AbstractThe majority of head and neck squamous cell carcinoma (HNSCC) is now classified into two major types: HPV-mediated [HPV(+)] and HPV-negative [HPV(−)]. Within this paradigm, the 8th edition TNM staging system effected modification about what is considered “locally-advanced” HNSCC. Two phase-III trials (RTOG 1016 and De-ESCALATE HPV) disappointingly showed that cetuximab is not as effective in HPV(+) oropharyngeal cancer (OPC) compared to cisplatin with radiotherapy. The recent NRG HN002 de-escalation trial demonstrated the presence of outcome heterogeneity within “low-risk” HPV(+) OPC, some of which continue to benefit from cisplatin combined with reduced-dose radiotherapy. Moreover, distant metastasis (DM) has consolidated its position as the leading cause of death in HPV(+) OPC and strategies to mitigate it are necessary. Unanswered questions and ongoing-emerging concepts exist in both HPV(+) and HPV– diseases. These include understanding the importance of risk under the rubric of extranodal extension (ENE), including degrees of pathological ENE (pENE), and emerging knowledge about radiologic ENE (rENE). Strategies addressing modification of biological phenomena have become paramount and includes hypoxia modification (such as smoking cessation). In addition, contemporary evidence suggests that immunotherapy improves survival in recurrent/metastatic settings, and it is now also being explored in primary disease presentations in combination with (chemo-)radiotherapy. Induction chemotherapy achieves DM reduction in nasopharyngeal cancer but has only been explored minimally in HPV(+) OPC. Evidence that loco-regional management can be de-intensified following a favorable response to induction treatment would provide an attractive option for HPV(+) OPSCC patients while also addressing risk of developing distant disease.

AbstractNaso-orbito-ethmoid (NOE) injuries constitute about 5% of facial fractures and most of them are as a result of road traffic accidents. Deformities in the region tend to be more cosmetically apparent. Considering the proximity of the area to critical structures like brain and eye, such injuries need to be thoroughly evaluated both clinically and radiologically to understand the extent of the injury as well as initiate emergency management if required prior to constituting a definitive treatment plan. CT scans are the most useful investigation modality. Identification and maintaining the attachment of Medial canthal ligament in appropriate position along with reducing / stabilizing the bony fracture is the key to getting good results in these fractures. Primary management of the deformity produces better results compared to secondary correction.

AbstractAdaptation is a highly contextual process, framed by institutions. When one group’s adaptation to climate hazards reduces another’s adaptive capacity, this is called divergent adaptation. The nuances of divergent adaptation are revealed in how institutions influence divergent adaptation outcomes, either to exacerbate conflict or to bring about greater peace and cooperation. By examining the sometimes conflicting adaptations of pastoralists and agro-pastoralists in Niger, this chapter describes the process of divergent adaptation through an institutional analysis from multiple scales. At the national scale, climate change adaptation policies and perspectives are entrenched in sedentarization politics vis-à-vis pastoral livelihoods. At the local scale, pastoralists take a defensive position as an adaptation pathway, to ensure secure passage with their livestock to water and pasture. However, in localities where local institutional actors have enhanced collective action arenas in which pastoral and agro-pastoral groups are represented, conflict dynamics are abated. Climate adaptation is not without conflict; however, local and sub-national institutions are critical to providing opportunities for collective action, cooperation, and peace in the context of divergent adaptation. Based on these findings, it is recommended that conflict and adaptation management and planning be integrated at multiple scales.

Abstract:
The explosive growth of business analytics has created a high demand for individuals who can help organizations gain competitive advantage by extracting business knowledge from data. What types of jobs satisfy this demand and what types of skills should individuals possess to satisfy this huge and growing demand? The authors perform a content analysis of 958 job advertisements posted during 2014-2015 for four types of positions: business analyst, data analyst, data scientist, and data analytics manager. They use a text mining approach to identify the skills needed for these job types and identify six distinct broad competencies. They also identify the competencies unique to a particular type of job and those common to all job types. Their job type categorization provides a framework that organizations can use to inventory their existing workforce competencies in order to identify critical future human resources. It can also guide individual professionals with their career planning as well as academic institutions in assessing and advancing their business analytics curricula.

Using succession planning as a developmental tool will allow the organization to determine what and whose knowledge is most important to capture and help them to identify critical positions. It is very important for organizations to grasp the meaning of succession planning before implementing a succession plan as a developmental tool for organizational sustainability. Based on the definitions of succession planning, this chapter defines succession planning as a proactive approach to managing talent, and the development of a leadership pipeline to meet an organization's need-based demand for sustainability.

In this chapter an attempt is made to focus on the ideological and practical implications of the new people-oriented forms of healthcare management variously described as strategic human resource management, high-performance management, human capital management, or high commitment management. The chapter details the managerial and academic claims concerning the management of human resources and goes on to emphasize the impact that the literature of Human Resource Management (HRM) has had on healthcare employees and healthcare organizations. The restrictions of the applied research approaches are also highlighted and the author argues that the managerial and post-structural literature is limited in that its conceptualization of HRM and changing performances is incomplete. Also considered is a range of literature that has been used to study individual and organizational change with a labour process perspective and explains why an approach based upon the study of the disparity between the “HRM rhetoric” and the day-to-day practice, as well as how people experience and respond to that disparity within a wider frame of social, political, and economic relations, is to be preferred. As a basis for further contextualization, the powerful and privileged positions of healthcare professionals and the distinctive character of their employment relationship are considered. The chapter concludes that a more critical analysis is necessary to challenge the way in which the concept of HRM is examined by healthcare management academics and practitioners.
