Demensia Alzheimer menempati urutan kesembilan penyebab kematian di Amerika Serikat. Demensia adalah kondisi yang sering dialami yang berhubungan dengan berbagai faktor dan gaya hidup terutama diet. Penelitian ini bertujuan untuk mengetahui hubungan defisiensi asupan riboflavin (vitamin B12) dengan demensia pada usia lanjut (usila). Penelitian ini menggunakan desain studi cross sectional dan dilaksanakanpada bulan September 2007 sampai dengan Januari 2008. Sampel penelitian adalah 141 lansia berumur lebih dari sama dengan 60 tahun yang diambil secara purposive sampling. Demensia diukur menggunakan kuesioner MMSE (² 24, skor maksimum 30) dan asupan riboflavin diukur menggunakan form Semi Quantitative – FFQ. Penelitian ini memperlihatkanbahwa 47,5% usila mengalami demensia. Hasil uji statistik menunjukkan bahwa terdapat hubungan yang signifikan antara umur, tingkat pendidikan, dan asupan riboflavin dengan kejadian demensia pada usila (nilai p < 0,05).Kata kunci: Demensia, defisiensi riboflavin, usia lanjutAbstractDementia Alzheimer’s was ranked the ninth leading cause of death in The United States. Dementia can not be avoided as related to several factors and lifestyle especially the diet. The objective of this research is to know relation the deficiency of riboflavine (vitamin B12) intake and incidence of dementiaat elderly. A cross-sectional study was conducted betweenSeptember 2007 and January 2008. The sample obtained was 141 elderly which it was conducted to purposive sampling. Dementia was measured by using questionnaire MMSE (² 24, maximum score was 30), and riboflavine intake was measure by Semi Quantitative – FFQ form. This study shows that dementia in elderly was 47,5%. Abstract When considering the various extrinsic variables that may affect disease vulnerability, it is valuable to study temporal ordering of factors to identify areas for disease intervention efforts. This study sought to inform improvement of networks for the purposes of education and health by attempting to better define the causal ordering of ethnicity, age, gender, education, disease burden, and dementia diagnosis with the Aging, Demographics, and Memory Study, a sub-study of the Health and Retirement Study. Participants and/or proxies self-reported total number of chronic conditions, subsequently regarded as disease burden. Assessments occurred over four waves; participants were not reassessed after dementia diagnosis. The current study categorized participants as demented (n=414), identified in any wave, or normal (n=117), identified in the final wave. Cognitively-impaired-not-demented and deceased participants were not considered due to lack of diagnosis. Cross-sectional weighting was used. A path model was developed; ethnicity, age, and gender were antecedent to education, and education was casually ordered before disease burden, which was antecedent to dementia diagnosis. A series of logistic and linear regression analyses were conducted. Results revealed that being non-white (Σβ=0.643, all p’s<.001), of older age (Σβ=0.250, all p’s<.001), female (Σβ=0.180, all p’s<.001), and having increased disease burden (Σβ=0.118, p<.001) all demonstrated a positive total effect on dementia diagnosis. Conversely, more years of education (Σβ=-0.245, p<.001) had a negative total effect on diagnosis. The education to disease burden pathway was non-significant. Ultimately, these results may indicate a need for dementia interventions that target those with low education or high disease burden.

ABSTRACT Objective: To investigate the most commonly used educational approaches in dementia training for primary health care professionals. Method: Integrative literature review, conducted between April and June of 2018, in PubMed, LILACS and IBECS databases. The descriptors used were: Training, Health Personnel, Dementia, Primary Health Care for PubMed; and the MeSH terms, Training Programs, Health Personnel, Dementia, and Primary Health Care for LILACS and IBECS. Results: The sample consisted of 13 articles; eight were published in the last five years (62%); seven articles with a quantitative approach (54%); seven articles produced on the European continent (54%), followed by five published on the North American continent (38%). All journals were from the health area (100%). Conclusion: Educational strategies were combined and used for education. Significant improvements in knowledge, skills, and attitudes of the teams with regard to professional management of dementias were evidenced.

Abstract:
Primary prevention will become increasingly important as dementia prevalence increases and effective retardive therapies are developed. To date, only one randomized controlled trial (involving treatment of systolic hypertension) has demonstrated that the incidence of dementia can be reduced. Physicians should remain alert to possible secondary causes of dementia and correct these whenever possible. Primary and secondary prevention of stroke should reduce dementia related to cerebrovascular disease either directly or as a comorbid factor in Alzheimer’s disease (AD). Epidemiological studies have revealed a number of risk factors for AD including genetic mutation, susceptibility genes, positive family history, Down’s syndrome, age, sex, years of education, head trauma and neurotoxins. In case-control studies non-steroidal anti-inflammatory medication and estrogen replacement therapy appear to decrease the relative risk of developing AD. Further research to develop and test preventative therapies in AD and other dementias should be strongly encouraged.

AbstractThe Clock Drawing Test (CDT) has been recognized as an effective tool for dementia detection. This study investigated the clock drawing performance of 240 non-demented elderly Korean people with a wide-range of educational levels and 28 patients with mild dementia of the Alzheimer’s type (DAT). We examined the effects of demographic factors, including education, and established norms for the elderly population. We found that the educational attainment and literacy status of older people influenced performance on the CDT significantly (p < .001). Furthermore, qualitative error analysis revealed that normal participants with low educational background committed errors similar to errors of the DAT patients. The DAT patients performed significantly worse than the non-demented participants in the CDT Total score (p < .001). However, the CDT has better criterion validity in participants with more than 6 years of education. In conclusion, the CDT performance in older people who are either illiterate or with 6 or less years of education should be interpreted with caution. Conceptual errors in the CDT can be the result of not only dementia but also lack of education. (JINS, 2010, 16, 1138–1146.)

Abstract:
Providing competent, high-quality, and person-centered care is important in the healthcare environment, including the care for the aging and training of those who provide care. A knowledge gap in dementia-specific training was identified at an assisted living facility. The purpose of this project was to answer the question whether a dementia-specific educational staff training program would improve staff members' knowledge and application of dementia-specific competencies as well as caring behaviors. Watson's theory of human caring, the theory of planned behavior, and Leininger's framework of culture care were used to inform this project. The educational project evaluated the success of implementing nurse caring behaviors and a dementia-specific training program. Data collection included analysis of the responses of 20 facility personnel to a survey validating the needs assessment. Results of pre- and posttesting of each educational module demonstrated a statistically significant improvement (p =.0001), and observation of staff-resident interactions demonstrated 100% achievement on the competency checklist across all project participants. Qualitative analyses of data gathered from participants' discussions resulted in 4 themes: (a) the characteristics of dementia care, (b) individual caregiver attitudes, (c) knowledge deficit, and (d) the importance of caring behaviors. Educational training methods were found effective to elicit nursing staff behavior change and improve understanding of the dementia patient and requisite care, which represents a significant positive social change and enhanced care for the dementia patient in the assisted living facility.

Abstract:
Dementia, a clinical condition that affects the psychological ability of patients, is distinguished by a significant overall decline in cognitive function that results in distorted perception. Guiding nursing practice in the long-term care (LTC) setting to decrease the unnecessary use of psychotropics is critical because doing so relates to the patients' quality of life and safety. In the LTC facility that served as the practicum site for this study, there was an observed overuse of psychotropic medications in the care of patients with dementia. The practice-focused question guiding this project asked whether a nursing staff development program would decrease the use of psychotropics in dementia patients. The purpose of the project was to inform nursing staff through an educational program on alternative methods to use when dementia patients exhibit increased disturbing behaviors. The conceptual framework for the project was the knowledge-to-action model. The nursing staff development program had a positive effect on the nursing staff as evidenced by a statistically significant improvement in knowledge and attitudes about the use of psychotropics in caring for dementia patients. The use of psychotropic in the dementia patient decreased from 22.32% to 15.77%, the lowest score achieved by the organization in 5 years. The dementia patients benefited from this project and its positive social change implications for nursing practice by decreasing dementia patients use of psychotropics, minimizing their side effects to the patients and providing an overall feeling of well-being.

Abstract:
The purpose of the research is to examine the family experience of dementia caregiving and design an educational intervention to assist family members in the caregiving role. Stress results when a caregiver confronts environmental demands (dementia behaviors) that threaten personal well-being. The perception of threat is the process of primary stress appraisal. Caregivers evaluate coping options, a process of secondary stress appraisal. Positive secondary appraisal (view the situation as amenable to change) has the potential to trigger re-appraisal of the primary threat, and to reduce overall stress. Education is one way to affect secondary appraisal. Adult learners benefit from progressive (problem solving) approaches using self-directed learning principles. With self-directed learning, the educator facilitates the learner’s access to the information. Distance education is a form of self-directed learning. Distance techniques include postal mail, facsimile, telephone, video, teleconferencing, satellite conferencing, and the Internet. The research question is: to what extent can distance education (by postal mail, passive Internet, and multimedia, interactive Internet) alter the primary and secondary stress appraisals of dementia caregivers? A total of 189 caregivers participated in the study. The study design, Switching Replications, includes three waves of measurement over a two month period. With the design, some participants receive the intervention upon entry into the study, while others wait until after the second wave of measurement. Three study hypotheses relate to the construct of primary appraisal and the dependent variable of threat perception. Analysis of Variance (ANOVA) results do not support hypotheses that posit the form of education affects threat perception. Four hypotheses address the construct of secondary appraisal and the dependent variable of home modifications. ANOVA results support hypotheses that posit the form of education affects home modifications. Active web education is statistically better than receiving no education or receiving information by postal mail. Assuming no cure or effective treatment, the number of people diagnosed with dementia will increase to 8.6 million by 2040 (General Accounting Office,1998). Despite the stress, families are and will likely remain the primary caregivers. Distance education merits further study as a way to reach and help family caregivers.

<p> Behavioral complications, such as Inappropriate Sexual Behavior (ISB), are developed in many cases of dementia. Sex, even for those with dementia, is a primary need, but supporting this need in an appropriate manner may overwhelm caregivers. The purpose of this project was to create a curriculum for Dementia Friendly Community (DFC) residential caregivers on the topic of sexual expression among older adults with dementia. By offering training on sexuality and dementia it is expected that caregivers can increase the DFC's provision of quality of life. This project presented an assessment tool and plan on how to manage ISB, activities relevant to the discussed topics, and curriculum evaluation forms. Expert reviewers offered recommendations to further improve the curriculum's quality.</p>

The purpose of this study was to understand the decision making process of caregivers placing their elderly family members in a nursing home facility. Experiential Learning Theory (ELT) was used, as well as the Critical Incident Technique (CIT). ELT was utilized in an effort to understand the learning that took place during the caregiving experiences, and CIT was used to better understand the critical incidents that led the caregivers to seek nursing home placement. A sample of twelve former informal dementia caregivers between the ages of fifty-seven and eighty-seven was drawn from the metropolitan Richmond, Virginia area. In-depth interviews were audiotaped and provided the primary source of data for this study. An interview protocol consisting of eleven open-ended questions derived from current dementia caregiving literature guided the conversation between the researcher and the caregivers in the sample. A constant comparison method was used in this study. The findings revealed that there are a variety of reasons why informal dementia caregivers seek nursing home placement for their family members. Themes related to the decision making process to seek nursing home placement include (1) dementia related behaviors, (2) safety concerns, (3) emotional and psychological burden, and (4) unexpected medical intervention. Indicators of each theme found in this study suggest that providing informal care for an individual with dementia can be very overwhelming and challenging. Although there were some positive aspects associated with this form of caregiving, such as feelings of pride and self-worth, the overall consensus from this study was that dementia caregiving is a very difficult experience in which the primary caregiver had to ultimately seek formal placement in a nursing home for their family member for a variety of reasons.

Abstract:
Dementia is a serious public health concern in the United States, with a prevalence of 5.2 million. There is currently no effective way to prevent or cure dementia, and the precise etiology is unknown, but it appears there are multiple risk factors. Prediabetes (PD) has been identified as a risk factor although the scientific evidence is conflicting. This study is important to those at high risk for dementia and to healthcare professionals who lack substantiated dementia prevention strategies. The purpose of this case control study was to determine whether PD is associated with dementia in adults aged 65-95 years and whether the association varies according to demographic (age, gender, race, and socioeconomic status [SES]) and health (atherosclerosis, body weight, cerebrovascular disease, dyslipidemia, hypertension, and stroke) risk factors. The ecosocial theory was selected to bridge the study findings to life-course exposures and risk factors. Cases (n = 574) and controls (n = 2,157) were sampled from a large ambulatory care dataset, and multivariable logistic regression was used to test the research hypotheses. No unadjusted association between PD and dementia was found (OR 1.08, 95% CI = .854, 1.241, p = .604). The regression analysis revealed no association between PD and dementia; however, atherosclerosis, hypertension, low body weight, and low/average SES were found to be significantly and independently associated with dementia. A stratified analysis revealed that race and SES did not alter the effect of PD on dementia. The implications for positive social change include the potential reduction of incident dementia through initiatives targeted toward demographic and health risk factors including atherosclerosis, hypertension, low body weight, and low/average SES.

Abstract:
Diabetes mellitus is still a very common disease in the world and affects the daily lives of patients negatively. Diabetes is also known to be associated with neurological diseases such as peripheral nerve diseases, stroke and dementia. Among these, the most common disease is a peripheral nerve disease, and it has been reported that poor diabetic control increases the risk of development and can be prevented by education of the patients. Vascular dementia is more common in patients with diabetes than Alzheimer's disease, and it is thought that cerebrovascular diseases may berelated to cognitive impairment in diabetes. Although the mechanisms by which diabetes affects the brain are not clearly revealed, it is thought that changes in vascular structure, insulin resistance, glucose toxicity, oxidative stress, accumulation of glycation end products, hypoglycemic episodes and amyloid metabolism are effective.The aim of this article is to describe the neurological complications of diabetes and to emphasize the importance of patient education, good diabetes control and early diagnosis in preventing these complications.
