This project aimed to determine if the attendance of Critical Care Paramedics at out-of-hospital cardiac arrests in Queensland was still associated with a survival benefit despite the diminishing gap in the skill set between Critical Care and Advanced Care Paramedics. AbstractMetamaterial-based devices have been extensively explored for their intriguing functions, such as cloaking, concentrating, rotating, and sensing. However, they are usually achieved by employing metamaterials with extreme parameters, critically restricting engineering preparation. In this chapter, we propose an optimization model with particle swarm algorithms to simplify parametric designs to realize bilayer thermal sensors composed of bulk isotropic materials (circular structure). For this purpose, the fitness function is defined to evaluate the difference between the actual and expected temperatures. By choosing suitable materials for different regions and treating the sensor, inner shell, and outer shell radii as design variables, we finally minimize the fitness function via particle swarm optimization. The designed scheme is easy to implement in applications and shows excellent performances in detective accuracy and thermal invisibility, which are confirmed by finite-element simulations and laboratory experiments. The optimization model can also be flexibly extended to a square case. This method can calculate numerical solutions for difficult analytical theories (circular structure) and optimal solutions for problems without analytical theories (such as square structure), providing new inspiration for simplifying the design of metamaterials in various communities.

It is well recognized that errors of communication are associated with causing harm to patients on the Intensive Care Unit (ICU). By means of presenting a patient-based narrative, this chapter looks at communication in intensive care medicine focusing attention on styles of communication that are useful in different areas of common practice. It must be accepted that communication needs to convey a message that sits within its own context. For instance, the way that proxy decisions are made for patients who lack capacity varies with both culture and region. Resources are variable, and this includes the time that can be allocated to communication. It is clearly beyond the scope of this chapter to offer guidance on exactly what information should be given to patients and their relatives, but we aim to highlight useful ways of making the communication of that information more effective. 03:00 in a District General Hospital Emergency department. The on-call intensive care doctor is fast-bleeped to the resuscitation bay to see a 35-year-old man called Stephen who has been brought in by ambulance from a roadside accident. He is conscious but distressed, with significant injuries to both legs and thorax. A ‘trauma team’ of doctors is assembled, plus the delivering paramedics, accident and emergency-qualified nurses and healthcare assistants. Communicating with teams in time-critical situations presents a clear challenge. Team members can be unknown to each other with an unknown skill mix, the clinical problems are undefined, different personnel have different—sometimes conflicting—motives, and goals, and clinical priorities can shift over time. The anaesthetist must be confident to communicate with authority in these circumstances. Under stress, most team members will respond well to someone else taking the lead. They will usually do what they are asked if they understand the instruction, are competent to undertake the task, and are not overloaded with other tasks. Closed-loop communication is a good way to keep communication efficient: ‘Someone get me a tube!’ can be misunderstood or ignored by most of the team. ‘Sarah, I want you to get a size 8 endotracheal tube from the trolley and test the cuff for me. Do you know how to do that?’ is specific, directed to a named individual and asks for confirmation of understanding and competence to complete the task.

This chapter critically analyses free-form generative parametric design techniques to evaluate the effectiveness of VPL (Visual Programming Language) systems, applied to existing high-geometric/formal complexity artefacts. The paneling of the South facade of the Institut du Monde Arabe (IMA) in Paris, designed by J. Nouvel, was chosen as a case history. This is to examine how through a complex kinematic modelling, the sunshine of indoor environments can be effectively controlled. The chapter focuses on reproduction, through reverse engineering techniques of the façade-type panel, through the most widely-used VPL platforms that determine algorithmic relationships. The generative parametric algorithms developed for the IMA Moucharabieh, indicate that identical rules can govern different geometries; in contrast, identical geometries can arise from completely different algorithmic formulae. Finally, the integration with the most widely-used BIM applications, is used to critically evaluate interoperable workflows.

Patients in critical condition need Physician’s supervision while they are in transit to the hospital. If the ambulance is linked to the emergency room, the physician can monitor patient’s vital signs and issue instructions to the paramedics for stabilizing the patient. When a disaster strikes, scores of people are transferred to the hospital in ambulances. During an emergency situation, the number of patients in critical condition reaches overwhelming proportions. In this chapter, we discuss the state of the art in transferring emergency medical data from the disaster site or ambulance to the hospital and outline some case studies. We present a scheme called MEDTOC (Medical Data Transmission Over Cellular Network) for transferring in-ambulance multiple patients’ data to the hospital by UMTS. This system enables the transfer of vital signs to the hospital in reduced and packed format using limited bandwidth wireless network. Medical data can be transmitted over 3G cellular network using various modes and quality of service parameters available in UMTS. This could help the physicians in monitoring several patients who are either in transit or at a triage unit on a disaster site. Results of the application of data reduction algorithm over CAN packets and feasibility studies in transfer of data over UMTS are presented and discussed.

The complexity of helicopter flight dynamics makes modeling and helicopter system identification a very difficult task. Most of the traditional techniques require a model structure to be defined a priori and in case of helicopter dynamics, this is difficult due to its complexity and the interplay between various subsystems. To overcome this difficulty, non-parametric approaches are commonly adopted for helicopter system identification. Artificial Neural Network are a widely used class of algorithms for non-parametric system identification, among them, the Nonlinear Auto Regressive eXogeneous input network (NARX) model is very popular, but it also necessitates some in-depth knowledge regarding the system being modelled. There have been many approaches proposed to circumvent this and yet still retain the advantageous characteristics. In this paper, the authors carry out an extensive study of one such newly proposed approach - using a modified NARX model with a II-tiered, externally driven recurrent neural network architecture. This is coupled with an outer optimization routine for evolving the order of the system. This generic architecture is comprehensively explored to ascertain its usability and critically asses its potential. Different implementations of this architecture, based on nature inspired techniques, namely, Artificial Bee Colony (ABC), Artificial Immune System (AIS) and Particle Swarm Optimization (PSO) are evaluated and critically compared in this paper. Simulations have been carried out for identifying the longitudinally uncoupled dynamics. Results of identification indicate a quite close correlation between the actual and the predicted response of the helicopter for all the models.

The case of a supercritical fluid heated from below (Rayleigh-Bénard) in a rectangular cavity is first presented. The stability of the two boundary layers (hot and cold) is analyzed by numerically solving the Navier-Stokes equations with a van der Waals gas and stability diagrams are derived. The very large compressibility and the very low heat diffusivity of near critical pure fluids induce very large density gradients which lead to a Rayleigh–Taylor-like gravitational instability of the heat diffusion layer and results in terms of growth rates and wave numbers are presented. Depending on the relative direction of the interface or the boundary layer with respect to vibration, vibrational forces can destabilize a thermal boundary layer, resulting in parametric / Rayleigh vibrational instabilities. This has recently been achieved by using a numerical model which does not require any equation of state and directly calculates properties from NIST data base (NIST, 2000) for instance.

The stability of the two boundary layers (hot and cold) is analyzed by numerically solving the Navier-Stokes equations with a van der Waals gas and stability diagrams are derived. The very large compressibility and the very low heat diffusivity of near critical pure fluids induce very large density gradients which lead to a Rayleigh–Taylor-like gravitational instability of the heat diffusion layer and results in terms of growth rates and wave numbers are presented. Depending on the relative direction of the interface or the boundary layer with respect to vibration, vibrational forces can destabilize a thermal boundary layer, resulting in parametric/Rayleigh vibrational instabilities. This has recently been achieved by using a numerical model which does not require any equation of state and directly calculates properties from NIST data base, for instance.

Abstract This chapter attempts to answer the core question in development economics: whether economic growth can alone eliminate or reduce poverty or whether governments should also focus on poverty alleviation through policies that directly help the poor. The literature has attempted to answer this using cross-country regression models. This chapter has critically reviewed these models. The objective of this review has been to understand whether the literature provides the answer to the core question. Unfortunately, the literature on this issue has generated more heat than light. It has offered conflicting answers, making an informed policy difficult. This chapter attempts to resolve these contradictory issues, using household surveys, following the non-parametric approach to estimating poverty elasticity for individual countries. The chapter has derived the growth and inequality elasticity for several poverty measures calculated from the individual country’s household surveys. This chapter develops a discrete-time decomposition that quantifies the contributions of economic growth and change in inequality on growth in poverty. Thus, this decomposition helps the governments understand whether they pursue economic growth first and foremost or focus on poverty alleviation policies that directly benefit the poor. The chapter presents a case study for China to understand how much economic growth and inequality have played a role in the country’s unprecedented poverty reduction.
