Full textThe face attractiveness is directly related to self-esteem and interpersonal relationships, involving different areas of health, such as medicine, dentistry and also Speech Pathology, but with different criteria. Considering that the muscular action involved in orofacial functions is dependent on the structure in which it operates, some studies have been conducted, linking form and function, but few relate to aesthetic muscular action involving the face and skin. Thus, this study sought to ascertain the influence of the orofacial functions in the aesthetic aspects of individuals with dentofacial deformity. We analyzed data from 36 subjects with average age of 27 years old. These were divided into three groups, matched according to the gender and age, participating 12 individuals with dentofacial deformity pattern II (DFD-II), another 12 pattern III individuals (DFD-III) and a group of 12 individuals with dentofacial balance, which constituted the Control Group (CG). The evaluation was performed by three expert examiners in Orofacial Motricity. The facial and orofacial myofunctional aspects analysis was done from photographs and recordings, according to the MBGR protocol. For the analysis of the aesthetic aspects it was used the Glogaus classification of wrinkles protocol, the photonumeric scale for classifying the marionette fine lines and the photonumeric escale for grooves in the musculature of the eyes. From the classification of the three examiners, the result obtained was a consensus. The comparison among the results from the different groups was performed applying the Kruskal Walliss test followed by Millers test, has been found a higher incidence of myofunctional alterations for DFD groups II and III compared to CG (p<0.05). In addition, it was studied the relationship between the myofunctional aspects and facial analysis, as well as the aesthetic and myofunctional aspects, the facial analysis and aesthetic aspects of the face, besides the correlation of the aesthetic aspects among themselves, using the Spearman correlation test. It was observed a significant correlation between the facial analysis (symmetry of the face and nasolabial angle) and the orofacial mobility, breathing, chewing, swallowing and speech functions (p<0.05), with no statistically significant correlation between facial analysis and aesthetic aspects, except for the nasolabial angle and furrows on the forehead (p<0.03). Between the aesthetic facial analysis and myofunctional aspects it was not found correlation statistically significant. There was a correlation between aesthetic aspects, namely: furrows in the procerus and vertical grooves on the lips, bumps on the musculature of the eyes and nasolabial folds, as well as grooves in the eyes muscles and marionette fine lines (p<0.01). Therefore, it is concluded that the orofacial functions influence the face symmetry, and it was also verified a relation between the nasolabial angle and the orofacial functions in individuals with dentofacial deformity. Furthermore, the aesthetics aspects of the upper third face correlated with the lower third, in the individuals in this study.

Ser portador de uma marca corporal depreciativa contribui para a estruturação de um esteriótipo, onde o estigmatizado, adequando-se ao que lhe é atribuído, sofre com a perda de sua própria identidade. Objetivo. Identificar as representações simbólicas referentes ao estigma em pacientes com câncer de cabeça e pescoço e deformidades faciais visíveis por onco-cirurgias. Metodologia. Estudo qualitativo, realizado no Hospital de Câncer de Pernambuco, no período de janeiro a maio de 2009. Resultados. A amostra constituiu-se de 30 indivíduos predominantemente do sexo masculino (53,3%), na faixa etária entre 41 e 50 anos de idade (23,3%), agricultores (40,0%) e com pouca escolaridade (46,6%). Em relação ao estigma dos pacientes por terem adoecido de câncer, identificou-se que eles consideravam a doença misteriosa e difícil de ser curada. Diferentes estratégias de enfrentamento da doença foram observadas em ambos os sexos a exemplo de: barganha com o divino, negar, esconder ou relativizar a gravidade do estado de saúde. Para o estigma frente à deformidade facial, este foi simbolizado como sinônimo de feiúra. Quanto à estigmatização percebido em familiares e amigos, a maioria dos homens interpretava os comportamentos como reveladores de apoio e suporte emocional. Já as mulheres demonstraram muito mais sensibilidade e perspicácia para identificarem comportamentos de estigma do tipo: pacto de silêncio ou verbalizações de pensamentos excessivamente altruístas imcompatíveis com o quadro clínico do doente. Para os estigmas observados em conhecidos e pessoas estranhas predominou o incômodo causado pelo olhar insistente, a compaixão e a curiosidade alheias. Conclusão: Superar comportamentos estigmatizadores representa um desafio considerável na sociedade estimulada pelas indústrias da beleza e da moda. Esforços serão precisos para se resgatar os valores éticos de igualdade, fraternidade, solidariedade e compaixão frente aqueles que apresentam diferenças indesejáveis

Full textThe masticatory function is the one of the main constituents of stomatognathic system and necessarily precedes the absorption of nutrients by the human organism. Bite force is a component of this function and a clinical parameter of this system\'s health. In dentofacial deformity, chewing is impaired due the imbalance of skeletal and muscular maxillomandibular relationships and of the occlusion. Three-dimensioal investigative approach is unique and has the advantage about the evaluation of facial morphology in the three planes of space, such as the configuration of the human face and dentofacial deformity. Therefore, the objective of this study was to analyze the variation of facial morphology in different three-dimensional measurements, as well as to verify possible asymmetry in bite force and the difference in the distribution of force in dental arc and occlusal contact area between subjects with dentofacial deformities class II, class III and control. The study was performed with 77 subjects who composed two different groups: dentofacial deformity (DDF) and control (CG). The group of dentofacial deformity (DDF) was subdivided according to its classification. Thus, 20 class II subjects (4 men and 16 women, mean age of 26 years old) were included in the DDF II subgroup, 32 class III (15 men and 16 women, mean age of 27 years old) formed DDF III, and 25 healthy volunteers (9 men and 16 women, mean age of 24 years old) comprised the CG. All were submitted to the threedimensional assessment of the facial morphology by laser scanning, bite force measurements by gnatodynamometer, analysis of occlusal force distribution and occlusal contact area by the T-Scan system. Comparisons between CG and DDF groups were performed by Analyses of Variance (ANOVA) for independent samples. Spearman\'s correlation test was applied to evaluate the possible association between bite force and anthropometric measurements, as well as multiple linear regression analysis to identify the anthropometric variables associated to the bite force. A significance level of 5% was adopted. In the investigation of morphology the subjects with dentofacial deformities presented specific features and compatible with the skeletal class II and skeletal class III, with increase of facial height, mainly in retrognathic subjects. The bite force was lower for DDF, did not differentiate between the groups and there were no asymmetries about them. The occlusal contact area was found to be poorer for subjects with class III deformity, but the occlusal force distribution did not show differences between the groups. It can be concluded the three-dimensional analysis method confirmed specific and relevant findings to the studied dentofacial deformities. And the facial proportions were considered factors influencing the magnitude of the bite force in subjects with deformity.

Full textThe facial morphology of children born with complete unilateral cleft lip and palate (CLP) have injuries of soft and hard tissues of nasomaxillary complex. The purpose of this research was to evaluate the bony structures of the midface of patients with complete unilateral CLP by cone beam computed tomography (CBCT) using the OsiriX software. The sample consisted of CBCT in 28 children with complete unilateral cleft lip and palate in which 19 on the left side and 9 on the right side, with a mean age of 9 years and 6 months. All CBCT were obtained immediately before the secondary alveolar bone graft after maxillary expansion. The structures analysis of the midface was performed using the OsiriXTM DICOM Viewer software, run on a personal computer MacBook Pro, with the operation system MAC OS X Version 10.6.8 Apple Inc. The hemifacial measurements were performed through the demarcation of seven pairs of bilateral points (Zf, Zy, Nm, Cn, J, Or and Po) and by the definition of the median sagittal and coronal planes. The facial asymmetry was verified becoming th side without cleft like the control side and the side cleft like the study side. The distance between each point was calculated in the referred plans for the sagittal and transverse dimensions, which the vertical was estimated by each point in the axial section. The data were tabulated and submitted to statistical analysis by test the Wilcoxon signed-stations, with a significance level p0.050. The results showed a tendency of the cleft side be smaller than the noncleft for transverse evaluations. About the laterality of the cleft it was verified that the clefts on the left show higher degree of asymmetry transverse to the variables Zf, Nm, Or and Zy. In the sagittal assessment, only Cn variable shows a statistically significant difference between the cleft side and noncleft, regardless of laterality of the cleft. Using estimated measurements with axial slices, due to the limitation of the software OsiriXTM, Cn also was the only variable to show statistical significance in the vertical assessment, independent of laterality of the cleft. It can be concluded that the software OsiriXTM has limitations in the assessments vertical. Patients with complete unilateral CLP had transverse asymmetries related to laterality of the cleft, mainly on the left side. The sagittal and vertical asymmetries were focused in the nasal region, independent of laterality of the cleft.

Resumo: Este estudo analisou traçados digitais obtidos por dois programas de computador (Dolphin Imaging® e Nemotec®) em comparação aos obtidos pelo método manual na análise de oito medidas cefalométricas (SNA, SNB, Co-A, Co-Gn, Altura facial anterior, A-Nperp, Pg-Nperp e Plano mandibular). Foram avaliados 30 pacientes leucodermas, sem distinção de gênero, com idade variando de 19 a 48 anos. Os pacientes eram portadores de retrognatismo mandibular e foram submetidos à cirurgia ortognática para correção, por meio da osteotomia sagital do ramo mandibular. Os traçados cefalométricos foram realizados em telerradiografias norma lateral obtidas uma semana antes da cirurgia. Os resultados foram submetidos à análise estatística. Foi utilizado à análise de coeficiente de correlação intraclasse (ICC), teste t ajustado para Tukey-Kramer e o teste Schuirmann. Os resultados mostraram que o método Manual e o método Nemotec® apresentaram excelentes confiabilidades em todas as medidas (ICC > 0,98). O método Dolphin Imaging® apontou baixa confiabilidade nas medidas Altura facial anterior (valor de ICC= 0,70), Co-A (valor de ICC= 0,47) e Co-Gn (valor de ICC= 0,49). Nas medidas A-Nperp, Pg-Nperp, Plano md, SNA e SNB não houve diferença nos três métodos estudados (p > 0,05); na medida altura facial anterior houve diferença entre os métodos Dolphin Imaging® e Nemotec®, mas não foram observadas diferenças em relação ao método manual (p > 0,05); nas medidas Co-A e Co-Gn foi observado que o método Dolphin Imaging® apresentou média significativamente inferior à dos demais métodos estudados (p > 0,05). O método Manual foi equivalente em 6 das 8 medidas (A-Nperp, Pg-Nperp, Plano md, SNA, SNB e Co-A), nas medidas altura facial anterior e Co-Gn não houve equivalência entre nenhum dos métodos (p < 0,01); não houve equivalência em nenhuma das 8 medidas do método Dolphin Imaging
Abstract: A-Nperp, Pg-Nperp e mandibular plane). Thirty patients (male or female), with age varying from 19 to 48 years old were analyzed. All patients were diagnosted with mandibular retrognathism and submitted to bilateral sagital mandibular advancement. All tracings were performed in lateral cephalometric radiographs obtained one week before surgery. The intraclass correlation coefficient analysis (ICC), the t test adjusted for Tukey-Kramer test and Schuirmann test were applied. The results showed that the manual tracings and Nemotec® tracing had excellent reliabilities for all measures (ICC > 0.98). The Dolphin Imaging® showed low reliability in anterior facial height (value of ICC = 0.70), Co-A (value of ICC = 0.47) and Co-Gn (ICC value = 0.49). In A-Nperp, Pg-Nperp, Mandibular plan, SNA and SNB there were no differences between the 3 tracings (p > 0.05), for the anterior facial height measures differences were found between the Dolphin Imaging® and Nemotec® tracings, but no differences were observed as compared to the manual tracing (p > 0.05), in Co-A and Co-Gn measures t Dolphin Imaging® presented a significantly lower mean than the other methods (p > 0.05). The manual tracings were equivalent in 6 of 8 measures (A-Nperp, Pg-Nperp, md Plan, SNA, SNB and Co-A), there was no equivalence between the methods in the anterior facial height and Co-Gn measures (p < 0.01). The Dolphin Imaging® method was not equivalent in any of the 8 measures. It was concluded that in the manual tracing only the Co- Gn, Pg-Nperp and SNB measures confirmed the diagnosis of mandibular retrognathism, and the Nemotec® software showed better results than the Dolphin Imaging® software

Full textUna deformidad se define como una forma o postura anormal de una parte del cuerpo debido a fuerzas mecánicas no disruptivas; surge con mayor frecuencia durante la vida fetal tardía debido a fuerzas mecánicas, afectando a menudo el sistema músculo-esquelético. Las deformidades dentofaciales pueden presentarse en forma aislada y afectar sólo la mandíbula, o bien, extenderse a múltiples estructuras craneofaciales, ser unilaterales o bilaterales, o expresarse en el plano facial vertical, horizontal o transversal. El caso clínico que se muestra, presenta a un paciente de sexo masculino con una deformidad dentofacial clase III producto de una proyección horizontal de la mandíbula, tratado en el servicio de cirugía oral y maxilofacial del HNAL y servicio de ortodoncia de la Clínica de Post Grado de la UNMSM, se realiza una revisión teórica de la patología, fase pr quirúrgica, fase post quirúrgica y seguimiento de 1 año post quirúrgico. El objetivo de este trabajo es describir las características clínicas de una deformidad dentofacial clase III y presentar el manejo ortodóntico quirúrgico de acuerdo al protocolo establecido de las instituciones participantes.

Full textOne of the important means to make an adequate diagnosis and treatment plan in orthodontics and maxillofacial surgery is the evaluation of the facial and skeletal pattern. Objective: The main purpose of this paper is to correlate both patterns using the Steiner analysis for hard tissue and Burstone and Legan for soft tissue on cephalometric radiographs of patients with Class II dentofacial deformity who attended the Institute of Maxillofacial Diagnostics in the year 2015. Materials and Methods: The sample consisted of 92 digital cephalometric radiographs (46 male and 46 female) between 18 and 45 years of age, analyzed with Planmeca Romexis® 4.0 software. Results: . The results indicated that in the male sex with a Class II skeletal pattern was 67.39%, while in the female sex with the same skeletal pattern was 39.13%. Conclusions: Based on the aforementioned data, it can be determined that sex influences the diagnosis and treatment plan.

Full textObjective: The objective of this study was to evaluate the correlation between the skeletal pattern and the facial pattern in cephalometric radiographs in patients with dentofacial deformities class I and III. Materials and methods: The following was a cross-sectional or transverse study. The sample consisted of 160 digital cephalometric radiographs (40 for each gender and class). The analysis was performed by identifying locations of cephalometric points using the VixWinPlatinium ® program, then hard tissues were further evaluated according to the Steiner analysis and Burstone-Legan for soft tissues. The data analysis was performed by means of descriptive statistics (mean ± of) and the correlation of both analysis by means of Pearson correlation. Results: The results indicated that there is no correlation between the skeletal and facial pattern in patients with dentofacial deformity class I. In men 0.15 (p = 0.353) and women -0.12 (p = 0.433). On the other hand, in patients with dentofacial deformity class III, correlation was obtained with 0.41 in women (p = 0.08) and in men 0.32 (p = 0.008). Conclusions: Based on the aforementioned data, it can be determined that sex is not a determining factor to find the diagnosis and treatment of patients with dentofacial deformities class I and III.

Se discute la opción de tratamiento, sus repercusiones estéticas, y su función masticatoria así como los resultados obtenidos.
