The innate immune system is the first line of host defense, and its ability to propagate antimicrobial and inflammatory signals from the cellular microenvironment to the tissue at-large is critical for survival. In a remarkably complex microenvironment, cells are constantly processing external cues, initiating convoluted intracellular signaling cascades, and interacting with neighboring cells to generate a global, unified response. At the onset of infection or sterile injury, individual cells sense danger or damage signals and elicit innate immune responses that spread from the challenged cells to surrounding cells, thereby establishing an overall inflammatory state. However, little is known about how these dynamic spatiotemporal responses unfold. Through the use GFP reporters, in vitro transplant coculture systems, and in vivo models of infection and sterile injury, this thesis describes identification of a gap junction intercellular communication pathway for amplifying immune and inflammatory responses, and demonstrates its importance in host innate immunity. The first section describes development of stable GFP reporters to study the spatiotemporal activation patterns of two key transcription factors in inflammation and innate immunity: Nuclear factor-KappaB (NFKB) and Interferon regulatory factor 3 (IRF3). Stimulation of NFKB-GFP reporters resulted in a spatially homogeneous pattern of activation, found to be largely mediated by paracrine action of the pro-inflammatory cytokine TNFa. In contrast, the activation of IRF3 was spatially heterogeneous, resulting in the formation of multicellular colonies of activated cells in an otherwise latent background. This pattern of activation was demonstrated to be dependent on cell-cell contact mediated communication between neighboring cells, and not on paracrine signaling. The second section describes the discovery of a gap junction intercellular communication pathway responsible for the formation of IRF3 active colonies in response to immune activation. Cell sorting and gene expression profiling revealed that the activated reporter colonies, collectively, serve as the major source of critical antimicrobial and inflammatory cytokines. Using in vitro transplant coculture systems, colony formation was found to be dependent on gap junction communication. Blocking gap junctions with genetic specificity severely compromised the innate immune system's ability to mount antiviral and inflammatory responses. The third section illustrates an application of the gap junction-induced amplification of innate immunity phenomenon in an animal model of sterile injury. Drug-induced liver injury was shown to be dependent on gap junction communication for amplifying sterile inflammatory signals. Mice deficient in hepatic gap junction protein connexin 32 (Cx32) were protected against liver damage, inflammation, and death in response to hepatotoxic drugs. Co-administration of a selective pharmacologic Cx32 inhibitor with hepatotoxic drugs significantly limited hepatocyte damage and sterile inflammation, and completely abrogated mortality. These finds suggests that co-formulation of gap junction inhibitors with hepatotoxic drugs may prevent liver failure in humans, and potentially limit other forms of sterile injury. In summary, this thesis demonstrates the development of novel tools for investigating the spatiotemporal dynamics of cellular responses, describes how these tools were utilized to discover a basic gap junction communication pathway critical in innate immunity, and provides evidence for the clinical relevance of this pathway in sterile inflammatory injury.

Abstract:
Weight bias and weight stigmatization are independent risk factors for poor health, and are brought up within health promotion, as focus areas when it comes to interventions targeting body weight (WHO, 2017; Pearl, 2018). Discourses within the society, can either reinforce weight bias and weight stigmatization towards people in larger bodies, or disrupt them. A gap in the literature exists, when it comes to health professionals and their means of communicating health on social media platforms, such as Instagram. This gap was the inspiration for the present study. The study sought to critically examine the discourses communicated by doctors and registered dietitians on the social media platform Instagram. With the specific focus of examining their presentation of body weight and health, the manifestations of roles and the discourses presented. The study was based on the theory of social semiotics, using critical multimodal discourse analysis, that include elements from the critical discourse analysis framework, by Fairclough (2010) (Machin & Mayr, 2012). The study found that the chosen health professionals generally presented weight bias and presented body weight as a sum of individual choices, as well as body weight as a personal responsibility. The health professionals used both visual and verbal techniques, to establish authority and power, and were generally promoting health as a commodity, as well as using their own body to promote the thin ideal. The strongest discourses present were those of healthism, paternalism and aesthetics defining health, findings that are supported within the literature, when looking at other health promoting entities, such as personal trainers.  The study brought forth important implications within health communication on social media platforms, thus that healthism is an area that is important to educate health professionals within, as well as there being basis to further investigate this notion. The study also brought  forth important considerations for ethics and validity within this type of research.

Abstract:
In 2011 the International Association of Athletic Federations (IAAF) published the Regulations on Hyperandrogenism, a health policy banning female athletes from track and field competition if their natural levels of testosterone were found to be higher than those of most female athletes. In 2014, Dutee Chand, a sprinter from India, was banned from competition based on these regulations. She appealed her ban in the Court of Arbitration for Sport (CAS) and as a result the 2011 IAAF Hyperandrogenism Regulations were suspended for two years. The issues at stake in the suspension of these regulations are, at their core, rhetorical issues related to health and medical technical communication: how information about health and medicine is communicated to stakeholders, the ethics of such communication, and the implications of such communication. They are also issues related to the medical regulation of sex and gender: Chand’s case is the latest in a history of sex verification testing of elite female athletes that began well before 2011. In this study I use feminist critical discourse analysis methods within the computer assisted qualitative analysis software program NVivo to analyze the 2011 IAAF Hyperandrogenism Regulations and the transcript of the CAS Award that suspended them. I argue that the 2011 IAAF Regulations and the CAS Award are an example of what I describe as a closed, Foucauldian system, which is not open to outside voices, stakeholders, expertise, or evidence. I also argue for the use of a heuristic alongside a feminist technical communication perspective on health and medical rhetorics that technical communicators might use to insert themselves into closed Foucauldian systems such as this one in order to enact positive change.

Abstract:
Constructions of race, reproductive health, and gender have been inextricably linked in the United States since the beginning of the nation. Today, these linkages remain evident in the marked racial and gender inequities in reproductive health outcomes that persist in the U.S. To better understand how these meanings and material outcomes are negotiated and produced by actors on the ground, this study asked: “How do reproductive healthcare providers (RHPs) communicate about the intersections of race, reproductive health, and policy?” I conducted semi- structures interviews with 24 RHPs, resulting in over 35 hours of recorded interviews. Drawing on critical-cultural communication, Reproductive Justice, Narrative Medicine, and Postcolonial theories, I developed a novel approach to narrative inquiry—Critical Narrative Analysis—to explore my data. Here, I present an in-depth analysis of 8 narratives shared by my participants. I conclude that participants communicated about race, reproductive health, and policy by engaging with dominant cultural narratives around these topics. While some participants contested dominant narratives, most upheld the foundational logics of oppressive systems in the stories they shared. To advance reproductive justice, I argue that new approaches to teaching clinicians, which engage with both narratives and sociopolitical structures affecting these narratives, are needed. By sharing my participants’ stories and contextualizing them within dominant narratives and social institutions, I aim to identify future research and practice opportunities for creating new stories about reproductive health and physician identity, stories which could suggest more equitable and just ways of doing reproductive health care.

Abstract:
Nonvocal alert patients in the intensive care unit setting often struggle to communicate due to inaccessible or unavailable tools for augmentative and alternative communication. A novel communication tool, the Manually-Operated Communication System (MOCS), was developed for use in intensive care settings for patients unable to speak due to mechanical ventilation. It is a speech-generating device designed for patients whose limited manual dexterity precludes legible writing. In a single-arm device feasibility trial, 14 participants (11 with tracheostomies, 2 with endotracheal tubes, and 1 recently extubated) used MOCS. Participants, family members, and observing nurses were interviewed whenever possible. Interviews included a modified version of the System Usability Scale (SUS) as well as open-ended questions; a qualitative immersion/crystallization approach was used to evaluate these responses. Participants with a tracheostomy and their family members/care providers rated MOCS on the SUS questions as consistently “excellent” (average rating across all groups was 84 +/- 17; all subgroups also rated the device highly). Through a qualitative interview process, these stakeholders expressed support for the use of MOCS in the ICU. Based on these data, MOCS has the potential to improve communication for nonvocal patients with limited manual dexterity.

Abstract:
The Swedish health care system does not offer care on equal terms for all its end-users. Discrimination toward patients can take the form substandard communication toward women or foreign born patients. Discrimination is also embedded in the organizational context. Health care is under pressure to increase efficiency and quality of care at the same time. There is a risk that demands for equality will be pushed aside. This thesis aims to contribute to our understanding of how discrimination is expressed in interpersonal- and organizational communication within health care, and highlight educational implications for health care practices. This thesis is comprised of three empirical studies and one conceptual study. In the first study, critical discourse analysis (CDA) is used to categorize gender patterns in communication between health care workers and patients, and finds that both patients and health care workers reproduced the gender order. Open questions created a setting less prone to be limited by gender stereotypes. In the second study, CDA is used and complemented with Linell’s dialogic perspective in order to explore whether patients who were native speakers of Swedish were constructed differently than those who were not, in patient-physician consultations. Findings indicated that the non-native speakers actually were model, participative patients according to patient-centered care. Notwithstanding this they were met by argumentation, whereas the more amenable native patients were met by accommodating responses. In the third study, qualitative content analysis is used to analyze how health care workers talked about patients in their absence. The results revealed that communication about patients who were perceived as not acting according to socially accepted gender norms contained negative and disparaging statements. The final study focused on Clinical Microsystems, a New Public Management-based model for multi-professional collaboration and improvement of health care delivery. Drawing on theories of New Public Management, gender, and organizational control, this study argues that the construction of innovative and flexible health care workers risks reproducing the gender order. The thesis concludes that gender and ethnic stereotypes are reproduced in health care communication, and that an efficiency-inspired organizational and institutional discourse may be an impediment to equal care. This calls for focus on learning about communication for prospective and existing health care workers in a multicultural health care context.

Abstract:
This study analyzed headlines in three influential newspapers to assess how those publications exercised media social responsibility in reporting the outbreak of Severe Acute Respiratory Syndrome (SARS). The situation posed the common media paradox where inherent news values encourage dramatic reports and escalation of conflict that, in tum, are criticized as irresponsible. The particular circumstances surrounding the SARS outbreak presented an opportunity for media social responsibility through public education and conveying perspective about risk from the disease. This study developed an analytical framework to evaluate the extent of fearful language, style of communication about risk of the disease, use of source material, and prevalence of journalistic normative behaviors. The analysis indicates that personal fear responses of journalists may influence the use of frightening language in news headlines. The study found that in early stages of reporting about the disease, journalists relied on overtly fearful language with little analysis or situational context. As the story became more familiar, even though the factual circumstances did not change, headlines revealed more efforts to communicate productively about risk and less use of explicit fearful terminology. However, most of the headlines demonstrated reliance on traditional news-gathering behaviors emphasizing conflict, controversy and human interest, rather than analysis and interpretation. Political controversy stemming from concerns about disease management by the Chinese government often prevailed over headlines representing socially responsible information about health protection or risk perspective. This study affirms the importance of media in public education during health crises. It also suggests journalists would benefit from better understanding risk communication principles and the influence of personal fear responses on their reporting. Additionally, the study demonstrates that the concept of media social responsibility deserves to be reconsidered in contemporary terms, to better guide both journalists and those charged with developing communication strategies during such circumstances.

Abstract:
This study explores how the complicated and nuanced identity theory, intersectionality, can be implemented as a communication strategy for sexual health professionals. From interviews with sexual health professionals in Oregon, this research indicates that through the adaptation of a reproductive justice lens professionals can practice intersectionality in their day-to-day work. Strategic communication has longed focused on the “cash value” of theory and suggested that theory is best when it can be applied in real world instances. This research identifies three strategies for application: the use of explicit language, the building of transformative coalitions, and the centering of marginalized voices, stories, and lived experience. This, in combination with an exploration into how sexual health professionals see their own professional and personal identities, indicates that there are very real world applications of intersectional theory that benefit practice.

Aim and research questions: This study investigates how the corona pandemic COVID-19 has been noticed and presented in the news reporting of a Danish newspaper and a Swedish newspaper in their digital news feed. The study is based on the following questions: ● What discursive themes can be identified in Danish and Swedish news reporting on COVID-19 in 2020? ● What are linguistic features used in the headline of the articles in Danish and Swedishnews media? ● How can the Danish and Swedish media reports on COVID-19 be explained by discourse analysis and theoretical framework? Method and material: The study has used a qualitative method with a critical discourse analysis that uses Norman Fairclough's three-dimensional analysis model to analyze a total of 48 articles from Aftonbladet and Kristeligt Dagblad. Main result: The study results show an overview of how news media reporting from Aftonbladet and Kristeligt Dagblad shapes the COVID-19 pandemic with four discursive themes: infection, information, restriction, and immunity. They shaped the discourses onCOVID-19 through a critical perspective in relation to the actions of politicians and authorities and their handling. Linguistic features are used to construct our society during the pandemic to capture readers' attention through the headlines. These have been in the form of metaphorical and charged words that reflect either negative or positive emotional reactions. It also highlights that the media has an influence on how things look; they can consciously or unconsciously make us feel a certain way. As a result, people perceive things differently and can sometimes misunderstand information.

In June of 2006, Merck gained FDA approval for Gardasil, a human papillomavirus vaccine. Shortly before, Merck also launched a large campaign to advertise the vaccine, which has continued through today. Because of the vast number of women the vaccine has the potential to affect, the product has become socially significant. Therefore, this study examines the Gardasil campaign and how it is persuasive. More specifically, this paper studies the rhetorical appeals Merck uses and explores the ethical strengths and limitations of the advertisements. The study explores how the Gardasil campaign blends elements of an informative Campaign with an advertising campaign, and the implications of doing so. Finally, based on Mercks blended campaign, this study makes suggestions for future blended campaigns.

Abstract:
Drawing on critical feminist narrative inquiry, we explore illness narratives of women cancer survivors living in Southern Central Appalachia via a daylong story circle (n = 26) and individual interviews (n = 3). In our article, we argue that participants functioned as illness genealogists as a consequence of their central location in families, as well as their location in a place (Southern Central Appalachia) characterized by what we call “cohabitating morbidities.” We coined this term to represent the experiences of women survivors living with multiple, sometimes simultaneously occurring illness experiences in their family systems. Finally, we reveal and explore rules that guide their survivorship experiences and storytelling, contending that study participants preserve their central location within family systems by decentering their own survivorship experiences and stories.

Inom intensivvården vårdas patienter med sviktande vitala funktioner där respiratorbehandling ofta kan vara en del av behandlingen. Invasiv respiratorbehandling leder till att patientens förmåga till verbal kommunikation upphör. Patienten befinner sig ofta i en utsatt situation där förlust av förmågan till verbal kommunikation kan leda till känslor av rädsla, oro och frustration. Patientens nedsatta kommunikationsförmåga ställer höga krav på vårdpersonalens förmåga att förmedla och tolka information.Syftet med studien är att beskriva vårdpersonalens upplevelser av att kommunicera med den vakna intuberade intensivvårdspatienten. Studien genomfördes som en kvalitativ intervjustudie med induktiv ansats. Specialistsjuksköterskor och undersköterskor på fyra olika intensivvårdsavdelningar intervjuades i fokusgrupper. Intervjumaterialet analyserades med kvalitativ innehållsanalys som resulterade i tre huvudkategorier samt åtta underkategorier. Intervjupersonernas upplevelser återges som svårigheter och frustration, där upplevelse av att inte nå fram samt hinder för kommunikation beskrivs. Strategier för att underlätta kommunikation behandlar de förhållningssätt och tekniker som vårdpersonalen använder i sin vardag och att använda sig av kommunikation som omvårdnadshandling handlar om att skapa trygghet och förståelse. I modern intensivvård tillåts ofta patienten vara så vaken som sjukdomstillståndet tillåter. Denna förändring har resulterat i ökad arbetsbelastning för vårdpersonalen. Strukturella brister observerades relaterat till det undersökta fenomenet, men även lösningar som i framtiden skulle kunna förbättra kommunikationen med vakna intuberade patienter. Studien skulle kunna bidra till ökad medvetenhet och förståelse kopplat till de utmaningar som vårdpersonalen dagligen ställs inför relaterat till kommunikation med vakna intuberade patienter.

Abstract:
Patients in intensive care are often intubated, and therefore unable to speak, which leads to difficulties in communication. The ICU is a very stressful environment and can be experienced as foreign and frightening by both patients and their close ones. The ICU nurse’s caring responsibilities includes both the care of the critically ill patient and the support of relatives who are often in shock. This balance is not always straightforward, and acting professionally in both instances can lead to problems. The professional competence of an ICU nurse has been described as a calm surface, beneath which rages a conflict between medical and ethical issues. As such, communicating professionally with relatives is a challenge that rivals the nurse’s medical responsibilities.The aim of the study was to describe the significance of the nurse's communication with close relatives in the ICU patient room.The study was conducted using a qualitative and inductive approach. A non-participant, semi-structured, observational method was used. Seven observations were conducted.The results of the study are presented in five categories: to prioritise attention; be available for relatives; listen, inform, be sensitive and responsive; to value the meeting and to juxtapose between compassion and objectivity. We have found that the nurse possess the power to make a difference in how the relation between the nurse and close ones turns out and also that several factors can affect their relation.

Objectives: The objectives of this research were to describe the interactions (formal and informal) in which macrocognitive functions occur and their location on a pediatric intensive care unit (PICU); describe challenges and facilitators of macrocognition using three constructs of space syntax (openness, connectivity, and visibility); and analyze the health care built environment (HCBE) using those constructs to explicate influences on macrocognition. Background: In high reliability, complex industries, macrocognition is an approach to develop new knowledge among interprofessional team members. Although macrocognitive functions have been analyzed in multiple health care settings, the effect of the HCBE on those functions has not been directly studied. The theoretical framework, “Macrocognition in the Health Care Built Environment” (m-HCBE) addresses this relationship. Methods: A focused ethnographic study was conducted, including observation and focus groups. Architectural drawing files used to create distance matrices and isovist field view analyses were compared to panoramic photographs and ethnographic data. Results: Neighborhoods comprised of corner configurations with maximized visibility enhanced team interactions as well as observation of patients, offering the greatest opportunity for informal situated macrocognitive interactions (SMIs). Conclusions: Results from this study support the intricate link between macrocognitive interactions and space syntax constructs within the HCBE. These findings help to advance the m-HCBE theory for improving physical space by designing new spaces or refining existing spaces, or for adapting IPT practices to maximize formal and informal SMI opportunities; this lays the groundwork for future research to improve safety and quality for patient and family care.

Much research about advertisements for menstrual products reveals the ways in which such advertising perpetuates shame and reinforces unrealistic ideals of femininity and womanhood. This study aims to examine the content of Instagram posts by four different menstrual product brands in hopes of understanding how these functions may or may not be carried out by social media posts by these brands as well. Building on the body of research about menstrual shame and advertising, I specifically ask: How do the Instagram pages for four menstrual product brands dissuade individuality; how do they prescribe femininity; and how do these functions differ across brands? From a liberal feminist perspective, the examined media exhibits some signs of progress—such as better racial representation—but overall maintains the status quo as to who should be using which products, what womanhood means, and what menstruation entails. These findings indicate that within menstrual product advertising, harmful gender, ability, race, class, and wealth stereotypes continue. Further research of a broader scope is needed to investigate changes on a larger scale, such as within advertising on other platforms and by more brands.

In eastern Kentucky, as in much of central Appalachia, current local storylines narrate the frictions and contradictions involved in the structural transition from a post-WWII Fordist industrial economy and a Keynesian welfare state to a Post-Fordist service economy and Neoliberal hollow state, starving for energy to sustain consumer indulgence (Jessop, 1993; Harvey, 2003; 2005). Neoliberalism is the ideological force redefining the “societal infrastructure of language” that legitimates this transition, in part by redefining the key terms of democracy and citizenship, as well as valorizing the market, the individual, and technocratic innovation (Chouliaraki & Fairclough, 1999; Harvey, 2005). This project develops a perspective that understands cancer prevention and control in Appalachiaas part of the structural transition that is realigning community social ties in relation to ideological forces deployed as “commonsense” storylines that “lubricate” frictions that complicates the transition.

As a committed social justice educator, I share in this dissertation a theoretically informed instructional autoethnography of my time teaching and researching as a Language Arts and Speech teacher across two different public high schools and two different school years. My story of learning to embody the values and practices of progressive teaching arises from the central research questions: "How can I, a self-identified progressive Language Arts educator committed to social justice, learn to implement critical, democratic, responsive, and holistic pedagogy as a public high school teacher in this particular region at this time in U.S. public education? And, within these particular schooling cultures, what aspects of these schooling environments support or inhibit my ability to perform as a progressive educator?" Responding to critiques of public schooling policy and practices, my work is grounded in theoretical commitments of progressive education articulated by the critical pedagogy of Paulo Freire and those North American educators who have brought his libratory praxis forward into what I call connected, social justice pedagogy.

Healthy eating has become an important subject in society, particularly in the media. Food has always been a complex area but in recent years it has become even more distinct. Based on a critical discourse analysis, the purpose of this study is to examine and disclose what discourses that appear in articles regarding dietary counselling in three swedish health magazines, ToppHälsa, MåBra and Womens Health & Wellness. Analyzing three articles in each magazine we identified several discourses. The most prominent is the discourse about wellbeing. The majority of the articles reproduce this discourse by associating healthy eating with wellbeing. As the research field is limited, this study contributes with findings in a area that still needs further research.

During the last decades Recognition of Prior Learning (RPL) has become a more frequently used method to recognise adult’s prior learning. This thesis analyses a process of RPL in health care, where health care assistants are assessed against subjects in the upper-secondary health care program. Prior research on RPL is to a high degree non-theoretical and focus is primarily on policy level research. This thesis adds to the field by progressing a critical social theory perspective on RPL. In the thesis the RPL process is analysed through Jürgen Habermas’ theory of communicative action and Axel Honneth’s Recognition theory. General questions posed are: What are the power issues in the RPL process? What implications does the tension between the lifeworld of work and the system of education have? What consequences does mutual understanding and communication have for the outcome of the RPL process? What part does recognition play for the participants? The results disclose the power relations that emerge in the relationship between participants and teachers. A caring ideology is developed and problematized. Further, the importance of mutual understanding between participant and teachers in the assessment of prior learning is discussed focusing on the consequences a lack of mutual understanding could have for the outcome in such assessments. On a macro level the analysis demonstrates the tension between the participants prior learning and the educational systems demand for formalising prior learning. In addition, analyses of a more developmental character that intends to show the potential for critical learning, change and recognition are progressed. The results suggest that communicative action can be used to develop RPL into processes focusing on critical learning and change. Recognition of traits and abilities could also enhance individual’s positive relations with the self. Such recognition could develop self-confidence and thus RPL could encourage learning and cultivate continuing self-realisation through work.
Validering har under de senaste decennierna blivit en alltmer använd metod för att erkänna vuxnas tidigare lärande. Denna avhandling analyserar en valideringsprocess inom omvårdnadssektorn för vårdbiträden med mångårig erfarenhet. Vårdbiträdena valideras mot ämnen inom omvårdnadsprogrammets karaktärsämnen. Tidigare forskning om validering är i hög grad icke-teoretisk och mycket fokus har legat på policynivå. Denna avhandling bidrar till forskningsfältet genom att problematisera validering utifrån ett kritiskt samhällsteoretiskt perspektiv. I avhandlingen analyseras valideringsprocessen utifrån Jürgen Habermas teori om det kommunikativa handlandet och Axel Honneths erkännandeteori. Exempel på mer generella frågor som ställs är: Vilka är maktfaktorerna i valideringsprocessen? Är handlingarna i processen koordinerade mot gemensam förståelse? Hur ser förhållandet mellan system och livsvärld ut? Vilken roll spelar erkännandet i validering för deltagarna? Resultatet synliggör de maktförhållanden som uppstår mellan deltagare och lärare i bedömningsprocessen. En omvårdande ideologi lyfts fram och problematiseras. Därtill diskuteras betydelsen av gemensam förståelse mellan deltagare och lärare vid bedömning, samt vilka konsekvenser brist på gemensam förståelse kan få. På ett makroplan åskådliggör analysen spänningen mellan deltagarnas erfarenheter och utbildningssystemets krav på formalisering. Vidare genomförs mer utvecklingsfokuserade analyser som vill visa på möjligheterna för kritiskt lärande och förändring i validering. Resultatet demonstrerar också, bland annat, betydelsen av att bli erkänd för sina förmågor och hur detta kan utveckla individens självuppskattning och främja lärande. I studien föreslås det att kommunikativt handlande kan utgöra en norm för hur validering kan utvecklas mot mer kritiskt lärande och positiv förändring. Men också att erkännande av tidigare förmågor kan gynna individens positiva relation till sig själv, hur detta främjar lärande, självuppskattning och hur validering därmed kan utgöra en grund för ett vidare självförverkligande genom arbete.

BACKGROUND: Pain assessment is a significant challenge in critically ill adults, especially those unable to communicate their pain level. At present there is no universally accepted pain scale for use in the non-communicative (cognitively impaired, sedated, paralyzed or mechanically ventilated) patient. Facial expressions are considered among the most reflexive and automatic nonverbal indices of pain. The facial expression component of pain assessment tools include a variety of facial descriptors (wincing, frowning, grimacing, smile/relaxed) with inconsistent pain intensity ratings or checklists of behaviors. The lack of consistent facial expression description and quantification of pain intensity makes standardization of pain evaluation difficult. Although use of facial expression is an important behavioral measure of pain intensity, precise and accurate methods for interpreting the specific facial actions of pain in critically ill adults has not been identified. OBJECTIVE: The three specific aims of this prospective study were: 1) to describe facial actions during pain in non-communicative critically ill patients; 2) to determine facial actions that characterize the pain response; 3) to describe the effect of patient factors on facial actions during the pain response. DESIGN: Descriptive, correlational, comparative. SETTING: Two adult critical care units (Surgical Trauma ICU-STICU and Medical Respiratory ICU-MRICU) at an urban university medical center. SUBJECTS: A convenience sample of 50 non-communicative critically ill intubated, mechanically ventilated adult patients. Fifty-two percent were male, 48% Euro-American, with mean age 52.5 years (±17. 2). METHODS: Subjects were video-recorded while in an intensive care unit at rest (baseline phase) and during endotracheal suctioning (procedure phase). Observer-based pain ratings were gathered using the Behavioral Pain Scale. Facial actions were coded from video using the Facial Action Coding System (FACS) over a 30 second time period for each phase. Pain scores were calculated from FACS action units (AU) following Prkachin and Solomon metric. RESULTS: Fourteen facial action units were associated with pain response and found to occur more frequently during the noxious procedure than during baseline. These included areas of brow raiser, brow lower, orbit tightening, eye closure, head movements, mouth opening, nose wrinkling, and nasal dilatation, and chin raise. The sum of intensity of the 14 AUs was correlated with BPS (r=0.70, P<0.0001) and with the facial expression component of BPS (r=0.58, P<0.0001) during procedure. A stepwise multivariate analysis predicted 5 pain-relevant facial AUs [brow raiser (AU 1), brow lower (AU 4), nose wrinkling (AU 9), head turned right (AU 52), and head turned up (AU53)] that accounted for 71% of the variance (Adjusted R2=0.682) in pain response (F= 21.99, df=49, P<0.0001). The FACS pain intensity score based on 5 pain-relevant facial AUs was associated with BPS (r=0.77, P<0.0001) and with the facial expression component of BPS (r=0.63, P<0.0001) during procedure. Patient factors (e. g., age, gender, race, and diagnosis, duration of endotracheal intubation, ICU length of stay, and analgesic and sedative drug usages, and severity of illness) were not associated with the FACS pain intensity score. CONCLUSIONS: Overall, the FACS pain intensity score composed of inner brow raiser, brow lower, nose wrinkle, and head movements reflected a general pain action in our study. Upper facial expression provides an important behavioral measure of pain which may be used in the clinical evaluation of pain in the non-communicative critically ill patients. These results provide preliminary results that the Facial Action Coding System can discriminate a patient’s acute pain experience.

The popular Social Networking Site, Facebook, offers its users the ability tocommunicate with others from all over the globe. Individuals can create a virtual identity for themselves enabling members to call, message, and locate others in a matter of seconds. The number of Facebook users appears to increase; yet, the number of members who die daily is not normally accounted. Facebook now permits the memorialization ofthe deceased’s profile. This allows members to continue commenting, sharing photos and videos, and visiting the deceased’s Facebook page. This trend led to the central question of this study, “What benefits, if any, are individuals receiving by utilizing Facebook status updates in order to cope with loss?” A gap in the literature exists pertaining to the creation of status updates for adults (25-64 years old) who have lost an immediate family member within the past year, as other studies have focused on adolescents’ and college students’ grieving processes on Facebook. As the principal investigator, I looked for what may or may not be different for individuals using Facebook status updates to cope after the loss of a loved one. I did so by interviewing seven participants, transcribing digital voice recordings, and using a grounded theory methodology to code and search for themes and patterns within the data. Participants were recruited using fliers, word of mouth, and emails sent to members of Nova Southeastern University. Findings from this study led to the discovery of the theory Social Grief after participants shared they received support, validation, and closure by using Facebook status updates to cope with the loss of an immediate family member.

The aim of this study is to analyze the functional beverage NOCCO’s Instagram account. The purpose is to expose how they use discourses of health and fitness in their marketing. In particular we analyze the social actors in the pictures. The theories in this study is based on a social semiotic theory of communication and an assumption that we live in The new public health era. The method applied is a qualitative multimodal critical discourse analysis (MCDA). The study examines how the social actors are used for marketing purposes, how they are represented in the pictures and in which settings they are placed in. The results shows that NOCCO uses discourses of health and exercise in their marketing on Instagram. NOCCO appears as a healthy product due to its association with exercise and fit social actors.

Aquest context de cultura del consum en què ens trobem, on la mercadotècnia ha aconseguit associar un determinat model corporal a l'èxit personal i social -relació que s'intensifica en l'adolescència, quan l'aspecte físic és objecte de la màxima atenció i preocupació- ha motivat la realització d'una intervenció específica des de l'educació física a través d'un procés d'investigació-acció.
L'objectiu de la recerca ha consistit en desenvolupar l'actitud crítica de l'alumnat cap al culte exacerbat al cos mitjançant el disseny, aplicació i avaluació d'un recurs educatiu multimèdia. Tot això en el marc d'una unitat didàctica de condició física i salut per a 4t d'ESO.
En la investigació s'analitzen els fonaments curriculars i pedagògics d'aquesta intenció educativa i es delimita conceptualment el principal objecte d'ensenyança: l'actitud crítica. Posteriorment es dissenya, es descriu i s'avalua un pla d'acció aplicat de manera sistemàtica i amb el rigor científic que requereix una recerca que naix i es desenvolupa en la pràctica.
El contexto de cultura del consumo en que nos encontramos, donde la mercadotecnia ha conseguido asociar un determinado modelo corporal al éxito personal y social -relación que se intensifica en la adolescencia, cuando el aspecto físico es objeto de la máxima atención y preocupación- ha motivado la realización de una intervención específica desde la Educación Física a través de un proceso de investigación-acción.
El objetivo de la investigación ha consistido en desarrollar la actitud crítica del alumnado hacia el culto al cuerpo exacerbado mediante el diseño, aplicación y evaluación de un recurso educativo multimedia. Todo ello en el marco de una unidad didáctica de condición física y salud para 4º de ESO.
En la investigación se analizan los fundamentos curriculares y pedagógicos de esta intención educativa y se delimita conceptualmente el principal objeto de enseñanza: la actitud crítica. Posteriormente se diseña, se describe y se evalúa un plan de acción aplicado de manera sistemática y con el rigor científico que requiere una investigación que nace y se desarrolla en la práctica.
The consumer society in which we live, together with certain merchandising campaigns, has resulted in the fact that people tend to associate more and more the idea of a "perfect body" with personal and social success. Needless to say, people are more and more concerned about their physical appearance during adolescence. All this has inspired a thorough study on this topic from the perspective of the physical education and by means of an "Action-Research" process.
The aim of this research was that some students in the fourth year of ESO (the last compulsory school year in the Spanish educational system) should acquire a critical attitude towards the body-building and body-care obsession. This study has been carried out by designing and using an Educational Multimedia Resource.
This educational research is to be justified in the curricular foundations of the Spanish educational system and it aims to define clearly the core of this study: the development of a critical attitude. Several teaching strategies are designed, described and assessed, so that students can develop this critical thinking.
All in all, the final goal is that students are able to develop a critical attitude while working from their own experiences.

Alors que beaucoup s'accordent sur l'opportunité représentée par ces communautés pour ses utilisateurs, les connaissances sur ce qui détermine l'action collective en ligne des patients ainsi que sur les fondamentaux de l'action collective en ligne dans ces espaces virtuels sont relativement peu développées. En conséquence, ce travail doctoral examine les raisons pour lesquelles les patients interagissent entre eux et comment ils procèdent. En nous appuyant sur le modèle du comportement orienté vers un but, la théorie de la valeur de l'attente, la théorie des forces du champ, les concepts de dons et les interviews menées, nous avons développé un modèle qui examine les interactions en ligne des patients dans un contexte d'action collective en ligne. Une approche multi-méthode, qualitative et quantitative, permet d'explorer les interactions des patients et de mesurer les déterminants de l'action collective en ligne sur ces espaces virtuels. L'analyse qualitative de 54 entretiens menés avec des patients, des proches de patients, des professionnels de la santé 2.0, des médecins et des soignants permet d'affiner le modèle de recherche, qui a ensuite été testé au travers d'une enquête quantitative auprès de 269 patients. Over the last few years, virtual patients’communities have been developing tremendously over the Internet. These Web 2.0 communities allow frequent interactions among patients, who can share health-related information within an interactive environment. While many agree on the opportunity represented by those communities for its users, we know very little about what determines patients’ online collective action, specifically on virtual communities as well as the fundamentals of online collective action in these virtual spaces. Accordingly, this doctoral work examines why patients interact with others and how they interact on topics related to their disease through these virtual communities. Drawing on the goal-directed behavior (MGB), the expectancy-value (EVT) theories, the field force theory, gift concepts and field interviews, we have developed a model for examining patients’ online interactions and identified gift-giving behaviors in the context of online collective action. A multi-method, qualitative and quantitative approaches, enables us to explore patients’ interactions and measures the determinants of online collective action on these virtual spaces. The qualitative analysis of 54 interviews conducted with patients, patient’s relatives, Health 2.0 professionals, doctors and caregivers allows refining the research model, which has then been tested through a survey handled with 269 patients, members of patient’s communities. This research contributes to IS research by increasing our knowledge regarding the individual dynamics and interactions that surround online patients’ communities

The objective of this thesis is to examine the meaning of the language used in an Entertainment-Education (E-E) radio serial drama RockPoint 256 (RP256) and its representation of People Living with HIV/AIDS (PLHA) and HIV/AIDS. The theoretical framework that is used to guide this study is the Cultural Studies perspective. Using Cultural Studies is instrumental to a study such as this that looks to find the source of meaning in RP256’s discourse (Hall, 1993, 105). The methodological research design used is discourse analysis that examines the oral and written data of RP256. Discourse analysis “focuses on the way language is used, what it is used for, and the social context in which it is used” (Punch, 1998, 226). There were two types of discourse analyses used. Gee’s (1999) discourse analysis is used to analyse the linguistic nature of the texts at a micro level while, Fairclough’s (1989) discourse analysis is used to observe the overarching meaning of the discourse found in RP256 at the macro level. The analysis of the data indicates that the representation of PLHA is a product of the societal and cultural markers that are a result of ideological labels given to HIV/AIDS and PLHA. The study concludes that PLHA are ostracised, stigmatized, live in poverty and are mostly women. The societal and cultural markers indicate that HIV/AIDS is still viewed as a plague and as a result PLHA are often silenced, marginalized, and discriminated against in Uganda.

abstract: Dominant discourses of health and fitness perpetuate particular ideologies of what it means to be “healthy” and “fit,” often conflating the two terms through conceptualizing the appearance of physical fitness as health. The discourse of healthism, a concept rooted in the economic concept of neoliberalism, fosters health as an individual and moral imperative to perform responsible citizenship, making the appearance of the “fit” body a valued representation of both health and self-discipline. This perspective neglects the social determinants of health and ignores the natural variation of the human body in shape, size, and ability, assuming that health can be seen visually on the body. Through a case study of one particular location of a popular commercial gym chain in an urban city of the Southwestern United States, this study employs a critical discourse analysis of the gym space itself including a collection of advertisements, photographs, and signs, in addition to participant observation and semi-structured interviews conducted with diverse women who exercise at this gym to explore how women resist and/or (re)produce discourses of healthism related to health, fitness, and body image. Ultimately, critical analysis shows that the gym itself produces and reifies the discourse of healthism through narratives of simultaneous empowerment and obligation. Though women in the gym reproduced this dominant narrative throughout their interviews, internal contradictions and nuggets of resistance emerged. These nuggets of resistance create fractures in the dominant discourse, shining light into areas that can be explored further for resistance practices through sense-making, necessitating a language of resistance.

EMS personnel experience emotionally charged calls, such as CPR, trauma, or domestic violence. This study examined the changes on communication by these events. Communication Complex metaframework allowed use of other disciplines. There is a lack of scholarship surrounding EMS communication. Research from other military and other fields was translated into the EMS community. Mental illness is a growing concern in EMS as 37% contemplate suicide and 6% complete it. Part of understanding the affect is an exploration of how the culture of EMS (i.e. training, traditions, machismo) shapes the way new EMS are acculturated. EMS have repeated exposure to trauma over a career. These exposures change communication patterns. Using a three-chapter autoethnography, I was able to examine my communication and mental status changes from rookie until retiring 14 years later with PTSD and constant suicidal ideation. Ethnographic interviews of veteran EMS provided insight into the old school ideology of emotional repression and shelving. I analyzed using the NREMT Patient Assessment skill sheet as a guide in a three-step process to discover and reassess themes. The primary survey indicated common job-related stressors- pedi calls and staffing problems. The secondary survey revealed themes of emotions, senses, and support. Finally, the reassessment revealed subtle changes in EMS culture, including decreased PTSD stigma, increased resiliency training, and increased administrative support. Future research could examine the effect of spousal support and changes in cultural emotional suppression. The goal is to develop programs to help allies understand the emotionality in EMS and create dedicated support structures to increase EMS mental health.

Today the medical disputes in Taiwan have occurred frequently. For most people, ECMO is regarded at a severe medical help symbol. However, there are many restrictions of using ECMO and the investment medical personnel and costs are huge. Therefore, this study tries to take ECMO as an example to explore the role of health communication in a medical environment and on critical care decision-making. This study uses theoretical sampling and selected twelve critical care team professionals as samples and conducted in-depth interviews using qualitative research. Grounded theory analysis was used for data analysis. Studies have shown that factors affecting the health of communication can be divided into the cognitive aspects of medical, demographic variables respect, health communication content and external factors. The models of today's critical care clinical decision-making are shared medical decisions, informed decision-making and professional mentor-based decision-making. Health communication will directly affect the critical care decision-making, such as trust , medical information asymmetry and other factors. The study suggests that cognitive aspects of health care can enhance the popularization of medical knowledge and understand the characteristics of health care can reduce the gap between expectation and clinical; Health communication process in view to complete and time permit to fulfill our obligations to communicate, By linking relevant team resources to ensure the quality of communication when necessary. Finally, we should establish a friendly medical treatment culture gradually improve today's health care environment and the relative collapse of the doctor-patient relationship, Reach patients, families and medical end-win situation.

Abstract:
This thesis investigates how body positivity and sustainability are discursively constructed by fast fashion brands. The brand chosen as a case study is H&M, as it describes itself as being both environmentally and socially involved. Yet, despite H&M's claims, during our preliminary research we were able to detect the lack of accessibility of sustainable collections to plus size customers. In this essay, we analyze how H&M communicates body inclusivity in its sustainability reports, website, and YouTube campaigns promoting conscious lines. Drawing on theories of representation, biopolitics, and post-feminism, we sought to critically analyze the previously mentioned material through a multimodal critical analysis. The results show how H&M discursively constructs health communication in different ways depending on which clothing line the brand is promoting and to which consumers it is being advertised. Class discourses were recognized, leading to exclusive access to sustainable clothing for higher income shoppers. We believe this work is relevant to opening up the discussion about democratization and making sustainable items fully accessible to everyone.

Objectifs: Les patients hospitalisés aux soins intensifs (SI) sont souvent victimes d’erreurs médicales. La nature interprofessionnelle des équipes de SI les rend vulnérables aux erreurs de communication. L’objectif primaire du projet est d’améliorer la communication dans une équipe interprofessionnelle de soins intensifs par une formation en simulation à haute fidélité. Méthodologie Une étude prospective randomisée contrôlée à double insu a été réalisée. Dix équipes de six professionnels de SI ont complété trois scénarios de simulations de réanimation. Le groupe intervention était débreffé sur des aspects de communication alors que le groupe contrôle était débreffé sur des aspects techniques de réanimation. Trois mois plus tard, les équipes réalisaient une quatrième simulation sans débreffage. Les simulations étaient toutes évaluées pour la qualité, l’efficacité de la communication et le partage des informations critiques par quatre évaluateurs. Résultats Pour l’issue primaire, il n’y a pas eu d’amélioration plus grande de la communication dans le groupe intervention en comparaison avec le groupe contrôle. Une amélioration de 16% de l’efficacité des communications a été notée dans les équipes de soins intensifs indépendamment du groupe étudié. Les infirmiers et les inhalothérapeutes ont amélioré significativement l’efficacité de la communication après trois sessions. L’effet observé ne s’est pas maintenu à trois mois. Conclusion Une formation sur simulateur à haute fidélité couplée à un débreffage peut améliorer à court terme l’efficacité des communications dans une équipe interprofessionnelle de SI.
Objective: Patient hospitalized in intensive care units (ICU) are often victims of medical errors. The interprofessional nature of ICU teams makes them particularly vulnerable to communication errors. The primary objective of this project is to improve interprofessional communication in an ICU team using a high fidelity simulation-based training. Method A prospective, double-blind, randomized controlled trial was conducted. Ten teams of six ICU workers completed three simulated scenarios of resuscitation. The intervention group was debriefed on communication skills, whereas the control group was debriefed on technical resuscitation skills. Three months later, teams completed a fourth simulation without debriefing. All simulation sessions were coded for quality, efficacy of communication and team information-sharing by four blinded observers. Results For our primary end-point, improvement in communication in intervention group wasn’t greater than in control group. A 16% increase in communication effectiveness was noted in ICU teams independently of the studied group. Nurses and respiratory therapists significantly increased their communication effectiveness after three sessions. The effect did not last to three months. Conclusion The combination of high-fidelity simulation training and communication-oriented debriefing can produce short-term gains in communication effectiveness of an interprofessional ICU team.

This study investigates the factors most likely to support effective communication in clinical placements for ESL nursing students in New Zealand. The experiences of ESL students as they complete a clinical placement are examined and compared with data from the students‟ clinical lecturers and preceptors. Data for the study was collected through interviews with individual students before, during and after their Transition to Practice placement. Interviews were also held with two of the students‟ preceptors, and a focus group was held with clinical lecturers who had experience of working with ESL students. Results from the study underline the importance of facilitating students‟ entry to the placement community of practice, and access to its interactions. A number of factors supported or inhibited students‟ participation and learning within the placement community. The study identified two critical factors intrinsic to the student, and two extrinsic to the student. Intrinsic factors included the student‟s proficiency with English language: in particular, the sophisticated sociopragmatic language skills used by nurses in their daily interactions. The student‟s use of learning strategies, including the proactive approach best suited to learning on placement, was also critical. Extrinsic factors likely to support the student‟s integration within the community of practice were the quality of the preceptor, in terms of attitude to and training for the preceptoring role, and the tone of the placement environment. The major outcomes of this study point to the importance of providing direct instruction for ESL students in the kinds of language and learning styles required for placement. There are also implications for the selection and training of preceptors and for the placing of students in appropriate clinical environments. The study concludes with suggestions for a specific communication programme for ESL students.

This study sought to examine linguistic and discursive strategies used to construct messages reflective of the implementation of the HIV and AIDS policy for Zimbabwe of 1999 by government and private newspapers. Such analysis was perceived to be important since media content has a bearing on Zimbabweans‘ perception and attitudes regarding HIV and AIDS prevention, treatment and control. The study was aimed at comparing messages from newspapers with views by the people of Zimbabwe regarding the implementation of the policy. Findings reveal that empowerment programmes particularly those targeting women and children are lagging behind as Zimbabweans, literature and newspaper data sources testify. In addition, information sources concur that cultural (For example, stigmatisation, polygamy, religious practices, spouse inheritance) and structural (For example, patriarchy, masculinity, bureaucracy, politics) are stumbling blocks that negatively affect the implementation of the policy. Further, even though private and government newspapers do not fully agree on the portrayal of human agents, there is a general consensus between newspaper reports and Zimbabweans that people still face socio-economic and econo-political challenges that militate against the smooth implementation of the HIV and AIDS policy. Government newspapers tend to downplay aspects which reveal inadequacies of government activities. The study notes this as betrayal of use of ideological squares both by government and private newspapers whereby certain aspects regarding the implementation of the policy are either downplayed or highlighted to influence perception. The study reveals that newspaper reports used nominalisation, quantification, positive politeness, thematisation, rhematisation, intertextuality, euphemism, proverbs, idioms, action verbs, metaphors and citation of experts as linguistic and discursive strategies both for agenda setting and building purposes regarding the implementation of the HIV and AIDS policy. Other devices used particularly in the encoding of Operation Murambatsvina are, claptraps, deictic referencing, personal pronouns, adjectives and direct speech. The study attributes problems regarding the Zimbabwean HIV and AIDS intervention model to the top – down approach inherent in the policy. Hence, the call for an adoption of an unhu/hunhu/ubuntu inspired bottom – up HIV and AIDS intervention model in Zimbabwe. This would inculcate pro-family, pro-village, pro-nation/people and ―servant leadership‖ (Mangena and Chitando, 2011) values in the fight against the pandemic through the embracing of Indigenous Knowledge Systems (IKS). Unfortunately, such values largely continue to elude the radar of the current top – down HIV and AIDS intervention model cuurently in use in Zimbabwe.

Abstract:

This dissertation, broadly, examines how power dynamics manifest during clinical interactions related to vague and gendered medical symptoms, such as pelvic pain. To examine this issue, I approach my research questions through multiple methods including a critical discourse analysis of the medical discourse on pelvic pain, a survey experiment administered to healthcare workers in the US, and a narrative analysis of pelvic pain patient experiences. While the focus of this research is on pelvic pain, the analyses presented here reach far beyond ideas of power dynamics and pelvic pain. Rather, the findings from this research speak to theoretical discussions medical sociologists have been having for decades. Specifically, findings provide new insight on: 1) the limits of evidence-based medicine as a biomedical paradigm, 2) how fundamental causes of health inequality intersect with each other as well as other factors, such as gender, important in predicting health outcomes and 3) how discussions of metamechanisms in fundamental cause theory can inform our understanding of the accumulation of cultural health capital. In providing such insight, this dissertation uses the case of pelvic pain to integrate multiple perspectives and theories in medical sociology to drive the field forward in a way that acknowledges the many ways power is simultaneously constituted in the clinical interaction. From the role of gender, race, and class in power relations, to the ways medical knowledge, discourse, and authority dictate the clinical interaction, this research covers a wide range of sociological theories and concepts. In doing so, this dissertation sheds new light on current understandings of power in the clinical interaction and its relationship to inequitable health outcomes in the US.

Exemplified by fire crews, SWAT teams, and emergency surgical units, critical teams are a subset of action teams whose work is marked by finality, pressure, and potentially fatal outcomes (Ishak & Ballard, 2012). Using communicative and temporal lenses, this study investigates how organizations prime and prepare their embedded critical teams to deal with improvisation. This study explicates how organizations both encourage and discourage improvisation for their embedded critical teams. Throughout the training process, organizations implement a structured yet flexible “roadmap”-type approach to critical team work, an approach that is encapsulated through three training goals. The first goal is to make events routine to members. The second goal is to help members deal with non-routine events. The third goal is to help members understand how to differentiate between what is routine and non-routine. The grounded theory analysis in this study also surfaced three tools that are used within the parameters of the roadmap approach: experience, communicative decision making, and sensemaking. Using Dewey’s (1939, 1958) theory of experience, I introduce a middle-range adapted theory of critical team experience. In this theory, experience and sensemaking are synthesized through communicative decision making to produce decisions, actions, and outcomes in time-limited, specialized, stressful environments. Critical teams have unique temporal patterns that must be considered in any study of their work. Partially based on the nested phase model (Ishak & Ballard, 2012), I also identify three phases of critical team process as critical-interactive, meaning that they are specific to action/critical teams, and they are engaged in by critical teams for the expressed purpose of interaction. These phases are simulation, adaptation, and debriefing. These tools and phases are then placed in the Critical-Action-Response Training Outcomes Grid (CARTOG) to create nine interactions that are useful in implementing a structured yet flexible approach to improvisation in the work of critical teams. Data collection consisted of field observations, semi-structured interviews, and impromptu interviews at work sites. In total, I engaged in 55 hours of field observations at 10 sites. I conducted 31 semi-structured interviews with members of wildland and urban fire crews; emergency medical teams; and tactical teams, including SWAT teams and a bomb squad. I also offer practical implications and future directions for research on the temporal and communicative aspects of critical teams, their parent organizations, and considerations of improvisation in their work.

abstract: At their cores, both rhetoric and public sphere theory have conceptualized how membership in public and counterpublic settings, as well as participation in public life and discussion, is cultivated, shared, contested, and shaped. Previous case studies on publics and counterpublics have looked at the experiences of individuals and collectives who enact practices in rhetorical invention that mark participation in public life. Much of public sphere scholarship focuses squarely on seasoned individuals in positions of authority and decision making in mainstream publics. Conversely, counterpublic spheres focus on the labor of individuals who have extensive experience in articulating discursive practices in response to dominant publics. However, a quietude that has permeated much of rhetoric and public sphere scholarship comes by way of the absence of youth-based voices in the public sphere. It is these same youths who are expected to lead the very publics that claim to represent them, yet do not afford them a mode of participation or agency in their own right. Given that studies in critical and vernacular rhetoric invest significant inquiry into the ways that marginalized communities enact responses towards dominant and mainstream ideologies, it is necessary to consider how these youthful perspectives contribute to rhetoric and the public sphere writ large. In an effort to inform the rhetorical tradition of its potential in accounting for the voices of youth, this study explores the ways in which youth speak, perform, and embody the various ways in which they belong to a public sphere. Through fieldwork in the LGBTQ youth organization One n’ Ten, I aim to speak to the ways in which rhetorical scholarship can begin to move towards a rhetoric of youth in public life. In this field, I utilize the concepts of enclaving and imagining in counterpublic spheres to examine the practices, discourses, and values that give rise to a queer counterpublicity that emboldens LGBTQ youth to speak and act in a way that honors their identities. Moreover, I draw on theories of critical and vernacular rhetorics to make sense of how One n’ Ten provides youth with opportunities to enact rhetorical agency conducive toward participation in public and counterpublic spheres. Finally, I discuss implications pertaining to how the experiences of young individuals stand to substantially inform theories in public, counterpublic, critical, and vernacular rhetorics, all of which contain opportunities to represent the experiences of both LGBTQ youth and youth writ large as members of public life.
