Full textThe purpose of the project was to create time-resolved three-dimensional sequences, so called 4D sequences, using computed tomography (CT) with low radiation dose and sufficient resolution and to start an implementation of this method at Karolinska University Hospital in Huddinge. To get as good result as possible a number of people with different medical professional background was involved. A phantom study was performed by moving a phantom with a constant velocity through the CT using a motor. After this, a pilot study was performed by scanning the right foot of two people who performed a movement for various lengths of time during a CT-scan. The scanning of the feet was done after the phantom gave adequate result with respect to image quality and radiation dose. To determine this, analyze of the images of the phantom and a calculation of the dose was made. The scanning of the feet generated interpretable images with sufficiently low radiation dose. This method will certainly be used in teaching and research purposes. It can also be used clinically at Karolinska University Hospital in Huddinge in the future, for example for the diagnosis and rehabilitation of tendons.

Full textIntroduction: The term colorectal cancer (CRC) means cancer of the colon and rectum.Colorectal cancer is the third leading cause of death worldwide with statistically rising mortality and affects thousands of people every year. The disease is investigated with diagnostic imaging where the most common is colonoscopy but also with computed tomography (CT) if the colonoscopy is incomplete or if the patient is unable to undergo a colonoscopy. The examination requires accurate bowel cleansing as well as a limited food intake, which leads to physical as well as psychological concern in the patient. Adequate information before, during and after the examination is necessary to facilitate the patient's mental and physical condition. Aim: The purpose of the study was to compile and illustrate the state of knowledge regarding patients' experiences before and during a CT colon (CTC) examination. Method: The literature study was conducted as a general literature review. Thirteen articles with both qualitative and quantitative approaches were included in the study after a quality review of all.Results: Four main findings were identified in the result. These consisted of physical efforts, concern and anxiety, embarrassment and reluctance for the study related to ethnicity and gender, and the acceptance of discomfort with the study itself. Conclusion: Many of the patients experienced pain during the examination which was related to bowel dilatation. Since many of the patients experienced that the examination was embarrassing and physically stressful, it is very important to have adequate information and a good dialogue. This can reduce their concerns, provide security and increase the manageability of the investigation. It is of utmost importance not to violate the patient's integrity and to provide encouragement during ongoing examination as well as after.

Full textIntroduction: For patients in need of urgent medical care, the time to diagnosis and treatment is an important factor. The role of the X-ray nurse is important in emergency diagnostics as they are responsible for conducting X-ray examinations. Today, mobile X-ray services, primarily conventional X-ray examinations, are for instance available for unstable patients. These services offer rapid diagnosis, but many urgent issues require examinations using a computed tomography (CT). Aim: The aim of this study was to compile the pros and cons of mobile CT in acute care diagnostics. Method: The study was conducted as a literature review. A systematic literature search was conducted in three medical databases. In total, ten quantitative articles were quality-reviewed and analyzed. Results: It appears in the results that the advantages of a mobile CT are several, such as reduced time for the diagnosis and treatment and reduced workload for the healthcare personnel. A mobile CT also counteracts transport-related complications in high-risk patients. However, it also states that the disadvantages, such as increased radiation dose, inferior image quality and increased costs, should be taken into account. Conclusion: The mobile CT has been shown to increase patient safety, which is an important factor in healthcare. The device, despite its drawbacks, has a great potential to facilitate diagnostics in patients in critical situations. Hopefully, this device will be further developed to be used regularly.

Full textComputed tomography (CT) scan of the colon has become a common investigation in radiology departments. Prior to this examination preparation in form of purgation and food restrictions are required. Information to patients facing this examination are of great importance for the results of preparations and thus for optimal investigation. The aim of this studie was describe whether patients feel that they received sufficient information about the bowel preparations before a CT scan of the colon. Another purpose of the study was to see whether patients understood the information they receive before the examination. Ten patients with an age between 62-81 years were interviewed after the CT colography examination at the radiology department in a hospital in the middle of Sweden for a period of two weeks. The collected material was then analyzed by qualitative content analysis. The interviewed patients felt that information on preparations for the CT colonography examination was sufficient. Patients can literally retell the information received but there is a lack of understanding of the information that deals with the definition of clear liquids and the approach of certain medications. Most patients understood the purpose of the bowel preparations performance before the examination. The study showed that there were unclarities in the written infomrmation given before the CT colonography examination that the patients did not understand, even though they considered that the information was sufficient. By taking into considerations the lack of information about bowel preparation that the studies results show there are subject to examination, preparation and implementation is carried out in the best manner.

Full textBackground: A hip fracture is occult when it is not visible on plain x-rays. About five percent of all cervical hip fractures are occult. To diagnose the occult hip fracture, the patient must undergo further examination with another modality. Computed tomography can be such a potential examination since it gives a clear picture of the patient’s anatomy. Delay to surgery increases the risk of mortality while the patient is hospitalized, and it is therefore important that the patient get the diagnosis as quickly as possible. Purpose: To create an overview of the knowledge situation regarding computed tomography in the diagnosis of occult hip fractures not detected by plain x-rays. Method: The study was conducted as a general literature review. Ten scientific articles of quantitative method were included. Results: Computed tomography did not detect all occult hip fractures and in many cases patients had to undergo further examination. The common meantime of the sensitivity based on four of the scientific articles was 83.3 % and the common specificity was 99.15 %. Of the remaining six articles computed tomography missed a total of 15 occult hip fractures in 595 patients. The meantime after the first examination with plain x-ray until the patient received a computed tomography differed between the patients and studies. The fastest meantime was 3 hours and 42 minutes. Conclusion: Computed tomography does not detect all occult hip fractures, and in many cases patients need to undergo further examination for the correct diagnosis.

Full textABSTRACT Background The number of people suffering from colorectal cancer increases every year and is the third most common cancer disease in Sweden. Therefore the demand increases for the examination methods in order to be able to make a correct diagnosis and start treatment. CT Colonography is one of the methods often used as a secondary method. Aim The purpose of this study is to examine what significance CT Colonography has in the investigation of colorectal cancer. Method This study is a systematic review. The collection of data were made from the databases PubMed and CINAHL. Studies which fulfil the inclusive criteria and answered the aim of this study were quality reviewed and analyized. Result CT Colonography can show colonic findings and is particularly useful to locate extracolonic findings. The method is important in the preoperative evaluation because of its exact localisation rate and is a good compliment to colonoscopy. CT Colonography is a reliable method with a sensitivity at 94–99.9 % which increases when CAD is used as a reading method, especially in the detection of pathology >10 mm. Conclusion CT Colonography is a valuable method in the investigation of colorectal cancer and other pathologies in the abdomen, colon and rectum. It’s a reliable method with a high sensitivity which increases with CAD as a reading method. A strong advantage of this method is that the whole abdomen is visible in the images so the localisation of extracolonic findings can be detected. Full textIntroduction: In health care, methods have been developed whereby the medical staff uses ionizing radiation, so-called X-ray technology. This type of ionizing radiation used in various X-ray machines is produced artificially. Cells in the process of dividing and cells that are developing quickly are more sensitive to radiation than fully developed cells. As a developing fetus has more cell division than younger people, adults and elderly, it is more sensitive to ionizing radiation. Examining pregnant women is a greater challenge for radiographers, having to take radiosensitive fetuses into account. A licensed radiology nurse is responsible for safe and secure care. Patient safety is one of the greatest responsibilities of the radiology nurse, striving for the best diagnostic image quality with the lowest possible radiation dose. Purpose: The purpose of this literature review was to compile an overview of which factors affect the radiation dosage transferred to the fetus, which radiation doses the fetus recives and the effects hereof, when using ionizing radiation in a radiology department. Method: The study was performed as a literature review. Eight scientific articles were reviewed, analyzed and the results were presented in categories. Results: Different factors affecting the radiation dose transferred to a fetus were identified. Fetal development stage, the fetal position, the pregnant woman's size and technical settings were the factors that were important to consider because they affect the radiation dose. Conclusion: By having knowledge of the factors affecting the radiation dose impacting the fetus the radiology nurse is able to work with focus on the patients safety and mediate security to the patient.

Full textStaff who has to attend CT-trauma examination is exposed to scattered radiation. The eye is sensitive to ionizing radiation and there is evidence that cataract and radiation are correlated. At CT-multitrauma long exposures and many projections is taken. The purpose of this study is to examine how much scattered radiation the staff is exposed to. How many times/year can one person in the staff attend a CT-trauma-examination before the ICRP recommendation dose of 20 mSv/year is reached? It is a quantitative study that has been done empirically at two hospitals with a phantom. A Siemens 128 slice and a Toshiba 80 slice were used in the measurements. A trauma protocol was used. One test measurement was done before the real measurements with dosimeters from Landuer placed at seven measure points at eye level at different distances from the gantry. The scattered radiation was highest close to the gantry and decreased with the distance except from two points that had lower measure results and this can be explained by the angle between the gantry and the dosimeter. It’s the radiographer’s responsibility to inform staff of the best place to stand at CT-trauma-examinations and to inform of the importance of alternating staff.

Full textSyftet med detta arbete var att utreda felkällor som förekommer vid användning av olika tekniker/modaliteter vid bäckenmätning. Då det finns risker med röntgenstrålning önskar man reducera stråldosen vid undersökningarna samtidigt som man vill använda sig av en enkel metod. De olika modaliteterna som jämförs är i) direktdigital detektor och bildplatta, ii) bildförstärkarteknikteknik, iii) CT. Den modalitet man använder sig av idag är direktdigital detektor och bildplatta. För att korrigera för förstoring som uppkommer vid röntgenundersökningen placeras en mätsticka i bildfältet. Om mätstickan vinklas mot sagittalplanet blir korrigeringen fel och detta visas i teori och praktiska försök. Bildförstärkare kan användas för att ta sidobilden. Bildförstärkarens geometriska egenskaper ger dessvärre en måttdistorsion så att objekt i det perifera bildfältet blir förstorade mer än objekt centralt belägna. Det har inte framkommit något bra sätt att korrigera för distorsionen. CT har inte använts vid bäckenmätningar på Helsingborgs lasarett tidigare. Att använda topogrammet vid CT- undersökning kan även det vara behäftat med längddistorsion och förslag ges hur man kan korrigera för felet. I CT-snittbilder fås ingen längddistorsion.

Full textBackground: Many patients who participate in CT-colonography find the preparations and the examination itself to be hard. A plain and well written information letter, as well as good communication and patient-centered care from the hospital staff could make the patient experience better. Aim: The aim of the study is to investigate how patients from two different hospitals perceive both the information letter and the oral information from the staff and compare to each other. Method: The study is a qualitative study. Patients have been asked to fill out a survey at the end of their examination. The patients have been strategically selected, inclusion- and exclusion criterias have been adapted. The result of the survey has been analyzed in SPSS. Results: In general the patients were pleased with the information letter. The patients at hospital B were to some extent more satisfied than the patients at hospital A. The majority of the patients from both hospitals were satisfied with the information from the hospital staff. Although the result showed some percentage difference between the hospitals, no significant difference was shown. Conclusion: Overall the participants of the study were satisfied with both the information letter and the information from the hospital staff. Although there were no significant differences, the result showed that the patients from hospital B were more satisfied than the patients from hospital A in all senses.

Full textIn the background the concept of multi-trauma is described and this is the most common cause of death in Sweden between the ages of 18–45. In multi-trauma situations occupational categories within the health care system work together for the rapid care of the patient and for the urgent diagnosis computed tomography is used. The purpose was to investigate radiographers’ experiences of the psychosocial and physical working environment in multi-trauma situations. The method was an empirical study based on mixed methods in which twelve radiographers from different hospitals participated in a web survey. The result showed that the working environment in multi-trauma situations is perceived to lead to increased stress and disturbed concentration at work, while the routines largely worked well. The ergonomic way of working was perceived as well-functioning but also not prioritized among the radiographers and in the physical environment space was lacking. The conclusion is that there is room for improvement for radiographers’ work environment in multi-trauma situations regarding psychosocial and physical work environment.

Full textHalvledardetektorer är av stigande intresse inom forskning för användning i fotonräknande datortomografi med spektral upplösning. För att erhålla en hög spatiell upplösning är det av intresse att hitta fotonens ursprungliga interaktionsposition. I detta arbete undersöks om maskininlärning kan användas för att erhålla en spatiell upplösning på subpixelnivå i en fotonräknande kiselstrippdetektor med 10 µm pixlar. Laddningsfördelningen från simulerade interaktioner i en, tre, och sju positioner inom var och en av tre pixlar undersöktes med hjälp av applikationen Classification Learner i MATLAB® för att bestämma den korrekta interaktionspositionen. Olika maskininlärningsmodeller tränades och testades för att maximera prestandan. När pulser från en och sju positioner inom pixeln användes, kunde modellen hitta den korrekta pixeln med en noggrannhet på 100% respektive 88.9%. Vidare kunde den korrekta positionen inom en pixel bestämmas med en noggrannhet på 54.0% och 29.4% när tre respektive sju positioner inom varje pixel användes. Resultaten visar att det skulle vara möjligt att förbättra den spatiella upplösningen med hjälp av maskininlärning.

Full textThe number of computed tomography (CT) examinations is increasing in Sweden and worldwide. A disadvantage of this method is high radiation doses that can cause stochastic effects (cancer) on patients. The most radiation-sensitive patients are children. They have faster cell division and longer life, which means that cancer has a greater chance of developing in the future. Aim: The aim of this study is to elucidate health risks that can occur in ionizing radiation in CT with focus on children and young adults. Method: A literature review was conducted where the articles were selected from the two databases PubMed and CINAHL. Ten articles were quality checked and the results of the selected articles were analyzed.Results: The results showed that CT examination conducted during childhood could lead to an increased risk of malignancy and deterministic injuries. There is a direct correlation between the amount of radiation dose, age and sex, when exposed to ionizing radiation from CT and the risk of cancer. The higher the dose and the younger the patient, and in girls overall, there is a greater risk of suffering from cancer. Conclusions: Due to health risks that occur after exposure to ionizing radiation, a great responsibility is required of both radiologists and X-ray nurses, to minimize the damage in children. Therefore, it is also of great importance that all investigations carried out within CT are justified and optimized so that the benefit of carrying out the investigation should be greater than the damage.
