Abstract:
Balloon pulmonary angioplasty (BPA) is a novel treatment option for patients with chronic thromboembolic pulmonary hypertension (CTEPH) who are not eligible for pulmonary endarterectomy (PEA) or suffer from persistent pulmonary hypertension after PEA. The aim of this study was to evaluate the real-life efficacy and safety of BPA in a consecutive group of patients who were diagnosed and treated in the national referral center for CTEPH in the Czech Republic. Here we report data from 160 BPA procedures performed in 64 patients. Efficacy analysis was performed in the subgroup of 25 patients who completed BPA series. Significant improvements were observed in New York Heart Association functional class (4% to 79% in I/II, p < 0.001), 6 min walking test distance (+54.3 m, p < 0.001), risk profile (15.8% to 68.5% with presence of 2/3 low risk criteria, p < 0.001), pulmonary artery mean pressure (−18%, p < 0.001), pulmonary vascular resistance (−32%, p < 0.001), stroke volume (+17%, p = 0.011) and quality of life (+37% in assessment of overall health status by a patient, p < 0.001). We observed 1 fatal periprocedural complication (1.6% of all 64 patients) and 19 BPA-related non-fatal complications (11.9% of all 160 interventions) that predominantly included hemoptysis (10.0% of all sessions). Overall survival at 12 months was 94.6%.

e20505 Background: 68% of cancer patients live at least 5 years after diagnosis and many are cured. Cancer survivors continue to need support. Music is energising, affirming and therapeutic. More funds are needed for cancer research so that more patients become long-term survivors. In 2012 Galway University Hospital founded 'Something To Sing About' (STSA.ie), a not-for-profit organisation to support cancer survivors and cancer research. Methods: Local publicity brings small bands of cancer survivors together rehearsing the same music at the same time every week (Wednesday 7pm) in a local hall, hospital or hotel, each with its own local musical director. The music is selected by a representative music committee and music therapist. Musical instruction is disseminated by the chief musical director via website and social media. A plenary rehearsal takes place every 3 months. All profits from events and music sales are allocated to cancer research projects through an open peer-reviewed grant-application process. Results: In 6 months STSA has grown from concept to network of 20 centres with total membership of 251 cancer-survivors. Immediate feedback has been extremely positive as assessed by personal communication and facebook activity (currently 1,025 ‘likes'). Over 100 singing-survivors participated in the first plenary rehearsal. All commercial venues have donated their meeting rooms gratis. Internationally there is one participating centre in Brisbane, Australia, with others signalling their intention to join in 2013 including: Memorial Sloan Kettering Cancer Center, New York; 14 breast cancer support centres in UK; and one centre in the Czech Republic (updated at meeting). STSA members are particularly keen on establishing links with other cancer patients abroad. Professional musicians are pledging support and suggesting fund-raising collaborations. Conclusions: Cancer survivors continue to need ongoing support and find the music therapy and group therapy provided by STSA most beneficial. Cancer survivors are very interested in supporting cancer research. STSA has the potential to become a major international cancer support network and cancer research foundation.

Abstract:
In the 1920 s, the politically heterogeneous Ukrainian emigration community in inter-war Czechoslovakia, with its back in World War I and losing national liberation competitions, desperately needed both physical and spiritual rest. However, the status of «emigrants» transformed the imagination of the natives about leisure and leisure. The recreational regulator was, on the one hand, the scientific and technical implications of the 'stormy twenties' and, on the other, the urgent need to keep 'one's band', that is, a collective form of rest and leisure. Ukrainian exiles visited various theatrical performances, book exhibitions, music concerts, sections and circles, and enjoyed excursions. Slowly, with some nuances, cinema was also part of the Ukrainian emigration leisure. It should be noted that the Ukrainian emigration in the inter-war Czechoslovakia, because of the 1920 s «quick return concept» and the priority, first of all, of its own political projects, did not leave any jobs or references to film vacations. The Great Depression of the 1930 s and the Losses dismissed the issue of leisure in general and film recreation in particular, making it difficult to physically work to survive. Contemporary scientific intelligence on the impact and role of cinema in the life of the Ukrainian emigration community in the interwar CSR is absent because of the fact that despite the status of Prague as a powerful political, cultural and scientific emigration center, it has not become a leading European cinema center, yielding here Berlin. Only those edited by Doctor of Historical Sciences, Professor IB Matyash, the book «Diplomatic History of Ukraine by Yevgeny Slabchenko (Eugene Deslav)», relevant to the subject of our intelligence, and the article by Roman Roslyak «Ukrainfilm».Based on the above, the purpose of the publication is to argue that, despite some efforts by the Ukrainian emigration community in the interwar CSR, a powerful European film center in Prague, unlike Paris, Vienna, Berlin and New York, never. The Ukrainian emigration community in the Czechoslovak Republic numbered about 20-22 thousand people. In its social composition, it was mainly peasant workers, the layer of her intelligentsia was small. Thus, most Ukrainian immigrants either used the moment to seek education in order to further have a higher social status or to work hard without being able to study. First of all, the Ukrainians, as a national community in exile with a lack of sufficient financial base, even in the «scientifically technical twenties» and «economically unstable thirties», were forced to stand in conservative positions on leisure issues. The first «moving pictures» appeared on the territory of the Czech Republic in 1896 in Prague and Karlovy Vary, thanks to the director and cameraman Jan Krzyzyniecki, who, since the second half of the 1890s, made several short documentary films. And a year later, one of the private American film companies came to the Czech Republic to start filming a black and white silent film in Bohemia. Whereas the first permanent cinema on Czech lands was started by the illusionist Victor Ponrepo (1858–1926) in 1907 in Prague. The Czechoslovakian film industry gained considerable momentum during the interwar period. Since 1921 professional film studios have started. And by 1932, the championship was kept by black and white silent films. The soundtrack of films in the Czechoslovakia began in 1930. In addition, in 1930 Czechoslovak authorities imposed a ban on the import of any German-language films. Prohibition of German-language film production leads to the fact that in 1933, the Czech studio «Barrandov Studio», established by the brothers Vaclav and Milos Havel in 1921, is firmly on its feet, and the number of cinemas is counted in 1938. 1824. However, there were attempts to create a quality Ukrainian emigration film product and, accordingly, Ukrainian (emigration) film studios in the Czechoslovak Republic. These attempts were linked to the names of Boris Khoslovsky and Roman Mishkevich. Khoslovsky since 1926 the head of production of advertising departments of the firm «Vira Film». Since 1928 organizer and owner of the «Mercury Film» Studio, specializing in the production of promotional films. Another Ukrainian film studio, «Terra Film», originated in Brno in the early 1930s on the initiative of Roman Mishkevich. This film studio tried to shoot science and plot (situational-natural) films. Until 1939, Myshkevich's firm remained the largest importer of motion pictures from the Czech Republic to Japan, China, India and Central America. Yet, to develop a powerful Ukrainian film industry in the Czechoslovakia proved impossible. There were several reasons for this. First, political (emigrant status) and economic (lack of sustainable financial flows from the Czechoslovakia) were hampered. Secondly, the Ukrainian film industry did not have the support of both Ukrainian and Czech (private) businesses. Thirdly, the low potential of Ukrainian film enthusiasts and the lack of professional education and experience were evident. Fourth, the Ukrainian Prague film production consisted mainly of documentaries, short films, plot films, reports and chronicles.

Abstract:
For centuries, art has played an important role as a tool of cultural diplomacy and even now represents an integral part of foreign cultural policy of different countries. Modern and contemporary fine art often reflects the current social and political issues, at both national and international levels. This thesis aims to prove that art, as a means of expression, has its place in international relations, in the context of cultural diplomacy. The intention of this thesis is to explore the many ways that fine art is applied as an instrument of cultural diplomacy and how the use of visual arts contributes to positive relations between nations and to the process of building a positive image of the state abroad. In the context of Czech cultural-diplomatic practice, the art and its employment is analysed on two levels - historical, that examines the use of art in the process of building the Czechoslovak state, and institutional, that introduces both state and private actors, who contribute greatly to the cultural diplomacy of the Czech Republic via the promotion of Czech fine arts. The final chapter focuses on the environment of New York as a world cultural and art center, and engages in representative Czech projects, activities, organizations and individuals who are successful advocates of Czech visual arts in the international context. The main contribution of this thesis is its aim to address an uncommonly debated topic - a specific area of artistic creation, its role and position in international relations and cultural diplomacy of the Czech Republic.

The purpose of this essay is to study New York Citys management of a larger crisis and to see how states are coping with stress. It is important to study how states manage stress so that states can learn to cope with larger crisis. That is important for the survival of the state and the protection of its citizens. David Easton is a political science author that write about this. There are four authorities under study in this essey and the following question will be poised for all of them : did the FDNY, the NYPD, the EMS and the PAPD manage the terroristattack against World Trade Center according to relevant crisis management?

This essay will be in the form of a case study and to be able to answer the specific questions I have studied three kinds of crisis management theories. I have taken some of the most central points from these theorys and used them to study the response of the different departements of New York City. The result of my study is that the different authorities did not act according to relevant crisis management. The biggest flaws where the lack of cooperation, the break down in almost all communications systems and the lack of preperation for large terroristattacks.

This thesis deals with the promotion of the Czech Republic abroad and in the American market, and focuses primarily on New York. The American public isn't very aware of Czech Republic, in fact still confusing it with Czechoslovakia, also the image of Czech Republic isn't very positive. All of this could be changed with effective promotion. Communication campaigns within the large American competition could be very expensive and therefore difficult to achieve, because of this it would be very important to focus on marketing tools which are budget friendly. The theoretical part of this thesis presents marketing and commercial communication, as well as preparation of communication campaigns and tools toward it. It also specifics on the promotion of tourism. The practical part of this thesis focuses on the current promotion of the Czech Republic abroad and in the American market, and problems the Czech Republic has to deal with, as well as offers possible solutions.

New York City is projected to add nearly one million residents by the year 2040. At a time when housing supply and affordability are a significant factor for global competitiveness, the city has implemented a variety of regulations and incentives to encourage new development. Current Mayor Bill de Blasio's housing strategy includes an initiative to rezone several neighborhoods to accommodate higher density, encouraging the private real estate development industry to build more units while requiring that a portion of those units be made permanently affordable. While rezoning actions are often studied years later in order to provide enough time to measure their success, the city's plan calls for as many as 15 neighborhoods to be rezoned within 10 years. A real-time analysis of an individual neighborhood rezoning proposal, approved during the time of this thesis, provides the ability to evaluate research questions related to how rezoning is being carried out now and how participants may alter their strategies going forward. The neighborhood of East Harlem, the third area in the city to undergo this rezoning process, is thus used as a case study for how rezoning is carried out, compromised and ultimately approved. The analysis reveals that the total amount of new residential development made possible through rezoning is limited compared to a "no action" scenario. Thus, the most significant impacts of rezoning are not to dramatically increase the number of new residential units to be built, but rather to require that a portion of those new units are made affordable through the introduction of the city's mandatory inclusionary housing program. Additionally, the rezoning process resulted in significant city commitments to public investments in the neighborhood. Yet, these commitments are not guaranteed within a specific timeframe and are almost entirely the responsibility of the public sector to implement. While the ability of rezoning to produce a significant number of new residential units is limited, rezoning will continue to serve as a primary means for the city to attempt to house its growing population.

New York City, arguably the world's financial capital and the world's biggest real estate market, and home to some of the finest medical and academic research centers houses a little over a million square feet of life science focused real estate. Despite tremendous academic research potential and financial wherewithal, the life science industry of the city is comparatively much smaller to other New York City industries. In addition, it is much smaller in comparison to life science industry in other parts of the country. This thesis investigates the New York City market as a possible location for developing life science focused real estate assets. As a first step, the research will focus on identifying and analyzing the key demand indicators to establishing the demand for life science focused real estate. Next, the thesis will focus on lab space as a real estate product to understand its main components and value drivers. Upon understanding the market and the product, the research will put forward possible strategies for developing lab buildings in the city. Further, in support of the development strategies the research will look to prove the financial feasibility of developing that life science real estate in the city. This would involve financial analysis and contrasting returns from life science assets against office assets. Lastly, through real options framework the study will go on to demonstrate the benefits of applying flexibility to real assets, while financially valuing this flexibility using the Monte Carlo analysis.

Real estate developers and investors have a vested interest in discovering new techniques for estimating the direction and magnitude of changes in residential rent within a neighborhood. This study hypothesizes, and finds evidence, that taxi activity is a proxy for changing income and neighborhood quality as well as an indicator of gentrification. Novel research is performed to determine if taxi activity is a significant predictor of rents in New York City at the neighborhood level. Nine OLS regression models are created using data about 1,466,234,991 taxi pickups and drop-offs, median rent, and median income across 188 neighborhoods in New York City in the years of 2010-2015. In all nine models, taxi activity is found to be a statistically significant predictor of rent at 99% confidence. This study finds that a I standard deviation positive shock in taxi drop-offs will result in a 0.009% 0.155% higher rent the next year on average.

A lack of quality housing affordable to the average worker near employment centers has long been an issue in American cities where the private production of housing for middle income families is restricted by market forces, zoning or physical boundaries. There are approximately 2.3 million middle income households in New York who earn between 80% and 150% of the Median Family Income who are priced out of market rate housing. These households are forced to relocate elsewhere or spend a daunting percentage of their time and income on housing and/or transportation. The high cost of land, labor and materials are further exacerbated by zoning regulations and entitlement review processes to result in a prohibitively high cost of housing production. Governments across the US and in New York have developed various types of policy strategies aimed at subsidizing development and increasing the affordability of housing. This thesis provides a summary discussion and perspective on the factors that increase the cost of housing production. It then reviews the different strategies utilized in reducing these costs, both nationally and locally in New York. Next it tests each strategy's effectiveness using a case study of a proposed development project in Brooklyn, NY. There are cost, constructability and physical constraints that govern a designer’s ability to utilize a particular material when laying out the framing for a new structure. The Krause Gateway Center in Des Moines Iowa</p><p>represented challenges that were not easily solved with one material or something previously seen in an office building. The plan layout, the extreme cantilevers and other geometric constraints demanded materials and options that are flexible and provide the opportunity to be built and modified in ways not</p><p>typically seen. Structural steel framing (after many rounds of test fits and studies of post tensioned concrete and other precast ides) was chosen for its ability to be built and designed to meet the needs of the building in ways other materials could not.</p>

<p>Bažant et al. have proposed a model of a gravity-driven collapse of a tall building which collapses in a progressive-floor collapse after the failure of a single storey. The model allows the re-computation of the structural resistance of the building once the downward movement of the building has been quantified. We give a physically more sound version of the collapse model, and determine the downward movement of the North Tower of the World Trade Center. Thereby we reproduce a value for the upward resisting force during the collapse that is similar to what has been achieved by Bažant et al. for the first three seconds of the collapse. However, our method of measurement also includes data up to 9 seconds after collapse initiation. These data show a much bigger upward resistance force between 4 and 7 seconds after collapse initiation.</p>

<p>The John F. Kennedy Center for the Performing Arts is located on the banks of the Potomac River in Washington, D.C. The Kennedy Center is home to the National Symphony Orchestra, the Washington National Opera and The Suzanne Farrell Ballet. In addition to being the nation's busiest arts facility, the Kennedy Center is also a "Living Memorial" to President Kennedy. The south expansion of the Kennedy Center, known as the REACH, provides much needed educational and rehearsal space, as well as a pedestrian link to the nearby Memorials.</p><p>The new structure combines practicality, versatility and innovation. Each surface and space created is unique, forming complex geometries and large span to depth ratios. The cast-in-place concrete structure is exposed on the interior or the exterior, including three white concrete pavilions rising out of the landscaped substructure. The pedestrian bridge connects the Kennedy Center to the river with over a 35-to-1 span to depth ratio. The REACH is scheduled to open in the Fall of 2019.</p>

<p>The Altstadtring-Tunnel is one of the essential east-west traffic routes in the city center of Munich and was constructed in the late 1960s. Segment 34 of the tunnel was built directly underneath the existing Prince-Carl- Palais, a historic building from 1804. Therefore 15 pre-stressed concrete girders with an effective depth of</p><p>3.5 m and a maximum span of up to 30 m were built which now form the tunnel roof slab. These girders were pre-stressed with steel nowadays well known for stress corrosion cracking. A recalculation of the slab showed that no ductile failure can be guaranteed in case of a progressive rupture of the tendons. Therefore, a concept for strengthening the slab was developed using concrete screw anchors as post installed bending and shear reinforcement. The concrete screw anchors are normally installed as anchoring elements in cracked and non- cracked concrete and are available with diameters up to 22 mm. Developing this concept further, it is straight forward to use these anchoring elements as post-installed reinforcement in existing concrete structures. This new strengthening system was developed at the University of Innsbruck in the last few years and can fulfill the special requirements of this project, such as installation of the strengthening system from underneath the tunnel slab during ongoing use of the structure. High strength steel with diameters of up to 63.5 mm will be used as post-installed bending reinforcement covered with a new shotcrete layer on the underside of the tunnel slab. In total 59.3 tons of new flexural reinforcement and 7199 concrete screws for strengthening the shear capacity of the girders will be used to ensure a ductile failure of the tunnel slab. The on-site work started in March 2019 and is expected to take two years to complete.</p>

<p>Advanced modeling of structures using combination of non‐linear finite element methods (NLFEM) and reliability analysis is a strong tool for realistic assessment of structures. NLFEM simulation has been recently a well‐established approach to the analysis of concrete structures since the response of the structure can be simulated quite realistically. In combination with fully probabilistic approaches, one can consider the randomness of input parameters such as material, technological and environmental characteristics that can have a direct impact on economic aspects during structural lifetime. However, guidelines fully describing NLFEM modeling of structures and safety formats are not available until now. In the framework of the European Project INTERREG AUSTRIA‐CZECH REPUBLIC "ATCZ190 SAFEBRIDGE", a number of existing bridges are carefully selected to be studied and modeled with NLFEM on deterministic and stochastic levels based on the upcoming Austrian standard ON B4008‐2. The assessment of structures will be described and documented in detail and the results will assist the development of a guideline. This guideline targets to help the engineering community perform accurate NLFEM analysis and to assist the structure's owners to check the accuracy of the assessment process. The current paper focuses on the presentation and discussion of statistical information about road and railway bridges provided by the main bridge operators in both countries. Moreover, the most commonly addressed structural characteristics of bridges within the program region are summarized and the further future steps of the project are briefly described.</p>

<p>Conventional single family houses in Poland are being built in the improved traditional construction technology with massive load-bearing structure created by masonry walls made of ceramic , silicate or aerated concrete blocks, concrete foundations, concrete rib-and-slab floor and pitched timber or concrete flat roof. Expanded polystyrene and mineral wool are being used as thermal insulation. Such solution is very time- consuming and costly due to more and more expensive manpower. The change in economy from communism to free market caused the beginning of modern thinking about construction technology for family house to be light, eco-friendly, innovative and low-cost. The definition of the term “affordable” in relation to single family housing, as well as the socio-economical background for the low-cost housing in Poland is presented. The paper presents proposal of low-cost family house construction built in light steel frame technology. The supporting structure is composed of steel frame made of cold-formed galvanized C shape profiles. Externally the steel frame is covered with cement bonded particle boards, covered with polystyrene insulation and finishing coat according to ETICS system. From the inside the steel frame is covered with plasterboards. The main thermal insulation of the building is created by mineral wool filling the interior spaces of walls, ceiling and roof panels. Comparison of the total construction costs of a 136 sq m building made in light steel frame technology with buildings of identical dimensions made in other, popular and available traditional technologies showed that a steel technology is approx. 10-20 percent cheaper. The paper presents also the comparison of the low-cost model steel structure houses worked out in Czech Republic, Portugal, Romania and Brazil, including technical parameters, structure type and cost of erection.</p>

<p>To deal with the high volume of traffic in the historic city center of Karlsruhe (Germany), the track system of the historic surface tram is moved underground.</p><p>First, seven underground stations are built, which are then connected by using a tunnel boring machine for excavation of the tracks. While the traffic continues on the surface, the new underground stations are being built in sections using the dig-and-cast construction method. Due to the high ground water level bore piles and diaphragm walls of reinforced concrete / concrete cut-off wall are used as excavation pit shoring. A grouted sealing blanket made using jet grouting processes serves as horizontal blanket.</p><p>The cover is made while the traffic continues overhead, to this end traffic routing of road and tram traffic is altered in several stages of construction. The subsequent removal of the soil is realized via this newly created tunnel system – this way no truck traffic has to pass through the city center.</p><p>The construction is a challenge to permit a limitation of the deformations in relation to the settlement of the immediately adjacent historic buildings and also in the logistics of the construction while keeping the traffic above ground running.</p>

<p>The development of urban infrastructure, especially the pedestrian network in Tokyo is listed up as the most priority measure which they have in one of the urban redevelopment step-up projects in the nationally strategic economic zone, Takeshiba. This is reported about the detailed design for the pedestrian deck which makes a connection between the train terminal station and the ship terminal station.</p><p>We have to create not only a new pedestrian network through an existing elevated traffic network that spreads in multiple layers in the center of Tokyo (it is so that threads pass through the eye of a needle), but also pile foundation through underground buried objects.</p><p>It must be made sure that there is little impact on the green called old Kyu Shiba Rikyu Japanese Garden which is rare in the center of Tokyo and is next to this pedestrian deck surrounded by skyscrapers.</p><p>Therefore we consider many design factors of the pedestrian bridge, for example, landscape deliberation, construction method through intersecting road, maintainability concerning usability, cost calculation, and so on. There are various contexts in the surrounding environment of this special situation. As a result, we completed this pedestrian deck that had some unique detail designs.</p><p>Then we designed not only sophisticated simple and also a well-balanced form for accomplishing a harmony between high-rise buildings and the Japanese garden.</p>

<p>Cable systems have numerous advantages, such as: large column-free areas, and reduced materials consumption, which reduces the load and the cost. Nevertheless, they are rarely used in long span roofs due to large deflections, and the insufficient space for end supports, or/and back-stayed cables. This work suggests the use of post-tension stress ribbon system in long span roofs in order to reduce the pull-out forces, deflections and concrete stresses compared to a conventional cable system. A comparison is carried out through meticulous and accurate finite element simulations, using SAP2000, implemented for the new +200m roof of Västerås Travel Center (Sweden), which will become one of the longest cable suspended roofs in the world, if not the longest. Results confirm the suitability and superiority of stress ribbon systems as it reduces concrete stresses, deflections, pull-out forces and vertical reactions. These reductions are found highly correlated to the applied prestressing forces.</p>

<p>Tuned mass dampers (TMDs) are widely used to mitigate wind-induced tall building vibrations. However, two major disadvantages of passive TMDs exist. The installation height of pendulum TMDs consumes expensive space and passive TMDs cannot cope with the time-varying excitation frequency of wind loading which will excite several structural modes. Also, the pendulum mass may impact the structure due to seismic excitation which necessitates huge snubbing systems. The presented adaptive TMD System addresses all these shortcomings. The installation height is reduced by up to 50 % by the inclination of the pendulum cables as this method enlarges the radius of the pendulum mass center being equivalent to the reduction of the natural pendulum frequency. The efficiency of the adaptive TMD is improved by adjusting the oil damper characteristics in real-time which allows reducing the pendulum mass up to 20 %. The overdamping control approach activated at wind return periods greater than 10 years reduces the maximum pendulum relative motion by up to 25 %. The adaptive TMD with inclined cables therefore minimizes the installation space which helps to maximize the economic benefit of the building.</p>

On May 7–8, 2001, the Population Council and the Center for Reproductive Health Research & Policy of the University of California, San Francisco, convened a technical seminar in New York City on the use of misoprostol for women’s health indications. The seminar was designed to provide a forum for researchers, providers, women’s health advocates, and educators to exchange information with the goal of advancing the potential of misoprostol to improve women’s health. Participants discussed the state of the art in research, examined current clinical use of misoprostol, and created strategies for the future. The first day focused on scientific and clinical aspects of misoprostol use. The second day’s discussion centered on the future of misoprostol for women’s health, including identifying priorities for research and the role of provider groups and women’s health and advocacy organizations in helping to ensure misoprostol’s continued, appropriate use. At the end of each session, the group had an opportunity to share ideas and discuss unanswered questions. This report covers the key issues raised by each speaker and highlights general areas of discussion among participants.
