Abstract This article reviews the common Current Procedural Terminology (CPT) codes for frequently performed breast imaging procedures. The CPT codes for breast radiology services, including the revised codes for mammography, computer-assisted detection (CAD), breast ultrasound, breast magnetic resonance imaging (MRI), and imaging-guided breast interventional procedures and the new codes for digital breast tomosynthesis (DBT) and marker placement in axillary lymph nodes are outlined. The application of basic CPT coding principles, such as the use of modifiers and the different structure of code families in breast radiology services, is also discussed. CPT codes are updated on an ongoing basis to reflect the evolution of medical practice, and the CPT codes for breast imaging procedures have changed significantly in the past 5 years. These modifications can affect the distribution of work and utilization of resources in radiology departments. A working knowledge of coding practices will facilitate compliance with billing regulations and help ensure breast radiologists are appropriately paid for their work.

Abstract Using whole-exome sequencing to examine the genetic causes of immune deficiency in 235 common variable immunodeficiency (CVID) patients seen in the United States (Mount Sinai, New York), 128 patients from Sweden, and 208 from Iran revealed 68 known disease-causing genes underlying this heterogeneous immune defect. The patients at the time of study ranged from 4 to 90 years of age. Overall, 31%, 36%, and 54% of the patients in the US, Swedish, or Iranian cohorts had mutations. The multiplicity of genes identified in the 571 subjects reflects the complex requirements of B-cell antigen signaling, activation, survival, migration, maturation, and maintenance of antibody-secreting memory B-cell populations to the plasma cell stage. For the US and Swedish cohorts, CVID subjects with noninfectious complications, lymphoid infiltrations, inflamatory conditions, or autoimmunity were somewhat more likely to have an identifiable gene, but in both cohorts, numerous subjects with these medical conditions had no potential gene that could be assigned. Specific clinical patterns of illnesses were also not linked to any given gene defect as there was considerable overlap in clinical presentations. These observations led to a new perspective on the complexity of the immunologic phenotype found in CVID syndrome.

This paper presents the mixed-mode effect on motor common mode current (CMC) in an adjustable speed drive (ASD). It is expected that the motor CMC is lower than inverter output CMC. However, the measurement result of the ASD shows that the AC motor input CMC is higher than the inverter output CMC in a frequency range. By using a mixed-mode (MM) 6-port networks, it is proven that the AC motor input CMC is higher due to an MM CMC which is generated from differential mode current.

In railway tractive vehicles, three-phase PWM (Pulse Width Modulation) inverters generate parasitic Differential-Mode Voltages (DMV) and Common-Mode Voltages (CMV). Parasitic voltages are a side effect of using the width modulation to shape the phase-to-phase inverter’s voltage. In this article, the authors present a mathematical description of the DM and CM voltages and carry out their spectral analysis. Based on the spectral harmonics analysis, the authors present a method for filtration of harmonics of DM and CM voltages aimed at limiting the capacitance parasitic currents: due to DM voltage – phase-to-phase parasitic current and CM voltage – ground parasitic currents. As the final result of the tests, almost complete elimination of leakage parasitic current form PE shock protection system was achieved.

When induction motor is directly driven by PWM power converter, high-frequency common-mode current will be generated inside the motor due to the high-frequency common-mode voltage load. So in this paper, on the basis of the analysis of the coupling effect of the parasitic elements on high-frequency current which include elements between induction motor windings, between the winding and the stator and between the winding and the rotor, According to the structural characteristics of the motor, it is obtained that the parasitic element between the first turns of the stator winding coil and ground can provide low impedance flow path for high frequency common mode current, most of the high-frequency common mode current drain into ground through this path, and the motor stator winding has the uniform transmission line characteristics. The correctness of the conclusions is verified by the experimental results of two different induction motor.

Abstract Bioimpedance measurement systems often use the Howland current sources to excite the biological material under study. Usually, difference or instrumentation amplifiers are used to measure the resulting voltage drop on this material. In these circuits, common mode voltage appears as artifacts in the measurement. Most researches on current sources are focused on improving the output impedance, letting other characteristics aside. In this paper, it is made a brief review on the load common mode voltage and output swing of various topologies of Howland current sources. Three circuits are proposed to reduce load common mode voltage and enhance load capability by using a fully differential amplifier as active component. These circuits are equated, simulated and implemented. The three proposed circuits were able to deliver an output current with cut-off frequency (-3dB) higher than 1 MHz for loads as big as 4.7 kΩ. The worst measured load common mode voltage was smaller than 24 mV for one of the circuits and smaller than 8 mV for the other two. Consequently, it could be obtained increases in the Common Mode Rejection Ratio (CMRR) up to 60 dB when compared to the Enhanced Howland Current Source (EHCS).

Understanding the rainfall-runoff behaviour of urban land surfaces is an important scientific and practical issue as storm water management policies increasingly aim to manage flood risk at local scales within urban areas, whilst controlling the quality and quantity of runoff that reaches receiving water bodies. By reviewing field measurements reported within the literature on runoff, infiltration, evaporation and storage on common urban surfaces, this study describes a complex hydrological behaviour with greater rates of infiltration than often assumed, contradicting a commonly adopted, but simplified classification of the hydrological properties of urban surfaces. This shows that the term impervious surface, or impermeable surface, referring to all constructed surfaces (e.g. roads, roofs, footpaths, etc.) is inaccurate and potentially misleading. The hydrological character of urban surfaces is not stable through time, with both short (seasonal) and long term (decadal) changes in hydrological behaviour, as surfaces respond to variations in seasonal characteristics and degradation in surface condition. At present these changing factors are not widely incorporated into hydrological modelling or urban surface water management planning, with static values describing runoff and assumptions of imperviousness often used. Developing a greater understanding of the linkages between urban surfaces and hydrological behaviour will improve the representation of diverse urban landscapes within hydrological models.

The operation and development of the economies of the EAEU countries is subject to the influence of external economic factors, while the single currency will eliminate the risks associated with the restriction of the use of the dollar due to sanctions and will reduce transaction costs. The article offers an approach for the formation of a single internal currency within the EAEU that is based on the following key provisions: firstly, in modern conditions, the role of digital financial assets (and, consequently, cryptocurrency) is gaining importance in economic regional interaction; secondly, the Russian Federation and the Republic of Kazakhstan have not fully legitimized digital financial assets, but partially recognize them, while preserving the resource-specific nature of the financial and economic sphere; thirdly, the global trend for the demand for an alternative type of money should be considered. The proposed approach is addressed to small regional economic and political associations, which makes it possible to fairly quickly consider current and future changes in pricing of one unit of currency. In addition, the proposed approach can be scaled based on the growing influence of the previous and new members of the regional economic association.

Screening is the application of a diagnostic test to a population of patients without suspicious clinical signs or symptoms of a disease. Applying screening protocols to detect cancer is especially attractive as cancer is a growing burden and earlier detection of malignancies can lead to better outcomes. While the initial excitement over screening for cancers has caused protocols to be put into practice, data analysis has cast doubt on the net benefit. Prostate-specific antigen (PSA) blood testing has been used as a screening test for prostate cancer. The harms of a positive PSA screen include invasive investigations and treatments with side effects such as infection and incontinence. The poor sensitivity and specificity of PSA screening caused the Canadian Task Force on Preventive Health Care to recommend against PSA as a screening tool. Similarly, mammography and ultrasound for detecting breast cancer lesions in asymptomatic women suffer from relatively low sensitivity and specificity. The harms of screening include emotional stress and invasive surgical procedures. Meta-analyses reveal that the risks outweigh the benefits for some age groups, but there may be a role in screening asymptomatic patients with a family history of breast cancer with more sensitive but more expensive modalities, such as magnetic resonance imaging (MRI). Finally, colonoscopy, which is the gold standard to diagnose colorectal cancers, has failed to show net benefit in screening. While screening protocols thus far have provided underwhelming results, the key in screening in the future may lie in using tests with high specificity and sensitivity, as in next generation screening modalities that rely on molecular markers and analytics.
