Full textABSTRACT This dissertation has the following specific aim: to measure outcomes of social work emergency certificates produced by a Mobile Crisis Team to determine the effectiveness of these certificates at linking clients to services. Linkage to services is a programmatic goal and is achieved by ensuring clients receive adequate crisis and follow up services as a result of Emergency Certificates. In doing so, risks of homicide, suicide and grave disability are mitigated. Under its administrative umbrella, Mobile Crisis Team operates a police-social work collaboration known as Crisis Intervention Team, and both are administered by the State of Connecticut Department of Mental Health and Addiction Services in southeastern Connecticut. The data comes from 233 consecutive cases that involved Emergency Certificate's to transport clients to the local emergency department for psychiatric evaluation and treatment. Data was gathered post-hoc by reviewing copies of each specific certificate and cross-referencing those certificates with an agency risk management report and an electronic data base that stores demographic data on all clients. Descriptive, bivariate and multivariable analysis, such as cross tabulations and binary logistic regression, were used to analyze the data in this study. Additionally, the chi-square automatic interaction detector (CHAID) was used to construct outcome trees to describe subgroups of interest. This research is a continuation of previously published research on the various permutations and outcomes of mobile crisis programs, and contributes two unique programmatic features: the use by social workers of a legally proscribed coercive tool to send people to the emergency department for assessment, and the effectiveness of police-social work collaborations compared to social work only referrals. Overall, linkage occurs in over 80% of cases, with significant predictors of linkage identified as substance abuse, Hispanic ethnicity, criminal justice involvement, suicide, affective disorders, and insurance status. Implications for program evaluation, future research and limitations of the study are also discussed

Full textThe purpose of this study is to develop a theoretical framework for social workers in children’s homes that may be used for play techniques during social casework intervention with a children’s home child. The influence that the placement in a children’s home has on the child, creates the context for this study. Specific behaviour- and emotional problems that exist within the child were identified and play techniques for the solving of these problems was described. The research was done based on an extensive literature study, which focused on the role and function of a children’s home, the needs, behaviour- and emotional problems of the children’s home child and the usage of a practice framework and play techniques by social workers. A combined qualitative and quantitative research method and an explorative and describing research design have been used in this study, since this combination resulted in reaching the goal of the study. The empirical research investigated the usage of play techniques by social workers during social casework intervention with a children’s home child. The overall sample consisted of the 23 children’s homes in the Cape Metropole where currently 31 social workers are employed. Semistructured questionnaires were used as an interview instrument with an availability test sample consisting of 18 social workers. In light of the findings derived from the literature study and empirical research, appropriate conclusions and related recommendations were made. The main conclusion of the study is that the participants use play techniques randomly and not in conjunction with a practice framework. The main recommendation of this study is that social workers should use play techniques during social casework intervention, based on a specific practice framework in order to assure responsiveness. Continuous education in this regard is the responsibility of each social worker working in a children’s home.

Full textThe aim of the proposed study is to evaluate demographic factors and treatment characteristics contained in the historical records of those treated as in-patients at the Sonstraal Adolescent unit of Lentegeur Hospital, during the period 1986 to 1990. This demographic study details the following aspects of the adolescent in-patients: size, that is, numbers of those admitted to the unit; composition, including age, sex and area. Treatment characteristics such as reasons for admission, diagnosis of psychopathology, referral agent on admission and discharge and length of stay in the unit are considered. The data for the study have been extracted from the clinical records contained at Sonstraal, namely , the 'Clinical Summary on Discharge' form. This form is completed by the therapist of each adolescent attending the unit. The EpiInfo computer programmes have been utilised to create a database and to select the appropriate procedures and statistics which form the basis for data analysis and interpretation. Data interpretation includes an analysis of the emerging trends and details the implications for policy issues, unit staffing and treatment options. Analysis of the trends and comparisons with literature findings have facilitated the generation of hypotheses which could be tested in future studies. This study therefore provides a working document for future prioritising and planning of in-patient, out-patient and community mental health services to adolescents, their families and communities. This involves recommendations for intervention and community involvement. In addition, the study provides a basis for future research into adolescent mental health care.

Full textEste estudo buscou resgatar a vivencia dos familiares e sujeitos em sofrimento psÃquico durante situaÃÃes de crise. Historicizando esses fatos foi possÃvel retratar um contexto de vida que se ramifica de diversas formas para alcanÃar um atendimento digno e resolutivo. Trata-se de estudo qualitativo que utilizou a HistÃria Oral TemÃtica como mÃtodo de apreensÃo das narrativas. As narrativas estÃo apresentadas na Ãntegra, com textualizaÃÃo do autor, cujo objetivo foi manter a singularidade e especificidade contidas no discurso. Como narradores, tivemos setes sujeitos, seis mulheres e um homem, estes foram identificados de diversas formas, tendo como ponto zero o Sistema de Atendimento MÃdico de UrgÃncia (SAMU). Para desvelar as ideias contidas nas narrativas, utilizei o referencial metodolÃgico de Paul Ricoeur que considera a compreensÃo como ordenaÃÃo do enunciado narrativo e quando esta estrutura organizacional nÃo se estabelece, nÃo à possÃvel compreender a narrativa, requer uma explicaÃÃo. Objetivando alcanÃar esta explicaÃÃo, optamos por utilizar aspectos codificados, explicaÃÃo causal do sofrimento mental a partir da visÃo dos sujeitos do estudo do reconhecimento da crise a busca por apoio terapÃutico e seus descaminhos, a rede de atenÃÃo em saÃde mental como recurso de apoio buscado pelos sujeitos, o cuidado e suas dimensÃes: perspectiva da famÃlia e da rede, o porvir na visÃo do familiar e do sujeito em crise. As narrativas revelaram sofrimento psÃquico marcado por multicausalidades, potencializado pelo meio ao qual o sujeito encontra-se inserido. A ausÃncia de rede de assistÃncia emergencial Ãs situaÃÃes de crise foi registrada em todas narrativas coletadas, tendo como foco principal a carÃncia de estruturas de retaguarda aos serviÃos substitutivos, o que gera sofrimento psÃquico em familiares, sujeito e vizinhos. Marcaram, tambÃm, os relatos excessivos de violÃncia praticados pelo sujeito em situaÃÃes de crise. A terapÃutica com ervas ganhou notÃria importÃncia nos relatos dos familiares, seguido pelos atos espirituais, medidas buscadas para suprir a ausÃncia da assistÃncia adequada. A partir deste contexto, o cuidado ganha relevante importÃncia no compreender as situaÃÃes de crise instaladas, uma vez que à possÃvel encontrar diversas formas de tratamentos, inclusive mÃtodos arcaicos à segregaÃÃo prisional. Por fim, encontrei nas narrativas a esperanÃa de cura ou tratamento que permita aos familiares e sujeitos retornarem a uma vida saudÃvel e normal. Evidenciei no estudo relatos peculiares da realidade vivenciada no contexto das situaÃÃes de crises psÃquicas, como a nÃo formaÃÃo da trÃade de cuidado sujeito, famÃlia e rede de atenÃÃo, que a despeito de a famÃlia perceber que o hospital psiquiÃtrico nÃo à a estrutura assistencial desejada, lanÃa mÃo deste por ser o Ãnico recurso existente e que à preciso rever o suporte de retaguarda dos novos serviÃos instalados pela Reforma PsiquiÃtrica.
This study ought to redeem experiences of families and individuals in psychological distress during crisis situations. Historicizing these facts, it was possible to portray a life context that branches in various ways to achieve a dignified and decisive care. It is a qualitative study that used thematic oral history as a method of seizure of narratives. The narratives are presented in full, with textualization of the author, whose objective was to maintain the uniqueness and specificity contained in the speech. As storytellers, we had seven subjects, six women and one man, these were identified in several ways, with the Zero point the System of Emergency Medical Service (SAMU). To disclose the ideas contained in the narratives, I used Paul Ricoeurâs methodological referential, that consider understanding as a ordination of the narrative enunciation and when this organizational structure is not established , it is not possible to understand the narrative, requires an explanation. In order to achieve this explanation, I chose to use aspects encoded as therapeutic measures and their diversity; care and its dimensions in the family perspective in relation to the health care network, the future vision of the family and the individual in crisis. The narratives revealed psychological distress marked by multi-causalities, enhanced through which the subject is inserted. The absence of network emergency assistance to crisis situations was recorded in all narratives collected, focusing mainly on the lack of structures to rear services, which generates psychological distress in relatives, individuals and neighbors. Marked also the reports of excessive violence by the subject in crisis situations. Therapy with herbs gained remarkable importance in the accounts of family members, followed by spiritual acts, measures sought to address the lack of adequate assistance. From this context, care gain relevant importance in understanding the crisis installed, since it is possible to find different forms of treatments, including archaic methods to segregational prison. Finally, I found in the narratives hope of cure or treatment that enables individuals and families return to a healthy and normal life. Found in the study reports the peculiar reality experienced in the context of psychic crisis situations, such as non-formation of the triad of subject care, and family care network, which in spite of the family realize that the psychiatric hospital care structure is not desired, makes use of this because it is the only resource available and it is necessary to review the support behind the new services installed by the Psychiatric Reform.

Full textIncorporation of a mental health focus into Early Intervention/Early Childhood Special Education (EI/ECSE) services is considered critical by many professionals due to the vulnerability of young children with disabilities to developing behavior problems, mental health disorders, and to experiencing maltreatment. This qualitative research study investigated ways personnel in EI/ECSE agencies in Oregon are addressing the mental health needs of the children and families they serve. How mental health is conceptualized by EI/ECSE providers, how well prepared providers are in addressing the needs, what kinds of strategies are used to serve children and families, and how adequately EI/ECSE families and children are served by the mental health system were explored. The study was divided into two parts, which included a state-wide study of 14 EI/ECSE agencies in five regions in Oregon, and an in-depth study of one EI/ECSE agency that has employed a mental health consultant. State-wide agencies were mainly located in rural parts of the state, although two education service districts in metropolitan areas were included. The perspectives of 10 mental health providers from community mental health centers were also collected. The most salient issues that surfaced in the state-wide study included the focus of EI/ECSE concern on behavioral and mental health problems in children ages three through five and strategies for related behavior management in classroom settings, the importance of collaboration with community agencies, and a desire for increased partnership with mental health. Interviews with both EI/ECSE and mental health agencies revealed problems in the mental health system including training of therapists in working with very young children, gaps in services for children who were not Medicaid-eligible and in services for parents with their own psychological issues, and in the capacity to serve all of the children who were referred. The in-depth study focused on one EI/ECSE agency that was developing staff competence in relationship-based work with families through support from a mental health consultant employed by the agency. Salient issues included funding for a mental health consultant in an educational agency, development of mental health services for early childhood through community collaboration, and supporting staff through reflective supervision.
