Full textThurston showed that for all but a finite number of Dehn Surgeries on a cusped hyperbolic 3-manifold, the resulting manifold admits a hyperbolic structure. Global bounds on this number have been set, and gradually improved upon, by a number of Mathematicians until Lackenby and Meyerhoff proved the sharp bound of 10, which is realized by the figure-eight knot exterior. We improve this result by proving a stronger version of Gordon’s conjecture: that excluding the figure-eight knot exterior, cusped hyperbolic 3-manifolds have at most 8 non-hyperbolic Dehn Surgeries. To do so we make use of the work of Gabai et. al. from a forthcoming paper which parameterizes measurements of the cusp, then uses a rigorous computer aided search of the space to classify all hyperbolic 3-manifolds up to a specified cusp size. Their approach hinges on the discreteness of manifold points in the parameter space, an assumption which cannot be made if the manifolds have infinite volume. In this paper we also show that infinite-volume manifolds, which must be Free Bicuspid, can have cusp volume as low as 3.159. As such, these manifolds are a concern for any future expansion of the approach of Gabai et. al

Full textIn this thesis, we study the construction and deformation of spherical-CR structures on three dimensional manifolds. In order to do it, we give a detailed description of the complex hyperbolic plane, its group of isometries and some geometric objects attached to this space such as bisectors and extors. We show a surgery theorem which allows to construct spherical-CR on Dehn surgeries of a cusped spherical-CR manifold : this theorem can be applied for the Deraux-Falbel structure on the figure eight knot complement and for Schwartz's and Parker-Will structures on the Whitehead link complement. We also define the character varieties for a real form of SL(n,C) for finitely generated groups as some subsets of the SL(n,C)-character variety invariant under an anti-holomorphic involution. We study in detail the example of the group Z/3Z*Z/3Z. These character varieties give deformation spaces for the holonomy representations of spherical-CR structures. With these deformation spaces and tools related to the visual spheres of a point in CP^2, we construct an explicit deformation of the Ford domain constructed by Parker and Will, which gives a spherical-CR uniformisation of the Whitehead link complement. This deformation provides infinitely many spherical-CR uniformisations of a particular Dehn surgery of the manifold, and spherical-CR unifomisations for infinitely many Dehn surgeries of the Whitehead link complement

Full textWe construct taut foliations in every closed 3-manifold obtained by r-framed Dehn surgery along a positive 3-braid knot K in S^3, where r < 2g(K)-1 and g(K) denotes the Seifert genus of K. This confirms a prediction of the L--space conjecture. For instance, we produce taut foliations in every non-L-space obtained by surgery along the pretzel knot P(-2,3,7), and indeed along every pretzel knot P(-2,3,q), for q a positive odd integer. This is the first construction of taut foliations for every non-L-space obtained by surgery along an infinite family of hyperbolic L-space knots. We adapt our techniques to construct taut foliations in every closed 3-manifold obtained along r-framed Dehn surgery along a positive 1-bridge braid, and indeed, along any positive braid knot, in S^3, where r < g(K)-1. These are the only examples of theorems producing taut foliations in surgeries along hyperbolic knots where the interval of surgery slopes is in terms of g(K)

Full textSymplectic instanton homology is an invariant for closed oriented three-manifolds, defined by Manolescu and Woodward, which conjecturally corresponds to a symplectic version of a variant of Floer's instanton homology. In this thesis we study the behaviour of this invariant under connected sum, Dehn surgery, and four-dimensional cobordisms. We prove a Künneth-type formula for the connected sum: let Y and Y' be two closed oriented three-manifolds, we show that the symplectic instanton homology of their connected sum is isomorphic to the direct sum of the tensor product of their symplectic instanton homology, and a shift of their torsion product. We define twisted versions of this homology, and then prove an analog of the Floer exact sequence, relating the invariants of a Dehn surgery triad. We use this exact sequence to compute the rank of the groups associated to branched double covers of quasi-alternating links, some plumbings of disc bundles over spheres, and some integral Dehn surgeries along certain knots. We then define invariants for four dimensional cobordisms as maps between the symplectic instanton homology of the two boundaries. We show that among the three morphisms in the surgery exact sequence, two are such maps, associated to the handle-attachment cobordisms. We also give a vanishing criteria for such maps associated to blow-ups

Full textThe aim of this thesis was to, through the analyses of different discourses surrounding sex reassignment try to understand how the genitals are linked to the social construction of sex and gender. I have interviewed psychiatrists, surgeons and people dealing with the laws surrounding sex reassignment surgery. I also interviewed people from organizations who represent transgender persons, transsexuals and former transsexuals. Most of the professionals indicate in the interviews that the genitals have an essential meaning in the decision-making regarding sex and gender. Even if they have an idea that the gender is “in the head”, it has to match the feelings for the genitals. There is a difference between the reasoning for FtM (Female-to-male-transsexual) and MtF (Male-to-female-transsexual). The penis is seen as the most essential sex marker and most of the professionals that were interviewed had difficulties in understanding how a person who wanted to keep the penis could have a gender identity as a woman. The vagina did not have the same status as a sex marker but instead the reproductive function from the ovaries and the uterus had almost the same status as a sex marker as the penis. For this reason the professionals that where interviewed had difficulties in understanding that a pregnant person could have a gender identity as a man. People from organizations who represented transgender persons, transsexuals and former transsexuals had some differing views. One considered that the whole body including the genitals was important for the feeling about ones gender while the other person expressed that the gender “in the head” was more vital for the decision and feelings of sex and gender.

Full textHealth professionals and the public often have negative attitudes towards patients who are overweight. The number of patients undergoing surgery for their obesity is increasing every year. A surgery for obesity is a major change for the patient and patients seek a lot of information before surgery. The perioperative dialogues have been tried in other patient groups before but not in patients with overweight. The overall aim of the study was to describe how anesthesia and theatre nurses through the continuity of the perioperative dialogue can alleviate suffering of the patient during a surgery for obesity. The study has a hermeneutic design. Data were collected with the help of the perioperative dialogue as documented on the nurse's meeting with 26 patients. Data were interpreted through a hermeneutic text interpretation.The results are described in two main themes To live in an overweight body and Continuity in the perioperative dialogue creates opportunities for the alleviation of suffering. Obesity hinders patients in daily life both mentally and physically, which is putting a strain on their forces, and is experienced as a suffering body. Through the perioperative dialogue continuity reduces anxiety and increases safety. Patients feel that they may be involved in planning, influencing and implementing their care. This participation reduces anxiety and therefore the suffering for the patient. The perioperative dialogue should be obvious as the confidence and safety created by the continuity and patients should be able to maintain its autonomy. Without continuity in the perioperative care disappear the patient's ability to participate in their care.

Full textBackground: In colorectal surgery ”Enhanced Recovery After Surgery” (ERAS) is a standardized pre-, intra- and postoperative concept aiming to enhance recovery after surgery. To achieve good outcome in postoperative care, early nutritional intake and early mobilisation has been identified as important factors as well as the patient´s involvement in their care. In order to develop postoperative nursing care and improve early mobilisation and early nutritional intake, factors affecting patient participation need to be identified. Aim: To describe patients experiences of the postoperative care after colorectal cancer surgery according to ”Enhanced Recovery After Surgery” with a specific focus on patient participation. Method: Semi structured interviews were conducted with individuals undergoing surgery according to ERAS (ten participants). The interviews were analysed using a qualitative content analyze method. Result: The individuals considered themselves well informed before surgery and thought they understood the concept, but the logbook´s function was found to be unclear. All patients were satisfied with the time in the surgical care department and the results showed that the relationship between the patient and healthcare staff are of great importance for postoperative recovery. Maintaining good nutrition postoperatively, however, is a challenge when many patients experienced nausea and had difficulty eating and drinking nutritional beverages. Conclusion: The ERAS concept contributes patient safety through continuity and clear guidelines, enabling participation and stimulating postoperative recovery. People with a weakened physical starting position, however, have a greater need for staff support and a person-centred approach would enable that, based on the ERAS concept. However the majority of patients suffer from postoperative nausea which has a negative impact on patient recovery but also on the experience as a whole, this problem has to be reviewed closer, but also improved if possible.

Full textKnowledge about the importance of nursing environment and patients’ participation in its design promote patients’ well-being. In person centred care, environment is an important component of the human healing process. The aim of this study was to describe the experience of the physical and psychosocial nursing environment in women who underwent day surgery due to breast cancer from registration to immediately before sedation. The method used was qualitative semi structured interview study. Strategic selection was used to recruit informants where eight patients participated. Qualitative content analysis was chosen to analyse the data. The results showed that the informants had a positive experience of the nursing environment at the department. Described experiences in the physical nursing environment varied but were generally positive. Clean and friendly environment was important for the informants’ experience. A “just like home” experience was described. Having a private room was highly appreciated by the informants, which they associated with experienced integrity. However, there were experiences of deficiencies in how integrity was preserved in the common waiting room due to its design. Experienced professionalism and skills in the nursing staff resulted in a sense of security and trust. The nursing staff’s good working methods and patient treatment created a sense of harmony and security that was experienced as calming. In conclusion, the department’s nursing environment affected the informants' experience generally positive. Although the experience of the physical nursing environment was described as not optimal, experience of the psychosocial nursing environment and good patient treatment outweighed the informants' overall experience.

Full textToday's society requires that each individual has to take decisions on all the choices that we face. When it comes to choices that either benefit or disadvantage our health the possibilities are endless. As a result of this there is a large part of the population that has increased in weight and might need help in making lifestyle changes. The purpose of this study was to investigate the factors that may have influenced and motivated the choice of going through a gastric bypass surgery, and to discern which changes following that kind of lifestyle change. Semi-structured interviews with the four participants were conducted, data were analyzed using qualitative content analysis. The result was partitioned into three main categories: Life before surgery, Factors that influenced and motivated to choose gastric bypass and Life after surgery. The findings displayed that the participants were motivated by their concern for the future health and their hopes for a changed future, the findings also showed that their choice was influenced by the surroundings. The surgery led to an improvement in mood and social life as well as an increase in quality of life for the participants.

Full textBackground: Acute postoperative pain is a common but also expected consequence after surgery that gives negative impact on the following postoperative recovery. Study objectives: The purpose was to investigate the average level of self-assessed intensity of pain that gave adult patients problem to rest, sleep or mobilize during the first postoperative day and if men/women, younger/older and surgical or orthopedic patients had different manageable levels of pain related to rest, sleep and mobilization during the early postoperative recovery. Design: A quantitative empirical descriptive study was performed from datasets from 479 elective surgical and orthopedic patients. Results: The result showed that the orthopedic patients self-assessed higher levels of postoperative pain than the surgical patients, but also that the surgical patients earlier got problems with recovery at low pain levels. Conclusions: The result underlines that pain is a subjective experience and that the pain treatment has to be individualized just to prevent the negative impact of pain but also to enhance the postoperative recovery and improve patient safety.

Full textAbstract During the years from 1986 until 1996 2019 thyroid gland surgeries have been performed at the university clinic charite', campus virchow-clinic. That means 3471 thyroid gland lobes have been treated. This paper is investigating the complications directly caused by the surgical procedures. Data gained directly during surgery and 6 month after surgery allow a very detailed view on the complications. There have been 4.5% transient recurrent nerve palsies and 0.7% permanent. Data based on the "nerves at risk" show 2.8% transient and 0.5% permanent palsies. A direct comparison of hemithyroidectomy and near-total thyroidectomy on the contralateral side show no significant difference in permanent nerve palsies. The hemithyroidectomy with contralateral near-total thyroidectomy was the surgical procedure with the lowest rate of complications compared to the thyroidectomy or the near-total thyroidectomy of both lobes. Postoperative Hypocalcemia was seen in 18,9% of all patients, but only 0.9% suffered from permanent hypocalcemia. The ligature of the lower thyroid artery close to the thyroid gland showed lower rates of hypocalcemia , but the results haven't been statistically significant. There has not been a higher rate of nerve palsies or a higher rate of relapses, so the ligature close to the gland is recommended. 149 operations were necessary due to recurrent growth of the thyroid gland. There have been126 real relapses, the rate of permanent nerve palsy was 1,6% for the "nerves at risk". Hypocalcemia was found in 23,8% of all cases, 0.8% were permanent. Patients older than 70 years do not have a higher risks to suffer from complications than younger patients, so thyroid surgery should also be performed for the elderly.

Full textThis kind of cancer is the ugly kind, it’s difficult to get used to. A qualitative study of women’s experiences of sexuality and sexual function after rectal surgery. Master Thesis in Sexology, 30 credits. Malmö University: Faculty of Health and Society, Department of Social Work, 2020. The purpose of the study was to investigate how women who have been treated for colorectal cancer experience sexuality and sexual function with focus on questions considering the physical body, internal processes such as thoughts and feelings, and external factors such as context. The ambition of the study is to contribute knowledge to professionals in healthcare who meet cancer patients in general. The empirical data in the study includes 7 semi-structured interviews that have been analysed through qualitative content analysis. The themes that emerged were; body, brain and the context, thoughts about normality and the experience of an abnormal body, the sexual personality, sex in a context; the distinction between desire and incentives, strategies for dealing with challenges and perspectives on healthcare. The study shows that individual variations based on women's cognitive schemas influence how sexually relevant stimuli are interpreted and affects the behaviours that follow, which is important to consider after cancer treatment. The study shows a connection between negative thoughts and perceived sexual problems. The informants presented a fear of being judged, by partners and in general. Cancer in the rectum was interpreted as stigmatizing by losing a function that previously could be discreetly controlled, several of the women felt that they were inhibited in their lives. Professional help was important for sexual rehabilitation and finding positive sexual strategies.

Full textUnderlying this work is a geometrical construction based on the possibly-singular core which arises from generalised hyperbolic Dehn surgery on M .Given a hyperbolic structure there is a natural spine for M whose complement is the interior of a ‘tube domain’ D analogous to a Dirichlet domain based on the singular core. This domain D is a hyperbolic solid torus and the hyperbolic structure on M can be recovered from D by identifying points of its boundary.
Conversely, we can try to reconstruct a hyperbolic structure on M from its ortholength invariant. To each point of P we give a representation π1(M) \to PSL2(C). We then describe a method for constructing a corresponding hyperbolic structure on M via its tube domain. This construction is quite general and it succeeds in many cases where traditional methods fail.

Full textLet M be an orientable and irreducible 3-manifold whose boundary is an incompressible torus. We are interested in immersed essential surfaces in closed 3-manifolds obtained from Dehn fillings on M. We show the following two things.
In Chapter 2, we suppose that M does not contain closed non-peripheral incompressible surfaces. We show that the immersed surfaces in M with the 4-plane property can realize only finitely many slopes. Moreover, we show that only finitely many Dehn fillings on M can yield 3-manifolds that admit non-positive cubing. This gives the first examples of hyperbolic 3-manifolds that cannot admit non-positive cubing.
In Chapter 3, we suppose M is hyperbolic. We show that all but finitely many Dehn fillings on M yield 3-manifolds that contain closed essential surfaces. Moreover, we give a bound on the number of exceptional Dehn fillings.

Full textWe present in this thesis a few properties of monotone Lagrangian submanifolds. We first solve a conjecture of Barraud and Cornea in the monotone setting by showing that the relative Gromov radius of two Hamiltonian-isotopic Lagrangians gives a lower bound on the Hofer distance between them. The general non-monotone case remains open to this day. We define all the structures relevant to state and prove the conjecture. We then define a new version of a Lagrangian Seidel morphism through the recently introduced Lagrangian cobordisms of Biran and Cornea. We show that this new version is chain-homotopic to various other versions appearing in the litterature. That all these previous versions are the same is folklore but did not appear in the litterature. We conclude with a conjecture claiming that an exact triangle obtained by Lagrangian surgery is isomorphic to an exact triangle of Seidel involving the symplectic Dehn twist.

Full textIntroduction: Operating theatre nurses have limited opportunities to meet the patient and studies focuses mainly on the intraoperative care. Operating theatre nurse meets the patient briefly just before the surgery together with other members of the surgical team. Aim: The aim of the study was to describe operating theatre nurse´s experiences of meeting with the patient during the perioperative nursing process.Method: The study was a qualitative interview study. In total six interviews with operating theatre nurses with at least three years of professional experience and were working at a hospital in central Sweden. The data was analysed through a content analysis. Results: The analysis revealed three categories with associated subcategories that described the operating theatre nurses´ meeting with the patient in the perioperative care: To establish a first contact – to make themselves known, to create a care relationship, making the patient involved in care; To ensure patient care – to prevent patient exposure to injuries, having the opportunity to evaluate the implemented care measures; To preserve patient privacy – wanting to do good. Conclusion: In the meeting with the patient a care relationship is created, which makes the patient involved in the care and the operating theatre nurse is given the opportunity to plan, to implement and to evaluate the implemented care measures, which contributes to the development of the patient´s care and the operating theatre nurse´s competence.
