Abstract:
The main purpose of the monograph is to identify the main patterns in assessing the operational reliability of metals on the basis of a mathematical model of unsteady deformation of an isotropic viscoplastic medium, with specified boundary, initial conditions and energy criterion of destruction. A physically based computational and experimental method for metal identification is formulated. Practical examples of the efficiency of the method for a wide class of materials are given: structural steels, titanium, aluminum and copper alloys. The assessment of the operational reliability of metals is based on a comparison of matrices containing seven parameters: tensile strength, yield strength, elongation, coefficient of dynamic viscosity, specific energy of fracture, rate of deformation, temperature. The book also pays a lot of attention to experimental data on the classical value of impact strength. It is shown that it actually reflects the properties of the material in the range of elastic-brittle state, but not in the scheme of a viscoplastic medium. It is intended for scientific and engineering workers, students involved in metal science and fracture mechanics.

Abstract:
Inflammatory back pain (IBP) refers to a collection of symptoms that may help identify patients with possible inflammatory spinal disease. A number of criteria sets have been proposed for IBP, which share common features such as: onset of symptoms aged <40 years, alternating buttock pain, improvement with exercise, worsening with rest, gradual onset, early morning stiffness, and improvement with NSAIDs. The IBP criteria were initially developed and validated in patients with ankylosing spondylitis, but have subsequently been shown to perform similarly in patients with axial spondyloarthritis (axSpA). The Berlin criteria demonstrated the highest specificity (84%) and the Calin criteria the highest sensitivity (92%). IBP criteria have been used in primary and secondary care referral strategies to facilitate the identification of patients with potential axSpA. It is important to note that IBP does not equate to a diagnosis of axSpA and many patients with IBP will not have axSpA.

Initially, the stuporous, excited (delirious mania), and febrile neurotoxic forms (neuroleptic malignant syndrome) of catatonia were recognized as distinctive. As catatonia became systematically defined by symptoms identified in lists of symptom rating scales, verified by benzodiazepine challenge tests, and validated by responses to treatments by benzodiazepines (lorazepam, diazepam) and electroconvulsive therapy (ECT), other syndromes met these criteria—anti-NMDAR encephalitis, self-injurious behavior (SIB), selective mutism, pervasive refusal syndrome, and akinetic mutism, among others. Such classification brought these syndromes within the catatonia variants, relieved by catatonia treatments. Such identification is feasible when clinicians consider the medical (Sydenham) model of diagnosis, the process of identification by symptoms, verification by objective tests, and validation by response to specific treatments.

Adjustment disorder is a psychiatric diagnosis that has undergone numerous iterations within the Diagnostic and Statistical Manual of Mental Disorders and is characterized by an immediate or almost immediate maladaptive or pathologic psychological response to a stressor. When the stressor is removed, there is the expectation that the patient returns to his or her previous level of functioning. This chapter provides an overview of its historical development, diagnostic criteria, potential controversies, epidemiology, neurobiology, comorbidities, and other differential diagnostic and treatment considerations such as ruling out depression and using assessment instruments. Acute on chronic liver failure is characterized by an acute deterioration of liver function in a patient with previously compensated cirrhosis secondary to a precipitating event. It is most commonly associated with a very poor prognosis and early identification of the precipitating cause is essential to successfully attempt to reverse decompensation. The most common precipitant is infection and a high index of suspicion is required. Other management is largely supportive with close attention to renal dysfunction being particularly important. All patients admitted to the intensive care unit with complications of cirrhosis warrant consultation with a transplant centre to determine whether they fulfil the criteria for transplantation and for expert advice.

It identifies key participants in Senate campaigns during this period—candidates for U.S. Senate, business interests, political party organizations, and state legislators—and constructs a model for how these stakeholders interacted with each other within the structure of legislative choice for U.S. senator along four dimensions—candidate identification, candidate nomination, election criteria, and system responsiveness. It also discusses how an anticipated indirect Senate election might have affected voter turnout in the preceding state legislative election.

Medulloblastoma is the most common malignant brain tumor in pediatric patients. Patients are presently stratified to either standard or high-risk groups based on clinical and pathologic criteria. Approximately 80% of patients with standard risk disease are cured of their primary disease. High-risk and recurrent disease groups have a poorer outcome; 5-year progression-free survival is only 65% with high-risk disease. Disease control after recurrence is very poor. Treatment is multimodal and also aims to limit short- and long-term toxicities. Recent identification of four molecular subtypes of medulloblastoma may change risk assignment and therapy. Addressing the medical and psychosocial issues of survivors continues to improve the quality of life for these patients beyond the disease’s treatment.

Metabolic syndrome is defined as the co-occurrence of metabolic risk factors for both type 2 diabetes and cardiovascular disease (i.e. abdominal obesity, hyperglycemia, dyslipidemia, and hypertension). Indeed, metabolic syndrome is an important risk factor for subsequent development of type 2 diabetes and/or CVD. Thus, the key clinical implication of a diagnosis of metabolic syndrome is the identification of a patient who needs aggressive lifestyle modification focused on weight reduction and increased physical activity. Multiple different phenotypes and ethnic-specific values for waist circumference are included within metabolic syndrome, with indications for differing treatment strategies. This book covers several aspects of metabolic syndrome, including its definition, diagnostic criteria, preventive measures, and treatment, as well as the possible association between ethnicity and the occurrence of metabolic syndrome.
