Waste transformation products find practical application in agriculture, industry and medicine. Abstract:
Calcium pyrophosphate (CPP) dihydrate crystals form extracellularly. Their formation requires sufficient extracellular inorganic pyrophosphate (ePPi), calcium, and pro-nucleating factors. As inorganic pyrophosphate (PPi) cannot cross cell membranes passively due to its large size, ePPi results either from hydrolysis of extracellular ATP by the enzyme ectonucleotide pyrophosphatase/phosphodiesterase 1 (also known as plasma cell membrane glycoprotein 1) or from the transcellular transport of PPi by ANKH. ePPi is hydrolyzed to phosphate (Pi) by tissue non-specific alkaline phosphatase. The level of extracellular PPi and Pi is tightly regulated by several interlinked feedback mechanisms and growth factors. The relative concentration of Pi and PPi determines whether CPP or hydroxyapatite crystal is formed, with low Pi/PPi ratio resulting in CPP crystal formation, while a high Pi/PPi ratio promotes basic calcium phosphate crystal formation. CPP crystals are deposited in the cartilage matrix (preferentially in the middle layer) or in areas of chondroid metaplasia. Hypertrophic chondrocytes and specific cartilage matrix changes (e.g. high levels of dermatan sulfate and S-100 protein) are related to CPP crystal deposition and growth. CPP crystals cause inflammation by engaging with the NALP3 inflammasome, and with other components of the innate immune system, and is marked with a prolonged neutrophilic inflitrate. The pathogenesis of resolution of CPP crystal-induced inflammation is not well understood.

Abstract:
The cortical silent period (cSP) refers to an interruption of voluntary muscle contraction by transcranial stimulation of the contralateral motor cortex. This article summarizes the physiology of cSP and gives guidelines as to how the cSP should be recorded and analysed, and refers to useful clinical applications. It describes the mechanisms at the spinal and supraspinal level can account for the cSP. It is currently thought that the cSP is mediated by gamma-aminobutyric acid receptors. cSP measurements are easy to obtain but require a standardized protocol to allow useful interpretation. Conditioning electrical stimulation of cutaneous nerves shortens the cSP. A conditioning transcranial magnetic stimulation pulse also affects cSP duration. The cSP duration is influenced by pathological conditions. cSP is an attractive probe to assess motor cortical inhibition in health and disease.

This chapter provides a broad descriptive background of central aspects of smallholder agriculture in six countries in sub-Saharan Africa (SSA). It offers an up-to-date picture of the current trends of crop production, area productivity, levels of commercialization, and sources of cash incomes among 2,500 farming households. Structured around smallholder production, commercialization, and diversification in the period 2002–15, the chapter points on the one hand at persistent challenges such as low crop yields, low levels of output per farm, and a high degree of subsistence farming, and on the other hand at positive change over time in terms of growth in crop production and increasing levels of commercialization. It points at large variations not only between countries and time periods but also at the village levels, where gaps in crop productivity between farms remain large. Implicitly it points at the potential yet to be exploited in the SSA smallholder sector.

This chapter provides an expository account of Indian appellate courts’ engagement with the Convention on the Rights of Persons with Disabilities (CRPD) and the developing case law on disability rights. As a dualist State, India has ratified but not incorporated the CRPD into its domestic law. This has not deterred frequent references to the CRPD in litigation at the highest level. The appellate courts—High Courts and the Supreme Court—have resorted to the CRPD in diverse ways. The analysis of the small but not insignificant body of case law shows that these instances can be classified into two broad themes of ‘citation’ and ‘interpretation’. In the final analysis, the overall impact of references to the CRPD can be considered largely positive but still modest in the absence of new legislation embracing the human rights framework and social model of the CRPD in India.

The Convention on the Rights of Persons with Disabilities (CRPD) became part of the Spanish legal system on 3 May 2008, being placed at the highest normative level. Accordingly, since then the CRPD has been directly applicable by different Spanish courts, which have to interpret fundamental rights in line with CRPD. Nevertheless, the application of CRPD by different Spanish courts has not followed a smooth path. This chapter examines how the CRPD has been used by different Spanish courts, from the lower courts to the Supreme Court and the Constitutional Court, focusing on the rights that have been most frequently referred to the courts: the rights to equality and non-discrimination; equal treatment as a person before the law; access to justice; personal liberty; honour, reputation and privacy; to education; an adequate standard of living; and participation in political and public life.

Central venous pressure (CVP) is at the crucial intersection of the force returning blood to the heart and the force produced by cardiac function, which drives the blood back to the systemic circulation. The normal range of CVP is small so that before using it one must ensure proper measurement, specifically the reference level. A useful approach to hypotension is to first determine if arterial pressure is low because of a decrease in vascular resistance or a decrease in cardiac output. This is done by either measuring cardiac output or making a clinical assessment blood flow. If the cardiac output is decreased, next determine whether this is because of a cardiac pump problem or a return problem. It is at this stage that the CVP is most helpful for these options can be separated by considering the actual CVP or even better, how it changed with the change in cardiac output. A high CVP is indicative of a primary pump problem, and a low CVP and return problem. Understanding the factors that determine CVP magnitude, mechanisms that produce the components of the CVP wave form and changes in CVP with respiratory efforts can also provide useful clinical information. In many patients, CVP can be estimated on physical exam.

Worldwide, colorectal cancer (CRC) is the third most common cancer in men and second in women, with annual estimates of 1.4 million newly diagnosed cases and over 690,000 deaths. Incidence rates relate closely to economic development. Although incidence rates have stabilized at a high level in most economically developed countries, they continue to increase in many traditionally low-risk countries, following the uptake of Western patterns of diet and physical inactivity. In principle, CRC is among the most preventable of all common cancers. Potentially modifiable risk factors include obesity, physical inactivity, high intake of red or processed meat, tobacco smoking, and heavy alcohol use. Several screening tests effectively reduce both the incidence and death rates of CRC through the detection of precancerous lesions and the treatment of early stage cancers. Despite the preventability of CRC, incidence rates over the last twenty years have decreased in only a few countries.

The UN Convention on the Rights of Persons with Disabilities (CRPD) has been in force for nearly a decade. This book examines how the Convention has been given effect and interpreted in thirteen different jurisdictions. It has two main interconnected aims. The first is to investigate and compare the way in which the CRPD has been interpreted and applied by courts in different jurisdictions. The second is to investigate and deepen understanding of the CRPD’s influence at the domestic level. The first of these aims situates this study within the emerging field of comparative international law—to which it offers the first major contribution addressing an international human rights treaty other than the UN Convention on the Elimination of all Forms of Discrimination Against Women (CEDAW). The second situates it within the field of disability law—to which it offers the first sustained analysis of how the CRPD influences domestic court judgments. Besides the thirteen jurisdiction-specific chapters (written by experts in both the CRPD and the particular jurisdiction in question), comparative analysis is provided in four chapters—addressing respectively the interpretation of CRPD provisions by domestic courts; the legal status of the CRPD in domestic law and its relevance to domestic case law; the uses made of the CRPD by domestic courts; and the judiciary’s role and perception of its relationship with the CRPD. The book also includes reflections on the implications of this study, and previous comparative international law studies of CEDAW, for human rights theory.

Castration-resistant prostate cancer (CRPC) represents a final stage of this malignancy for many men and is defined as the progression of prostate cancer despite castrate levels of testosterone. CRPC may present as a rising PSA, the development of new metastases, or worsening of known metastases. Recent advances have resulted in five new treatments for CRPC: the immunotherapy sipuleucel-T; the cytotoxic cabazitaxel; the androgen biosynthesis inhibitor abiraterone acetate; the radioisotope radium-223; and the antiandrogen enzalutamide. These have all improved overall survival in randomized phase III studies for patients with metastatic CRPC. Furthermore, multiple agents and combinations are currently in late-stage clinical testing. Men with advanced prostate cancer represent an important population for clinical and translational research and clinical trial participation should be considered as part of standard care.

Examples of this include U.S. savings rates at historic lows, poor retirement prospects for citizens in developed countries, and the disparaging gap between investor returns and market returns. Research indicates that individuals working with a financial advisor generally receive better results than those who do not. It is now recognized that cardiovascular disease (CVD) is partly a paediatric problem, i.e. the onset begins in childhood, although clinical symptoms may not become apparent until later in life. Therefore, from a primary prevention point of view, the extent to which physical activity or physical fitness in childhood may deter this process is of utmost importance. Although physical activity and CRF at a young age have not been directly linked to the incidence of CVD, evidence thus far supports cardiovascular health benefits of early higher physical activity and CRF levels on cardiometabolic risk factors like obesity, blood pressure, insulin resistance, and their maintenance throughout the course of life. By affecting these intermediary pathways, lifelong (high-intensity) physical activity may also deter the age-related decreases in CRF and related signs of premature arterial ageing.

The EU Capital Requirement Regulation (CRR) [8] and of EBA Regulatory Technical Standard for prudent valuation [15], published on Jan. 1st, 2014 and Jan. 28th, 2016, respectively, constitute the EU Prudent Valuation Framework. The CRR, art. 34, requires to Institutions a prudent valuation of positions measured at fair value and the deduction of the resulting Additional Valuation Adjustments (AVAs) from the Common Equity Tier One (CET1) capital. The art. 105 disciplines the AVAs intended to achieve an appropriate level of certainty in prudent value. The EBA RTS [15] allow two approaches to prudent valuation. The simplified approach, applicable by small financial institutions (with total absolute fair-valued assets and liabilities below EUR 15 billions), prescribes a very simple total AVA equal to 0.1% of the total fair value. The core approach, compulsory for institutions above the EUR 15 billion threshold, prescribes the calculation of 9 AVAs, referring to different sources of valuation uncertainty, as the excess of valuation adjustments required to achieve the prudent value with 90% level of confidence. Five out of nine AVAs include a 50% weight to take diversification into account and avoid double counting effects. Those positions for which a change in their fair value affects only partially the CET1 may be partially excluded from the AVAs calculation.

Abstract:
Community-acquired pneumonia (CAP) is defined as an acute infection of the pulmonary parenchyma in someone who has not recently had close contact with the health care system. Common presentations include abrupt fever and chills, productive cough, purulent sputum, dyspnea, pleuritic chest pain, and the absence of rhinorrhea and sore throat. Prompt antibiotics and admission to the correct level of care are essential in emergency management. Chest radiographs and dry computed tomography (CT) scans can be extremely useful in confirming the diagnosis of CAP in immunocompetent patients. Assessment of the airway, breathing, and circulation is essential. Adequate fluid resuscitation, early appropriate antibiotics, and careful attention to monitoring are still mainstays of treatment. Clinical decision tools like CURB-65 and PORT score can help identify low-risk patients when making decisions about whether or not to admit.
