Abstract:
Introduction. Along with an increasing number of cancer patients, the need for cytostatic drugs is also increasing. Nursing staff are the largest professional group exposed to the potential dangers of these substances. Aim of the study. Assess the awareness of nursing staff who have direct contact with cytostatic drugs in the use of personal protective equipment (PPE). Material and methods. The research group consisted of 101 nurses routinely exposed to cytostatic drugs. A diagnostic survey and questionnaire technique were used along with the author’s original questionnaire. Results. Of the respondents, 58.42% (n=59) never used protective shoes while dealing with cytostatics, while 53.4% (n=54) never used long-sleeved, waterproof uniforms; 49.50% (n=50) did not apply half masks, and 34.65% (n=35) failed to protect their eyes with protective glasses. The most common cause of not using the protective equipment was identified as lack of time (72; 71.29%). Deficiency of training on protective measures while working with hazardous cytostatics was cited by 37.62% (n=38) as the reason for their behavior, while almost 22% of them claimed that their employer did not provide them with a sufficient amount of protective equipment for individual use. The older, more experienced and higher-educated the staff, the higher awareness among them about the need for using PPE. Conclusions. Higher-educated and more experienced nursing staff should constitute the source of ‘good practices’ and educate younger undergraduate colleagues theoretically and practically. Employers and management staff should provide employees with more training on the correct application of protective measures and increase the intensity of control of the use of personal protective equipment.

Abstract:
Increased oxidative stress and indoleamine-2,3-dioxygenase (IDO) activity have been reported in cancer, but their relationship with chemotherapy remains unknown. The aim of the present study was to examine wether the chemotherapy treatments used in colorectal cancer had an additional effect on oxidative stress and on IDO activity. Plasma samples were collected from 27 colorectal cancer patients on cytostatic treatment, 27 with cytostatic drugs plus monoclonal antibodies (cytostatic-Mabs) and 15 non-treated patients. All patients with colorectal cancer had high plasma malondialdehyde (MDA), thioredoxin (Trx) levels, and elevated IDO activity in plasma (IDOp) and in dendritic cells (IDOc). This study shows that treatment with cytostatics have an effect on oxidative stress by increasing MDA levels and by decreasing Trx levels and IDO activity. However, treatment with cytostatic-Mabs showed no effect on MDA levels but decreased Trx levels, and the IDO activity showed values similar to the healthy group. Significant correlations between plasma IDO activity and the levels of Trx (r = 0.2062, p < 0.05) and MDA (r = 0.2873, p < 0.005) were observed. Furthermore, our study suggests that IDO activity measured as kynurenine levels could be used as a marker of the response to the chemotherapy treatments, although further studies are necessary.

Abstract Hyperthermic intraperitoneal chemotherapy (HIPEC)—heated, intra-abdominal chemotherapy—has become the treatment of choice for treating peritoneal metastases from ovarian, stomach or colorectal cancers. HIPEC has several advantages and disadvantages. The major benefit is minimal systemic toxicity for the patient, but there is a risk of occupational exposure for operating room staff. We have not found any reports of workers with chronic aplastic anaemia as a result of exposure to cytostatic fumes during HIPEC. The aim of this case report is to raise the awareness of potential negative health effects of inhalation exposure to cytostatic drugs. We present a rare case of a 43-year-old woman, suffering from aplastic anaemia as a long-term consequence of exposure to cytostatics. During the HIPEC procedure, surgical revision of the peritoneal cavity was undertaken which resulted in release of cytostatic fumes. Despite awareness of the health effects of occupational exposure to cytostatic drugs and well-developed procedures for safely handling them, unexpected exposure may occur causing serious medical conditions. These may develop in sensitive subjects although accidental high-level exposure may lead to unexpected long-term consequences in all workers. Medical staff need to be informed of the risks of HIPEC and safety guidelines to reduce the risk of exposure.

Abstract:
A retrospective study on Acute Non Lymphoblastic Leukemia (ANLL) was conducted at the Sub Division of Pediatric Hematology, School of Medicine, University of North Sumatera/Dr. Pirngadi Hospital Medan, in a period of 5 years (1983-1988). There were 18 cases consisted of 14 (77. 78%) males and 4 (22 .22%) females with the age group of 0-2 years: 6 (30%), 2-8 years: 9 (50%), 8-15 years: 3 (30%). By the FAB classification, they were of FAB M-1: 1 (5.55%), FAB M-2: 1 (5.55%), FAB M-3: 1 (5.55%), FAB M-4: 2 (11.12%) and FAB M-6: 13 (72.23%). Only 7 (38.88%) were treated with cytostatics while the others received only supportive therapy. The result of cytostatic treatment was unsatisfactory: 4 (57.14%) died within the first 2 months of treatment, 3 (42.86%) discontinued their cytostatics treatment.

Abstract:
— In this study, we’ve evaluated the myelotoxic effect of treating Walker-256 carcinoma with cytostatics, followed by correction of thrombocytopenia with liposomal and liposome-free mexidol. The study included 60 rats, which, depending on the type of treatment, were divided into 5 groups. We recorded the greatest increase in the number of platelets on the background of liposomal mexidol both on the 3rd and 7th day after the start of chemotherapy. At the end of the monitoring, the myeloprotective effect was 30% higher in the liposomal mexidol compared to its non-liposomal form. Conclusion: Activation of thrombocytopoiesis on the background of cytostatic therapy helps to reduce the complications related to the use of chemotherapy.

Abstract:
Metallothioneins (MTs) are small cysteine-rich proteins involved in a number of pathophysiological processes. Particularly their linkage to cancer processes has been vastly studied and it is well known that MTs can inactivate metal-based cytostatics or scavenge free radicals. These processes result in pronounced chemoresistance and a poor prognosis for patients. Despite this knowledge, involvement of specific (sub)isoforms into this phenomenon requires further elucidation. Our results identified CisPt as the cytostatic which provoked the highest cell death exp followed by CarboPt and OxaliPt. Fluorescence microscopy visualized the oxidative cell stress. After application of 24hIC50 values, the reactive oxygen species (ROS) were visually produced. Our results also showed that MT 1, 2 and 3 expression manifested the highest qPCR activity after CisPt treatment (both healthy and cancer cells). While evaluating the expression of the protein level in the healthy and cancer cells treated by the cytotoxics we concluded that for MT1 and MT3 it was low under all three cytostatic treatments. Cancer cells had higher protein expression levels than healthy cells. In case of MT1/2 for cancer cells was highest under CisPt treatment.

Abstract One main problem of tumor therapy is the resistance of malignant cells to cytostatics due to high expression of efflux transporters. Whereas the role of these efflux transporters for tumor cell resistance is well established, little is known about uptake transporters, which may increase the sensibility of tumor cells for cytostatics. In the present study we addressed the interaction of cytostatics established for the treatment of lymphoma, namely melphalan, chlorambucil or bendamustine with human Organic Anion Transporter (OATs), which belong to the Solute carrier (SLC) gene family. We selected these cytostatics, because they show structural similarity to p-aminohyppurate (PAH), the model substrate of OATs. OATs are mainly expressed in the kidney, where they are responsible for the excretion of endogenous and exogenous organic anions like urate or various drugs e.g. diuretics. Initially, we examined the cis-inhibitory effect of melphalan, chlorambucil and bendamustine on OAT1-mediated [3H]PAH uptake as well as OAT3- and OAT4- mediated [3H]estrone sulfate uptake in HEK293 cells, which were stably transfected with these transporters. Melphalan did not show any significant inhibitory effect on all tested OATs. 100 μM chlorambucil reduced OAT1-, OAT3- and OAT4-mediated uptake of PAH or estrone sulfate down to 14.6 ± 0.17%, 16.3 ± 4.0% and 66.0 ± 1.4%, respectively. 100 μM bendamustine inhibited only OAT3-mediated estrone sulfate uptake up to 91.9 ± 0.5% compared to control cells. OAT1- or OAT4- facilitated transport of PAH and estrone sulfate remained unchanged by bendamustine, suggesting that bendamustine interacts exclusively with OAT3. To determine the affinity of OAT3 for bendamustine and chlorambucil, we performed concentration dependent inhibition of OAT3-mediated estrone sulfate uptake and calculated the Ki values for both cytostatics. Dixon-Plot evaluation confirmed a competitive inhibition of OAT3 by bendamustine as well as chlorambucil. The results demonstrated higher affinity of OAT3 for bendamustine with a Ki value of 2.7 μM than for chlorambucil, showing a Ki value of 38.2 μM. To elucidate the expression of OATs in lymphoma cell lines, we performed RT-PCR experiments. Our data demonstrate high expression of OAT3 in all cell lines compared to lymphocytes isolated from a normal person. No expression of OAT1 and OAT4 was observed any lymphoma cell lines. The expression of OAT3 in B-cell lymphoma cell lines Karpas, Raji, SudHL4 and T-cell lymphoma cell lines L428, Jurkat and Hut78 was quantified by real time PCR. The highest expression of OAT3 was observed in the order Jurkat>Hut78>SudHL4>L428>Raji>Karpas. The expression of OAT3 was confirmed by real time PCR in four patients with chronic lymphocytic leukamia. OAT3- dependent cytostatic effects of bendamustine was examined by [3H] thymidine incorporation. 30 min incubation of OAT3-expressing HEK293 cells with 10, 50 or 100 μM bendamustine significantly reduced the proliferation of transfected versus non-transfected cells. We conclude that the molecular background for the cytostatic efficiency of bendamustine in lymphoma cells is due to 1) the expression of OAT3 in lymphoma cells and 2) a the high affinity of OAT3 for bendamustine.

A cutaneous melanoma mouse model was used to test the efficacy of a new therapeutical approach that uses low doses of cytostatics in conjunction with mild whole body microwave exposure of 2.45 GHz in order to enhance cytostatics antitumoral effect.Materials and Methods.A microwave exposure system for C57BL/6 mouse whole body microwave irradiation was designed; groups of 40 mice (males and females) bearing experimental tumours were subjected to a combined therapy comprising low doses of dacarbazine in combination with mild whole body irradiation. Clinical parameters and serum cytokine testing using xMAP technology were performed.Results.The group that was subjected to combined therapy, microwave and cytostatic, had the best clinical evolution in terms of overall survival, tumour volume, and metastatic potential. At day 14 the untreated group had 100% mortality, while in the combined therapy group 40% of mice were surviving. Quantifying serum IL-1β, IL-6, IL-10, IL-12 (p70), IFN-γ, GM-CSF, TNF-α, MIP-1α, MCP-1, and KC during tumorigenesis and therapy found that the combined experimental therapy decreases all the inflammatory cytokines, except chemokine MCP-1 that was found increased, suggesting an increase of the anti-tumoral immune response triggered by the combined therapy. The overall metastatic process is decreased in the combined therapy group.

Cytostatics are toxic pharmaceuticals, whose presence in surfaces puts healthcare workers at risk. These drugs might also end up in hospital effluents (HWW), potentially damaging aquatic ecosystems. Bicalutamide is a cytostatic extensively consumed worldwide, but few analytical methods exist for its quantification and most of them require advanced techniques, such as liquid chromatography mass spectrometry (LC-MS), which are very complex and expensive for large monitoring studies. Therefore, a simple but reliable multi-matrix high performance liquid chromatographic method, with fluorescence detection, was developed and validated to rapidly screen abnormal concentrations of bicalutamide in HWW and relevant contamination levels of bicalutamide in indoor surfaces (>100 pg/cm2), prior to confirmation by LC-MS. The method presents good linearity and relatively low method detection limits (HWW: 0.14 ng/mL; surfaces: 0.28 pg/cm2). Global uncertainty was below 20% for concentrations higher than 25 ng/mL (HWW) and 50 pg/cm2 (surfaces); global uncertainty was little affected by the matrix. Therefore, a multi-matrix assessment could be achieved with this method, thus contributing to a holistic quantification of bicalutamide along the cytostatic circuit. Bicalutamide was not detected in any of the grab samples from a Portuguese hospital, but an enlarged sampling is required to conclude about its occurrence and exposure risks.

Abstract:
Working group for safe handling with cytostatic drugs has been formed by the Ministry of Health, and it consists of professionals from IORS, Federal Bureau of Weights and Measures, Industrial Medicine, Institute of Hematology, Military Medical Academy, and Crown Agents. The aim of this working group is to prepare procedures for safe handling with cytostatic drugs, as well as program for educational seminar for nurses, medical technicians, and pharmaceutical technicians. The procedures will serve as a guide of good practice of oncology health care, and will refer to all actions that health care professionals carry out from the moment of drugs arrival to the pharmacy to the moment of their application. In the first segment of this procedure, general rules are given for working with cytotoxic agents, control for risky exposures, safe system of work, control of working environment, monitoring of the employees' health condition adequate protection in the working environment, protective equipment of the employees (gloves, mask, cap, eyeglasses, shoe covers, coats and chambers for vertical laminary air stream). Storing of cytostatics, procedure in case of accident, and waste handling and removal are also described in this segment. Fifty-three standard operational procedures are described in detail in the second segment. Training scheme for preparation of chemotherapy is given in the third segment - education related to various fields and practical part, which would be carried out through workshops, and at the end of the course participants would pass a test and obtain certificate. After the procedures for safe handling with cytostatics are legally regulated employer will have to provide minimum of protective equipment, special rooms for the drugs dissolving, chambers with laminar airflow, 6 hours working time, rotation of the staff working with drugs dissolving in intervals of every five years, higher efficiency, better health control. In conclusion this specific field of work requires great psychological and physical efforts, and know-how. When taking care of ourselves, we are taking care of the others, as well.

Abstract:
Trois approches synthétiques de la cytostatine, produit naturel inhibiteur de la protéine-phosphatase 2A et possédant des propriétés anti-tumorales, ont été étudiées. Les deux premières stratégies, reposant sur la formation de la liaison C7-C8 par métathèse croisée ou par oléfination de Horner-Wadsworth-Emmons, se sont révélées inappropriées pour accéder au produit naturel. Une troisième stratégie, impliquant la création de la liaison C8-C9 par addition d’un organolithien sur un aldéhyde, a conduit à un intermédiaire avancé dont la transformation en cytostatine avait été décrite. Une synthèse formelle convergente de la cytostatine a donc ainsi réalisée. Une approche synthétique du bisélide E, produit naturel marin, a été également étudiée. Celle-ci implique la formation de la liaison C8-C9 par couplage pallado-catalysé entre une oléfine gem-dichlorée et un organométallique engendré à partir d’un iodure d’alkyle. Les fragments C1-C8 et C9-C19 ont été synthétisés. Une métathèse cyclisante a permis de former la lactone insaturée du fragment C1-C8 et le tétrahydrofurane présent dans le fragment C9-C19 a été élaboré par une réaction d’hétéro-Michael.

The work analyzes trends in medicines used in specialized medical centers from ATC (Anatomical-Therapeutic-Chemical Classification System) L01 in Czech Republic from perspective of the State Institute of Drug Control (SUKL) and of Czech Health Insurance Companies (Vseobecna zdravotni pojistovna Ceske republiky as the biggest one, and rest joined in Svaz zdravotnich pojišťoven Ceske republiky). SUKL perspective considers medicines delivered to healthcare facilities and the other side providing information regarding payments for L01 medicines between years 2012-2015. Conclusions presented show slightly decreasing trend in terms of number of packages delivered (935 ths. in 2012 and 931 ths. in 2015). On the other hand, value of medicines has increased from 5 067 mil. CZK in 2012 to 6 568 mil. CZK in 2015. The increase in value and decrease in volume in the same time (while prices remain stable or decreasing) indicates trend in increasing size of packaging or entry of new strength (with more active ingredient and therefore more expensive) e.g. entry of Herceptin 600 mg in 2014. This paper further identifies the most expensive medicines in ATC L01 group and starts a discussion about possible reasons of discrepancies between different sources of data which are for the most expensive medicines in hundreds of mil. CZK in value and ths. of packages in terms of volume (e.g. Avastin in 2014: difference 104 mil. CZK and 5 598 packages).

Kroppens organ består av många miljoner celler, som har sin uppgift i det organ de befinner sig i. När en cell dör ersätts den med en ny som har samma uppgift. Cellerna samarbetar och kommunicerar via olika signaler, vilket är en förutsättning för att kroppen ska fungera. Cancercellerna delar på sig snabbt vilket leder till celltillväxt. cancer är den vanligaste dödsorsaken i Sverige, efter hjärt- och kärlsjukdomar. Syftet med studien är att beskriva patienters upplevelser av cytostatikabehandling och dess biverkningar vid olika cancersjukdomar och hur livskvalitet påverkas. För att på ett tydligt sätt beskriva och belysa patienters upplevelser av cytostatikabehandling, gjordes en litteraturstudie som är baserad på kvalitativa ansatser. Resultatet består av tre huvudkategorier och sju subkategorier. De fyra huvudkategorierna är: kroppsliga förändringar, emotionella förändringar och sociala konsekvenser. Våra resultat behandlar hur patienten upplever och påverkas av cytostatika behandling. Detta påverkar patienter och förändrar deras dagliga liv psykisk, fysisk och socialt. Patienter upplever att de blir trötta och inte klarar av sina dagliga sysslor. De upplever också att den fysiska förmågan blir sämre som följd av behandlingen. Detta påverkar även patientens sociala delar av livet, till exempel minskad kontakt med vänner. Därför är det viktigt att sjuksköterskan är lyhörd och medveten om patientens behov och i ett tidigt skede skapar en bra vårdrelation med patienten. För att sjuksköterskan ska minska lidandet för patienten borde hon eller han ge adekvat information och även adekvat kunskap om olika behandlingar.

Bakgrund: Idag erbjuder sjukhus i Sverige gruppträning som rehabilitering för cancerpatienter i syfte att minska eventuella behandlingsframkallade biverkningar. Träning kan bromsa sjukdomsförlopp, förhindra spridning av sjukdomen och minska sjukdomsrelaterad ångest och oro. Det anses vara en brist på evidens gällande cancerpatienters upplevelser kring träning i grupp som rehabiliteringsform.   Syfte: Syftet är att undersöka hur patienter med cancersjukdom upplever gruppträning, under eller efter cancerrelaterad behandling.    Metod: I en kvalitativ intervjustudie med en induktiv ansats inkluderades sex kvinnliga intervjupersoner. Det utfördes sex semistrukturerade intervjuer. Intervjuerna utfördes på patienter som tidigare har genomgått en hel gruppträningsperiod handledd av en fysioterapeut. Materialet analyserades enligt kvalitativ innehållsanalys.     Resultat: Intervjuernas innehåll delades upp i 8 underkategorier och 4 huvudkategorier. Huvudkategorierna bestod av Motivation att träna, Träningsmiljö, Hinder för träning och Upplevda positiva effekter efter avslutad träning   Slutsats: Resultatet av studien visar vikten av det sociala stödet och att det upplevdes spela en nyckelroll för intervjupersonernas trygghet och förståelse. Positiva effekter upplevdes av träningen i sig, såsom bättre styrka, kondition och högre energinivå. Författarparet identifierade att informationsbrist, brist på energi och negativa tankar kunde upplevas som centrala negativa faktorer som i sin tur kunde påverkat motivationen att träna.
Background: Hospitals in Sweden offers group exercise as a possibility for rehabilitation to patients with cancer to aim the possible side effects cancer treatment may induce. Exercise may delay, or stop, proliferation of the disease and may even reduce cancer related depression and anxiety. It is considered that it lacks evidence describing the experiences cancer patients have after using exercise as rehabilitation form.   Aim: The purpose is to investigate how patients with cancer experience group exercise, during or after completed cancer treatment.   Method: In a qualitative interview study with an inductive approach six female interview persons were included. Six semi-structured interviews were performed. The authors interviewed patients who completed a whole group exercise-period supervised by a physical therapist. The results were then analysed using a qualitative content analysis.   Result: The interview results was divided into 8 sub-categories and 4 main categories. The main categories were Exercise motivation, Exercise environment, Exercise obstacles and Experienced positive effects after completed exercise.   Conclusion: The results shows that the importance of the social environment plays a key role according to the experience of support and understanding. It was also described that physical effects from the exercise itself was present. The individuals felt both stronger and experienced having a higher energy level. The authors did identify that lack of information, lack of energy and negative thoughts were described as obstacles, these obstacles were the main negative factors that could lower the individual’s motivation towards the exercise.

Abstract:

Bakgrund: Hand-fot syndrom är en form av perifer sensorisk neuropati orsakad av cytostatikabehandling. Syndromet kan ge biverkningar såsom smärta, obehag, domningar, svullnad och nedsatt balans. Utvärderade behandlingsmetoder saknas.

Syfte: Undersöka hur tolv veckors sjukgymnastisk behandling med långvågsdiametri, interferens och balansträning påverkar biverkningar i fot/underben orsakad av cytostatikabehandling hos sju patienter med hand-fot syndrom.

Metod: Gruppstudie, kvasiexperimentell resultatstudie. Sju patienter deltog. Variabler som mättes var, smärta, obehag, domningar och balans. Tre mätningar utfördes, före, efter samt åtta veckor efter interventionen. Självrapporterad skattning och fysisk mätning användes.

Resultat: Gruppens smärta, obehag och domningar minskade vid samtliga mätningar. För smärta visade mätning efter intervention samt åtta veckor efter signifikans (p=0,027), (p=0,042). Obehag visade signifikans efter interventionen (p=0,018). Domningar visade ingen signifikans. Balans visade signifikans i: Skärpt Romberg, höger, blundande, åtta veckor efter interventionen (p=0,043). Skärpt Romberg, vänster, blundande, efter interventionen (p=0,027), åtta veckor efter interventionen (p=0,028). Stående på ett ben, höger, blundande, efter interventionen (p=0,042), åtta veckor efter interventionen (p=0,027). Inga mätningar visade försämringar.

Slutsats: Restultaten visade att behandling med långvårdsdiametri, interferens och balansträning minskade smärta, obehag, domningar och delvis förbättrade balans vid hand-fot syndrom. Dock går det inte att påvisa vilken behandlingskomponent som påverkat mest. Ytterligare studier behövs för att ge resultat större giltighet.

Three measurements were carried out, before, after, and eight weeks after the intervention. Self-reported estimates and the physical measurement were used.

Results: The group's pain, discomfort and numbness decreased in all measurements. For pain measurement after the intervention and eight weeks after showed significance (p = 0,027),(p = 0,042). Discomfort showed significance after the intervention (p = 0,018). Numbness showed no significance. Balance showed significance in: Sharpened Romberg, left, eyes closed, eight weeks after intervention (p = 0,043). Sharpened Romberg, left, eyes closed, after the intervention (p = 0,027), eight weeks after intervention (p = 0,028). Standing on one leg, the right, eyes closed, after the intervention (p = 0,042), eight weeks after intervention(p = 0,027). No measurements showed deterioration.

Conclusion: The results showed that treatment with short-wave diathermy, interference and balance training reduced pain, discomfort, numbness and partial improvements in balance in hand-foot syndrome. However, it is not possible to demonstrate which treatment component that affected the most. Further studies are needed to produce results more valid.

Ibland behövs skyddsisolering på sjukhus med risk för vårdrelaterade infektioner. Syftet var att ur ett sjuksköterskeperspektiv belysa omvårdnaden under skyddsisoleringen samt vilka infektionsförebyggande åtgärder som är av betydelse. Metoden var en litteraturstudie som bygger på 17 vetenskapliga artiklar. Resultatet visar olika omvårdnadsaspekter som är betydelsefulla vid neutropeni. Olika infektionsförebyggande åtgärder för att hindra endogen och exogen smitta belystes. Smittspridningen förhindras genom användning av basala hygienrutiner och handdesinfektion. Patienter med neutropeni som skyddsisoleras på sjukhus är en utsatt och svårt sjuk patientgrupp vilket gör det svårt att randomisera tillräckligt stora, väldesignade och jämförbara studier. Det saknas forskning inom omvårdnaden. Säkra rekommendationer med hög trovärdighet kan inte ges med denna bakgrund. Behov finns att kritiskt utvärdera de insatser som görs idag och att utveckla forskningen av omvårdnaden av patienter med cytostatikaorsakad neutropeni.

Malmö University: Health and Society, Department of Nursing, 2008.Neutropenia is a serious complication among cancer patients who receive chemotherapy and can cause life threatening cross infections and sepsis. Protective care at hospital is sometimes needed with risk for cross infections. The purpose of this review was to highlight the nursing care of patients in protective care and to indicate the most important actions to prevent cross infections. A literature review was used as method based on seventeen scientific articles. The result showed actions of nursing care that are important during protective care at hospitals. The result also showed interventions to prevent cross infections caused of endogenous and exogenous source. There are few well designed studies and there is a lack of comparable empirical evidence. These patients are very ill and vulnerable and it is difficult to perform randomized studies with enough sample size to get well designed and comparable studies. Therefore recommendations can not be given that are based on high level of evidence. There is a need to develop evidence based nursing methods and to identify and evaluate nursing interventions so that the best possible care of patients with chemically induced neutropenia in protective care is given.

Cancer är en av de vanligast förekommande sjukdomarna i Sverige. Som behandling för cancer ges vanligen cellhämmande läkemedel, cytostatika. Biverkningar under cytostatikabehandling är svåra att undvika och några av de vanligaste är utmattning, illamående, kräkningar, håravfall och mukosit. Att drabbas av svår sjukdom kan leda till stort lidande hos människan. Genom att skapa sig en uppfattning om hur dessa biverkningar påverkar patienternas dagliga liv kan sjuksköterskan möta patienten i sitt lidande. Syftet med denna litteraturstudie är att med inspiration av Evans (2003) analys av kvalitativa artiklar, beskriva hur patienter upplever att biverkningarna av cytostatikabehandling påverkar deras dagliga liv. Analys av artiklarna resulterade i tre kategorier och fem underkategorier. Huvudkategorierna är: Kroppen sviker, Sociala förluster och Emotionell instabilitet. Resultatet visar att patienter som genomgår en cytostatikabehandling påverkas på många olika plan i livet.

Abstract:
This work deals with studying a molecular mechanism of anticancer effect of a new platinum - based drug. The qualities of still unknow dinuclear platinum komplex (BBR3571-DACH) were parallelly studied together with clinically time-tested and used mononuclear platinum complex DACH. Earlier essays demostrated, that DNA is the critical target for the cytostatic activity of platinum compounds. Altered properties of DNA and binding characteristics of these two platinum compounds were monitored by several different bioanalytical methods (differential pulse polarography, UV-VIS spectrophotometry, fluorescence spectrophotometry, CD spectroscopy and electrophoresis) after modification of DNA by both of platinum complexes. For the compounds BBR3571-DACH and DACH it was determined that the DNA binding is rapid and bifunctional. The stabilizing effect on DNA was significant particularly after modification of DNA by dinuclear komplex, while denaturating effect wasn´t proved at all. The results also indicate that dinuclear platinum complex BBR3571-DACH probably does not participate on formation of long-range cross-links like other early studied polynuclear platinum complexes.

Abstract:
Following the administration of cytostatic drugs, an increase in thromboembolic phenomena has been described in cancer patients. Such hemostatic alterations may be related to degree of hipercoa-gulability observed following chemotherapy, in comparison to previous levels. In terms of the fibrinolytic system, however, no - clearly defined alterations have been detected. We studied the - changes in plasmatic coagulation and fibrinolysis in 40 patients with non-operable stage III and IV lung cancer (30 epidermoid - ad 10 microcytic neoplasms) following cytostatic chemotherapy. Two studies were done on each patient, i.e., one at the time of diagnosis, -and the other 48 hours after completing the first chemotherapeutic cycle. The results show significant (p 0,05) post-chemotherapy increases in fibrinopeptide A (FPA) levels (pre: 2.95 ± 3.98, post: 8.15 ± 9.40 ng/ml), as well as a decrease in fibrinolytic activity reflected by a drop (p 0.01) in functional tissue plasminogen activator (t-PA) (pre: 1.53 ± 1.66, post: 0.91 ± 0.95 ng/ml). Morever, a tendency towards reduced euglobulinic precipitate lysis on fibrin agar was observed (pre: 122.8 ± 85.7, post: 105 ± 71.5%). The other parameters evaluated, i.e., antithrombin III, plasminogen immunologic t-PA and functional PA inhibitor - (PAI) showed no significant changes.We have also studied the potential accumulative effect of three chemotherapy courses and the results were compared to the situation at the time of diagnosis. A significant increase p 0.01 in functional PAI has been observed (pre: 1.85 ± 2.38, post: 5.41 ± 3.74 U/ml). The possible participation of tumor mass in the elevation of these parameter was considered: but no relation betwen tumor mass and increase PAI have been detected.Chemotherapy is apparently capable of conditioning a decrease in fibrinolytic activity in these cancer patients, this could be related to the enhanced tendency of these patients to developing - thromboembolic phenomena following cytostatic chemotherapy.
