Abstract:
Purpose Research surrounding psychological capital (PsyCap) continues to expand as scholars and organizational leaders look to the strengths of positive organizational behavior (POB) and its effects in the workplace. Although previous research underscores connections between PsyCap and various organizational outcomes, there remains little research on its actual development. The purpose of this paper is to determine the efficacy of a six-week work-site intervention in improving PsyCap. The study included 49 individuals (n=32 assigned to the treatment group and n=17 to the comparison group). Using a pretest/posttest experimental design, the study showed that the intervention was efficacious in helping employees to develop greater PsyCap. Design/methodology/approach This study was part of a larger mixed methods design aimed at determining the efficacy of a work-site intervention in improving the PsyCap of employees. This design was chosen because it maximizes the strengths of both qualitative and quantitative methods while minimizing each of their weaknesses (Creswell and Plano Clark, 2007; Tashakkori and Teddlie, 1998). The specific mixed methods design implemented was the explanatory design using the participant selection model. This is a two-phase, sequential approach which began with a quantitative analysis and then drew upon those data to determine individuals who will be asked to participate in a follow-up study. Findings The purpose of this study was to determine the impact of a work-site intervention on the participants’ PsyCap in determining its developmental capacity. Previous studies have demonstrated strong correlations between PsyCap and many of the positive outcomes managers and supervisors desire for their employees –high productivity, low absenteeism, positive organizational citizenship, etc. (Luthans et al., 2013; Youseff and Luthans, 2007). With the results of the present study showing significantly greater improvement for the treatment group than their counterparts in the comparison group, it demonstrates that PsyCap can be developed and, consequently, these positive outcomes can accessed by a focused work-site intervention. Originality/value This paper builds on work already being done but focuses on the following significant gap in the literature: Can PsyCap be developed using work-site interventions? This study provides a compelling answer to that question and thus pushes the study of POB forward in its development.

Abstract:
To analyze the outcome of urgent carotid endarterectomy (CEA) performed within less than six hours in patients with crescendo transient ischemic attack (TIA) and stroke in progression. From January 1998 to December 2008, 58 urgent CEAs were done for acute neurological ischemic events – 46 patients with crescendo TIA and 12 patients with stroke in progression. Brain computed tomography (CT) was done prior and after the surgery. Disability level was assessed prior to and after urgent CEA using modified Rankin scale. Median follow-up was 42.1 ± 16.6 months. In the early postoperative period stroke rate was 0% for the patients in crescendo TIA group while in patients with stroke in progression group 3 patients (25%) had positive postoperative brain CT, yet neurological status significantly improved. Mid-term stroke rate was 2.2% in crescendo TIA group and 8.3% in stroke in progression group. In the early postoperative period there were no lethal outcomes, mid-term mortality was 8.3% in stroke in progression while in crescendo TIA group lethal outcomes were not observed. In conclusion, based on our results urgent CEA is a safe and effective treatment option for patients with crescendo TIA and stroke in progression with acceptable rate of postoperative complications.

A study of 32 piano sonatas by Beethoven reveals a significant asymmetry between increasing and decreasing dynamics. Specifically: (1) crescendos are more frequent than diminuendos, (2) crescendos tend to last longer than diminuendos, (3) large changes of dynamics tend to involve reductions in loudness, and (4) crescendos will more commonly follow low dynamic levels than will diminuendos follow high dynamic levels. These results support a "ramp archetype" of musical dynamics in which the music tends to build in a gradual way, but tends to subside relatively quickly. The results parallel a previous study showing an identical textural asymmetry in the evolution of polyphonic sonorities.

Abstract:
A 51-year-old man presented with vertigo, slurred speech and left facial droop. He had been previously diagnosed with transient ischaemic attack (TIA) and had a prior lacunar infarct. Imaging showed heavy symmetrical calcification in the globus pallidus, frontal white matter and cerebellar dentate nuclei/deep white matter. The imaging was pathognomonic for Fahr’s disease and diagnosis was confirmed when other secondary causes of hypercalcemia were excluded. Fahr’s disease is a rare, autosomal dominant, neurological condition characterised by primary brain calcification. Patients present with progressive neurological and psychiatric symptoms; commonly, Parkinsonian movement disorders, seizures, headaches, dysarthria, cognitive decline, psychosis and personality changes. There is an association with intracerebral ischaemic events. This case supports a growing body of anecdotal evidence of this association and is the first in which crescendo TIA may be the initial presentation of Fahr’s disease. Referral for genetic counselling and symptomatic relief for neurological symptoms are the main management strategies.
