Abstract:
Existential distress among caregivers of patients with all sites and stages of cancer is common and may serve as a driving mechanism of caregiver burden. Concurrently, the caregiving experience is an opportunity for meaning-making and growth. To date, no empirically supported treatments specifically target meaning-making and existential distress among cancer caregivers. To address this critical gap in the literature, meaning-centered psychotherapy has been adapted for delivery to cancer caregivers (meaning-centered psychotherapy for cancer caregivers (MCP-C)). This chapter presents the rationale and overview of MCP-C, and it highlight key elements of the intervention that uniquely target the specific existential needs of cancer caregivers. A case example of an MCP-C group member is presented to illustrate key MCP-C therapeutic techniques. It also discusses challenges of delivery of MCP-C to this vulnerable population and current MCP-C trials that address these barriers to delivery and utilization.

This commentary focuses on Article 2.1.8 of the UNIDROIT Principles of International Commercial Contracts (PICC) concerning acceptance of an offer within a fixed period of time and how such time is to be calculated. Art 2.1.8 stipulates that a period of acceptance fixed by the offeror begins to run from the time that the offer is dispatched. A time indicated in the offer is deemed to be the time of dispatch unless the circumstances indicate otherwise. In general, deadlines for acceptance may be either absolute or relative. Art 2.1.8 is concerned only with relative deadlines, for only relative deadlines leave anything for calculation. In cases where the offer has arrived but its date is manifestly wrong, it may be sufficient to lead evidence of the probability of the date of dispatch by working back from the date of delivery.

Evidence demonstrating the efficacy of treatment and prevention programs for eating disorders is accruing. However, the common face-to-face delivery of these interventions has a number of limitations, including high cost and limited accessibility. E-mental health, referring to the use of information and communication technology—particularly the Internet—in interventions for mental health disorders, has the potential to overcome these barriers and enhance the treatment and prevention of eating disorders. To date, the limited number of evaluations have documented small to moderate effect sizes in the improvement of eating disorder symptomatology through Internet-based treatment and prevention. Beyond efficacy, major questions remain regarding content, structure, and modes of delivery of Internet-based interventions; suitable diagnostic tools and safety measures; and cost-effectiveness, dissemination, and implications for public health programming. These aspects deserve attention in future research before widely recommending Internet-based interventions for eating disorders.

This chapter reviews the importance of delivering community-based systems and EBIs with fidelity (i.e., in accordance with their implementation requirements) and sustaining these interventions over time. The chapter describes the training and technical support provided in CTC to ensure that coalitions take necessary actions to maintain their functioning in the long term and deliver EBIs in adherence to their core components and to their intended recipients. It is especially important that coalitions collect data on coalition functioning and EBI delivery and use these data when problems are identified. Examples of how CTC coalitions in the United States and other countries have engaged in these efforts are highlighted.

This article focuses on flexible Bayes regression of epidemiologic data involving pregnancy outcomes. It first provides an overview of finite mixture models and nonparametric Bayes methods before discussing some of the possibilities focusing on gestational age at delivery, DDE and age data from the Longnecker et al. (2001) study. More specifically, it examines how risk of premature delivery is impacted by maternal exposure to the pesticide DDT. The results showcase the use of Bayesian analysis in epidemiological studies that collect continuous health outcomes data, and in which the scientific and clinical interest typically focuses on the relationships between exposures and risks of an abnormal response, corresponding to an observation in the tails of the distribution. The article also highlights the limitations of current standard approaches that can be overcome by means of Bayesian analysis using density regression, mixtures and nonparametric models, as developed and applied in this pregnancy outcome study.

This chapter focuses on delivery considerations for patients with intracranial masses, spinal cord injuries, CNS vascular abnormalities, and neuromuscular disorders. Special attention is paid to issues regarding pre-delivery planning, optimal timing of delivery, preferred mode of delivery, initial assessment on labor and delivery, and postpartum considerations. For patients with neurologic disorders in pregnancy data regarding optimal mode of delivery is often limited due to the rare nature of some conditions. This chapter highlights the most recent evidence and strongly emphasizes the need for a multidisciplinary approach with close collaboration between anesthesiology, neurology, and neurosurgery to achieve optimal pregnancy outcomes.

Myanmar has been linked to alleged past and potential future mass atrocities by international non-governmental organizations and, at times, the UN Special Rapporteurs on Human Rights. To date there has been no international commission of inquiry, let alone any referral to the International Criminal Court. Looking beyond controversial efforts to justify the forcible delivery of assistance following Naypyidaw’s problematic response to Cyclone Nargis, this chapter contextualizes and describes the extent of alleged R2P atrocities in Myanmar, outlines how the Myanmar government as well as the United Nations and regional organizations such as ASEAN and other key actors in the international community have implemented R2P either directly or indirectly, and offers a provisional explanation as regards the limited nature of the international community’s response to concerns that Naypyidaw has at least at times failed to exercise its R2P in the contexts of internal armed conflict and communal violence.

THE ROOTS of implementation science (IS) in cancer in some sense date back to the earliest days of uncovering cancer’s etiology, diagnosis, prevention, and treatment, although it was not called that. Indeed, unlocking the mysteries of cancer and determining effective ways to intervene began not in the lab but, rather, the clinic. As Mukherjee recounted in the seminal work, The Emperor of All Maladies, 1 cancer had been the subject of clinical examination for centuries, and the drive to optimize care began in those early days. As opposed to the largely separate worlds of research discovery and care delivery that exist today, scientific research and cancer treatment coexisted. In addition, epidemiologic observations of risk factors affecting oncogenesis developed targets for what types of prevention programs needed to be implemented. Naturally, the challenges of what exactly to implement and how best to implement have been with us throughout time.

Big data promises to harness the power of advanced computing to transform health and health care, including cancer research and care delivery. In health care, big data can be generated by administrative and clinical processes, by patients and families, and by machines. Ultimately, the goal of big data is to transform data into actionable knowledge with attention to four dimensions: person-level data collection; data access, exchange, and aggregation; population-level analytics; and provider, researcher, or patient-facing clinical decision support. A fabric of trust forms the basis for policies for governance, privacy and security, and confidentiality. This chapter offers several examples of the application of big data along the cancer care continuum, ranging from primary prevention through diagnosis, survivorship, and end-of-life care. Challenges to the effective collection and use of big data include its integration with health care delivery; interoperability; and the need for validated, well-designed informatics tools.
