Dehydration is linked to dysphagia and impaired cognition which are conditions frequently encountered by speech-language pathologists (SLPs) working in health care. The SLP can play a role in dehydration prevention by participating in hydration management with individual patients and by addressing institutional hydration practices. This article highlights the need for awareness of predisposing factors that make older people susceptible to dehydration, the detection of signs and symptoms of dehydration, and the implementation of preventative strategies.

Sport is not in itself the cause of enhanced mortality. During physical activity physiological changes, such as increased catecholamine levels, acidosis, and dehydration, can trigger cardiac arrest in those young athletes who are affected by underlying cardiovascular disorders. Because sports can prompt adverse events from underlying cardiac disease, pre-participation screening has been advised as strategy in the prevention of sudden cardiac death (SCD) in competitive athletes. SCD is rare in young healthy athletes, but remains a tragic and counter-intuitive event, subject to persistently high public visibility, triggering of emotion, and media attention, with potential legal liability considerations. Pre-participation screening in children and adolescent athletes can be done by an experienced specialist (paediatrician, family doctor, cardiologist, or sports medicine specialist); it includes family and personal history, physical examination (including blood pressure), and resting 12-lead ECG. All these issues are discussed in this chapter.

Constipation is frequent in the palliative care population. Constipation can present with a variety of symptoms, including nausea, vomiting, overflow diarrhea, abdominal distention, and pseudo-obstruction. Common causes are metabolic disorders, dehydration, autonomic failure, medications such as opioids, intra-abdominal tumors, and immobility. Constipation is often undiagnosed and is the cause of significant distress in patients with life-threatening diseases and at the end of life. Malignant bowel obstruction can present as severe constipation. Structural disorders such as intraluminal and extraluminal bowel lesions caused by primary or metastatic tumor affect peristalsis and the maintenance of normal bowel function. The management and prevention of constipation involve adequate hydration, avoidance of excessive dietary fiber, laxatives, prokinetic agents, and adjustment or discontinuation of constipating medications. In the case of pure opioid-induced constipation, the mu opioid receptor antagonist can be helpful, but it must be avoided in patients with a history or clinical signs of bowel obstruction.

It covers, causes, priorities, and community-based and clinic programmes, including immunization and the Integrated Management of Childhood Illness (IMCI). It emphasizes ways in which diarrhoea, pneumonia, and malaria can be managed at community and family level. It highlights the dangers and types of diarrhoea, its causes, and treatment. It includes a detailed section on dehydration and oral rehydration therapy (ORT and ORS), appropriate rehydrating foods, and how these can be embedded into use by families. It describes acute respiratory infection (ARI), its causes, prevention, diagnosis, and treatment. Importantly, it describes how community health workers (CHWs) and community members can assess ARI severity and how it can be best treated, including the careful use of community-based and home-based antibiotics in remoter areas. Finally, the chapter addresses malaria diagnosis and treatment, particularly how a community can set up a step-by-step control programme.
