Abstract:
Le cellule dell'organismo umano sono provviste di un recettore per il calcio extracellulare (CaSR) la cui espressione tissutale è ridotta nei pazienti nefropatici cronici. Esso ha un ruolo chiave nella regolazione della secrezione del PTH da parte delle cellule paratiroidee e del riassorbimento del calcio da parte delle cellule tubulari renali. Recenti studi biochimico-cellulari hanno osservato che questo recettore ha peculiari caratteristiche funzionali, mentre la ricerca farmacologica ha messo a disposizione farmaci agonisti dell'attività del CaSR (calciomimetici), che sono oggi usati per la cura dell'iperparatiroidismo secondario e che possono avere effetti sulla malattia cardiovascolare e ossea nel paziente nefropatico cronico. A tale riguardo, gli studi EVOLVE e ADVANCE sono stati dedicati a capire il ruolo del calciomimetico cinacalcet nella prevenzione dello sviluppo delle calcificazioni arteriose e degli eventi cardiovascolari nei pazienti nefropatici. Promettenti prospettive sono, perciò, state aperte dalle ricerche in questo recettore e per il trattamento dei malati nefropatici cronici.

Abstract:
Sono stati valutati gli aspetti morfologici delle flogosi croniche rino-sinusali alla TC, eseguita secondo un protocollo iniziale standardizzato, in funzione delle alterazioni anatomo-patologiche presenti, tenuto conto che le nuove tecniche chirurgiche più conservative presuppongono un dettagliato bilancio spaziale pre-operatorio. La TC è tecnica in grado di documentare ispessimenti anche minimi della mucosa rino-sinusale, così come manifestazioni reattive ossee di tipo litico o sclerotico ed eventuali componenti essudative associate. Il dato fondamentale, tuttavia, da analizzare durante il corso dell'esame è rappresentato dall'integrità o coinvolgimento delle regioni del complesso ostio-meatale e del recesso sfeno-etmoidale. Le flogosi croniche semplici sono state suddivise a seconda dei dati anatomo-morfologici emersi in tre forme, in accordo con i dati recenti della letteratura: infundibolare, ostiomeatale e sfenoetmoidale. Tra le flogosi croniche complicate sono stati considerate le poliposi erosive e le cisti. Le poliposi erosive rappresentano la forma sintomatica più frequente, di cui sono state valutate sia forme iniziali, sia forme più avanzate, sia forme recidive, sia complicanze ascessuali. Nell'ambito della patologia cistica, le cisti mucose e sierose sono risultate assai frequenti, ma raramente sintomatiche, mentre forme flogistiche croniche cistiche di più raro riscontro quali il mucocele e mucopiocele sono state evidenziate in pazienti portatori di sintomatologia oculare ad andamento lentamente ingravescente. Infine nelle forme croniche complicate sono state inserite le rare flogosi iatrogene.

Introduction. The cases of reactivation of chronic HBV hepatitis were described in rheumatology and then in hematology. Usually, HBV relapses are described after the third cure of biological therapy in both rheumatologic and hematologic pathology. Purpose. The assessment and monitoring of developments in the treatment of a woman with chronic HBV reactivated after biologic therapy with Rituximab for a small B-cell non-Hodgkin’s lymphoma who developed also chronic kidney disease. Results. We present the case of a 36 years old woman with reactivation of chronic HBV hepatitis after biological therapy with Rituximab (cure 9) for non Hodgkin’s lymphoma with small B-cell. We initiate lamivudine therapy for one month; thereafter we continue with entecavir (dose adjusted to the glomerular filtration rate), together with Rituximab, with favorable outcome both in hematological pathology, as well as in chronic HBV hepatitis, respectively in the biochemical and virology response. Conclusions. In our case, the patient followed the trend of most cases described in the literature, with return to normal ALT values and viral negativity, but without HBs seroconversion.

Abstract:
Il trattamento poli-farmacologico ha determinato, nel corso degli anni, un significativo rallentamento della progressione della malattia renale cronica verso lo stadio di uremia terminale, ma siamo ancora distanti dallo sviluppo di interventi terapeutici in grado di bloccare questo inesorabile e irreversibile processo. Studi clinico-patologici hanno chiaramente dimostrato che il principale elemento coinvolto nel danno renale è la fibrosi tubulo-interstiziale e che il meccanismo patogenetico alla base di questa condizione ha inizio in larga parte nel compartimento tubulare. In particolare, il processo di transizione epitelio-mesenchimale gioca un ruolo importante nella genesi del danno cronico. Durante questo processo, le cellule epiteliali tubulari subiscono un incremento significativo di markers di superficie di natura mesenchimale e, grazie al rimodellamento del citoscheletro e alla degradazione della membrana basale, sono in grado di migrare nell'interstizio dove svolgono un ruolo chiave nel processo patogenetico. In questo contesto, sembra avere un ruolo chiave l'enzima eparanasi, una endo-β-D-glucuronidasi che taglia le catene dell'eparan-solfato a livello di siti specifici intracatena, e partecipa attivamente alla degradazione e al rimodellamento della matrice extracellulare. La degradazione dei vari costituenti dell'ECM, inclusi i proteoglicani eparan-solfato fa-vorisce il rilascio di fattori trofici quali il FGF-2 che induce l'espressione dei marcatori mesenchimali alfa-SMA, VIM e FN, porta alla degradazione della membrana basale mediante la secrezione di metalloproteinasi della matrice ed aumenta la motilità cellulare. L'epressione dell'eparanasi è regolata da fattori di trascrizione, dalla metilazione del DNA e da varie molecole endogene. L'importanza di questo enzima è stata confermata clinicamente dal riscontro di una sua iperespressione in preparati istologici di biopsie effettuate in soggetti affetti da nefropatie croniche (per esempio, nefropatia diabetica). Pertanto visto l'importante ruolo dell'eparanasi sono in fase di standardizzazione numerose strategie per inibire la sua espressione genica e/o la sua attività enzimatica. Infine, è stato proposto il suo possibile utilizzo come biomarker di progressione del danno tubulo-interstiziale da utilizzare routinariamente in ambito nefrologico.

Food allergies have become a public health problem, exceeding 3.27-5% of the local population. Knowledge of food allergens is required. Material and methods. Our study comprised 100 patients with chronic urticaria caused by food sensitization, selected from a total of 252 patients with chronic urticaria different causes. Discussions. Chronic urticaria study group had a food issue and awareness (40%); sensitization was caused by foods known as sensitizing: egg, fish, shellfish, strawberries, nuts, seeds, raspberries, strawberries, soy, celery; the land was hereditary and personal allergic to very high percentage (79%); changes in quality of life was found in 80%. Repeated and abusive food consumption with irregular mealtimes is another contributing factor. Conclusions. Chronic urticaria with food sensitization is one of the manifestations of pathology that can be assigned to XXI century diseases. Cooperation between the family doctor, dermatologist, allergist is required.

Abstract:
La TC cone beam (CBCT) rappresenta un interessante strumento diagnostico specialmente nei bambini per lo studio del basicranio anteriore e laterale, in particolar modo per la bassa esposizione a radiazioni tipica della metodica. Lo scopo del presente studio è stato quello di individuare le potenziali indicazioni per la CBCT nei bambini e il confronto rispetto alle indicazioni nell’adulto. Sono stati analizzati un totale di 554 pazienti (range 0-18 anni, media 10.36) sottoposti a CBCT da Gennaio 2004 a Giugno 2013 presso il Dipartimento di ORL della Clinica Universitaria di Marburg, allo scopo di effettuare un analisi retrospettiva dei parametri utilizzati e delle indicazioni all’esame. I dati provenienti dai bambini sono stati inoltre confrontati con un set di dati di 1730 adulti sottoposti a diagnosi mediante CBCT presso lo stesso dipartimento e già precedentemente pubblicati. Le differenti indicazioni più frequentemente rilevate all’esecuzione dell’esame negli adulti, rispetto ai bambini hanno riguardato il basicranio anteriore, e in particolare: trauma del volto (60.4%) vs. rinosinusite cronica (54.8%), ostruzione respiratoria nasale (13.9% vs. 13.0%) e rinosinusite cronica (12%) vs. trauma del volto (10.8%). Per quanto riguarda il basicranio laterale le indicazioni principali sono state il colesteatoma (20.3%) vs. controllo del posizionamento dell’impianto cocleare (31.2%), otorrea cronica (17.5%) vs. colesteatoma (20.9%) e il controllo del posizionamento dell’impianto cocleare (11.8%) vs. otite media cronica mesotimpanica (6.8%). La CBCT è una metodica efficace per lo studio delle strutture ossee nell’adulto e nei bambini. I parametri di impostazione dipendono principalmente dalla regione oggetto di studio. Uno dei principali obiettivi è ridurre l’esposizione alle radiazioni sia nei bambini che negli adulti.

Rettocolite Ulcerosa e Morbo di Crohn, note come Malattie Infiammatorie Croniche Intestinali, sono largamente diffuse nei paesi occidentali. L'eziologia è multifattoriale e comprende una predisposizione genetica e squilibri immunologici del tratto digerente che attivano il processo flogistico della parete intestinale. La terapia delle Malattie Infiammatorie Intestinali comprende amino salicilati, cortisonici, immunosoppressori, ciclosporina e agenti biologici, farmaci gravati da una grave tossicità a lungo termine e da fenomeni di resistenza. Dal momento che granulociti e monociti attivati, insieme a citochine proinflammatorie e alla deregolazione dell'attività dei linfociti T regolatori (T®), hanno un ruolo cruciale nell'infiammazione cronica intestinale, l'aferesi selettiva dei monociti e dei granulociti, una tecnica che rimuove i leucociti attivati dal sangue in regime di circolazione extracorporea, potrebbe rappresentare un presidio terapeutico sicuro ed efficace. Vari studi multicentrici sull'efficacia terapeutica della granulocitoaferesi hanno dimostrato che questa rappresenta un'opzione sicura per i pazienti resistenti alla terapia farmacologica oppure un trattamento ben tollerato in associazione con protocolli terapeutici tradizionali, capace di indurre periodi di remissione clinica prolungati e una significativa riduzione dell'assunzione di cortisonici. Ulteriori studi sono necessari per definire meglio la frequenza del trattamento, i volumi ematici da processare, la migliore terapia farmacologica da associare alla granulocitoaferesi e la sua efficacia in altre patologie autoimmuni.

La ricerca si č posta come obiettivo la valutazione dell'efficacia della terapia gruppoanalitica a tempo limitato su un campione di soggetti con DCA afferenti al Programma di trattamento dell'AUSL di Parma. L'indagine, di tipo quantitativo, ha misurato il cambiamento nel tempo di diverse variabili sia individuali (valutazione del funzionamento globale, sintomatologia clinica, autoefficacia percepita, qualitŕ della vita, autostima) a livello relazionale (Carta di rete) e a livello gruppale (questionari clima di gruppo e di gradimento dell'esperienza). Inoltre, si č voluto verificare se una terapia gruppoanalitica a tempo limitato di 6+6 mesi possa produrre dei sostanziali cambiamenti nelle aree sopraelencate per i diversi sottotipi di DCA. I risultati sottolineano come le modificazioni siano piů evidenti per la Bulimia confermando le ipotesi che alla base di questa sindrome vi siano strutture di personalitŕ piů flessibili rispetto all'Anoressia e ai BED. I gruppi a tempo limitato di 6+6 mesi sono parsi uno strumento terapeutico adeguato per una casistica di pazienti con disturbi alimentari cronici solo rispetto alla sintomatologia di facciata ma č troppo breve perché si possa verificare un cambiamento profondo e permanente della personalitŕ e delle modalitŕ relazionali in pazienti con disturbi alimentari cronici.

Abstract:
Background and aim Ulcerative Colitis (UC) and Crohn’s Disease (CD), collectively labelled as inflammatory bowel disease (IBD), are idiopathic, chronic inflammatory disorder of the bowel with a remitting and relapsing course. IBD are associated to poor emotional functioning and psychological distress. We have investigated the brain involvement in patients with IBD using functional magnetic resonance imaging (fMRI). Materials and methods We developed an emotional visual task to investigate the emotional functioning in 10 UC patients and 10 healthy controls (HC). Furthermore, we have compared the brain stress response between a group of 20 CD patients and a group of 18 HC. Finally, we evaluated potential morphological differences between 18 CD patients and 18 HC in a voxel based morphometry (VBM) study. Results We found brain functional changes in UC patients characterized by decreased activity in the amygdala in response to positive emotional stimuli. Moreover, in CD patients, the brain stress response and habituation to stressful stimuli were significantly different in the medial temporal lobe (including the amygdala and hippocampus), the insula and cerebellum. Finally, in CD patients there were morphological abnormalities in the anterior mid cingulated cortex (aMCC). Conclusion IBD are associated to functional and morphological brain abnormalities. The previous intestinal inflammatory activity in IBD patients might have contributed to determine the functional and morphological changes we found. On the other hand, the dysfunctions of the brain structures we found may influence the course of the disease. Our findings might have clinical implications. The differences in the emotional processing may play a role in the development of psychological disorders in UC patients. Furthermore, in CD patients, the different habituation to stress might contribute to stress related inflammatory exacerbations. Finally, the structural changes in the aMCC might be involved in the pain symptoms associated to the bowel disorder.

Abstract:
Questa tesi descrive lo stato delle conoscenze degli effetti tossici da micotossine su animali d allevamento, ed alcuni esperimenti condotti per valutare gli effetti indotti da esposizione cronica da micotossine sui ruminanti. Uno studio è stato condotto su 15 aziende specializzate in sistemi di produzione intensiva di carni bovine, situate nel Nord Italia (province di Verona e Mantova), con l'obiettivo di individuare i rischi d esposizione a contaminazione da micotossine. Alcuni metodi di laboratorio sono stati sviluppati: un metodo per la determinazione di ocratossina A (OTA) accumulata in tessuti e organi; Un metodo per valutare gli effetti delle fumonisine sulla biosintesi delle basi sfingoidi sfingosina (So) e sfinganina (Sa); Un metodo per rilevare l addotto AFB1-albumina. La razione totale mescolata (TMR) è risultata positiva alla AF e FB. Tra i singoli alimenti, il mais e la semola glutinata di mais sono stati i principali responsabili della contaminazione del TMR. Il livello di contaminazione è positivamente correlato al contenuto di umidità di mais. Il metodo per la determinazione dell OTA nei tessuti ed organi ha mostrato un buon recupero medio. L'analisi del rapporto Sa/So nel sangue non ha mostrato alcun effetto negativo delle fumonisine sulla biosintesi lipidica. L'addotto AFB1-albumina è risultato positivo per il 18% dei campioni totali di sangue.
The thesis describes the state of knowledge about toxic effects of mycotoxins on farm animals, and some experiments conducted to assess effects induced by chronic exposure to mycotoxins on ruminants. A field study for was carried out on 15 farms specialised for intensive beef production system, located in Northern Italy (provinces of Verona and Mantova), with the aim to identify risks of exposure to mycotoxins contamination. Some laboratory methods were performed: a method for the detection of ochratoxin A (OTA) concentration in tissues and organs; a method for evaluating the effects of fumonisin on biosynthesis of the two sphingoid bases sfingosine (So) and sphinganine (Sa); a method to detect the AFB1-albumin adduct. Total mixed rations (TMR) resulted positive for AF and FB contamination. Among single feedstuffs, corn and corn gluten feed were the main responsible for TMR contamination. Level of contamination was positively related to corn moisture content. The method for the determination of OTA in tissue and organ showed a good mean recovery. The analysis of ration Sa/So in blood did not show any negative effect by fumonisin on the lipidic biosynthesis. The AFB1-albumin adduct was positive on 18% of total blood samples.

L’aumento dei costi in sanità, l’aumentata prevalenza delle patologie croniche, le disuguaglianze attuali, evidenziano la domanda crescente di una popolazione fragile che richiede una risposta globale ai bisogni della persona nel suo insieme, attraverso la costruzione di un sistema sanitario integrato che ne garantisca una presa in carico efficace. Riuscire a gestire le patologie croniche in modo appropriato è la sfida a cui sono chiamati i professionisti socio-sanitari; ma quali sono gli elementi per riuscirci? Le evidenze scientifiche dimostrano che è fondamentale l’integrazione tra i professionisti e lo sviluppo dei Percorsi Diagnostici Terapeutici Assistenziali (PDTA). In quest’ottica, in Italia e in particolare in Emilia-Romagna e nell’Azienda USL di Bologna si sono succeduti, e ancora si stanno evolvendo, diversi modelli di organizzazione per migliorare la gestione appropriata delle patologie croniche e l’aderenza alle linee guida e/o ai PDTA. Il ruolo del medico di medicina generale (MMG) è ancora fondamentale e il suo contributo integrato a quello degli gli altri professionisti coinvolti sono imprescindibili per una buona gestione e presa in carico del paziente cronico. Per questo motivo, l’Azienda USL di Bologna ha sviluppato e implementato una politica strategica aziendale volta a disegnare i PDTA e incoraggiato la medicina generale a lavorare sempre di più in gruppo, rispetto al modello del singolo medico. Lo studio ha individuato nelle malattie cardiovascolari, che rimangono la causa principale di morte e morbilità, il suo focus prendendo in esame, in particolare,lo scompenso cardiaco e il post-IMA. L’obiettivo è verificare se e quanto il modello organizzativo, le caratteristiche del medico e del paziente influiscono sul buon management delle patologie croniche in esame valutando la buona adesione alla terapia farmacologica raccomandata dalle linee guida e/o PDTA dello scompenso cardiaco e post-IMA.
The rising health care costs, the increasing prevalence of chronic conditions, the existing inequalities in access to care highlight the growing demand for a response to the needs of a fragile population, through the construction of an integrated health system that could ensure an effective provision of care. Managing chronic diseases appropriately is a challenge for health care professionals, but which are the key elements to achieve it? Scientific evidence indicates that the integration between professionals and the development of Integrated Care Pathways are fundamental. In Italy and especially in Emilia Romagna and in Local Health Authority of Bologna, different models of organization have been implemented and are still evolving to improve the management of chronic diseases, the appropriateness and adherence to guidelines and/or pathways. The role of the general practitioner is still vital and his/her contribution together with that of other health care professionals are essential for the appropriate treatment of chronic diseases. . For this reason, the Local Health Authority of Bologna has pursued a policy aimed at developing integrated care pathways and encouraged general practitioners to work together rather than running individual practices. The study has chosen cardiovascular diseases, and specifically heart failure and acute myocardial infarction, as its focus because these diseases are the leading cause of death and morbidity. Introduzione: L’incontinenza urinaria (IU) è ad oggi un problema riconosciuto e riportato in letteratura da vari studi, anche se la sua prevalenza risulta variabile (27-74%). La sua causa principale, nelle pazienti con fibrosi cistica, è la tosse cronica, che porta ad una progressiva debolezza dei muscoli del pavimento pelvico. L’obiettivo della tesi è di valutare la prevalenza dell’IU nelle pazienti con fibrosi cistica seguite al Centro Regionale di Cesena, determinare la presenza o meno di correlazione tra incontinenza ed età, indice di massa corporea e funzionalità polmonare e confrontare i risultati ottenuti con quelli presenti in letteratura, con lo scopo di pianificare la presa in carico riabilitativa. Materiali e metodi: Il questionario ICIQ-UI SF è stato somministrato durante le visite di routine alle pazienti di età maggiore o uguale a 10 anni. Sono inoltre stati raccolti i dati clinici di ciascun paziente: altezza, peso, FVC, FEV1 e MEF 75/25. Risultati: 49 pazienti hanno partecipato allo studio. 20 di queste sono risultate incontinenti e sono significativamente più grandi di età rispetto alle continenti (p=0,01). Esiste inoltre una correlazione tra incontinenza e FEV1 o MEF 75/25, sebbene non sia statisticamente significativa (p=0,06). Non è stata trovata alcuna correlazione tra IU e BMI o FVC. Tutte le donne incontinenti hanno riportato IU da stress; le situazioni che più comunemente causano le perdite di urina sono tosse/starnuti, attività fisica e risate. Conclusioni: L'incontinenza urinaria è una condizione frequente e sottovalutata che colpisce comunemente le donne con fibrosi cistica, sebbene questo problema non sia spesso riportato dalle pazienti al medico o al fisioterapista, forse a causa dell’imbarazzo. È però importante identificare le pazienti con incontinenza perché semplici esercizi per rinforzare i muscoli del pavimento pelvico possono migliorare la situazione. Indagare la presenza dell’IU dovrebbe diventare parte delle visite di routine.

Background: Cardiovascular disease (CVD) is a common cause of morbidity and mortality in childhood chronic kidney disease (CKD). Left ventricular hypertrophy (LVH) is known to be one of the earliest events in CVD development. Left ventricular diastolic function (DF) is thought to be also impaired in children with CKD. Tissue Doppler imaging (TDI) provide an accurate measure of DF and is less load dependent than conventional ECHO. Aim: To evaluate the LV mass and the DF in a population of children with CKD. Methods: 37 patients, median age: 10.4 (3.3-19.8); underlying renal disease: hypo/dysplasia (N=28), nephronophthisis (N=4), Alport (N=2), ARPKD (N=3), were analyzed. Thirty-eight percent of the patients were on stage 1-2 of CKD, 38% on stage 3, 16% on stage 4. Three patients were on dialysis. The most frequent factors related to CVD in CKD have been studied. LVH has been defined as a left ventricular mass index (LVMI) more than 35.7 g/h2,7. Results: Twenty-five patients (81%) had a LVH. LVMI and diastolic function index (E’/A’) were significantly related to the glomerular filtration rate (p<0.003 and p<0.004). Moreover the LVMI was correlated with the phosphorus and the hemoglobin level (p<0.0001 and p<0.004). LVH was present since the first stages of CKD (58% of patients were on stages 1-2). Early-diastolic myocardial velocity was reduced in 73% of our patients. We didn’t find any correlation between LVH and systemic hypertension. Conclusion: ECHO evaluation with TDI is suggested also in children prior to dialysis and with a normal blood pressure. If LVH is diagnosed, a periodic follow-up is necessary with the treatment of the modifiable risk factors (hypertension, disturbances of calcium, phosphorus and PTH, anemia ).

Chronic diseases have been described as “the biggest public health threat society has ever faced” (World Economic Forum, 2010). The forecast from both the World Health Organization and the World Bank is that the morbidity and mortality rate of these diseases will impact severely on low and middle-income countries (WHO, 2005). The burden of disease in South Africa comprises both communicable and non-communicable burden maladies, including high rates of HIV/AIDS and tuberculosis (TB). A high incidence of injury, peri-natal and maternal disorders also contributes to the health burden (Mayosi, Flisher, Lalloo, Sitas, Tollman & Bradshaw, 2009). The country is approaching a demographic transition with an accelerating ageing population. The factors contributing to the ageing of the population differ from those of the rest of the world (Haldenwang, 2009). The Western Cape Province is unique in its position as the province in South Africa with the highest life expectancy, a population change rate that is double the national average, and a higher than average older population (Provincial Government of the Western Cape, Department of Health. 2010. Strategic Plan 2010-2014). An extensive literature survey employing an integral environmental scanning approach was used to determine the impact of ageing on the burden of disease in the Western Cape. The connection between ageing, chronic disease and physical functionality was explored in a non-standardised questionnaire and in the literature survey. An evaluation of the Western Cape Department of Health Strategic Plan 2010-2014 found that non-communicable diseases and mental health issues that constitute about 28 per cent of the burden of disease (WHO, 2008) are not considered in future planning. This study shows that the Western Cape Province has limited time to prepare for the expanding and accelerating ageing population. The analysis of the questionnaire and the literature survey forms the foundation for the building of scenarios to visualise possible health futures in 2030 in the Western Cape. The desirable scenario is a balance between life expectancy and health expectancy.

Sanmao (1943-91), author of over 19 books, is well known in Chinese-speaking communities for her travel writing. The present work offers a critical introduction to Sanmao’s life and work as well as an English translation of three selections from her most recognized travelogue, among both general readers and critics, Stories of the Sahara (1976). This text recounts her experience of travelling in the Western Sahara with her husband José María Quero y Ruíz from Spain. Chapter 1 introduces Sanmao’s career, her travel narratives, and the extant scholarship on her work to the English-speaking audience. More specifically, it highlights her time living in the Western Sahara among three cultures and languages—Chinese, Spanish, and Sahrawi—and contextualizes Stories of the Sahara, especially drawing attention to moments that require special care when the text is moved from Chinese to English. Next, this chapter focuses on the central role that language and translation play in Sanmao’s travel writing. This analysis is informed by Roman Jakobson’s classification of translation as used to study travel literature by Michael Cronin. I provide a discussion of my choices concerning translating the texture of the Western Sahara and the linguistic aspects of Sanmao’s writing, as well as the characteristics of Sanmao’s legacy that I attempt to emphasize through my translation. Chapter 2 includes my English translation of three texts from Stories of the Sahara. A brief introduction and a short conclusion open and close this thesis.

This chapter looks at cronyism as a pillar of Morocco’s political economy and studies its manifestation and impact in the manufacturing sector. Cronyism and patronage have helped the Makhzen to strengthen its control on Moroccan society and prevent any countervailing power from taking root, especially in the economic sphere. After Morocco became independent, the state–business nexus was consolidated through “Moroccanization,” import-substitution strategy, and privatization. The network of cronyism and patronage was reinforced and extended during the liberalization era. This chapter uses a unique database to identify politically connected firms and measure their performance in comparison with non-connected ones. Political privileges are granted to connected firms through preferential access to finance, protection from foreign competition, and discriminatory implementation of rules and regulations. This chapter also provides some illustrative evidence showing that political connectedness tends to impede firms’ dynamics and innovation.

This chapter studies business elites in Morocco to illustrate the importance of the control of the financial sector for the domination of the royal monarchy and its allies. It first highlights the role of banks in the history of Moroccan capitalism, from the setting up of a French protectorate over Morocco to the dominance of some major family groups in a private oligopoly. It then describes the importance of the financial sector in Moroccan cronyism and the growing role it has come to play in Morocco’s recent strategy of exploring African markets. Using a network analysis of board members of Moroccan listed companies, the chapter shows the centrality of finance compared to other key sectors of the economy. It argues that crony interlockers, who are members of royal foundations and represent an institutional investor, or a holding family that owns a bank and/or an insurance company are central actors in the network of listed companies. Finally, the chapter highlights how the financial sector is deployed in the post-Arab Spring strategy of expansion in sub-Saharan markets.

The growth in the popularity of Wales as travel destination in the late eighteenth century is sketched, while the relative ‘invisibility’ of Wales in travel writing as well as in scholarship is noted. ‘Europe’ is presented as a fluid entity, and the ‘nationalities’ of the travellers discussed is problematized (e.g. a number of the French travellers studied identify as Breton, and the notion of ‘Germany’ encompasses numerous states and political alliances over time). Since Wales’s ‘Celticness’ is a major theme for travellers throughout the periods under discussion, the changing uses of the term ‘Celtic’ (and its derivatives) are explored. Wales is positioned as a case study or an exemplar of a particular type of relationship between peripheral and hegemonic culture(s), through a discussion of general theoretical issues surrounding the ethics of travel, the contact zone and the notion of the travellee. This draws on work by Cronin on minorities, Forsdick on ethics, Pratt on the contact zone and travellees, and Urbain on endotic/exotic travel.

This chapter describes in depth how MBS and MBZ have established their control through building highly personalized political patronage networks. Firstly, their creation of an ‘inner circle’ of closest relatives is considered, including most prominently their full brothers and (in MBZ’s case) their adult sons. Secondly, their creation of a parallel circle of non-ruling family technocrats and gatekeepers is discussed, including key friends and cronies who have been entrusted with major political and economic portfolios. Thirdly, the special circumstances surrounded the treatment of the formal heads of state is explained--in MBS’s case his father Salman bin Abdul-Aziz Al-Saud (the King of Saudi Arabia), and in MBZ’s case his older half-brother Khalifa bin Zayed Al-Nahyan (the ruler of Abu Dhabi and President of the UAE). Fourthly, the management of other influential ruling family members is studied, including the co-option or coercion of half-brothers and uncles (and in MBZ’s case senior members of other UAE ruling families). Finally, MBS and MBZ’s supervision of other, lesser ruling family members is reflected upon, including the small minority who have either been actively repressed or have flown into exile.

This paper concerns the application of flatness principle to fractional systems. As soon as the path has been obtained by flatness, a new robust path tracking based on CRONE control is presented. The flatness concept in path planning is used when the trajectory is fixed (in space and in time), to determine the controls to apply without having to integrate any differential equations. A lot of developments have been made but, in the case of non integer differential systems (or fractional systems), few developments are still to be made. So, the aim of this paper is to apply flatness principle to a fractional system and to define a robust path tracking by CRONE control strategy. Firstly, we remind flatness principle definitions used in control’s theory. We study the fractional systems dynamic inversion. A robust path tracking based on CRONE control is presented. Finally, simulations with two different controllers (PID and CRONE) illustrate the path tracking robustness.

In a vibration isolation context, fractional derivative can be used to design suspensions which allow to obtain similar performances in spite of parameters uncertainties. This paper presents the synthesis and the achievement of a new Hydractive CRONE suspension system. After the study of the different constraint in suspension in the first paper, the ideal transfer function of the hydractive CRONE suspension is created and simulated in different case. Then a method to determine the technological parameters is proposed. A parallel arrangement of dissipative and capacitive components and a gamma arrangement are compared. They lead to the same unusual performances: the stability degree robustness and the rapidity robustness.

Shaping command input or preshaping is used for reducing system oscillation in motion control. Desired systems inputs are altered so that the system finishes the requested move without residual oscillation. This technique, developed by N.C. Singer and W.P. Seering, is used for example in the aerospace field, in particular in flexible structure control. This paper presents the study of ZV shaper for explicit fractional derivative systems (generalized derivative systems). A robustness study of ZV shaper is then presented and applied to improve second generation CRONE control response time. Results in simulation and on a DC motor bench are given.

Not only is Abe Shinzō on the way to becoming Japan’s longest-serving Prime Minister in the country’s history. With more than 1 million followers on Twitter and slightly less than 600 hundred thousand fans on Facebook, he is by far the most successful Japanese political leader on social media. Commentators have described Abe’s turn to social networking services (SNS) as a “revenge” against “traditional” media against the background of a growing use of SNSs by other major Japanese political actors. At any rate, particularly through Facebook, combining text and pictures of himself on and off duty, Abe has successfully established his own mode to communicate with and “exhibit” himself to voters, citizens and the global community of netizens. This paper aims to address the following research question: on which themes and key concepts is this “presentation of the self” based? In other words, how is the Prime Minister communication staff constructing Abe’s “social” image and to which audience is this aimed? Based on Goffman’s theorization and later application of his work on the study of online social interactions, this paper illustrates the strive to ensure the consistency of Abe’s use of the SNS with previously expressed concepts and ideas (e.g., in the 2006 book “A Beautiful Country”), with the aim of pleasing the “bubble” of like-minded individuals constituting Abe’s (online) support base, and avoid issues that might possibly harm the Prime Minister’s reputation. Abe’s Facebook activity (a combination of text and pictures) during a critical time in his second tenure (2017), in which he faced cronyism allegations while coping with gaffes and scandals involving cabinet members, provided a case in point for multimedia content analysis.

A state equation can be called the foundation of modern control theory. However, the relationship between fractional derivatives and the state equation has not been examined sufficiently. Consequently, a systematic procedure referred to by every researcher on the fractional-calculus side or control-theory side has not yet been established. For this study, therefore, involvement of fractional-order derivatives into a state equation is demonstrated here for ready comprehension by researchers. First, the procedures are explained generally; then the technique to incorporate the fractional-order state-vector into a conventional state equation is given as an example of the applications. The state-space representation in this study is useful not only for modeling a controlled system with fractional dynamics, but also for design and implementation of a controller to control fractional-order states. After we complete installation of the basic parts, we can apply the benefits of modern control theory, including robust control theories such as H-infinity and μ-analysis and synthesis in their integrities, to this fractional-order state-equation.

Abstract:
The “Institut de Radioprotection et de Suˆrete´ Nucle´aire”, as the technical support of the French Safety Authority, carries out studies and research to analyze and assess the safety of all nuclear plants. In this frame IRSN studies the feasibility of modeling Material Testing Reactor core with SIMMER-III code, for simulation of reactivity initiated accidental transients. The SIMMER-III multi-physics code system was initially developed for mechanistic safety analyses of liquid metal cooled fast reactors while employing coupled spatial neutron kinetics and thermal hydraulics models. Neutronics and thermal-hydraulics SIMMER-III models have been extended to safety analyses for water cooled and moderated reactors. The use of a code like SIMMER-III requires approximations; it computes a simplified R-Z geometry and chemistry description of the core that must be validated. The methods applied consist here in developing models of the same reactor on several scales of detail. The first step is the validation of the cross section condensation for deterministic APOLLO2 calculation against Monte Carlo TRIPOLI4 2D model. Temperature effects, kinetic parameters and void coefficients on the whole core are then calculated on a 2D APOLLO2 model, using the Method of Characteristics. These parameters are also computed with a 3D combined transport and diffusion calculations by means of APOLLO2/CRONOS2 calculations, validated against a TRIPOLI4 3D precise reference model. The final step is the validation of the simmer-like R-Z geometry in APOLLO2 Sn and Pij. Finally, an R-Z geometry has been computed in SIMMER-III, for the calculation of the kinetic parameters and temperature coefficients. This validation method has been applied to Jules Horowitz Reactor, a French Material Testing Reactor currently in commissioning by the CEA. This leads to conclude that discrepancies due to simplifications are acceptable. Moreover SIMMER-III shows quite a good agreement with CEA ring calculation on the kinetic parameters. Concerning neutronics feedbacks coefficients, further analyses remain necessary.
