Abstract:
Home treatment as an alternative to acute adult in-patient care is part of the National Health Service's Plan for mental health services in the UK. As a form of crisis intervention, it benefits from an understanding of, and ways of working with, the social systems relevant to the patient in crisis. This article describes the social systems intervention process as an alternative to admission and also considers its application in achieving early in-patient discharge.

Abstract:
IntroductionCompulsory admissions are associated with feelings of fear, humiliation and powerlessness. The number of compulsory admissions in Germany and other high-income countries has increased in recent years. Peer support has been shown to increase the self-efficacy of individuals with mental health conditions in acute crises and to reduce the use of coercive measures in clinical settings. The objective of this study is to reduce the number of compulsory admissions by involving peer support workers (PSWs) in acute mental health crises in outreach and outpatient settings.Methods and analysisThis one-year intervention is an exploratory, cluster randomised study. Trained PSWs will join the public crisis intervention services (CIS) in two of five regions (the intervention regions) in the city of Bremen (Germany). PSWs will participate in crisis interventions and aspects of the mental health services. They will be involved in developing and conducting an antistigma training for police officers. The remaining three regions will serve as control regions. All individuals aged 18 and older who experience an acute mental health crisis during the operating hours of the regional CIS in the city of Bremen (around 2000 in previous years) will be included in the study. Semistructured interviews will be conducted with PSWs, 30 patients from control and intervention regions, as well as two focus group discussions with CIS staff. A descriptive comparison between all participants in the intervention and control regions will assess the proportion of compulsory admissions in crisis interventions during the baseline and intervention years, including an analysis of temporal changes.Ethics and disseminationThis study was approved by the Ethics Committee of the University of Bremen (file 2022-09) on 20 June 2022. The results will be presented via scientific conferences, scientific journals and communicated to policy-makers and practitioners.Trial registration numberDRKS00029377.

Abstract:
This paper describes a state- and county-sponsored program that provides preventive and crisis-intervention services in three schools serving severely emotionally disturbed students. Students in these schools typically have histories of previous mental health diagnoses and treatment, often including hospitalization and/or residential or correctional placements, with little or no follow-up mental health services in the community. These students tend to have untreated disorders such as Attention Deficit Hyperactivity Disorder, mood disorders, and chemical dependency but have “fallen through the cracks” of the mental health system and remain at high risk for deterioration and out-of-home placements. This project utilizes a county social worker in the schools to coordinate services, intervene in crisis situations, and facilitate appropriate treatment for the students by acting as a liaison with students, special educators, school social workers, parents, child protection and child welfare services, the correctional system, and mental health providers. The program is described in detail, with examples of types of interventions and recommendations for future programs.

Abstract:
Counseling skills were evaluated in a telephone counseling service for Vietnam veterans. Thirty simulated crisis calls were made to telephone counselors by experienced clinical psychologists. The counselors' responses were rated on dimensions that indexed general counseling skills, knowledge of veteran needs, and provision of appropriate advice. Whereas the majority of callers demonstrated adequate counseling skills, many lacked knowledge of veterans' posttraumatic stress, common veteran terminology, and the nature of veterans' experiences. The findings suggest that telephone counseling services that target specific populations should employ comprehensive training to ensure that counselors possess adequate and relevant knowledge about callers and their presenting problems. The simulated caller paradigm appears to be an effective paradigm for training and evaluation of telephone counselors.

Abstract:
Purpose Data shows that there is an increasing number of young people in the UK needing access to mental health services, including crisis teams. This need has been exacerbated by the current global pandemic. There is mixed evidence for the effectiveness of crisis teams in improving adult functioning, and none, to the authors’ knowledge, that empirically examines the functioning of young people following intervention from child and adolescent mental health services (CAMHS) crisis teams in the UK. Therefore, the purpose of this paper is to use CAMHS Crisis Team data, from an NHS trust that supports 1.4 million people in the North East of England, to examine a young person's functioning following a crisis. Design/methodology/approach This service evaluation compared functioning, as measured by the Outcome Rating Scale (ORS), pre- and post-treatment for young people accessing the CAMHS Crisis Team between December 2018 and December 2019. Findings There were 109 participants included in the analysis. ORS scores were significantly higher at the end of treatment (t(108) = −4.2046, p < 0.001) with a small effect size (d = −0.36). Sixteen (15%) patients exhibited significant and reliable change (i.e. functioning improved). A further four (4%) patients exhibited no change (i.e. functioning did not deteriorate despite being in crisis). No patients significantly deteriorated in functioning after accessing the crisis service. Practical implications Despite a possibly overly conservative analysis, 15% of patients not only significantly improved functioning but were able to return to a “healthy” level of functioning after a mental health crisis following intervention from a CAMHS Crisis Team. Intervention(s) from a CAMHS Crisis Team are also stabilising as some young people’s functioning did not deteriorate following a mental health crisis. However, improvements also need to be made to increase the number of patients whose functioning did not significantly improve following intervention from a CAMHS Crisis Team. Originality/value This paper evaluates a young person’s functioning following a mental health crisis and intervention from a CAMHS Crisis Team in the North East of England.

The Crisis Intervention Team (CIT) program was first developed to reduce violence in encounters between the police and people with mental illness as well as provide improved access to mental health services. Although there is overwhelming popular support for this intervention, scant empirical evidence of its effectiveness is available—particularly whether the program can reduce the use of force. This investigation seeks to fill this gap in the literature by exploring the factors that influence use of force in encounters involving people with mental illness and evaluating whether CIT can reduce the likelihood of its use.

Recently, concern has been expressed by mental health professionals, MIND, and the police regarding the management of Section 136 referrals, which at present varies according to the local psychiatric services available. In the majority of London Metropolitan Boroughs a person deemed to be in need of care and control is taken first to a police station where the necessary documentation is completed and then transported, often a considerable distance, to a mental hospital for the purpose of assessment. This can result in lengthy delays in a police cell for the patient, time-consuming negotiations with hospitals for the police, and problems completing the assessment procedure due to lack of availability of social workers. The importance of considering alternatives, particularly with the move towards community care, has been stressed.

Abstract:
There are four main components in this thesis: a literature review of program evaluation, a description and discussion of the current status of program evaluation in the crisis intervention literature, results and discussion of the formative evaluation which is the primary element of the thesis, and a report on the use of the Goal Attainment Follow-up Guide (GAFG) (Kiresuk & Sherman, 1968) and the Brief Derogatis Psychiatric Rating Scale (B-DPRS) (Derogatis, 1978) for community-based mobile crisis intervention programs. The data for the evaluation were gathered using both quantative and qualitative methods. There were 150 participants in the study: 89 females and 61 males. The mean age was 35. The GAFG was completed by 81 of the participants; 33 of the participants were administred the B-DPRS. There were three major findings in this evaluation. the participants contacted significantly more community agencies and spent less time in hospital after using the crisis program and the GAFG and B-DPRS were found to be unsuitable as outcome instruments for a community-based mobile crisis program.

Abstract:
Aim/Purpose: This research paper reports on the evaluation of a mental health self-management program provided to immigrant nurses working at various rural South Australian aged care services. Background: The residential aged care staffing crisis is severe in rural areas. To improve immigrant nurses’ employment experiences, a mental health self-management program was developed and conducted in rural and regional health care services in South Australia. Methodology: A mixed approach of pre- and post-surveys and post workshop focus groups was utilized with the objectives of exploring the experiences of 25 immigrant nurses and the impact of the mental health program. Feminist standpoint theory was used to interpret the qualitative data. Contribution: A new learning environment was created for immigrant nurses to learn about the theory and practice of maintaining and promoting mental health. Findings: Statistical tests showed a marked difference in responses before and after the intervention, especially regarding knowledge of mental health. The results of this study indicated that a change in thinking was triggered, followed by a change in behaviour enabling participants to undertake self-management strategies. Recommendations for Practitioners: Include expanding the workshops to cover more health care practitioners. Recommendations for Researchers: Feminist researchers must actively listen and examine their own beliefs and those of others to create knowledge. Extending the program to metropolitan areas and examining differences in data. E technology such as zoom, skype or virtual classrooms could be used. Impact on Society: The new awareness and knowledge would be beneficial in the family and community because issues at work can impact on the ability to care for the family, and there are often problems around family separation. Future Research: Extending the research to include men and staff of metropolitan aged care facilities.

Abstract:
In the year prior to the pandemic, 1 in 3 adolescents in the US reported experiencing persistent feelings of isolation and anxiety, a 40% increase since 20091. Schools around the country recognize their role in assisting with this crisis and have increased their mental health services and staff. Although beneficial, these services require spatial compliments to increase their effectiveness and reach. Large scale renovations or entirely new constructions are often needed but require years of planning and substantial budgets to implement. How can we immediately begin to spatially address this crisis? Small scale interventions can be quickly designed and implemented with minimal disruptions to school systems, structures, and budgets. Acting together as a supportive network, these spaces can provide students, staff and faculty with accessible, restorative spaces that prioritize and support mental health. By providing schools and other shared spaces of our built environment with this type of micro restorative space, we hope to build resiliency through small scale, accessible and restorative interventions with the goal of maintaining and improving individual and community health and wellbeing. This paper focuses on the design, construction, and effectiveness of a small-scale restorative space within a main academic building on the grounds of a public university.

Abstract:
The aim of this paper is to propose an interventionist approach for transforming a School Health Center (CSE) in São Paulo, Brazil. The Center is in a situation of crisis, providing precarious health services due to limited human and material resources. In the article we argue that the solution for the crisis depends on how it is interpreted. We present an ethnographic narrative constructed collectively by researcher and practitioners through data from interviews and a mini-intervention. The aim of this narrative was to provide a better understanding about the development of the activity of health care and education/research conducted at the CSE. As it will be argued this hypothesis is the first step towards a broader on-going intervention using a method called Change Laboratory, which is based on an Activity Theoretical approach. The narrative shows that the School Health Center’s object (primary and secondary health care) and the object of the Faculty of Public Health (research and education) have changed towards different developmental directions. The integration between these two objects is narrower than when the Center was first created, and this can be a key source of conflicts between CSE by the Faculty. The results suggest that a more effective solution to the crisis would involve the reconstruction of the object motivation (product service) produced in the activity.

Research on social-emotional health and resilience of Latvian teachers was conducted as part of the ERASMUS+ project “Supporting teachers to face the challenge of distance teaching”. The aim of this study is to assess teachers’ social-emotional health and resilience to reveal those areas which require significant support and development in the Covid-19 pandemic situation. In the article, the concepts used in the project – social-emotional health, resilience, covitality will be theoretically analysed to substantiate the structure of the empirical study. Teachers’ mental health was tested using Social-emotional Health Survey– Teachers SEHS-T (Furlong et al., 2017; Furlong et al., 2014; Furlong & Gajdošová, 2018, as mentioned in Lapiņa, 2021) and Resilience Scale RS 25 (Wagnild & Young, 1993; Wagnild, 2009; Wagnild, 2016) with supplementary questions. 636 teachers of general and vocational schools took part in the research. The results identified that positive teachers’ strengths are self-regulation, empathy, and cognitive reappraisal. However, the teachers demonstrated limits in resilience as such, and in some scores of SEHS-T, as in covitality domain Belief in Others, especially in institutional and colleagues’ support, and in Engaged Living – gratitude and zest. The identified weaknesses and limits will be used as a foundation for preparing further intervention activities – a digital psychological support programme for strengthening the teachers’ resilience and mental health in general.

Addressing the significant mental and physical healthcare needs of Veterans requires innovative strategies to enhance access to evidence-based care. The integration of Virtual Human (VH) agents into Mobile Health (mHealth) applications presents a promising opportunity to overcome barriers associated with suicide prevention and connect with Veterans. The Battle Buddy (BB) project was conceived as a mobile wellness and suicide prevention application, empowering Veterans with an always-available resource concierged by an engaging and supportive conversational VH agent. Human-centered design is essential in the development of all interactions focused on the persuasive strategies of (1) personalization, (2) self-monitoring, (3) tunneling, (4) suggestion, and (5) expertise. Veterans can interact with the BB VH during daily check-ins, learn about mental health and wellness strategies, participate in interactive activities, increase self-awareness of their current status, and build and work safety plans in times of suicidal crisis. BB is designed to provide the Veteran with easy access to a suicide prevention ecosystem in which a wealth of evidenced-based interventions will be delivered in a non-stigmatizing fashion by a computer-based dialogue system with virtual embodiment, utilizing various multi-modal language cues such as text, speech, animated facial expressions, and gestures to interact with users. This paper explores our human-centered design process for the BB feature set to target the negative effects of social isolation and loneliness, conditions that challenge Veteran healthcare and suicide prevention.

The COVID-19 pandemic is most notably a physical health crisis, but it strongly affects mental health as well. Social isolation, job and financial losses, uncertainty about the real impact of the crisis, and fear for physical well-being affect the mental health of many people worldwide. These stressors can increase emotional distress and lead to depression and anxiety disorders. At the same time, there are enormous challenges on the health care side. People in need of mental health support have been increasingly confronted with limitations and interruptions of mental health services in many countries. In May 2020, the United Nations already warned that the COVID-19 pandemic has the seeds of a major mental health crisis if action is not taken. The panel discussed and analysed mental health in times of the COVID-19 pandemic with reference to South Africa, Nigeria, Germany and Spain.

This Evidence Check commissioned by NSW Ministry of Health aimed to evaluate delivery of physical health interventions for people living with a mental illness, delivered though community mental health programs. The review found that sufficient evidence exists to support a number of interventions, with further evaluation; and identified and describes key characteristics for effectiveness such as duration of the intervention and mode of delivery (e.g. face-to-face or telephone, group or individual). The supported interventions and/or actions included: multi-strategy lifestyle behaviour change interventions; care delivery models including peer-led self-management and staff delivered interventions; integration of new physical health care models or initiatives; referral to other services (e.g. telephone Quitline); assessing barriers and enablers prior to implementation; and the involvement of peer workers and consumers in design and delivery.

Background. Mental health mobile applications (apps) have the potential to expand the provision of mental health and wellness services to traditionally underserved populations. There is a lack of guidance on how to choose wisely from the thousands of mental health apps without clear evidence of safety, efficacy, and consumer protections. Purpose. This Technical Brief proposes a framework to assess mental health mobile applications with the aim to facilitate selection of apps. The results of applying the framework will yield summary statements on the strengths and limitations of the apps and are intended for use by providers and patients/caregivers. Methods. We reviewed systematic reviews of mental health apps and reviewed published and gray literature on mental health app frameworks, and we conducted four Key Informant group discussions to identify gaps in existing mental health frameworks and key framework criteria. These reviews and discussions informed the development of a draft framework to assess mental health apps. Iterative testing and refinement of the framework was done in seven successive rounds through double application of the framework to a total of 45 apps. Items in the framework with an interrater reliability under 90 percent were discussed among the evaluation team for revisions of the framework or guidance. Findings. Our review of the existing frameworks identified gaps in the assessment of risks that users may face from apps, such as privacy and security disclosures and regulatory safeguards to protect the users. Key Informant discussions identified priority criteria to include in the framework, including safety and efficacy of mental health apps. We developed the Framework to Assist Stakeholders in Technology Evaluation for Recovery (FASTER) to Mental Health and Wellness and it comprises three sections: Section 1. Risks and Mitigation Strategies, assesses the integrity and risk profile of the app; Section 2. Function, focuses on descriptive aspects related to accessibility, costs, organizational credibility, evidence and clinical foundation, privacy/security, usability, functions for remote monitoring of the user, access to crisis services, and artificial intelligence (AI); and Section 3. Mental Health App Features, focuses on specific mental health app features, such as journaling and mood tracking. Conclusion. FASTER may be used to help appraise and select mental health mobile apps. Future application, testing, and refinements may be required to determine the framework’s suitability and reliability across multiple mental health conditions, as well as to account for the rapidly expanding applications of AI, gamification, and other new technology approaches.

This Evidence Check was commissioned by the Ministry of Health to identify validated health assessment tools for physical health, mental health, development, and family violence for the Wellbeing and Health In-reach Nurse (WHIN) Coordinator program. This program is a partnership between NSW Health and the NSW Department of Education which places nurses in NSW schools to identify the health and social needs of students and coordinate early intervention and referral to services and programs. Validated assessment tools will help the nurses to identify children at risk of academic, behavioural, emotional or health-related difficulties. This Evidence Check also aims to describe how they are used in clinical practice and barriers and enablers to their effective use. Seventy-two assessment tools were found, but coverage of the areas affecting children’s wellbeing was uneven. Mental health had several promising tools, as did development for younger children. However, physical health and family violence did not have well-validated tools. There was little information on how they were used in clinical practice. Enablers for use included minimal training requirements, ease of administration and ready availability. Use of parental as well as teacher reports was seen as valuable. There is a need for further work on tools for physical health and family violence. There is also a need for information on the practicalities of the chosen tools (user acceptability, licensing, costs, and training requirements) and for clear practice guidelines.

One of the key lessons of the 2007-2008 global financial crisis is the importance of financial market infrastructures (FMIs) as a pillar of financial stability. Before, the role of financial market infrastructures, namely the provision of trading, clearing, settling, recording, and compressing services for transactions between financial institutions (FIs) was often taken for granted. This was reflected in FMIs having often been referred to as the financial system’s plumbing, including by the Federal Reserve’s 14th chairman (Bernanke, 2011)—a clear reference to the critical yet concealed importance of FMIs in the safe and efficient functioning of financial markets. Today, it is clear that the failure of an important FMI will almost certainly lead to systemic instability in financial markets. Given this, it is evident that FMIs are critical infrastructures; that is, based on a definition by the European Commission (2008), FMIs can be considered systems that are essential for the maintenance of vital societal functions, health, safety, security, economic or social well-being of people. In light of this importance, it’s perhaps surprising that the literature about financial networks has addressed the importance of FMIs rather recently and sparingly. The archetypical financial network, composed of FIs as elements (the nodes) that are interlinked through different types of relations (e.g., exposures, payments, ownership, common holdings), has been complemented by the introduction of FMIs as an additional layer that provides a medium for FIs to interact. As highlighted in Berndsen, et al. (2018), a network of FIs that does not include FMIs is a logical network—one that displays bilateral relations despite those requiring the intervention of an FMI to exist. And that’s why the plumbing reference is particularly illustrative: when looking at the floor plan of a house, the plumbing is a critical additional layer hidden beneath the first—immediately visible—layer; in a building, carelessly knocking down a wall could have a disastrous effect on the supply of water, electricity, gas, communications within the apartment and even to others above and below–not to mention the effect on the structural integrity of the building. However, there are further layers beneath those containing FIs and FMIs. In fact, a financial network composed of FIs and FMIs is still a logical network, as the connections between FIs and FMIs also require the intervention of other elements to exist. Those elements provide the physical connection that enables the interlinkages among FIs and FMIs, in the form of wired (e.g., cable) or wireless (e.g., radio waves) connections. That is, as stated by Berndsen, et al. (2018), the interdependence of financial markets with physical networks, such as power and communication networks, make those networks critical infrastructures and obvious candidates for examining the stability of financial systems from an operational perspective.

Children and adolescents under the age of 18 years comprise 34% of the population, and child and adolescent Mental, Neurological and Substance Use (MNS) disorders are common and an important cause of disability and distress. Providers at all levels should be competent to screen for and identify common childhood mental disorders, which usually present with developmental delay, emotional and/or behavioural symptoms and/or learning difficulties. Given the burden of disease, the fact that many mental disorders begin in childhood, and that early intervention is most effective in childhood and adolescence, it is essential that effective promotion, prevention, treatment, care and rehabilitation interventions should be provided for children and adolescents. In this webinar, hosted by the Academy of Science of South Africa, leading experts in child and adolescent mental health presented their priorities for child and adolescent mental health services and research in South Africa.
