Abstract:
A fully and symmetrically matched, third-order doubly asymptotic computational boundary (DAA3) is presented in both operator and matrix forms. Its performance is then compared with that of other DAA boundaries for the spherical geometry, which admits an exact series solution. Based on stability, accuracy and passivity as performance criteria, DAA3 is found to be superior to all other DAAs formulated to date. However, there are other third-order forms that are nearly as good, and are easier to implement in boundary-element form. The existence of both DAA2 and DAA3 boundaries enables the analyst to assess solution accuracy for a given problem through the comparison of results produced with these higher approximations of different order.

Abstract:
The objective of this study was to validate the shelf-life of marinated and frozen chicken tenderloins. Treatments were randomly assigned to the age of the tenderloins post-harvest, days aged (DA): DA4, DA5, DA6, DA7, and DA8. Microbial analyses were used to analyze the growth of aerobic, psychotropic, and lactobacilli bacteria to assess the shelf-life of bulk-packaged chicken tenderloins. Tenderloins were sampled fresh, then vacuum tumbled in a marinade. After marination, the tenderloins were sampled with the remaining tenderloins packaged and frozen (−25 °C). After freezing the chicken tenderloins were slacked in a refrigerated cooler (2.2 °C) for up to 132 h (h) and sampled at 36 h, then every 24 h following. After marination, each treatment significantly (p < 0.05) decreased in aerobic and psychotropic counts except DA4. During slacking, no treatment crossed the threshold of 106 CFU/mL (Log 6) set for this study. Though none crossed the threshold, treatments DA4, DA5, and DA6 had significant (p < 0.05) increases in aerobic bacteria after 7 days of age. The psychotropic bacteria continuously grew at each sampling period, with DA4 and DA5 surpassing the other treatments (p < 0.05) at 108 h and 132 h reaching 105 CFU/mL. Every treatment remained below the spoilage threshold, suggesting that this method of storage is suitable for chicken tenderloin shelf-life.

Abstract:
For assessing physiological maturity through pod and seed developmental patterns in local landraces of green gram (Vigna radiata L. Wilczek), a sufficient number of flowers were tagged on the day of anthesis for all the genotypes. Ten fruits were harvested per replication for each genotype of five harvesting stages starting from 7 days after anthesis and at seven-day intervals. The harvested pods and seeds were utilized for the study. Enhancement in pod length, weight, no of seed/pod and 1000 seed weight was noted up to 28 DAA in most of the genotypes. Continuous increase in seed fresh weight was noted up to 28 DAA whereas dry weight continued to increase till 35 DAA in correspondence with loss in moisture. Maximum dry matter accumulation was between 14 DAA-21 DAA. Significant moisture loss was recorded for all genotypes from 21 DAA-35 DAA. The highest germination was achieved at 35 DAA for all genotypes except V1 and V4 for which the highest germination was recorded at 28 DAA. Enhancement in shoot length was up to 35 DAA for all genotypes whereas for root length it was till 28 DAA for most of the genotypes. Vigour Index 1 was noted to be constantly enhanced with progress in development up to 35 DAA for most of the genotypes whereas an increase in vigour index II was 28 DAA except V5. Germination and vigor index decreased after 28 DAA in 2 genotypes and 35 DAA for 3 genotypes indicating attainment of physiological maturity between 28-35 DAA.

Abstract:
Background: Despite the global increase in popularity for the use of the Direct Anterior Approach (DAA) for Total Hip Arthroplasty (THA), the current beliefs and reasons for its use and disuse amongst South African orthopaedic surgeons is not well understood. Objective: To determine the perspectives regarding DAA compared to other surgical approaches to THA. Methods: We conducted an anonymous online survey sent to all current members of the South African Orthopedic Association (SAOA). Results: The response rate was 24.25% (n=194). There were 76 (39.18%) respondents that have performed DAA (DAA Performers) and 118 (60.82%) that have never performed DAA (DAA nonperformers). A proportion of 50% and 11.84% of DAA performers were between 30 to 45 years and older than 60 years of age, respectively (p<0.000). The DAA is the preferred approach to THA for 36.84% (n=28) of DAA performers, whilst 63.16% (n=48) prefer an alternative approach. Both DAA performers who prefer DAA and those who prefer alternative approaches consider DAA more satisfactory for length of hospital stay (p<0.000) and short-term functional outcomes (p=0.002) compared to other surgical approaches. For DAA non-performers, the primary reasons for not performing DAA THA were inexperience in surgical technique (72.09%), increased operative time (12.79%), unsatisfactory outcomes (27.91%) and learning curve (67.44%). There were 5 (18.52%) and 30 (70.33%) surgeons who have been in clinical practice for more than 10 years that do and do not intend to use DAA in the future, respectively (p<0.000). Conclusion: The trends of the adoption of the DAA by South African orthopaedic surgeons show increasing popularity, similar to patterns seen in the United Kingdom and the United States of America. Younger surgeons are more likely to perform the DAA THA whilst more experienced surgeons are less likely to deviate from their current standard of practice.

Abstract:
Background Unrestricted access to direct-acting antiviral (DAA) therapy for hepatitis C virus (HCV) infection has been available in Australia since March 2016. Individuals with HIV–HCV co-infection are at a greater risk of liver fibrosis progression. This study estimated DAA treatment uptake among individuals with HIV–HCV co-infection, during the first year of DAA treatment access in Australia. Methods: Pharmaceutical Benefits Scheme (PBS) data on dispensed DAA and antiretroviral therapy (ART) prescriptions from March 2016 to March 2017 were used for analysis. Results: During March 2016 to March 2017, a total of 935 individuals with HIV–HCV co-infection were receiving ART and initiated DAA treatment, with 93% to 97% completing their prescribed course. Estimated DAA treatment uptake in the HIV–HCV-infected population was 41% (935/2290). Most were men (94%). Median age was 50 years. DAA treatment was initiated by specialists in 64% of cases (n = 602), and by general practitioners (GPs) in 25% of cases (n = 238). The proportion of individuals initiated on DAA by GPs increased from 20% in March–April 2016 to 26% in January–March 2017. Most specialists (77%) and GPs (72%) initiated DAA treatment for one to three patients. Among individuals initiated on DAA by GPs, 68% received their ART prescription from the same GP. Conclusions: A high level of DAA treatment uptake and completion was observed among individuals with HIV–HCV co-infection during the first year of DAA treatment access. The proportion of individuals prescribed DAA by GPs increased over time; this is important for broadened access.

Abstract:
The thorny issue of membrane biofouling in membrane bioreactors (MBR) calls for new effective control measures. Herein, D-amino acid (DAA) was employed to mediate MBR membrane biofouling by inhibiting biofilm information and disintegrating formed biofilm. Different DAA control ways involving membrane property, DAA-adding timing, and DAA-control mode were explored through experiments and the multiple linear regression model and the response surface methodology. The optimized DAA control ways were acquired, involving DAA used as an active agent, and the DAA-adding timing of 4 h cultured before running, as well as both hydrophilic and hydrophobic membrane, resulting in an approximately 40.24% decrease in the membrane biofouling rate in comparison with the conventional MBR. DAA is an efficient membrane biofouling mediating approach for MBR under optimized control ways combination and a facile solution for solving membrane biofouling in actual membrane systems.

Abstract:
Double aortic arch (DAA) is an extremely rare congenital anomaly that can be divided into right dominant, left dominant, and balanced DAA according to the relative size of the two arches. The incidence of balanced DAA is only 5% among double arch anomalies. DAA is symptomatic only when it produces symptoms secondary to compression of the trachea or esophagus. DAA is rarely associated with other congenital heart diseases. In this report, we present a rare case of asymptomatic DAA combined with Tetralogy of Fallot (TOF) in an 8-month-old girl.

Abstract:
Gastric cancer is a malignant tumor with a high incidence and mortality rate worldwide. Nevertheless, anticancer drugs that can be used for gastric cancer treatment are limited. Therefore, it is important to develop targeted anticancer drugs for the treatment of gastric cancer. Dehydroabietic acid (DAA) is a diterpene found in tree pine. Previous studies have demonstrated that DAA inhibits gastric cancer cell proliferation by inducing apoptosis. However, we did not know how DAA inhibits the proliferation of gastric cancer cells through apoptosis. In this study, we attempted to identify the genes that induce cell cycle arrest and cell death, as well as those which are altered by DAA treatment. DAA-regulated genes were screened using RNA-Seq and differentially expressed genes (DEGs) analysis in AGS cells. RNA-Seq analysis revealed that the expression of survivin, an apoptosis inhibitor, was significantly reduced by DAA treatment. We also confirmed that DAA decreased survivin expression by RT-PCR and Western blotting analysis. In addition, the ability of DAA to inhibit survivin was compared to that of YM-155, a known survivin inhibitor. DAA was found to have a stronger inhibitory effect in comparison with YM-155. DAA also caused an increase in cleaved caspase-3, an apoptosis-activating protein. In conclusion, DAA is a potential anticancer agent for gastric cancer that inhibits survivin expression.

Abstract:
This study investigated the vasoactive effects of des-aspartate-angiotensin-I (DAA-I) in male Wistar rats on whole body vascular bed, isolated perfused kidneys, and aortic rings. Dose–response curves to DAA-I were compared with those to angiotensin II (Ang II). The Ang II-type-1 (AT1) receptor blocker, losartan, was used to evaluate the role of AT1 receptors in the responses to DAA-I. Studies were also conducted of the responsiveness in aortic rings after endothelium removal, nitric oxide synthase inhibition, or AT2 receptor blockade. DAA-I induced a dose-related systemic pressor response that was shifted to the right compared with Ang II. Losartan markedly attenuated the responsiveness to DAA-I. DAA-I showed a similar pattern in renal vasculature and aortic rings. In aortic rings, removal of endothelium and nitric oxide inhibition increased the sensitivity and maximal response to DAA-I and Ang II. AT2 receptor blockade did not significantly affect the responsiveness to DAA-I. According to these findings, DAA-I increases the systemic blood pressure and vascular tone in conductance and resistance vessels via AT1 receptor activation. This vasoconstrictor effect of DAA-I participates in the homeostatic control of arterial pressure, which can also contribute to the pathogenesis of hypertension. DAA-I may therefore be a potential therapeutic target in cardiovascular disease.

Abstract:
Background and Aims: The impact of antibody responses following direct-acting antiviral (DAA) therapy in hepatitis C virus (HCV)-infected recipients before and after liver transplantation (LT) is still undetermined. Methods: In this observational cohort study, we aimed to explore the association between changes in anti-HCV antibody titers following pre-LT DAA therapy and allograft injury, including biliary complications (BCs) and acute cellular rejection (ACR). Results: A total of 153 cases were enrolled from January 2015 to February 2021. Serum anti-HCV antibody titers were assessed before and after (day 30) LT. Among all recipients, 31/153 (20.3%) had pre-LT DAA therapy (the DAA group) and 122/153 (79.7%) did not undergo pre-LT DAA therapy (the DAA-naïve group). A higher incidence of post-LT BCs was observed in the DAA group (p = 0.028). Compared with the DAA-naïve group, the DAA group had a significantly higher mean level of anti-HCV titer upregulation (p = 0.0024); furthermore, among the recipients with BCs (n = 28) and ACR (n = 41), those in the DAA group exhibited significantly higher mean levels of anti-HCV antibody titer upregulation (p < 0.005). Conclusions: In conclusion, we speculate that the upregulation of anti-HCV antibody titers, which might have been induced via the restoration of HCV-specific immune responses through pre-LT DAA therapy, was associated with post-LT allograft injury.

Abstract:
Background Hepatitis C (HCV) in South Africa is incompletely characterised and understood. Epidemiological and clinical data will better inform our understanding and assist national policy decision making. On the background of more than two decades of clinical challenges in HCV management, the advent of direct acting antivirals (DAA) now makes HCV elimination plausible. To better understand the base from which we come, we elected to review and characterise our HCV experience at Groote Schuur Hospital (GSH) in the Pegylated interferon (Peg-IFN) and Ribavirin (RBV) management era. Methods Patients with chronic HCV attending GSH Liver Clinic from 2002 to 2014, were included, in the analysis. Relevant data were extracted from a registry and existing clinical records accessed. Two brands of Peg-IFN were available and those treated with the first generation add-on protease inhibitor, telaprevir, were included. Results 238 patients were included in the analysis, median age of 47 (IQR 37-58) years, men 60.5%. Men were significantly younger than women, 43.5 (35-52) vs 55 (42-64) years, respectively, p< 0.0001. Ethnically, the majority were white (55.9%) or mixed-ancestry (21.8%), 16.4% were HIV co-infected, 3.7% hepatitis B (HBV) co-infected and 0.4% triple infected with HCV, HBV and HIV. The most likely mode of HCV acquisition was blood/blood product exposure prior to 1992 (32.8%) and injecting drug use (IDU) 17.6%, while 30.3%, had no clear risk factor identifiable. Genotypes (GT) 1 to 5 were observed with GT-1 (34.9%) predominating. In those biopsied, (n=90), 30% ≥F3 fibrosis, with 15.6% cirrhotic. With IL28B polymorphisms, heterozygous CT (23.9%) and CC genotype (15.5%), were most frequent. 32.6% accessed Peg-IFN/Ribavirin-based therapy, 6.5% (n=5) with add-on telaprevir. GT-1 (35.1%) was most prevalent in the treatment group, followed by GT-3 (26%) and GT-5 (18.2%); 10% were HIV co-infected. Overall SVR rate was 75.3% with 37% of GT-1 not achieving SVR; 49.4% experienced adverse events including cytopaenias (32.5%) and depression (15.6%) with 15.6% requiring erythropoietin for anaemia and 15.6% GM-CSF for neutropaenia. Conclusion HCV patients in the Peg-IFN/Ribavirin management era typified the epidemiology of HCV. GT distribution was pangenotypic and treatment outcomes were encouraging despite treatment challenges. Patient selection, IL28B and sensible cytopaenia support, likely accounted for this. However numbers treated were limited and the DAA era of therapy allows for a rapid expansion of therapy with now growing numbers of patients and a changing local epidemiology.

Jayaraman, Jayakumar. "Dental age assessment (DAA) : development and validation of reference dataset for southern Chinese and its application to East Asian populations." Thesis, The University of Hong Kong (Pokfulam, Hong Kong), 2014. http://hdl.handle.net/10722/207191.

Abstract:
Age assessment which is an integral part of forensic and clinical practice when assessed using the extent of dental development has proven to be more accurate than other methods. Variations in dental development have necessitated the construction of ethnic specific reference datasets (RDS) to ensure accurate age assessments. Age estimated from ethnically different RDS in southern Chinese subjects has been shown to be inaccurate. A systematic review and meta-analysis from the most commonly used French-Canadian dataset revealed consistent over-estimations of age of global population groups, inferring the need for population specific RDS. A study which compared a group of 400 five years old children born in the 1980s and the 2000s demonstrated that children born in recent decades have more advanced dental development. Hence, only the most recent samples were included in the construction of a RDS for southern Chinese using dental panoramic radiographs of 2306 subjects. The reference dataset was subsequently validated on 484 subjects of southern Chinese origin by conducting dental age assessments (DAA) using un-weighted and weighted methods of dental age calculations. Paired t-test demonstrated that all methods of assessments were able to accurately estimate the age (p>0.05). The overall age differences ranged from -0.01 to 0.11 years for males and -0.03 to 0.10 years for females respectively. In addition, to test the accuracy of different ethnic datasets, 266 southern Chinese subjects for whom age had been estimated using the UK Caucasian and French-Canadian datasets were re-scored using the southern Chinese RDS. The latter was able to estimate the age of 80% of the subjects within a range of 12 months and the importance of population specific reference standards was elucidated. The validated southern Chinese RDS on dental development can thus be used to estimate the age of children and young adults of southern Chinese origin. This RDS was also tested for applicability on the records of 953 subject obtained from Japan, Thailand and Philippines. A similar method of validation was conducted and the southern Chinese RDS estimated the age of Thai males, Filipino and Japanese subjects with a reasonable degree of accuracy. The genetic similarity between the southern Han Chinese and the other East Asian population groups may account for the obtained accuracy. The secular trend study was the first of its kind study in Asia that demonstrated advanced dental maturation in children born in recent decades. Natural calamities that strike East Asia leave thousands of people missing. In those circumstances, dental age assessment using the southern Chinese RDS would help in the process of identifying deceased victims. Furthermore, only half of the children in the world below the age of five years are registered; thus the need for determining age is of foremost importance to safeguard them against age specific crimes. Methods of establishing reference datasets and conducting accurate age assessments that have been investigated and tested in this study indicate that the methodology can be applied to any ethnic population group in the world.
published_or_final_version
Paediatric Dentistry
Doctoral
Doctor of Philosophy

Abstract:

Abstract:
The hepatitis C virus (HCV) treatment landscape has rapidly changed over recent years. The development of direct-acting antiviral (DAA) agents that specifically target various steps in the HCV lifecycle has revolutionized therapeutic options for patients with HCV, with the development of highly effective and well-tolerated oral interferon-free (IFN-free) regimens. There are several DAAs that have been approved and others that are currently in development, targeting different nonstructural HCV or host proteins that are essential for HCV replication. Data on virological response in different HCV genotypes and patients subgroups are well established in the literature, while results on clinical outcome are much more limited. In this thesis, I will describe real-life virological and clinical outcome with DAA therapy in a large cohort of patients with HCV treated in clinical practice in the Veneto Region. For this purpose, we have used the NAVIGATORE web-platform that is recording and monitoring all HCV-DAA based treatments in the Veneto region and we have assessed virological and clinical outcomes of DAAs treatment in 3 subgroups of patients: 1) HCV infected cirrhotics (both compensated and decompensated) to clarify drugs efficacy and clinical outcomes, including long term benefits of treatment, 2) Patients with extrahepatic manifestations due to HCV and 3) patients with HCV aged > 70 years. Our analysis has been based on a cohort of 2946 cirrhotic patients. DAAs were safe and efficacious in these patients, including those with advanced, or decompensated cirrhosis. SVR rates for most genotypes were excellent, but the initially used Sofosbuvir plus Ribavirin regimens were suboptimal in HCV-1 and HCV-3 infected patients. Efficacy was not significantly affected by common comorbidities but it was reduced in patients with Child B cirrhosis. Severe adverse event were unusually: tolerability and safety were excellent in patients with Child A cirrhosis, while were reduced in Child B patients, particularly when treated with Ribavirin containing regimens. Patients with decompensated cirrhosis often showed an improvement in Child scores and patients with compensated cirrhosis showed a reduced incidence of complications, compared to the expected rates in the natural history of untreated disease. The "de novo" incidence of HCC was also reduced during and after DAA and the incidence of HCC recurrence not increased during and after DAAs. A subgroup of patients developed an aggressive type of HCC during therapy, and this was more frequent in non-responders, an observation that certainly deserves further evaluation. Extrahepatic manifestations of HCV infection also improved with successful antiviral therapy. Moreover, a sub-analysis in a group of patients over 70 years, the "elderly group", confirmed excellent safety and efficacy of DAAs also in this setting.

Abstract:
Review question / Objective: To assess the possible cause of events, the incidence of grade 3 hyperglycemia after treating Sofosbuvir/Velpatasvir/Voxilaprevir in HCV infections. Condition being studied: Sofosbuvir, velpatasvir, and voxilaprevir (SOF/VEL/VOX) is an effective, safe rescue therapy regimen for patients have previously been treated failure. Initiating Direct-Acting Antiviral (DAA) treatment for HCV infection with diabetes have experienced hypoglycemia, it could improve insulin resistance due to clean HCV. However, some studies shown that SOF/VEL/VOX has Grade 3 hyperglycemia adverse events. This finding contradicts that other DAAs studies. Information sources: Conducting a comprehensive literature search on the pubmed, Cochrane, clinicalkey, Embase, and MEDLINE electronic databases.
