health outcomes and costs. Implementation steps included (1) estimation of the healthcare demand and pharmacy resources, (2) definition of the workflow of the CPCP, (3) definition of the treatment care plan, for which tools were developed to support pharmaceutical validation, detection, and management of ADEs and drug–drug interactions, and (4) program assessment in terms of safety and cost-effectiveness. The pharmacists’ interventions performed, severity of errors intercepted, and patients’ satisfaction with the CPCP were also assessed. This CPCP demonstrates that the involvement of the pharmacist throughout the care plan prevents harmful medication errors in this population (0.1 per patient) and prompts significant cost savings (€1.2 million for 1,930 treated patients). Conclusion The implementation of a CPCP developed by hospital pharmacists for patients treated with DAA for HCV infection is an effective approach for preventing harmful medication errors and improving cost- effectiveness.

Abstract. It is known that the ionospheric cross polar cap potential (CPCP) saturates when the interplanetary magnetic field (IMF) Bz becomes very large. Few studies have offered physical explanations as to why the polar cap potential saturates. We present 13 events in which the reconnection electric field (REF) goes above 12mV/m at some time. When these events are examined as typically done in previous studies, all of them show some signs of saturation (i.e., over-prediction of the CPCP based on a linear relationship between the IMF and the CPCP). We show that by taking into account the size of the magnetosphere and the fact that the post-shock magnetic field strength is strongly dependent upon the solar wind Mach number, we can better specify the ionospheric CPCP. The CPCP (Φ) can be expressed as Φ=(10-4v2+11.7B(1-e-Ma/3)sin3(θ/2)) {rms/9 (where v is the solar wind velocity, B is the combined Y and Z components of the interplanetary magnetic field, Ma is the solar wind Mach number, θ=acos(Bz/B), and rms is the stand-off distance to the magnetopause, assuming pressure-balance between the solar wind and the magnetosphere). This is a simple modification of the original Boyle et al. (1997) formulation.

Major depressive disorder (MDD) put caregivers of palliative cancer patients (CPCP) and the patients at risk for various physical and psychological complications. This study aimed to determine the prevalence of MDD and its contributing factors among CPCP. The diagnosis of MDD was determined using the Mini International Neuropsychiatric Inventory. Pro forma and validated questionnaires were used to assess sociodemography, caregiving and patient factors, coping strategy, and social support. About 6% of CPCP had MDD. Predictors for MDD were coping by behavioural disengagement and positive reframing. These coping strategies are crucial for CPCP to have optimum mental health status. The structure and the conform ational mobility of the η5-bonded and of the chelating ligands of the dithiolenato complexes are discussed on the basis of their temperature-dependent 1H NMR spectra. The mass spectra show metastable transitions for the fragmentation processes.

Background and Objectives: Fee for service (FFS), the dominant payment model for primary care in the United States, compensates physicians based on volume. There are many initiatives exploring alternative payment models that prioritize value over volume. The Family Medicine for America’s Health (FMAHealth) Payment Team has developed a comprehensive primary care payment (CPCP) model to support the move from activity- and volume-based payment to performance-based payment for value. Methods: In 2016-2017, the FMAHealth Payment Team performed a comprehensive study of the current state of primary care payment models in the United States. This study explored the features, motivations, successes, and failures of a wide variety of payment arrangements. Results: The results of this work have informed a definition of comprehensive primary care payment (CPCP) as well as a CPCP calculator. This quantitative methodology calculates a base rate and includes modifiers that recognize the importance of infrastructure and resources that have been found to be successful in innovative models. The modifiers also incorporate adjustments for chronic disease burden, social determinants of health, quality, and utilization. Conclusions: The calculator and CPCP methodology offer a potential roadmap for transitioning from volume to value and details how to calculate such an adjustable comprehensive payment. This has impact and interest for all levels of the health care system and is intended for use by practices of all types as well as health systems, employers, and payers.

The seasonal variability of gelatinous zooplankton (siphonophores, medusae, and thaliaceans) abundance was investigated at Cabo Pulmo National Park (CPNP) from weekly zooplankton samples collected throughout 2014. The Gulf of California had prolonged warming during 2009-2019, with 2014 as the anomalously warm year preceding El Niño 2015-2016 compared to the 2003-2020 SST time series. Gelatinous zooplankton accounted <1% of the entire zooplankton community abundance at CPNP during 2014, suggesting a low influence of predation pressure upon their zooplanktonic and micronekton preys. Siphonophores (57%), thaliaceans (42%), and medusae (1%) were present throughout the year. The abundance of gelatinous zooplankton had a significant negative association with sea surface temperature and a positive association with sea surface chlorophyll-a concentration and velocity and direction of the wind, increasing their abundance during October after the hurricane season. The gelatinous zooplankton species assemblage at the coastal CPNP was similar but less abundant than the gelatinous zooplankton species assemblage observed in the oceanic region of the southern Gulf of California during summer 2014. Tropical species Diphyes dispar, Abylopsis tetragona, Chelophyes contorta, and Thalia spp. numerically dominated the gelatinous zooplankton community associated with a regional heatwave period recorded during 2014. Caregivers of palliative cancer patients (CPCP) who are depressed put both the patients and them at risk for serious physical and psychological complications. This study investigated the prevalence of depression and its contributing factors among the CPCP in Malaysia using the diagnostic tool and validated questionnaires. About 6% of CPCPs was diagnosed to have a major depressive disorder (MDD). Coping by ‘behavioural disengagement’ increased the odds for MDD while using ‘positive reframing’ was protective for MDD. The CPCP should be trained with beneficial types of coping strategy to help them reduce the burden of caregiving and to ensure optimum mental health status. Variation of the cross polar cap potential (CPCP) with the interplanetary electric field (IEF), the merging electric field EKL, the Polar Cap North (PCN) magnetic index, and the solar wind-magnetosphere coupling function EC of Newell et al. (2007) is investigated by considering convection data collected by the Super Dual Auroral Radar Network (SuperDARN) in the Northern Hemisphere. Winter and summer observations are considered separately. All variations considered show close to linear trend at small values of the parameters and tendency for the saturation at large values. The threshold values starting from which the non-linearity was evident were estimated to be IEF*~EKL*~3 mV/m, PCN*~3–4, and EC*~1.5×104. The data indicate that saturation starts at larger values of the above parameters and reaches larger (up to 10 kV) saturation levels during summer. Conclusions are supported by a limited data set of simultaneous SuperDARN observations in the Northern (summer) and Southern (winter) Hemispheres. It is argued that the SuperDARN CPCP saturation levels and the thresholds for the non-linearity to be seen are affected by the method of the CPCP estimates.

El CPCN-PM se localiza entre los 600 y 950 m de altitud; presenta clima templado superhúmedo a húmedo, lluvias de 2.250 mm anuales y temperatura promedio anual de 14,5ºC; están presentes tres regiones fitoecológicas principales, floresta ombrófila mixta, floresta ombrófila densa y sabana. Los taxones hallados son: Rotadiscus amancaezensis (Hidalgo, 1869), Lilloiconcha tucumana (Hylton Scott, 1963), Radiodiscus cuprinus Fonseca & Thomé, 2000, Radiodiscus tenellus Hylton Scott, 1957, Radiodiscus promatensis sp. nov., Zilchogyra gordurasensis (Thiele, 1927) y Zilchogyra zulmae sp. nov. La conchilla de R. promatensis sp. nov. es muy pequeña, casi plana, con ombligo muy desarrollado, abertura reniforme, con estrías espirales en la teleoconcha y 58-62 costillas axiales en el último anfracto. La conchilla de Z. zulmae sp. nov. es troquiforme alta, con cinco anfractos con fuertes y espaciadas costillas axiales (25-38), ombligo amplio y perspectivo y abertura subcircular angulosa. La fauna de gasterópodos carópidos del CPCN-PM tiene tres especies conocidas sólo del área (R. cuprinus, R. promatensis sp. nov. y Z. zulmae sp. nov.), una, que se distribuye también en Paraguay (R. tenellus) y tres (L. tucumana, R. amancaezensis y Z. gordurasensis), comunes a otras áreas de América del Sur, principalmente la región de las Yungas.

Resumo Analisou-se a resolutividade da atenção básica em saúde bucal nos municípios do estado da Paraíba, Brasil. Trata-se de um estudo transversal de abordagem indutiva, com procedimento comparativo e técnica de documentação indireta. O indicador de resolutividade (IR) constituiu da razão entre o número de tratamentos concluídos e primeiras consultas odontológicas programáticas dos municípios paraibanos (n = 223) entre 2011 e 2014, utilizando dados coletados da plataforma Datasus/TABNET. Foram consideradas variáveis explicativas: cobertura de primeira consulta odontoló gica programática (CPCOP), cobertura de equipes de atenção básica (CEAB), cobertura de equi pes de saúde bucal (CESB), cobertura de equipes de saúde da família (CESF), percentual de exodontia (PE), Produto Interno Bruto (PIB), Índice de Desenvolvimento Humano Municipal (IDHM) e coeficiente de Gini (CG). Os dados foram analisados por estatística descritiva e regressão múltipla binomial negativa (α = 0,05). As medianas do IR em 2011, 2012, 2013 e 2014 foram, respectivamente, 20,4, 17,5, 15,2 e 15,7. Houve associação positiva do IR com CPCOP (RP = 1,14; IC = 1,02-1,28), CEAB (RP = 1,02; IC = 1,01-1,03), e negativa com ano (RP = 0,83; IC = 0,74-0,94). A resolutividade em saúde bucal na atenção básica é influenciada por fatores relacionados à cobertura.

Feeding and hygiene measures at each visit were scored and the scores summed up for the entire follow-up period.SettingHabro and Melka Bello districts, Ethiopia.SubjectsRandomly selected mothers with a child aged 6–12 months (n1790).ResultsA total of 1199 mothers, 629 in the control and 570 in the intervention areas, were assessed at all visits and included in the analysis. Mothers in the intervention area showed higher scores than those in the control area regarding meal frequency (difference: 1·04, 95 % CI 0·35, 1·73), composite feeding score_1 (difference: 1·25, 95 % CI 0·37, 2·13; a summing score of currently breast-feeding, meal frequency and dietary diversity) and composite feeding score_2 (difference: 1·40, 95 % CI 0·49, 2·32; a summing score of meal frequency and dietary diversity). However, there were no differences in the scores of breast-feeding, dietary diversity and hand washing between the two areas (allP>0·05).ConclusionsThe CPNP programme was effective in improving some child feeding behaviours in rural Eastern Ethiopia.

We report on fabrication and characterization of two different opal-like structures: (i) crystal exhibiting mechanochromism, i.e. change of colour when subjected to mechanical stress, composed of closely packed colloidal polystyrene particles (CPCP) embedded in a poly-dimethylsiloxane (PDMS) elastomeric matrix; (ii) metallo-dielectric systems (MDCS), based on the realization of inverse silica opal and following attachment of gold nanoparticles on the silica network of the inverse colloidal structure. Optical measurements, performed on the two structures, have demonstrated that: (i) when an horizontal strain is applied on the CPCP embedded in PDMS, a blue shift of the diffraction peak occurs as a function of the applied strain and (ii) MDCS have unique optical properties that combine the localized surface plasmon resonance (LSPR) of gold nanoparticles (Au NPs) with the photonic band gap features of colloidal crystal structures. Finally, preliminary results on MDCS used as SERS substrate evidence a higher increase of the Raman signal in respect to that observed for others metallic structures.

To elucidate the relationship between chronic pain conditions with cast immobilization and autonomic function, we investigated the functional changes of the autonomic nervous system in conscious rats with chronic post-cast pain (CPCP) induced by a two-week cast immobilization of one hind limb. We telemetrically examined the time courses of systolic arterial blood pressure (SBP), heart rate (HR), and the middle-frequency (MF) component obtained from the power spectral analysis of SBP variability as a vasomotor sympathetic index. We also investigated the baroreflex sensitivity to phentolamine, an α-adrenoceptor antagonist, and the SBP and HR responses to a low ambient temperature (LT; 9.0 ± 0.2°C) exposure, a sympathetic stimulant. Rats exposed to cast immobilization exhibited mechanical allodynia lasting for at least 10 weeks after cast removal in the calf area (skin and muscle) of the bilateral hind limbs. Under resting conditions, the SBP, HR, and MF components were significantly increased during cast immobilization (all p < 0.001). Following cast removal, these parameters gradually decreased and within 1 week reached lower than baseline levels, lasting for over 10 weeks. Phentolamine administration (10 mg/kg, intraperitoneally) significantly decreased the SBP before and during cast immobilization (before, p < 0.001; during, p = 0.001) but did not lower the SBP after cast removal. The baroreflex gain after phentolamine administration, calculated as the HR increase divided by the SBP reduction, was significantly increased after cast removal (p = 0.002). The SBP increase on LT exposure was significantly greater after cast removal than that before cast immobilization, suggesting hypersensitivity to sympathetic neurotransmitters. These results revealed that, in the CPCP model, sympathetic activation was augmented during cast immobilization, which then decreased after cast removal and remained below normal levels with persisting pain behaviors. Additionally, the responsiveness of the autonomic nervous system was impaired in the CPCP model.

Building confidence is essential for qualified nurses undertaking numeracy assessments. This qualitative research study explores self-reported levels of confidence in nurses at various stages of the life-course, including when undertaking a numeracy test as part of the Community Practitioner Nurse Prescribing (CPNP) qualification. An hour-long, semi-structured focus group was conducted by the authors, and the resulting data was thematically analysed. The authors explored the experience of CPNP students returning to a learning and exam environment, and how this experience impacted on their confidence. Overall, a high level of confidence in the use of numeracy in clinical practice, but strong test anxiety, was identified in all participants. Participants reported an increase in confidence levels following the successful achievement of the numeracy test, with some going on to display advanced numeracy skills in clinical practice. Teaching staff have a responsibility to support returning learners to build confidence throughout this process and the authors conclude with some suggestions of how to support teaching and learning in this setting.

Abstract Cephalopod paralarval species richness, abundance and size structure were surveyed wduring an anomalous warm period (2014–2017) in the Cabo Pulmo National Park (CPNP), Gulf of California, Mexico. Paralarval taxa from six families (Cranchiidae, Enoploteuthidae, Ommastrephidae, Onychoteuthidae, Argonautidae and Octopodidae) were identified. Most taxa were of tropical biogeographic affinity and oceanic habitats. Highest paralarval richness occurred during spring associated with the northward movement of Tropical Surface Water, while the lowest was recorded during autumn with the southward flow of the Gulf of California water mass. Although 89% of the paralarvae were collected at or close to their species hatching size, none of the paralarval taxa showed a consistent seasonal spawning period. A canonical correspondence analysis showed three taxonomic assemblages: Argonauta and Helicocranchia–Onychoteuthis groups correlated with northward currents and high zooplankton biovolumes (ZB) and SD complex–Abraliopsis group with southward currents and intermediate ZB. Our Helicocranchia pfefferi paralarvae are the first recorded for the Gulf of California. At least 11 cephalopod taxa reproduce in CPNP. This area represents a suitable spawning habitat for cephalopods of socio-economic value.
