O debate entre J. Searle e D. Dennett acerca da possibilidade técnica de uma Inteligência artificial gira em torno de uma única questão: estados mentais expressam algo intrínseco, privado e subjetivo, experimentado em primeira-pessoa ou não passam de entidades obscurantistas, frutos da ignorância e, em princípio, reduzíveis à objetividade científica da neurociência? Searle defende a primeira tese, enquanto Dennett argumenta pela segunda. O texto expõe a posição de Dennett, a crítica de Searle e se direciona para uma nova solução, que defende um monismo integral, no qual mente e cérebro podem ser vistos como duas dimensões de uma mesma realidade. Nesse sentido, o projeto de Inteligência artifical precisará revisar o conceito de “físico” se quiser avançar em suas pretenções, incluindo nele conceitos hoje antagônicos como intencionalidade, liberdade e subjetividade. Abstract: The debate between J. Searle and D. Dennett about the technical possibility of an artificial intelligence revolves around a single issue: mental states express something intrinsic, private and subjective, experienced in first-person or are merely obscurantist entities, results from ignorance and, in principle, reducible to scientific objectivity of neuroscience? Searle argues for the first position, while Dennett argues for the second. The text exposes the position of Dennett, the criticism of Searle and is directed to a new solution, which advocates an integral monism, in which mind and brain can be seen as two dimensions of the same reality. In this sense, the project of artificial intelligence need to review the concept of "physical" if it wants to advance its claims, including today antagonistic concepts such as intentionality, freedom and subjectivity. When industrialist John D. Rockefeller visited the new open-pit mining operation of his competitor, Daniel J. Guggenheim, in Bingham Canyon, Utah, in 1910, he declared with genuine envy and admiration,“it's the greatest industrial sight in the whole world.” What most impressed Rockefeller were the massive steam shovels that had revolutionized the process of copper extraction by enabling firms to mine and smelt tons of previously worthless low-grade copper ore. Equally impressive was the fact that, where skilled American miners had very recently toiled underground in search of rich veins of copper, unskilled immigrants now worked aboveground, loading tons of newly blasted copper ore onto train cars. Rockefeller was not alone in expressing wonder at this new man-made marvel. Hundreds of sight-seeing tourists also traveled to the mine by train each week to experience the thrill of industrial America's newfound ability to move mountains. Like Rockefeller, they saw nothing but progress and modernity in the great open-pit mine.

Sie gehen auf den Kantor Johann Nicolaus Mempell (1713-1747) in Apolda zurück, der möglicherweise ein Schüler von J. P. Kellner war. Johann Gottlieb Preller aus Oberroßla bei Apolda (1727-1786, ab 1753 Kantor und Organist in Dortmund) und vielleicht ein Schüler von J. N. Mempell und J. T. Krebs senior, kombinierte diese Gruppe von Handschriften mit Kopien, die er in den Jahren 1743 bis 1749 selbst angefertigt hatte. Abstract Breast cancer is a heterogeneous collection of diseases grouped by hormone receptor status or by expression of key subtype-determining genes. Breast cancer subtypes, in particular basal-like breast cancer and the luminal breast cancer subtypes, differ by hormonal receptor status, proliferation, genomic instability and mutational signatures, treatment response, and prognosis. The highly distinct signatures of basal-like and luminal breast cancer suggest that they may have different cells of origin within the breast duct. As part of the Washington University Human Tumor Atlas Network (WU-HTAN) program, we generated multi-omic data for 53 samples from 37 breast cancer tumors and 4 normal adjacent tissues. Genomic subtyping was applied to both bulk and single-nucleus RNA sequencing. Analysis of single-nucleus gene expression and chromatin accessibility in epithelial cells underscores similarities between basal-like breast cancer and luminal progenitor cells within the breast duct, and between luminal breast cancer and mature luminal ductal cells. Objectives: There are no current guidelines for optimal management of first-time patellar dislocation in children and adolescents. The objective of this study was to develop consensus-based guidelines for management of first-time dislocation in adolescents. Methods: A 29-question, case-based, multiple-choice survey was developed after 2-rounds of iterations by 20 members of the Patellofemoral RIG. The survey contained two case scenarios of first-time patellar dislocation in adolescents - one with and one without an osteochondral fracture. The survey was administered to primary sports medicine and orthopedic surgeons of the PRISM Society in April 2020 and the responses were analyzed. Consensus-based guidelines were generated when at least 66% of the respondents agreed. Results: 81/276 (29%) members responded to the survey and 6 consensus-based guidelines were generated from those responses (Table 1).There was no consensus between conservative (60%) and operative (40%) treatment of contralateral patellar dislocation. Free-text analysis revealed several variables that influence the decision-making process. Conclusions: There are changes in trends related to management of first-time dislocation in adolescents. MRI, though not recommended for all patients with first-time dislocation, was obtained by 51% of respondents. The survey provides details of conservative treatment, including the role and duration of bracing and physical therapy. For first-time dislocation with an osteochondral fracture, concomitant patellar stabilization is preferred over isolated fixation. Simultaneous guided-growth for genu valgum correction is recommended. There were several variables that influenced the decision-making process and there were several areas of discordance. Reducing the extracellular Mg2+ concentration ([Mg2+]o) to 0.1 mM evoked an aberrant pattern of glutamatergic activity in the synaptic network formed by rat hippocampal neurons grown in primary culture. This treatment resulted in a significant increase in neuronal death when maintained for 20–24 h; 0.1 mM [Mg2+]o elicited a stable and repetitive series of intracellular Ca2+ concentration ([Ca2+]i) spikes as indicated by indo-1-based microfluorimetry. Fura-2-based digital imaging experiments found that the [Ca2+]i spikes were synchronized for all the neurons in a given field. Thus electrophysiological recordings from individual cells were reasonable representations of the field as a whole, enabling correlation of electrical activity to viability. Underlying each [Ca2+]i spike was an intense burst of action potentials. Whole cell voltage-clamp experiments showed that a burst was composed of fast action currents superimposed on a slow inward current. The N-methyl-d-aspartate (NMDA) receptor antagonist CGS19755 (10 μM) blocked [Ca2+]i spiking, the slow inward current, and the cell death induced by low [Mg2+]o. The L-type Ca2+ channel antagonist nimodipine (10 μM) blocked [Ca2+]i spiking, all synaptic activity, and the cell death induced by low [Mg2+]o. The non-NMDA receptor antagonist 6-cyano-7-nitroquinoxaline-2,3-dione (CNQX, 10 μM) exerted variable effects on [Ca2+]i spiking and blocked the slow inward current only when the cells were held at a relatively negative holding potential. CNQX did not afford any protection from 0.1 mM [Mg2+]o-induced neurotoxicity. [Ca2+]i imaging experiments showed that CNQX inhibited [Ca2+]i spiking in a subset of neurons within an active network. Thus, the neurons that were insensitive to CNQX appear to be those that were destined to die. We characterized an in vitro model that allowed us to correlate specific electrophysiological components of glutamatergic synaptic activity to the subsequent viability of the network. A slow NMDA receptor-mediated inward current was required to elicit [Ca2+]i spiking and neurotoxicity. Non-NMDA receptors did not contribute to synaptically mediated cell death in this model. An L-type Ca2+ channel antagonist was neuroprotective when used at concentrations that blocked synaptic activity, suggesting that dendritic L-type Ca2+ channels present a useful target for neuroprotective drugs.

The literature surrounding entropy changes accompanying degradation is scarce and limited to solely graphite anode cells. Meanwhile, graphite-silicon blends become frequent in commercial applications due to their considerable capacity advantage. The lithiation/delithiation results in volume changes of the silicon particle, which has been reported to cause an increased hysteresis [1] of the open circuit potential (OCP). Our hypothesis is that entropy reflects certain morphological changes occurring within the electrode and consequently, entropy hysteresis is also higher for electrodes containing silicon. We further postulate that entropy hysteresis increases with cycle age. If the hypothesis is correct, entropy measurement will offer a unique insight into battery degradation among commonly used differential voltage analysis (DVA) and incremental capacity analysis (ICA). To test our hypothesis, we adapted an accelerated entropy measurement method proposed by Osswald et al. [2] on high-energy NCA/Gr-Si cylindrical cells, with ~10 wt % Si and ~90 wt % Gr anode composition. The cells were divided into two groups; the first group was stored at an elevated temperature to act as an example of accelerated calendar ageing, while the second group experienced cycle ageing. Subsequently, we performed DVA and ICA to provide a direct comparison with the entropy results and checked for correlation. In accordance with the hypothesis, the entropy behaved similarly to the OCP. Entropy hysteresis remained stable for calendar aged cells (Fig. 1 a) but increased considerably for cycled cells (Fig. 1 b). Silicon volume expansion and its 'breathing' effect [3,4] caused charge entropy to increase with cycle age. Graphite particles experienced breaking and cracking, which prompted a decrease in discharge entropy during cycling. These combined effects led to the observed rise in entropy hysteresis over time. A direct comparison of entropy profiling with DVA revealed alike characteristics. Based on abrupt energy level changes accompanying phase transitions, entropy profiling was successfully used to track ageing markers, aiding recognition of a loss of active material on positive (LAMPE) and negative (LAMNE) electrodes as well as loss of lithium inventory (LLI). Both DVA and entropy profiling revealed that LLI was the main degradation mode for the calendar aged cell, while LAMNE combined with LLI for the cycled cell. Plotting entropy against voltage allowed for additional observations. Horizontal shift towards higher voltages occurred due to the rise in internal resistance but also LLI. While some authors [5] successfully obtained information about LAMPE and LAMNE from ICA, and an analogy can be performed for entropy profiling, it is difficult to draw definitive conclusions from these results. The fact that entropy profiling reflects microscopic changes occurring within electrodes, and considers also ageing markers, makes it a unique, non-invasive tool among ICA and DVA. However, its application is not straightforward and needs further validation. Abstract Targeted therapies for molecularly-defined subtypes have led to immense clinical benefit for many cancer types but have generally not been successful for pancreatic cancer. Given that the mainstay of treatment remains multi-agent chemotherapy with FOLFIRINOX or gemcitabine/nab-paclitaxel, there remains an urgent need to identify novel actionable vulnerabilities for subsets of PDAC patients. Toward this end, we conducted an integrative, genome-scale examination of genetic dependencies and cell surface targets for PDAC by leveraging CRISPR and RNAi screening data from The Cancer Dependency Map Project, genomic data of bulk patient tumors from The Cancer Genome Atlas, and custom single-nucleus RNA-seq of a 43-patient cohort comprised of untreated and treated specimens. Our results re-affirm the prominence of Ras/MAPK signaling and a synthetically-lethal interaction between VPS4A/B, but also reveal recurrent susceptibilities to genes within the fatty acid metabolism, vesicular transport and exocytosis, and nucleobase synthesis pathways that otherwise have minor to moderate depleting effects on the majority of cell lines. Aberrations in frequent tumor suppressor genes and chromosomal arm-level variations appear to modify the strength of dependencies, including that of KRAS, CCND1, and GPX4, and may serve as predictive biomarkers of response. In addition, we leveraged mRNA profiling of bulk primary tumors as well as metastatic organoid models to conduct a genome-wide search for cell surface targets that are highly-expressed in tumors while lowly or not expressed in other toxicity-prone, non-malignant tissues. These putative drug targets do not need to be cancer dependencies and can be compatible with antibody-based therapeutic strategies that leverage alternative modes of cellular toxicity. Our approach identifies MSLN, NECTIN4, TROP2, and other antigens which have previously been shown to be largely tumor-specific but also reveals the expression of multiple putative targets within the CEACAM, claudin, and tetraspanin families. Finally, molecular subtyping efforts over the past decade have yielded classical and basal-like as consensus subtypes with variations therein, but genetic dependencies and cell surface expression patterns unique to either are insufficiently understood. We identified CLDN18, CEACAM5, and CEACAM6 as cell surface antigens for the classical subtype and MSLN, AQP5, and SLC6A14 for basal-like. Dependency on TLK2 and CCND1 is associated with the basal-like and classical subtype, respectively. BackgroundImmune checkpoint inhibitor therapy, or ICI, is currently the most successful treatment option for patients with renal cell carcinoma (RCC). However, only 20% of patients have a durable response,1 driving a significant need to improve treatment outcomes. The tumor microbiome has recently been shown to play a role in chemotherapy-based treatment outcomes, but, to our knowledge, no study has explored its role in response to ICIs.2–4MethodsTumor samples were collected from 22 patients with RCC as a part of the Total Cancer Care program at The Ohio State University Comprehensive Cancer Center. Raw RNA-seq reads from these biopsies, as well as data on the responses to ICI therapy were collected. Response evaluation was based on RECIST v1.1 criteria with complete or partial response, or stable disease classified as ”Responders,”, and progressive disease classified as ”Non-responsders”. The RNA-seq reads were processed through a pipeline developed by the Spakowicz lab, known as ExoTIC (Exogenous sequences in Tumor and Immune Cells), to carefully identify exogenous sequences.5 6 Reads that don’t align to the human reference genome are meticulously filtered of (1) common laboratory contaminants, (2) taxa that inversely correlate with input RNA quantity, and (3) taxa commonly found in the negative controls of microbiome experiments. DESeq2 was used to perform a differential abundance analysis on the comparison groups at every taxonomic level.ResultsThe 22 patients with RCC range from 22 to 74 years of age at diagnosis, are 72.7% male, and 54.5% responded to ICIs. Exogenous taxa are identified in the tumor RNAseq, including bacteria, fungi, and viruses (figure 1). Within the tumors responsive to immunotherapy, there was found to be a significant enrichment of certain microbial species, including Bacillus thuringiensis, Comamonas testosteroni, Colletotrichum higginsianum, and Elaeis guineesis. Comparatively, the cohort of non-responsive tumors was found to have a significant enrichment of Candidatus Promineofilum breve, Clostridioides difficile, Nocardia cyriacigeorgica, Streptomyces sp. Abstract Background: Breast cancer patients with brain metastases have a high unmet clinical need and improved management strategies are needed. There has been interest in studying CDK 4/6 inhibitors in the management of breast cancer brain metastases. A phase II study has shown abemaciclib to have activity in the management of hormone receptor (HR)+/HER2- brain metastases. Pre-clinical data suggests a potential synergy with CDK inhibitors and radiation therapy. Stereotactic radiosurgery (SRS) is a cornerstone in the management of limited brain metastases. We hypothesize treatment with abemaciclib and SRS will be safe and improve intracranial progression free survival (PFS) compared to abemaciclib alone. Trial Design: The study is designed as a prospective, single-arm, nonrandomized, open-label, phase I/II trial of abemaciclib and endocrine therapy with SRS among patients with HR+/HER2- metastatic breast cancer brain metastases. Treatment will be initiated with one week of abemaciclib followed by stereotactic radiation to sites of brain metastases or post-operative cavities with continued abemaciclib. Safety will be monitored initially by a 3+3 design. If unexpected neurologic toxicities are noted, the dose of radiation therapy will be reduced. This will be followed by a phase II study to evaluate intracranial PFS. Eligibility: Eligible patients include those that are HR+/HER2-, ≥18, ECOG ≤2 with ≤15 breast cancer brain metastases with measurable disease per Response Assessment in Neuro-Oncology Brain Metastases (RANO-BM) criteria. Specific Aims: The primary objective of the phase I study is to evaluate the safety and feasibility of abemaciclib and SRS to sites of brain metastases in the management of HR+/HER2- metastatic breast cancer with brain metastases. The primary objective of the phase II portion is to determine PFS intracranially. Secondary objectives include evaluation of extracranial PFS, local and distant intracranial control, and overall survival. Statistical Methods: Safety and feasibility will be monitored in the phase I study using a 3 + 3 design followed by a phase II study to assess intracranial PFS. The phase II study is designed as a single-arm, two-stage trial using the Restricted-Kwak-and-Jung’s method. In the first stage, a total of 21 patients will be enrolled. If pre-specified endpoints are met, an additional 10 patients will be enrolled in the second stage. Neste trabalho, objetivou-se analisar as transformações ocorridas no uso da linguagem por parte de seus usuários tendo como base o período correspondente ao início dos anos 90, momento histórico em que a internet ainda não havia sido popularizada no mundo, em comparação ao ano de 2017, período marcado pelo amplo acesso à internet, principalmente nos países mais desenvolvidos. Para tal, realizou-se uma investigação tendo como base o COCA (Corpus of Contemporary American English) com o intuito de se verificar, através da associação de palavras com seus colocados, como alguns termos eram utilizados antes da popularização da internet e após o mesmo fenômeno. Através da análise estatística dos insumos, foi possível identificar que certos termos da língua (neste caso da língua inglesa) passaram a ser utilizados mais frequentemente para expressar algo relacionado à tecnologia, tendo sido os sentidos anteriores rebaixados, nesta transformação semântica, a uma frequência menor ou muito menor de uso após a realidade do acesso amplo à internet, o que representa uma transformação léxico-semântica propiciada por um fenômeno de alcance global que influencia a vida das pessoas de modo a ressignificar o uso que fazem do mundo e consequentemente a metalinguagem que utilizam nas trocas que realizam com o mesmo.REFERÊNCIASBENSON, M., BENSON, E., ILSON, R. Background:Remission, LDA and LDAS have been proposed as treatment goals for SLE. However, the independent impact of these states on damage accrual has not been fully evaluated.Objectives:To determine the independent impact of remission (both off & on treatment), LDA, and LLDAS on damage accrual.Methods:We studied a long-term longitudinal multinational SLE cohort, including patients completing at least two annual assessments. Remission off-treatment was defined as a SLEDAI (excluding serology) =0, without prednisone and immunosuppressive (IS) drugs. Remission on-treatment was defined as a SLEDAI (excluding serology) =0, prednisone daily dose<=5 mg/d and maintenance IS drugs. LDA was defined as a SLEDAI (excluding serology) <=2, without prednisone or IS drugs. LLDAS was defined as a SLEDAI <=4 with no activity in major organ systems, with no new features of lupus disease activity compared to the previous assessment, prednisone daily dose<=7.5 mg/d and maintenance IS drugs. Antimalarials were allowed in all groups. Damage accrual was ascertained with the SLICC/ACR damage index (SDI). Univariable and multivariable generalized estimated equation (GEE) negative binomial regression models were used. To create mutually exclusive groups, disease activity was divided into five states: remission off-treatment, remission on-treatment (minus remission off treatment), LDA (minus remission), LLDAS (minus remission and LDA) and not-optimally controlled. The proportion of the time that patients were in the specific state at each visit since cohort entry was determined. Possible effect modifiers and confounders adjusted for included sex, age at diagnosis, race/ethnicity, education, baseline disease duration, follow-up time, the highest-ever glucocorticoid dose prior to cohort entry, antimalarials and SDI. Time-dependent covariates were determined at the same annual visit as disease activity state; the outcome was the increase in the SDI and it was assessed at the subsequent visit.Results:There were 1,652 patients, 1464 (88.6%) were female, mean age at diagnosis was 34.6 (SD 13.4) years and mean baseline disease duration was 5.5 (SD 4.1) months. Patients had a mean follow-up of 6.5 (SD 4.3) years, 11686 visits were included. 763 patients (46.2%) had an increase in SDI score ≥1 during follow-up. 2483 (21.2%) of the visits were classified as remission off-treatment, 2276 (19.5%) as remission on-treatment, 544 (4.7%) as LDA, 657 (5.6%) as LLDAS and 5726 (49.0%) as not-optimally controlled. Being in remission off-treatment, remission on-treatment, LDA and LLDAS were predictive of a lower probability of damage accrual [remission off-treatment IRR=0.403, 95% CI 0.301-0.541); remission on-treatment IRR=0.313 (95% CI 0.218-0.451) LDA: IRR=0.469 (CI 95% CI 0.272-0.809); LLDAS IRR=0.440 (95% CI 0.241-0.803)]. Introdução: Disfunção Temporomandibular (DTM) é o termo para designar um quadro de desorganização neuromuscular identificada pela presença de cefaleias crônicas, sons na articulação temporomandibular, restrições dos movimentos mandibulares, hiperestesia e dor nos músculos da mastigação, da cabeça e do pescoço. Objetivo: realizar uma revisão de literatura sobre a DTM. Material e Método: foi feita uma seleção de artigos científicos a partir das bases de dados LILACS e SCIELO utilizando os descritores “Articulação Temporomandibular”, “Transtornos da Articulação Temporomandibular” e “Dor Facial”, usando como critérios de inclusão trabalhos brasileiros e inglês publicados em português e inglês no período de 2000 a 2018. Dos 798 artigos encontrados e delimitados pelos critérios inclusivos, foram selecionados 56 artigos como amostra, que apresentaram a temática elencada para a pesquisa e que foram discutidos nas seguintes sessões: a) Conceitos e epidemiologia; b) Etiologia; c) Sintomatologia; d) Diagnóstico; e) Tratamento. Conclusão: as causas da DTM são multifatoriais e seu diagnóstico deve ser minucioso. Observa-se a importância da anamnese para coleta de dados sintomatológicos da doença e o estudo de cada caso para melhor adequar a técnica de tratamento a ser utilizada. Há a necessidade de avaliações clínicas multidisciplinares nos indivíduos identificados com DTM para que o tratamento seja otimizado, minimizando a morbidade e diminuindo os custos do tratamento. BackgroundDurvalumab consolidation after chemoradiation for unresectable stage III non-small cell lung cancer (NSCLC) improves overall survival. However, up to 25% of patients progress within 18 months following durvalumab consolidation. Little is known regarding the genomic determinants of response to therapy.1 2MethodsWe retrospectively reviewed medical records of 76 patients with stage III NSCLC who received definitive chemoradiation and durvalumab consolidation between 2015–2020 at a large tertiary academic center. Tumor characteristics, molecular profiling, and clinical outcomes including response, progression-free survival (PFS), and overall survival (OS) were documented in an IRB-approved database. Outcomes were assessed by molecular alterations identified from diagnostic biopsy samples using Kaplan-Meier analysis.ResultsOf 76 patients with stage III NSCLC treated with definitive chemoradiation and durvalumab consolidation, 74 were evaluable for PFS and OS. Median age at diagnosis was 66.5 years and 43% were women (n=32). Histology included adenocarcinoma (55%, n=41) and squamous cell carcinoma (32%, n=24). Median follow-up time was 23.0 months from start of durvalumab. The cohort’s median PFS was 15.9 months with 36 patients having documented radiographic progression. Overall survival for the cohort was 32.0 months with 28 deaths. Molecular profiling was performed at time of diagnosis in 35 patients (47%), of which 30 had adenocarcinoma histology. 18 patients had KRAS mutations including KRAS p.G12C (n=8), which were mutually exclusive with 8 patients who had other clinically targetable alterations (EGFR mutations n=1, ALK fusion n=1, RET fusion n=1, MET exon 14 skipping mutation n=1, or ERBB2 mutation n=4). Three patients had non-targetable mutations (BRAF non-p.V600E, STK11, KEAP1) and the remaining six patients lacked an identifiable alteration. There was no significant difference in PFS (p=0.92 by log-rank) or OS (p=0.36 by log-rank) between patients with KRAS mutations, other targetable alterations, non-targetable mutations, or those without molecular profiling. Within patients with KRAS mutations, there was no significant difference in PFS (p=0.33 by log-rank) or OS (p=0.69 by log-rank) when comparing KRAS p.G12C to non-p.G12C mutations.ConclusionsOur study of real-world cohort of patients with stage III NSCLC examined genomic determinants of response to treatment with definitive chemoradiation and durvalumab. Results from this retrospective study suggest that patients with KRAS-mutated tumors derive similar benefit from therapy than patients with other targetable, non-targetable or no identifiable genomic alterations. Future directions for this cohort include analysis of post-progression therapy, subgroup analysis comparing genomic alterations to patterns of progression, and examination of molecular signatures of patients with progression.ReferencesAntonia SJ, Villegas A, Daniel D, et al. The random first order transition theory (RFOT) describing the transition from a supercooled liquid to an ideal glass has been actively developed over the last twenty years. This theory is formulated in a way that allows a description of the transition from the initial equilibrium state to the final metastable state without considering any kinetic processes. The RFOT and its applications for real molecular systems (multicomponent liquids with various intermolecular potentials, gel systems, etc.) are widely represented in English-language sources.
